Background: Acute and transient psychotic disorders (ATPDs) have benign course and takes place in the existence of noticeable and exceptionally traumatic stressors (direct temporal association between stressors and occurrence of ATPD) which necessitate clinical investigations. Stressors are also known to affect phenomenology and its content. Aims and Objectives: This study aims to study the socio-demographic characteristics, clinical variables, the role of stressors and phenomenology in ATPDs patients. Methodology: It is a cross-sectional and observational study. This study was done on 150 patients diagnosed on the basis of International Classification of Disease-10 diagnosis criteria of ATPDs after getting an informed consent from them. Patients’ information was recorded on socio-demographic and clinical profile sheet. Thereafter, Presumptive Stressful Life Event Scale and Positive and Negative Syndrome Scale (PANSS) scales were applied to assess the stressors and phenomenology, respectively. Results: Most of the patients were female and were between 15 and 35 years of age. Mean age of patients was 32.45 (standard deviation = 11.25) years. Majority of the patients were educated, married, and unemployed. A large amount of the patients belong to nuclear family and were resident of rural areas. Most of the patients presented within 2 weeks of the onset of the illness without any past or family psychiatric history. In 75% patients, stressors precede the onset of the illness. The average value of PANSS was higher in female. Sleep disturbances, delusions, hallucinations, poor rapport, lack of insight, and concrete thinking were the most commonly observed phenomenology in these patients. Conclusions: Maximum patients who developed the illness had psychological stressor/s in the past year (mainly before 2 weeks) of the onset of the illness. It helps to consider the person’s life events as a trigger for illness and make decisions regarding treatment accordingly.
IntroductIonThe term "nevus variqueux osteohypertrophique" was first used by Maurice Klippel and Paul Trénaunay, around a century ago, to describe two patients who had hemangiomatous lesions of the skin along with asymmetric soft tissue and bone hypertrophy. 1 Klippel-Trénaunay syndrome (KTS) was later used to describe the triad of capillary malformations, venous malformations, and soft tissue or bony hypertrophy. 2An incomplete form of KTS is defined as having at least two of the three cardinal features.Frederick Parks Weber later noticed the presence of these observations with arteriovenous fistulas, which is known as Klippel-Trénaunay-Weber syndrome. 3 The cause of the syndrome is not known, but it has been suggested that it is caused by a somatic mutation associated with angiogenesis and vasculogenesis during the embryonic period. 4 If the patient has bleeding from capillary or venous malformations or persistent embryonic veins, they may present with intramucosal, intramuscular, intraspinal, or intracerebral hemorrhage. 5 Only two other cases of KTS that resulted in hearing loss have been reported.We describe a case report of KTS in a 5-year-old boy associated with bilateral conductive hearing loss.
Kimura's disease (KD) is a chronic inflammatory disorder of the lymph node which is very rare in the Indian population. A 34-year-old female presented with left postauricular region swelling for the past 3 years at an outpatient department. On histopathological examination, it was diagnosed as KD. It should be kept in mind when treating a patient with lymphadenopathy and eosinophilia or a high immunoglobulin E level. This unique case report highlights this impressive clinical entity.
Background: Psychiatric disorders have been rising in last few years and emerging as leading cause of disease burden. Psychiatry as a discipline of medicine carry a lot of stigma due to which there is a lack of young medical students or professionals in choosing psychiatry as a career. Aims and Objectives: To study the impact of psychiatry posting on attitude towards psychiatry and estimation of likelihood of Psychiatry as a career choice in female interns. Materials and Methods: A cross sectional observational study of 112 female interns who met the Inclusion criteria. A written informed consent was obtained from the interns. A selfassessable ATP-30 scale was used to assess the attitude of the interns, this questionnaire was filled by the interns on the first day and after the two weeks of their posting in psychiatry department. The data collected was analysed with the help of SPSS version (21.0) info. Results: Ninety-eight interns were considered for further analysis because 14 female interns fulfilled the exclusion criteria. More than 95% interns reported to have positive attitude towards psychiatry throughout the posting. Results showed a statistically significant (<0.01) positive attitude towards major areas in psychiatry except only psychiatric hospitals and treatment. Only 33 female interns chose psychiatry as a career choice after the posting of two weeks which is not statistically significant, while around 40% interns denied for it. Conclusion: Interns showed highly positive attitude towards psychiatry. The positive attitude of interns towards psychiatry would go a long way towards reducing the stigma attached to people with mental illness and mental health professionals.
The pterygopalatine fossa (PPF) is a challenging space and pathological processes in this anatomical space are uncommon. This report presents a case of glomus tumor right PPF in a 65 years old female patient who underwent tumor removal by endoscopic transnasal-transmaxillary approach with preoperative selective embolization of the right internal maxillary artery.
Tuberculosis of the nose is so rare that it has become a forgotten entity among the clinicians. The clinicians fail to diagnose primary nasal TB as symptoms and signs of this specific nasal inflammation mimic other nonspecific nasal inflammatory conditions and therefore, its diagnosis and treatment is often delayed. We came across a case of primary nasal TB in a 65 years old female presented with complaints of nasal obstruction, epistaxis and anosmia since past 3 years. Anterior rhinoscopy showed friable nasal mass in both nasal cavities. Computed tomography scan showed heterogeneously enhancing soft tissue in nasal cavity with destruction of the nasal septum and hard palate. Histopathological examination and GeneXpert confirmed mycobacterium tuberculosis. The patient was put on antituberculous therapy.Nasal endoscopy and tissue biopsy play important role in diagnosis of the disease. GeneXpert test not only has good sensitivity and specificity for the diagnosis of EPTB but also perfectly fits the requirements of the Indian health care setting.
Background: Organophosphate (OP) poisoning is a common cause of significant mortality and morbidity all over the world. In Asia, OP poisoning is the most common form of fatal self harm. Previous studies have suggested hearing loss due to OP poisoning. The aim of the study was to study audiological assessment in the patients with OP poisoning.Methods: 26 patients (age <50 years) of OP poisoning over a period of one year were included in the study following ethical approval from Institute Ethics Committee. 25 healthy subjects served as control. Each subject was subjected to brainstem evoked response audiometry (BERA) examination.Results: 50% patients with OP poisoning were aged between 20-29 years. Male outnumbered females in the patients (M:F- 17:9) as well as controls (M:F- 17:8). Only absolute wave-v latency (ms) was statistically significantly prolonged in cases when compared with controls in both right (0.035) as well as left ears (0.048). We found no statistical significant difference in interpeak wave I-III, III-V, I-V latency of both ears in cases when compared with controls. On second assessment in the patients, we observed a statistical significant decrease in absolute wave III and V latency, and interpeak I-III interval in right ear when compared with wave latency at first assessment. In left ear on second assessment, a significant decrease in absolute wave I, III, and V latency was observed in the patients.Conclusions: OP compounds may affect entire neural auditory pathway. Further studies with a large sample size are required for the assessment.
