Both frequent reason for consultation and cause of absence at work, low back pain is a notorious public health concern. The Belgian Health Care Knowledge Centre (KCE) has been surrounded by 31 clinicians from different disciplines to develop a guideline covering the entire management of low back and radicular pain. A recently published guideline of the British National Institute for Health and Care Excellence (NICE) was largely used as a starting point and adapted to the Belgian context.

Divers aspects des relations entre l'immunohématologie et les tumeurs sont passés en revue.Les relations entre groupes sanguins et maladies malignes, et les déficits immunitaires associés aux tumeurs, sont rapidement cités. Les aspects suivants sont développés :Les auto-anticorps associés aux leucémies lymphoïdes chroniques peuvent être de type IgG et avoir une spécificité dans le système Rhésus ; il peut s'agir également d'agglutinines froides de nature IgM. L'association agglutinine froide anti-i/réticulosarcome est possible. Des auto-anticorps peuvent être également rencontrés dans la maladie de Hodgkin, les thymomes, les lymphosarcomes, les tumeurs de l'ovaire.Dans certains cas, les auto-anticorps semblent être produits par la tumeur elle-même. A l'opposé, d'autres observations suggèrent que la tumeur a stimulé la production de ces anticorps.Les premières descriptions de ce type d'anomalie concernent l'extrême affaiblissement de l'antigène A des hématies de sujets atteints de leucémie aiguë avec parfois un accroissement de la réactivité H. Ceci suggère qu'il s'agit bien d'une perte d'activité de la glycosyltransférase spécifique de A, produit direct du gène A. Ces modifications ne sont pas limitées aux antigènes A et B. Elles sont dans tous les cas acquises, polyclonales et indépendantes.Cette perte antigénique peut aboutir, à l'extrême, à un aspect de mosaïque érythrocytaire où une population d'hématies paraît entièrement dépourvue d'un antigène. En dehors de cas exceptionnels, il n'a pas été possible d'établir de relations entre le mosaïcisme érythrocytaire et des anomalies chromosomiques.Il est vraisemblable qu'il n'existe pas de véritable néoantigène mais que les néoantigènes apparents soient la conséquence d'une perte antigénique ou d'un réarrangement de la membrane cellulaire. Ainsi, certains cryptantigènes à l'état normal, tels T et Tn, ont été trouvés à la surface de cellules de tumeurs du sein.D'autres antigènes ayant une activité pseudo-A (chez des sujets O) ont été décrits par différents auteurs.Il existe des lectines parmi lesquelles la Concanavaline A et la lectine du germe de blé, qui agglutinent certaines cellules tumorales alors qu'elles sont sans effet sur les cellules normales correspondantes. Cependant, l'obstacle fondamental à une utilisation thérapeutique des lectines est qu'il s'agit de protéines d'origine animale ou végétale donc de molécules immunogènes.
