Complex continuous care process of patients suffering from thalassemia is largely due to a collection of a large volume of diverse data, which makes traditional paper-based approach burdensome. Adoption of electronic medical records (EMR) system is the best solution in this situation because of the documented benefits in previous studies. Unfortunately, current implementation rate of the system, especially in Malaysia, found to be poor. Studies showed that it is related to low acceptance level towards the system. Previous studies recommend collaboration with targeted end-users during system development stages. Therefore, this article investigated and proposed a framework for online Thalassemia medical record management system which developed with the involvement of the targeted users, which are doctor and nurse. Paediatric Day Care Unit in Hospital Sultanah Aminah was selected for the setting of this study. In this study, previously gathered user requirement was analysed to produce the system framework, which then was used to develop the prototype. Usability and acceptance testing of the system prototype was conducted, from which questionnaires and interviews were given to 3 doctors and 3 nurses. System Usability Scale (SUS) was used and the mean final score was 68.75 with standard deviation 5.352, which is slightly above average. The results also support the previous study which found a significant negative correlation between SUS score and age. The results of the user acceptance testing (UAT) showed a very positive and encouraging feedback from the users. In conclusion, the framework proposed in this study can be accepted and used as a reference for any effort to develop a similar system.
Hospital International Accreditation is a lengthy process, in which total integration of the hospital's management system is required in order to complete the process. Difficulties in obtaining data from all units and departments, matching between existing documents with the accreditation elements, and comparing the visual aspect of the hospital real condition with the requirement of the accreditation are factors that cause delay for the hospital getting international accreditation. In order to overcome this issue, a computer guided hospital accreditation management system is developed. The system has been developed based on the guideline of Joint Commission International Accreditation Standards for Hospitals 6th Edition 2017. There are four sections included in this system. The elements are classified according to conventional hospital management system including finance, human capital, business process, facility, clinical process, and information system. We classified the elements in the JCI Accreditation Standards according to the conventional hospital management system elements. In order to obtain the JCI by section, input from document review, field survey, system study, and automatic electronic measurement are applied. The user interface is implemented in the mobile apps. MIT App Inventor 2 software is used to implement this system. GUI for data input and result visualization has been also implemented in the mobile apps. The functionality test of the system is conducted. This includes the output-input relation and JCI score accuracy. We have also compared duration for data input and processing to produce output between the conventional method and the new computer guided method. Test results showed that the system is able to produce JCI score according to manual input with shorter input and processing time. This shows that the system is applicable for hospital management to accelerate the process of getting JCI accreditation.
Thalassemia is an inherited blood disorder that requires lifelong disease management and therefore poses a pecuniary challenge in public health management in Malaysia. Constant monitoring of patients throughout their life requires novel adaptation of new technologies to enhance the effectiveness and success of the complex disease treatment regimen. In this paper we present a prototype mobile health (mHealth) application that will assist patients, care workers and medical practitioners in administering thalassemia disease management. The ubiquity and proximity of smartphones as a personal device will allow it to take up an assistive role in the management process. The user requirement analysis revealed that not all of the disease's information is available for stakeholders are suitable for the application and the alert function that works both ways between the patient and doctor is desirable. Operational limitations such as patient confidentiality, minor (underage) patients and analogous legacy registry has to be taken into account. The development of this prototype is the first for thalassemia and the finding of this study will assist future developers embarking on designing blood related disease management applications which share a similar process flow and data requirements.
The care process of patients suffering from thalassemia involves continuous and complex care procedures, which produce a large volume of diverse data. This makes the management of the patient records using paper-based system cumbersome. Information technology (IT) is an essential factor in the administration of healthcare where it can improves health care quality and service. Besides that, it provides more accurate and timely information regarding patient care. Consequently, the adoption of IT tools such as Electronic Medical Records (EMR) in hospitals is a solution to reduce some barriers in the healthcare services. This solution may improve the effectiveness of chronic disease management such as thalassemia, since a lot of the patient’s data will be stored and retrieved due to the frequent monitoring and treatment. In order to facilitate successful adoption of an EMR, involvement of end-users would be essentially required during the designing, implementation and usage phase. In this study, an EMR specifically design for thalassemia patient management is proposed to be implemented in Hospital Sultanah Aminah, Johor, Malaysia. The user requirement analysis (URA) of the proposed system, Online Thalassemia Management System (OTMS), is investigated and discussed in this article.
