Background: The severe manifestation of coronavirus disease 2019 (COVID-19) is known to be mediated by several cytokines and chemokines. The study aimed to compare the early cytokine profile of mild and severe COVID-19 patients to that with COVID-19-like symptoms and tested negative for Severe Acute Respiratory Syndrome Coronavirus-2 in the Reverse-Transcriptase Polymerase Chain Reaction (RT-PCR) test.Methods: This was a prospective, observational study on COVID-19 patients admitted to King Khalid University Hospital, King Saud University Medical City from June to November 2020. Clinical and biochemical data were collected from hospital charts. Blood samples were collected at the time of hospital admission to measure cytokines. A Cytokine and Growth Factor High-Sensitivity Array was used to quantitatively measure cytokines.Results: The study included 202 RT-PCR-positive individuals and 61 RT-PCR-negative individuals. C-Reactive protein (CRP) and Interleukin-10 (IL-10) levels were found significantly elevated in the RT-PCR positive group compared to the RT-PCR negative group (p=0.001). Patients with severe COVID-19 had significantly longer median hospital stays than those with mild COVID-19 cases (7 vs 6 days). They also had higher CRP and Vascular Endothelial Growth Factor (VEGF) levels and lower Interleukin-4 (IL-4) levels compared to the mild cases. CRP, interleukin-6, IL-10, VEGF, and Monocyte Chemoattractant Protein-1 (MCP-1) levels were significantly elevated in men and IL-10 was significantly higher and interleukin-8 was significantly lower in women compared to negative controls. Elevated Interferon-& Gamma; (IFN-& gamma;) and IL-10 levels were seen in mild COVID-19 cases and elevated level of MCP-1 was seen in severe COVID-19 cases when categorized according to the length of stay in the hospital.Conclusion: CRP and IL-10 levels were elevated in the RT-PCR positive group. People with severe COVID-19 had higher CRP and VEGF levels and lower IL-4 levels. Elevated IFN-& gamma; and IL-10 levels were seen in mild COVID-19 cases and elevated level of MCP-1 was seen in severe COVID-19 cases when categorized according to the length of stay in the hospital.
Background Post-acute coronavirus disease 2019 (COVID-19) syndrome, also known as long COVID, is a prolonged illness after the acute phase of COVID-19. Hospitalized patients were known to have persisting symptoms of fatigue, headache, dyspnea, and anosmia. There is a need to describe the characteristics of individuals with post-COVID-19 symptoms in comparison to the baseline characteristics. Purpose To investigate the clinical and biochemical characteristics of people who recovered from COVID-19 after 6 months of discharge from the hospital. Methods This was a prospective follow-up investigation of hospitalized and discharged COVID-19 patients. Adult patients admitted to King Saud University Medical City, Riyadh, Saudi Arabia, with laboratory-confirmed COVID-19 and discharged were recruited. The baseline demographic information, comorbidities, vital signs and symptoms, laboratory parameters, COVID-19 therapy, and outcomes were collected from the medical records. Blood samples were collected for cytokines estimation. A detailed interview about signs and symptoms was undertaken during the follow-up. Results Half of the followed-up people reported experiencing at least one of the COVID-19-related symptoms. The mean blood pressure was found higher in follow-up. People with the symptoms were characterized by low lymphocyte count, lower serum calcium levels, and hyperglycemia compared to people without any post-COVID-19 symptoms. Cytokines IL-8, VEGF, and MCP-1 were higher in people with the most frequent symptoms. Conclusion People with post-COVID-19 symptoms were characterized by lower lymphocyte count, lower serum calcium levels, and hyperglycemia compared to people without symptoms. Individuals with the most frequent post-COVID-19 symptoms had higher baseline pro-inflammatory, chemotactic, and angiogenic cytokines.
Context: Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease that can affect all organs and cause considerable morbidity and mortality. The estimated prevalence of SLE in the central region of Saudi Arabia is 19.28/100,000 individuals. Approximately half of those will eventually develop clinically significant renal disease. Therefore, screening for renal involvement is essential for early diagnosis and optimal disease management. Aims: This study aims to develop a pathway that can facilitate the early diagnosis and management of lupus glomerulonephritis in the Kingdom of Saudi Arabia. Methodology: The Saudi Society of Rheumatology in collaboration with the Saudi Society of Nephrology and Transplantation and under the supervision of the Saudi Commission for Health Specialties formed a committee involving rheumatologists, nephrologists, and a renal pathologist. Multiple workshops were conducted to adapt the 2019 Update of the Joint European League against Rheumatism and European Renal Association–European Dialysis and Transplant Association recommendations for the management of SLE, as well as The Kidney Disease: Improving Global Outcome 2020 using the ADAPTE process. Results: This document includes recommendations related to screening for renal involvement, renal pathology and classification, initial and subsequent therapy for lupus nephritis (LN), the treatment of refractory and relapsing LN, and the management of advanced renal disease. Recommendations concerning pregnancy and postpartum care are also included. Conclusion: We developed a guideline for the management of adults with LN based on recommendations developed elsewhere. This guideline was adapted to the local context of our health-care system.
Objective: To evaluate the effect of obesity on peritoneal dialysis (PD) including technique survival, incidence of complications and mortalityDesign: Retrospective cohort analysisSettings: Nephrology unit at King Saud University in Riyadh, Saudi Arabia between January 1st, 2005 and December 31st, 2014Subjects: Ninety-eight patients treated in the PD unit. Out of these, 36 had healthy weight and 28 were overweight, while 34 patients were obese.Intervention: As a retrospective study, no intervention in patient medication or dialysis was performed.Main outcome measure: The main outcome measure was patient mortality or cessation of PD (transplant or transfer to hemodialysis (HD)). Other measures included infection and complications.Results: The mean age was 52.14 years (range: 16-77 years) for obese patients compared with 49.83 years (range: 16-84 years) in overweight and 47.53 years (range: 19-86 years) in normal weight patients. Peritonitis rate was 0.38 per patient years in normal weight, 0.22 in overweight and 0.35 in obese patients. The mortality rate was 0.09 per patient years in normal, 0.07 in overweight and 0.06 in obese patients. Six (15%) patients in the normal weight group shifted to HD, compared to 7 (25%) overweight and 14 (32.5%) obese patients (P=.045). Six (15%) patients in normal weight group underwent renal transplant, compared to 2 (7.1%) overweight and 4 (9.3%) obese patients (P=.020).Conclusion: Our results revealed no significant differences in terms of infections, complications and patient mortality. However, obese patients had higher transfer to HD and lower renal transplant.
During the current COVID-19 pandemic, timing and preparedness for healthcare facilities are fundamental. This descriptive article narrates the Department of Internal Medicine at the College of Medicine and King Saud University Medical City (KSUMC) response before and during the COVID-19 outbreak by focusing on the preparedness and mitigation plans the department implemented for its clinical services. A COVID-19 taskforce was created to implement the pathways for healthcare workers' workflow organization, plan the effective use of isolation facilities and personal protective equipment, and enhance liaisons between departments in the medical city. Telehealth was utilized for outpatient services, and medications were delivered to patients via courier. Aerosol-generating procedures were conducted after thoroughly reviewing the clinical indications and time-related risks. Medical day unit services followed a strict multistep screening protocol and rescheduled nonurgent investigations and therapeutics when possible. The Department of Internal Medicine prepandemic preparedness helped significantly mitigate the pandemic impact on KSUMC. Prioritizing in-hospital clinical care according to the patients' conditions, full adherence to infection control measures, clear and efficient communication between departments, and utilizing telehealth were the key elements in maintaining state-of-the-art clinical care during the pandemic.
BACKGROUND:COVID-19 is diagnosed using RT-PCR assays of samples from nasal and oropharyngeal swabs. People with negative RT-PCR often presented with clinical manifestations of COVID-19. The data on such patients are lacking. The present study aims to characterize the patients who were suspected COVID-19 cases and tested negative in RT-PCR compared to patients who had been tested RT-PCR positive. METHODS:This is a retrospective, observational study of adult suspected and confirmed patients of COVID-19 admitted to King Saud University Medical City, Riyadh, Saudi Arabia, from 1st March 2020 until 30th November 2020. Laboratory confirmation is done through nasal/pharyngeal swab specimens, tested positive in RT-PCR assay. Patients with initial negative RT-PCR test results were assessed again within 48-72 h to avoid false-negative results. Patient data were extracted from the electronic medical files of each included patient using a predesigned case report form. RESULTS:The study included 488 (80.93%) patients with RT-PCR swab results positive, and 115 (19.07%) patients who were negative. Respiratory rate and diastolic blood pressure were higher among the swab-positive cases. More number of swab-negative patients had comorbidities such as coronary heart disease, chronic kidney disease, and carcinoma. Fever, cough, and shortness of breath were reported higher among the swab-positive cases. ALT and AST, and LDH levels were found higher among RT-PCR-positive patients. Serum creatinine, blood urea nitrogen and troponin were more elevated in RT-PCR-negative patients. Antibiotics, anticoagulants, and corticosteroids were used more by swab-positive patients. Significantly higher number of RT-PCR-positive patients required proning, high-flow nasal cannula, non-invasive mechanical ventilation, and invasive mechanical ventilation. Acute cardiac ischemia and death were found to be similar among the patients. However, deaths occurred significantly earlier among the swab-positive cases when compared to the swab-negative group. CONCLUSION:Distinctive symptoms and markers of COVID-19 are more frequent among patients who had RT-PCR-positive results.
Our objective is to study the outcomes and complications of peritoneal dialysis (PD) including comparison of self-care PD with home-care assisted PD during a five-year period. A retrospective study of PD data at King Saud University-affiliated hospital in Riyadh from January 1, 2009, to December 31, 2013. One hundred and eleven patients were included (female 55%). The average age was 47.4 (1-83) years. Twenty-one (18.91%) patients were on continuous ambulatory PD and 90 (81.08%) on automated PD. The mean time on PD was 23.5 (3-60) months. At the end of five years, 47 (42.34%) patients were continuing on PD, 12 (10.81%) had renal transplant, 33 (29.73%) patients were transferred to hemodialysis, and two (1.8%) patients were transferred to other centers. Seventeen patients died during this period giving a mortality rate of 7.13 deaths/100 patient-year during the five-year period. Six patients died due to cardiovascular causes, while five had sepsis. There was one death each due to prostate cancer, hyperoxaluria, and toxic epidermal necrolysis. Three patients died suddenly at home. Peritonitis rate was one episode/35.28 patient/month or one episode/2.94 patient/year. We compared the results for patients doing the dialysis themselves [56 (50.45%)] "self-care PD" to 55 (49.5%) patients assisted by a family member or other caregivers "assisted PD." We found no significant difference in the incidence of complications, technical outcome, mortality, and peritonitis episodes. However, we found a high prevalence of diabetes mellitus and significant increase in exit site infection in assisted PD. Our study suggests that PD patients in Saudi Arabia have a good overall outcome. Furthermore, assisted PD showed good patient and technique outcome.
Objectives: To assess the prognostic value of IgG4 reactivity in biopsy-proven cases of idiopathic membranous nephropathy (iMN) and to outline its potential in guiding therapy. Methods: A retrospective study of biopsy-proven iMN cases from January 1997 to August 2013 was undertaken. Patients were identified, and an extensive analysis of the clinical and histological parameters were performed. The primary endpoint was a worse renal outcome, which was defined as doubling of the serum creatinine baseline value. Results: The study included 52 patients, with mean age of 38.0 years. The median baseline creatinine was 112.7 ± 36.3 umol/l among those with positive staining for IgG4 and 67.4 ± 19.3 umol/l among those with negative staining for IgG4 (P=0.5). There was no significant difference in the 24 hr urine protein between IgG4 positive and IgG4 negative staining cases (P=0.375). The probability of doubling serum creatinine was similar among those with or without IgG4 deposition. Follow up of the patients revealed that 23.4% of those with positive IgG4 and 14% of those with negative IgG4 had deterioration of the renal function. The prevalence and severity of tubulo-interstitial inflammation was not statistically different between the two groups. Conclusion: There was no relationship between IgG4 positivity and the severity of clinical and/or histological parameters in patients with iMN. IgG4 reactivity had no impact on the long-term outcome of the patients. Further studies are needed to outline the potential use of targeted therapy against IgG4 auto-antibody in a certain category of patients with membranous nephropathy.
