LEARNING OUTCOME: To list three advantages of using bioelectrical impedance analysis (BIA) for assessing the effect of nutrition therapy in older adults
LEARNING OUTCOME: To identify three key elements of the Level II Nutrition Screen (LII) that predict post hospitalization (p̄-hosp) weight loss or hypoalbuminemia.
With the implementation of Diagnostic Related Groups (DRGs) as the basis of hospital reimbursement, the average length of hospital stay has declined. As a result, elderly patients are being discharged from short-stay hospitals more rapidly than those with similar illnesses in the past. This may result in more frequent need for early non-elective readmission. Predicting which elderly persons may be readmitted will enable preventative interventions to reduce overall health care costs. The purpose of this study was to determine predictors and assess predictive models of early non-elective hospital readmission. The sample consists of 134 male and female Medicare patients who were found to be nutritionally compromised at the time of discharge. The study is a secondary data analysis utilizing a repeated measures design of multiple variables representing demographics, anthropometries, laboratory values, and impairment in Activities of Daily Living (ADLs). Data have been collected at four time points: at hospital discharge, and during dietitian or nurse home visits at one week, one month, and three months post discharge. Bivariate logistic regression analysis reveal that serum albumin and change in weight measured at one month post discharge are highly significant predictors of readmission. There is a 31% decrease in the log odds of readmission associated with every 0.1 unit increase in serum albumin at one month post discharge (Wald χ 2 = 8.2748, p=0.0040), such that an albumin value of 3.3mg/dL relates to a 51% probability of early non-elective readmission compared to an albumin value of 3.7mg/dL with a 20% probability of readmission. Serum albumin at one month post discharge accounts for approximately 28% of the variance in non-elective readmission (McFadden's R 2 = .2753). For every kilogram of weight loss, there is a 75% increase in the odds of readmission (Wald χ 2 = 5.1757, p=0.0229). Change in weight accounts for 8% of the variability in non-elective readmission (McFadden's R 2 = .0843). It is of interest that serum albumin measured at the time of hospital discharge was not significantly associated with non-elective readmission in this population. This suggests that an evaluation of serum albumin and change in body weight should be routine during early post-hospitalization office visits to identify those at increased risk of early non-elective readmission.
The collisional activation (xenon) decomposition of actinomycin V, dactinomycin, 7-nitrodactinomycin, 7-aminodactinomycin, and a mixture of dactinomycin, actinomycin C1 and actinomycin C2 was studied using a triple-quadrupole mass spectrometer with fast atom bombardment (xenon) ionization. Fragmentation pathways of the structurally significant ions were rationalized by assuming an initial McLafferty rearrangement at the ester linkage between the methylvaline and the threonine of both rings to form linear pentapeptides, followed by fragmentation in either pentapeptide yielding typical sequence ions and sequence cleavages from both rings.

The alkylation of thallous cyclopentadienide (1) has been shown to give 5-substituted cyclopentadienes without the complications due to prototropic shifts which accompany alkylation of alkali metal cyclopentadienides.1 In seeking a route to the interesting (CH)12 hydrocarbon2 11,12-di-hydrosesquifulvalene (3), it appeared likely that alkylation of 1 with tropylium fluoroborate (2) would afford a ready means of access to this system.
AbstractThe mass spectra of a series of hippuric esters have been determined. A variety of novel rearrangements induced by the presence of a formed benzyl carbonium ion has been rationalized with the aid of high resolution measurements and isotopic and substituent labeling. An eight centered rearrangement is proposed for the observed transfer of oxygen from a nitro group to a double bond.
Mass spectra of cyclopenin (I), viridicatin (III), 3-benzylidene-3,4-dihydro-4-methyl-1H-1,4-benzodiazepin-2,5-dione (VI) and derivatives of these systems were measured. Fragmentation schemes are proposed based on high resolution data, metastable peaks and, where appropriate, on deuterium labeling. The decomposition of I is found to follow pathways different in several respects from those proposed by Luckner et al. on the basis of low resolution spectra.
The mass spectra of several isofuroquinoline alkaloids (N- methylfuroquinol-4-ones) including the new N-prenylfuroquinol-4-one, acrophylline, have been recorded. The fragmentation of hexahydroacrophylline and related 3-ethyl-4-hydroxyquinol-2-ones have also been determined by deuterium and oxygen-18 labelling and high resolution measurements.
Three alkaloids have been extracted from the leaves of A. haplophylla. One is the known 4-hydroxy-2,3-dimethoxy-10-methylacridone. The other two are the new alkaloids acrophylline, 9-(3-methylbut-2-enyl)-7- methoxyfuro[2,3b]quinol-4-one, and acrophyllidine, 9-(3-hydroxy-3- methylbutyl)-7-methoxyfuro[2,3-b]quinol-4-one. These structures have been determined and confirmed by synthesis of a reduction product.
