In clinical skills training objective structured clinical examinations (OSCEs) are the method of choice to foster learning. Hence, implementing them is challenging and expensive. Thus it is investigated how an alternative procedure that keeps OSCE's essential elements, but assigns less stations to each student, works in terms of validity and justifiability.Data of n = 694 students, each taking five tasks drawn semi-randomized out of a pool of 26 tasks, strictly aligned with learning objectives, are analyzed. Despite unsurprisingly low overall reliability, a justifiable pass/fail decision is possible for 480 students (69 %). The remaining group (n = 210, 30 %) is indeed larger than with longer OSCEs, and would need ongoing assessment. The tasks' psychometric quality contributes to exams construct validity. Resource preserving short practical skills assessment is educationally valid and feasible with MedUni Vienna's human medicine cohorts of n = 680.
Background. Relevant literature reports no increase in individual scores when test items are reused, but information on change in item difficulty is lacking. Purpose. To test an approach for quantifying the effects of reusing items on item difficulty. Methods. A total of 671 students sat a newly introduced exam in four testing shifts. The test forms experimentally combined published, unused, and reused items. Figures quantifying reuse effects were obtained using the Rasch model to compare item difficulties from different person samples. Results. The observed decrease in mean item difficulty for reused items was insignificant. Students who self-scheduled to the last test performed worse than other students did. Conclusion. Availability of leaked material did not translate into higher individual scores, as mastering leaked material does not guarantee transfer of knowledge to new exam items. Exam quality will not automatically deteriorate when a low ratio of randomly selected items is reused.
Background: Observation, rating, and grading are parts of the routine of the faculty when in contact with medical students. Most interestingly, there is a dearth of proper instructions on rater training for objective structured clinical examinations (OSCEs) in medical education literature.Aims: We assembled 12 tips for implementing a structured rater training program. To highlight these tips, we provided examples of our implementation of rater training for an OSCE in 2009.Methods: We developed a tailored rater training program by compiling information from medical education and other related research disciplines.Results: The structured rater training at hand proved feasible, and the acceptance rate of this training by examiners was high.

This created the need to standardize the process more and more. On the initiative of the German skills labs the German Medical Association Committee for practical skills was established and developed a competency-based catalogue of learning objectives, whose origin and structure is described here.Goal of the catalogue is to define the practical skills in undergraduate medical education and to give the medical schools a rational planning basis for the necessary resources to teach them.Methods: Building on already existing German catalogues of learning objectives a multi-iterative process of condensation was performed, which corresponds to the development of S1 guidelines, in order to get a broad professional and political support.Results: 289 different practical learning goals were identified and assigned to twelve different organ systems with three overlapping areas to other fields of expertise and one area of across organ system skills. They were three depths and three different chronological dimensions assigned and the objectives were matched with the Swiss and the Austrian equivalent.Discussion: This consensus statement may provide the German faculties with a basis for planning the teaching of practical skills and is an important step towards a national standard of medical learning objectives.Looking ahead: The consensus statement may have a formative effect on the medical schools to teach practical skills and plan the resources accordingly.[german] Einleitung: Angestoßen durch die Änderung der Approbationsordnung haben die berufspraktischen Kompetenzen in Deutschland eine höhere Priorität erhalten und werden in den medizinischen Fakultäten deswegen vermehrt vermittelt. An 25-year-old man was buried by an avalanche during off-slope skiing. He was rescued by his companions and resuscitated by mouth-to-mouth ventilation. The emergency physician from a helicopter based emergency medical service placed two venous lines in both external jugular veins and secured the airway with a tracheal tube. When transferred to the emergency department an additional central venous catheter was inserted via his right femoral vein. The subsequent computed tomography scan revealed several small air bubbles adjacent to the endothelium of the brachiocephalic vein. In an experimental setting, it was shown that air could enter the circulation via a central venous catheter within a few seconds, but measured values of embolising air were smaller than the calculated values when applying the law of Hagen-Poiseuille. Nevertheless, it is important to keep the lumens of a central venous catheter filled with saline before any manipulation in order to prevent or attenuate venous air embolism.
Die Skills-line des 2. Studienjahres hat ein Gesamtstundenausmaß von 4 SWS (entspechend 60 Ausbildungsstunden). Es werden insgesamt ca. 600 Studierende in 60 Kleingruppen a 10 Teilnehmern unterrichtet. Weiters inkludiert sind 120 Minuten Einführungsvorlesungen und freie Übungsmöglichkeiten außerhalb des angeleiteten Unterrichts. Die Line besteht aus 5 Lineelementen, den "ärztlichen Grundfertigkeiten“, der "ärztlichen Gesprächsführung 1 und 2“, der "physikalischen Krankenuntersuchung“, sowie dem "Famulaturpropädeutikum“.
[english] Objectives: The checklist „Professional Communication in Medicine“ serves as a tool for teaching and training medical students in communication skills, it has also been developed in order to evaluate the students’ performance at the end of their second year at the Medical University of Vienna. The checklist consists of three parts: 26 items referring to the contents of the interview according to George Engel’s ten step model of anamnesis, 10 items referring to the style of communication and 12 items deal with the relationship established. Methods: Teachers of communication skills in medicine were asked to check the list for objectivity (interrater- reliability). In a first step (learning), after they got to know how to handle the list, they were trained in applying this checklist with the help of videos – each presenting a complete anamnesis. The testing that followed looked at the items in terms of their level of consensus. Results: On the whole it can be said that a good level of consensus – in around 75 percent of all items - was reached in assessing all three parts, i.e. style of communication, contents and relationship. The highest levels of consensus were observed in part B: style of communication, the lowest in section C: relationship. Conclusion: Results have turned out satisfactorily as far as they provide consistent forms of assessment for the major part of items. However, improvements need to be made in areas dealing with question-building, psychosocial aspects but also personal development. All in all, the checklist can be seen as an essential contribution to quality assurance in medical communication, involving both, students and teachers. [german] Zielsetzungen: Die Checkliste PK „Professionelles ärztliches Kommunikationsverhalten“ ist als Unterrichtsmittel, für gezieltes Training und die Evaluation kommunikativer Fertigkeiten, im medizinischen Unterricht an der Medizinischen Universität Wien (MUW), entwickelt worden. Die Checkliste umfasst drei Bereiche: Gesprächsinhalt (Anamnese), Gesprächsstil und Beziehung/Kontakt. Methodik: Zur Überprüfung von Gütekriterien wurden Lehrende der skills line „ärztlichen Gesprächsführung“ herangezogen. Nach einer Einschulung in die Handhabung der Checkliste trainierten sie in einem ersten Schritt (Lernphase) die Anwendung der Checkliste anhand von Videofilmen. In der anschließenden Testphase wurde die Bewertungen der Items hinsichtlich deren Übereinstimmung erfasst. Ergebnisse: Insgesamt stimmten die Bewertungen für alle drei Bereiche -Gesprächsstil, Gesprächsinhalt sowie Beziehung und Kontakt - bei drei Viertel aller Items (rund 75 %) sowohl in Lern- wie in Testphase überein. Die höchste Übereinstimmung erzielten Items des Bereichs B (Gesprächsstil), die niedrigste Items des Bereichs C (Beziehung/Kontakt). Schlussfolgerung: Die Ergebnisse sind zufrieden stellend, in so ferne als für einen großen Teil der Items konsistente Einschätzungen vorliegen. Optimierungen in der Fragenkonstruktion ebenso wie in der Lehre sind vor allem in Bereichen, in denen psychosoziale Themen angesprochen werden, notwendig. Die Checkliste kann einen wesentlichen Beitrag zur Qualitätssicherung von Lehr- und Prüfungsbedingungen kommunikativer Fertigkeiten im medizinischen Unterricht leisten.
