This book examines the perceived social effects of keepers of on an online journal, or blog. Users of a popular blogging site were surveyed in order to determine how they felt that using the site has impacted upon their family, friend and self relationships. Participants were asked 70 questions to the ascertain possible social impacts of keeping a blog. 322 respondents answered the survey which was mix of multiple choice and open-ended questions, and the data provided was analysed using both quantitative and qualitative methods. The results illustrated that keeping an online journal does impact these users, with respondents citing both positive and negative effects. This book examines an under-investigated area of social science research, that is, the intersections of online and offline life, and the inherent social implications.
HISTORY 15 y/o male high school student sustained a knee injury while playing basketball. As he forcefully contracted his quadriceps in an attempt to jump, an opponent was standing on his planted foot. He felt sharp pain in front of his knee, fell to the ground, & was unable to walk. Moderate swelling was noted. He was evaluated in the emergency room, placed in a knee immobilizer & given crutches. Later that evening his swelling increased & pain remained severe through the night. PHYSICAL EXAMINATION The next day revealed a large amount of anterior, proximal tibial swelling & ecchymosis, as well as swelling about the medial & lateral aspects of the patella. No apparent intraarticular effusion or obvious deformity, but landmarks were obscured by the swelling. Patellar tendon was grossly intact. He was unable to actively contract his quadriceps. He had almost full passive extension of the knee but flexion > 10 degrees was limited by pain. Detailed examination of the knee was limited due to intense pain & guarding. Neurovascular examination was WNL. DIFFERENTIAL DIAGNOSIS Isolated tibial tubercle avulsion Complete patellar tendon rupture Proxial tibial physeal injury Patellar tendon periosteal sleeve avulsion TESTS AND RESULTS Lateral radiograph of the knee: § Patella alta § Subtle irregularity of tibial tubercle § Soft tissue swelling anterior to tibial tubercle MRI of knee (extended down to patellar tendon insertion): § Mild superior elevation of tibial tubercle § Patellar tendon intact § Extensive anterior soft tissue swelling without joint fluid Surgical exploration: § Large avulsion of periosteal sleeve (distal to patellar tendon insertion) with retraction & redundancy of the periosteum into the apophyseal disruption § Patella was significantly elevated § Small partial detachment of patellar tendon FINAL/WORKING DIAGNOSIS: Patellar tendon periosteal sleeve avulsion injury TREATMENT/OUTCOMES Re-attachment of periosteum & avulsed portion of patellar tendon Intraoperative re-establishment of normal patellar height confirmed radiographically with comparison view of opposite knee Repeat x-rays at 2 days confirmed normal symmetric patellar height Knee brace locked in full extension for four weeks PT for progressive ROM starting at 4 weeks post-op X-rays at 6 weeks showed normal patellar height as well as healing of tibial tubercle At 8 weeks post-op, patient was doing well, knee range of motion was 0–120 degrees, and he could perform a straight leg raise without difficulty PT was continued for ROM & strengthening, progressing activities as tolerated Returned to basketball when strength was 85% that of contralateral leg
