Das Pankreaskarzinom ist durch einen hochmalignen Phänotyp sowie eine sehr limitierte Therapierbarkeit gekennzeichnet. Ein gravierendes Problem, das sich bei der konservativen Behandlung dieses Tumors ergibt, ist eine bestehende oder erworbene Resistenz gegenüber Chemotherapeutika, wobei diesbezüglich dem Transkriptionsfaktor NFkB eine wichtige Rolle zukommt. So konnten wir zeigen, dass Pankreaskarzinomzelllinien durch Sekretion von IL1ß über einen autokrinen Mechanismus verfügen, der in einer permanenten NFkB-Aktivierung und damit in einem chemoresistenten Phänotyp resultiert. Das Pankreaskarzinom ist weiterhin durch eine massive Durchsetzung mit mesenchymalen Stromazellen charakterisiert, deren Rolle in Zusammenhang mit Chemoresistenz bislang kaum untersucht ist. Um den Einfluss des Stromas auf die Sensitivität von Pankreaskarzinomzellen gegenüber Zytostatika zu bestimmen, wurden humane Pankreaskarzinomzelllinien und murine Pankreasfibroblasten, die durch einen myofibroblastischen Phänotyp (hohe Vimentin- und α-SMA-Expression) gekennzeichnet sind, in einem Transwell-Cokultur-System untersucht. Mittels Durchflusszytometrie konnte gezeigt werden, dass, im Gegensatz zu anderen transformierten Zelllinien, in Pankreaskarzinomzelllinien, die sensitiv auf eine Vielzahl an Zytostatika reagieren, nach 48h Cokultivierung mit Fibroblasten kaum Apoptose induziert wurde. Der Grad der Resistenz war abhängig von dem Verhältnis von Fibroblasten zu Tumorzellen, wobei schon ein 1:1 Verhältnis zu einer 50% geringeren Sensitivität gegenüber Zytostatika führte als bei monokultivierten Tumorzellen. Gelshift Assays zeigten eine in den Tumorzellen nach Cokultur deutlich erhöhte NFkB -Bindungsaktivität, und mittels ELISA konnte in Überständen von cokultivierten Tumorzellen signifikant erhöhte IL1ß-Spiegel nachgewiesen werden. IL1ß-Rezeptor-Blockierung oder NFkB-Inhibition hoben den desensitivierenden Effekt durch die Fibroblasten auf und resultierten in einer deutlich erhöhten Anzahl apoptotischer Tumorzellen. Diese Daten zeigen, dass Stromafibroblasten durch Induktion von NFkB und Sekretion von IL1ß in Pankreaskarzinomzellen die Sensitivität gegenüber Zytostatika herabsetzen, was die Bedeutung von Tumor-Stroma-Interaktionen bei der Chemoresistenz dieses Tumors unterstreicht.
Hintergrund & Zielsetzung: Infektionen sind häufige Komplikationen einer Pankreatitis und bestimmen massgeblich den klinischen Verlauf. Aufgrund der relevanten Sekretion proinflammatorischer Zytokine sind Myofibroblasten des Pankreas möglicherweise von entscheidender Bedeutung bei den lokalen und systemischen entzündlichen Reaktionen, die eine Pankreatitis begleiten. Die Bedeutung der pankreatischen Myofibroblasten bei angeborenen Immunantworten als Teil des immunologischen Abwehrsystems ist bisher jedoch nicht charakterisiert. Ziel der Studie war es daher, die Expression und Regulation von Toll-like Rezeptoren (TLR) sowie TLR-assoziierter Adapterproteine in humanen Pankreasmyofibroblasten zu analysieren und diese Rezeptoren funktionell zu charakterisieren.
Das NFκB-Zielgen IEX-1 (Immediate early gene X-1) spielt eine wichtige Rolle während Zellwachstum und zellulärer Stressantwort, wobei die hierbei zugrundeliegenden Mechanismen noch weitgehend unklar sind. Eine wesentliche Eigenschaft von IEX-1 besteht in einer modulierenden Wirkung auf Zellzyklus und Apoptose. Anhand stabil transfizierter HEK-293 Zellen, die zum einen IEX-1 konditionell (tetrazyklinabhängig) überexprimieren, zum anderen aber ribozymvermittelt IEX-1-defizient sind, ließ sich zeigen, dass die apoptosesteigernde Wirkung von IEX-1 mit verminderter NFκB Aktivität einhergeht. So war mittels Gelshiftassay in IEX-1-überexprimierenden 293-Zellen eine niedrigere NFκB-Bindungsaktivtät zu beobachten, sowie mittels Luziferase-Assay eine deutlich reduzierte transkriptionelle Aktivität – z.B. unter TNFα oder IL1β Behandlung. Umgekehrt war diese NFκB-Aktivierung sehr viel stärker in 293-Zellen mit ausgeschalteter IEX-1 Expression. Während in den IEX-1 überexprimierenden Zellen die Degradation des NFκB-Inhibitors IκB deutlich vermindert war, ließ sich mittels Westernblot in 293-Zellen mit herabgesetzter IEX-1 Expression eine deutlich beschleunigte IκB-Degradation beobachten. Weiterhin war mittels Westernblot von IκBα-Immunpräzipitaten eine erniedrigte IκBα-Polyubiquitinierung unter IEX-1 Expression festzustellen, wie auch eine herabgesetzte Aktivität des 26S-Proteasoms (mittels fluorometrischem Proteasomassay). Demgegenüber war in den ribozymtransfizierten 293-Zellen sowohl das Ausmaß der IκBα-Ubiquitinierung als auch die Aktivität des 26S-Proteasoms signifikant erhöht. Diese Ergebnisse deuten daraufhin, dass IEX-1 durch Modulation der IκBα-Ubiquitinierung und -degradation zu einer Verminderung der NFκB-Aktivität führt. Da IEX-1 selbst ein NFκB-Zielgen ist, könnte es Teil eines gegenregulatorischen Prozesses von NFκB sein und hierdurch eine Verminderung NFκB-vermittelter Apoptoseresistenz bewirken.
Pankreasadenokarzinome weisen zahlreiche molekulare Veränderungen auf, die die Chemoresistenz gegenüber Zytostatika bedingen. Veränderungen in anti-apoptotischen Signaltransduktionswegen scheinen hierbei eine entscheidende Bedeutung zu haben. So konnte gezeigt werden, dass in einer Reihe von Pankreaskarzinomzellinien eine hohe konstitutive NFkB-Aktivität die Resistenz gegenüber TopoisomeraseII-Inhibitoren vermittelt und in anderen Zellinien Akt/PI3K an der Chemoresistenz beteiligt zu sein scheint. In der vorliegenden Arbeit sollte geklärt werden, ob NFkB und/oder Akt/PI3K Pankreaskarzinomzellinien gegenüber dem in der Therapie des Pankreaskarzinoms am häufigsten eingesetzten Zytostatikum Gemzar schützt. Anschließend sollte die Eignung dieser Mechanismen als molekulares Ziel einer kombinierten Chemotherapie in vitro und in vivo untersucht werden. Mittels FACS-Analysen gelang es uns nachzuweisen, dass Zellinien mit hoher basaler NFkB-Aktivität, im Gegensatz zu Zellinien mit niedriger basaler NFkB-Aktivität gegenüber Gemzar resistent sind. EMSA und Luciferase-Assay zeigten, dass Gemzar in allen Zellinien eine dosisabhängige NFkB-Induktion bewirkt. Die Tatsache, dass durch NFkB-Inhibition die resistenten Zellen auch gegenüber Gemzar-Konzentrationen sensitiviert werden, die nicht in der Lage sind, NFkB zu induzieren, belegt die Theorie, dass hauptsächlich die konstitutive NFkB-Aktivität für die Resistenz gegenüber Gemzar verantwortlich ist. Im Gegensatz hierzu bestand zwischen der Akt-Aktivität und der Gemzar-Resistenz keine Korrelation. Weiterhin hatte eine pharmakologische PI3K/Akt-Inhibition keine Auswirkungen auf die Sensitivität gegenüber diesem Zytostatikum. Die auf diesen Daten basierenden in vivo Untersuchungen in einem subkutanen SCID-Maus Modell bestätigten, dass durch eine NFkB-Inhibition mittels Sulfasalazin eine signifikante Verbesserung der Ansprechrate von Capan-1 oder PancTu-1 Tumoren auf Gemzar erzielt werden kann. Es ist somit festzuhalten, dass durch eine spezifische NFkB-Inhibition Pankreaskarzinom-Zellinien in vitro und in vivo gegenüber Gemzar sensitiviert werden. Dies lässt die Hoffnung zu, dass die Kombination von Zytostatika mit klinisch etablierten NFkB-hemmenden Mitteln eine neue Therapieoption des Pankreaskarzinoms darstellen könnte.

Hintergrund: Die Rolle des antioxidativen Transkriptionsfaktors Nrf2 in der Tumorigenese wird kontrovers diskutiert. Galt Nrf2 zunächst als ein molekulares Ziel in der Chemoprävention – auch mit günstiger Wirkung gegenüber Tumoren – so deutet die Tatsache, dass viele Tumoren eine gesteigerte Nrf2 Aktivität zeigen, auf dessen Beteiligung in der Tumorigenese hin. Kürzlich konnte im Kolonkarzinom ein Zusammenhang zwischen erhöhter Nrf2-Aktivierung und einer daraus resultierenden, verstärkten proteasomalen Genexpression und Proteasomaktivität gezeigt werden. Eine Ursache dieses Nrf2-Effektes in kolorektalen Tumoren könnte persistierender oxidativer Stress im Darmepithel während chronisch entzündlicher Darmerkrankungen (CED) sein.
Evidence accumulates that the transcription factor nuclear factor E2-related factor 2 (Nrf2) has an essential role in cancer development and chemoresistance, thus pointing to its potential as an anticancer target and undermining its suitability in chemoprevention. Through the induction of cytoprotective and proteasomal genes, Nrf2 confers apoptosis protection in tumor cells, and inhibiting Nrf2 would therefore be an efficient strategy in anticancer therapy. In the present study, pancreatic carcinoma cell lines (Panc1, Colo357 and MiaPaca2) and H6c7 pancreatic duct cells were analyzed for the Nrf2-inhibitory effect of the coffee alkaloid trigonelline (trig), as well as for its impact on Nrf2-dependent proteasome activity and resistance to tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) and anticancer drug-induced apoptosis. Chemoresistant Panc1 and Colo357 cells exhibit high constitutive Nrf2 activity, whereas chemosensitive MiaPaca2 and H6c7 cells display little basal but strong tert-butylhydroquinone (tBHQ)-inducible Nrf2 activity and drug resistance. Trig efficiently decreased basal and tBHQ-induced Nrf2 activity in all cell lines, an effect relying on a reduced nuclear accumulation of the Nrf2 protein. Along with Nrf2 inhibition, trig blocked the Nrf2-dependent expression of proteasomal genes (for example, s5a/psmd4 and α5/psma5) and reduced proteasome activity in all cell lines tested. These blocking effects were absent after treatment with Nrf2 siRNA, a condition in which proteasomal gene expression and proteasome activity were already decreased, whereas siRNA against the related transcription factor Nrf1 did not affect proteasome activity and the inhibitory effect of trig. Depending on both Nrf2 and proteasomal gene expression, the sensitivity of all cell lines to anticancer drugs and TRAIL-induced apoptosis was enhanced by trig. Moreover, greater antitumor responses toward anticancer drug treatment were observed in tumor-bearing mice when receiving trig. In conclusion, representing an efficient Nrf2 inhibitor capable of blocking Nrf2-dependent proteasome activity and thereby apoptosis protection in pancreatic cancer cells, trig might be beneficial in improving anticancer therapy.
Das Pankreasadenokarzinom weist in der meist palliativen Therapiesituation unter allen bestehenden Radio-Chemotherapieoptionenen eine infauste Prognose auf. Die ausgeprägte Apoptoseresistenz stellt hierbei ein zentrales Problem in der Behandlung dieses Malignoms dar und begründet die Notwendingkeit neuer, molekularer Therapieansätze.
Pancreatic stellate cells have been investigated mostly for their activation process, supposed to support the development of pancreatic disease. Few studies have been presented on reversal of the activation process in vitro. Thiazolidinediones (TZDs) have been used as antidiabetics and have now been reported to exert antifibrotic activity. We tested effects of natural and synthetic ligands of peroxisome proliferator-activated receptor gamma (PPARγ) on human pancreatic fibroblastoid cells (hPFCs) in search for specificity of action. Ciglitazone, as a prototype of TZDs, was shown to have reversible growth inhibitory effects on human pancreatic fibroblastoid cells/stellate cells. Cells treated with ciglitazone for three days showed enhanced lipid content and induction of proteins involved in lipid metabolism. Collagen synthesis was reduced in hPFC. Interaction of PPARγ with DNA binding sites upon ligand binding was shown by gel shift analysis. These findings point toward a potential for adipocyte differentiation in human pancreatic fibroblastoid cells.
The cancer/testis antigen (CT) family is defined by its specific expression pattern. In most cases CT antigens are expressed in normal human tissue only in germline cells and some tumors. Because of their restricted expression pattern CT antigens are regarded as potential targets for vaccine immunotherapy. Little is known about the functions of the various CT antigens. Some of these CT antigens (CT 7; MAGE A3/6) seem to be involved in the dysregulation of cell-cycle control and increased cell proliferation. The monoclonal antibody Ki-A10 detects a nuclear antigen with a unique distribution pattern in normal human tissues and tumors. The antigen is now characterized as cancer/testis antigen 45 (CT45). The accurate localisation of CT45 could provide details about its function. Therefore immunfluorescence stainings with subsequent confocal laser microscope analysis were performed to deliver precise data about the nuclear localisation of CT45. Different human tumor celllines (L428, HT1080, WS1-CLS, SW872) and 5-aza-2'-deoxycytidine treated human peripheral blood lymphocytes were stained with mab Ki-A10. All stainings showed an irregular and dotlike pattern, which is wellknown for nuclear speckles (interchromatin granule clusters). Nuclear speckles are dynamic structures known to harbour many proteins of the pre-mRNA splicing machinery. In addition CT45 shows a clear homology to a DEAD-Box protein (DDX26). DEAD-Box proteins are generally believed to be RNA helicases, which in turn are localised in nuclear speckles. Due to these results we believe that CT45 could be involved in pre-mRNA splicing.
Das duktale Pankreasadenokarzinom weist nach wie vor eine nahezu infauste Prognose auf und zählt zu den häufigsten Ursachen krebsbedingter Todesfälle. Die ausgeprägte Chemoresistenz stellt ein wesentliches Problem in der Behandlung des Pankreaskarzinoms dar, wobei dem Transkriptionsfaktor NF-κB eine zentrale Bedeutung zukommt. Kürzlich konnte gezeigt werden, dass der Verlust der Expression des early response Gens IEX-1 mit einer deutlich schlechteren Prognose bei dieser Tumorentität einhergeht. Ziel der Studie war daher die molekularen Mechanismen der durch IEX-1 vermittelten Tumorsuppression und eine mögliche Verbindung mit der NF-κB vermittelten Chemoresistenz zu klären.

BACKGROUND & AIMS:The identification of precursor lesions has a great impact on the understanding of tumorigenesis. Precursor lesions of endocrine tumors are known to occur in the setting of the MEN1 syndrome. The aim of this study was to test the hypothesis that MEN1-associated duodenal gastrinomas originate from diffuse preneoplastic gastrin cell changes. Precursor lesions may precede the development of duodenal gastrinomas because, in contrast to sporadic gastrinomas, these tumors are usually multiple.METHODS:The distribution of endocrine cells in the nontumorous duodenal tissue was analyzed qualitatively and quantitatively for 25 patients operated on for a duodenal gastrinoma. MEN1 status was assessed clinically and by polymerase chain reaction-based mutational analysis.RESULTS:Fourteen of 25 patients with gastrinoma had proliferative, hyperplastic lesions consisting of gastrin cells in the nontumorous duodenal mucosa, similar to the gastric enterochromaffin-like cell lesions observed in chronic atrophic gastritis. All patients with Zollinger-Ellison syndrome with proven MEN1 had such proliferative gastrin cell lesions, and all patients with Zollinger-Ellison syndrome without precursor lesions were MEN1 negative.CONCLUSIONS:Duodenal gastrinomas in MEN1, but not sporadic duodenal gastrinomas, are associated with proliferative gastrin cell changes within the nontumorous mucosa. It is likely that these lesions precede the development of MEN1-associated duodenal gastrinomas.
Immediate early gene X1 (IEX-1) represents a stress response gene involved in growth control and modulation of apoptosis. Here, we report a detailed analysis of IEX-1 with respect to its intracellular localization. By means of confocal laser scanning microscopy, a green fluorescent protein-IEX-1 fusion protein transfected into HeLa cells, as well as endogenous IEX-1, could be detected in distinct subnuclear structures. This particular subnuclear localization of IEX-1 was not observed with a green fluorescent protein-IEX-1 fusion protein lacking a putative nuclear localization sequence, along with a decreased effect on apoptosis. Double immunofluorescence staining revealed a partial co-localization of endogenous promyelocytic leukemia protein (PML) and IEX-1 in these subnuclear structures. Nuclear localization of IEX-1 is also enhanced upon treatment of cells with leptomycin B, an inhibitor of the nuclear exporter CRM1. These observations indicate that IEX-1 is specifically shuttled to and from the nucleus. Overexpression experiments using PML isoforms III and IV revealed distinct intranuclear interaction of IEX-1 and PML. Coprecipitation experiments showed physical interaction between IEX-1 and PML. The close structural relation of IEX-1-containing nuclear subdomains and PML nuclear bodies suggests a function of IEX-1 related to the multiple functions of these unique subnuclear regions, particularly during stress response and growth control.
Persistent hyperinsulinemic hypoglycemia (PHH) in adults that is not caused by an insulinoma is a rare and not well-characterized disease that has been named nesidioblastosis. In this study, we defined and scrutinized criteria for its histologic diagnosis, assessed its relative incidence, and discussed its pathogenesis. In pancreatic specimens from 15 adult patients with PHH in whom no insulinoma was detected and in 18 adult control patients, the endocrine tissue was screened for islet and beta-cell changes. The diagnostic reliability of the findings was checked by an interobserver analysis. The relative frequency of the disease was assessed in a series of 232 patients with PHH. Finally, genetic analysis of the menin gene was performed. Among the various indicators of islet changes, beta-cell hypertrophy characterized by enlarged and hyperchromatic beta-cell nuclei was the most significant and diagnostic finding in patients with PHH. The interobserver analysis revealed 100% specificity and 87.7% sensitivity. The hyperfunctional state of the beta-cells was not associated with changes in the subcellular distribution of insulin and proinsulin, proliferative activity, or mutations of the menin gene. Our results indicate that diffuse nesidioblastosis in adult patients with PHH resembles that seen in neonates suffering from PHH. The most important criterion for the diagnosis is the beta-cell hypertrophy. As approximately 4% of adult patients with PHH are affected by diffuse nesidioblastosis, this disease is not as rare as it has been thought to be. Pathogenetically, the defective insulin secretion could be based on a molecular defect.
Early lymph node metastasis is common in pancreatic ductal adenocarcinoma (PDAC). The present study has examined the relationship of lymphatic spread to lymph vessel development and the expression of lymphangiogenic cytokines in a series of well-characterized PDACs. The hot spot method revealed the intratumoural and peritumoural lymphatic vessel density (LVD) to be slightly higher in PDACs than in the normal pancreas. The average intratumoural LVD, however, was strikingly decreased. There was no overexpression of vascular endothelial growth factor (VEGF)-C and VEGF-D in PDACs compared with the normal pancreas. LVD and expression of lymphangiogenic cytokines were not related to any of the biological tumour features or to patient survival. Three orthotopic nude mouse PDAC models did not reveal any increase in tumour-associated LVD, despite a high rate of lymph node metastasis. Lymph vessel proliferation was comparable in PDAC and chronic pancreatitis, in both humans and mice. In conclusion, increased lymphangiogenic activity is not required for and does not significantly affect the lymphatic spread of PDAC. The reduced number of human and murine intratumoural lymph vessels indicates that lymphatic metastasis takes place predominantly via peritumoural lymphatic vessels. The weak expression of lymphangiogenic cytokines in neoplastic cells and lymphatic vessel proliferation in peritumoural regions and chronic pancreatitis indicate that inflammation may be the reason for the low rate of lymphangiogenesis.
