IntroductionCertain neuropsychiatric symptoms (NPS), namely apathy, depression and anxiety demonstrated great value in predicting dementia progression representing eventually an opportunity window for timely diagnosis and treatment. However, sensitive and objective markers of these symptoms are still missing.ObjectivesTo investigate the association between automatically extracted speech features and NPS in early-stage dementia patients.MethodsSpeech of 141 patients aged 65 or older with neurocognitive disorder was recorded while performing two short narrative speech tasks. Presence of NPS was assessed by the Neuropsychiatric Inventory. Paralinguistic markers relating to prosodic, formant, source, and temporal qualities of speech were automatically extracted, correlated with NPS. Machine learning experiments were carried out to validate the diagnostic power of extracted markers.ResultsDifferent speech variables seem to be associated with specific neuropsychiatric symptoms of dementia; apathy correlates with temporal aspects, anxiety with voice quality and this was mostly consistent between male and female after correction for cognitive impairment. Machine learning regressors are able to extract information from speech features and perform above baseline in predicting anxiety, apathy and depression scores.ConclusionsDifferent NPS seem to be characterized by distinct speech features which in turn were easily extractable automatically from short vocal tasks. These findings support the use of speech analysis for detecting subtypes of NPS. This could have great implications for future clinical trials.DisclosureNo significant relationships.
BACKGROUND:The primary progressive aphasia (PPA) diagnosis trajectory is debated, as several changes in diagnosis occur during PPA course, due to phenotype evolution from isolated language alterations to global cognitive impairment. The goal of the present study, based on a French cohort, was to describe the demographics and the evolution of subjects with (PPA) in comparison with Alzheimer's disease (AD) on a period of 7 years.METHODS:We conducted a repeated cross-sectional study. The study population comprised individuals with PPA and AD diagnosis (N=167,191) from 2010 to 2016 in the French National data Bank (BNA). Demographic variables, MMSE scores, diagnosis status at each visit and prescribed treatments were considered.RESULTS:From 2010 to 2016, 5186 patients were initially diagnosed with PPA, 162,005 with AD. Compared to AD subjects, significant differences were found concerning age (younger at first diagnosis for PPA), gender (more balanced in PPA), education level (higher in PPA) and MMSE score (higher of 1 point in PPA). Percentage of pending diagnosis, delay between first consultation and first diagnosis and the number of different diagnoses before the diagnosis of interest were significantly higher in PPA group compared to AD group. Pharmacological and non-pharmacological treatments were significatively more recommended following PPA than AD diagnosis.CONCLUSION:This study improves the knowledge of PPA epidemiology and has the potential to help adopting appropriate public health service policies. It supports the hypothesis that PPA is diagnosed later than AD. The PPA diagnosis increases the prescription of non-pharmacological treatments, especially speech and language therapy (SLT) that is the main treatment available and most effective when at the initial stage.TRIAL REGISTRATION:ClinicalTrials.gov identifier NCT03687112.
The combined provision of an arising number of dementia cases, and the substantial absence of effective treatments, led the scientific community toward the identification of early phases of this condition. Such an effort aims at the recognition of therapeutic windows and the characterization of the disease’s different grades. In the last years, Motor and Cognitive Dual-Tasks (MCDT) have been widely used to address the early diagnosis of several neurocognitive disorders, among which dementia. Here we present different protocols: the walking MCDT, the toe-tapping MCDT, and the forefinger-tapping MCDT. Moreover, each task has been performed under different cognitive conditions: no cognitive effort, counting backwards by 1, 3, and 7. In this work, we report the results obtained through the combination of different motor and cognitive tasks, and we present 2 brand-new MCDT protocols, attempting to identify a sweet-spot for early diagnosis of dementia.
Le but de cette présentation est d'illustrer notre contribution à l'élaboration de méthodes d'apprentissage statistique robustes et interprétables, afin d'analyser des données issues d'essais cliniques pour l'étude de pathologies telles que l'apathie ou la maladie d'Alzheimer. En particulier, nous nous intéresserons à des approches détectant automatiquement la relation jointe entre biomarqueurs de grande dimension tels que les images médicales, l'information génétique ou des tests psychologiques. Nous présenterons également nos travaux portant sur la modélisation de l'évolution de la maladie d'Alzheimer à partir de données cliniques longitudinales acquises sur une courte durée. Nous verrons comment nos méthodes permettent d'identifier différents types de progressions de biomarqueurs impliqués dans le processus pathologique. Ces méthodes peuvent être appliquées sur des données de grande dimension, notamment des images médicales acquises selon différentes modalités. Enfin, nous montrerons comment ces méthodes peuvent être utilisées comme référence statistique afin d'évaluer et de prédire le stade de la maladie à partir de données cliniques non étudiées par nos algorithmes.
Aujourd’hui, il y a un besoin croissant d’harmonisation et d’innovation dans les évaluations neuropsychologiques. Étant donné que les outils de diagnostic actuels sont parfois invasifs (scan), coûteux (imagerie) et longues (évaluation neuropsychologique classique), de nouvelles méthodes écologiquement valides et sensibles sont nécessaires pour améliorer l’accessibilité en tant que dépistage de première ligne dans la population fragile. Les technologies numériques intelligentes peuvent servir d’outils supplémentaires non invasifs et/ou rentables, permettant d’identifier les sujets au stade préclinique ou aux premiers stades cliniques par exemple de la démence ou outres troubles psychiatriques qui pourraient convenir à un essai clinique, ainsi que de surveiller plus continuellement la trajectoire des maladies. De plus, des mesures plus normalisées pourraient entraîner une réduction des erreurs dues aux annotations humaines, ce qui faciliterait de meilleures comparaisons de données entre les sites cliniques. La mise en œuvre de ces nouvelles méthodes de mesure pourrait faciliter le diagnostic précoce et des stratégies de prévention et de traitement des troubles cognitifs et psychiatriques potentiellement plus efficaces. Toutefois, avant de les appliquer à la pratique et aux essais cliniques, ces outils devraient être examinés dans les cohortes importantes en cours. En s’appuyant sur les procédures existantes, des tentatives ont été faites pour améliorer et réinventer les métriques classiques en utilisant l’intelligence artificielle – analyse et tests informatisés, jeux sérieux, capteurs portables, analyse de la parole et de l’image ou crayons intelligents. Alexandra König, chercheuse/psychologue à l’INRIA et au CoBTeK lab, présentera l’application sur iPad « Δelta » permettant aux cliniciens de faire passer et d’analyser automatiquement des tests neuropsychologiques classiques même à distance à l’aide de l’intelligence artificielle (IA) et l’analyse automatisée du discours. Un exemple d’une analyse psycholinguistique informatisée d’une entrevue clinique sera également présenter.
Les émotions négatives (frustration, anxiété, apathie) sont des symptômes fréquemment retrouvés dans la maladie d’Alzheimer. Il est important de réduire ces symptômes afin de permettre notamment un meilleur accès aux capacités mémorielles et cognitives. L’immersion en réalité virtuelle permet de diminuer et d’isoler contre les stimuli externes qui peuvent induire des émotions négatives. Le voyage virtuel est une méthode qui permet de mobiliser l’attention du sujet et de raviver son intérêt et sa curiosité.

Les technologies de l’information et de la communication (TIC) sont des solutions non invasives et qui ont montrées une utilité pour identifier les sujets aux premiers stades cliniques des maladies neurodégénératives [1], [2]. Les recherches actuelles s’orientent sur l’intérêt des ICT à un stade pré clinique, comme marqueur d’évolution, au cours des essais thérapeutiques et dans les troubles psychiatriques [3], [4]. Cette session à pour objectif d’illustrer les scénario d’utilisation d’outils numériques novateurs, qui pourraient être utilisés pour le dépistage à grande échelle et pour le suivi des patients dans les essais cliniques. Léa Domain, interne de psychiatrie au centre hospitalier Guillaume-Régnier de Rennes présentera les résultats préliminaires de l’étude DEFLUENCE. Cette étude a pour objectif de déterminer si les altérations qualitatives aux tests de fluences verbales mesurées de façon automatisée peuvent constituer un bio marqueur pronostic de l’évolution de la dépression. Alexandra König, neuropsychologue et chercheuse au laboratoire CoBTeK présentera l’application Δelta sur tablette qui permet aux cliniciens de faire passer et d’analyser automatiquement des tests neuropsychologiques classiques même à distance à l’aide de l’intelligence artificielle (IA), de l’analyse automatisée de l’expression faciale et de la voix. Un exemple d’une analyse psycholinguistique informatisée d’une entrevue clinique sera également présentée. Clément Abi Nader, doctorant dans l’équipe Epione INRIA présentera les travaux portant sur la modélisation de l’évolution de la maladie d’Alzheimer à partir de données cliniques longitudinales acquises. Cette approche consiste à développer des algorithmes intégrant des données hétérogènes (imagerie, données biologiques, capteurs, données cliniques).
The aim of this study is to develop a generative and probabilistic statistical learning model for the joint analysis of heterogeneous biomedical data. The model will be applied to the investigation of neurological disorders from collections of brain imaging, body sensors, biological and clinical data available in current large-scale health databases. The resulting methodological framework will be tested on the UK Biobank, as well as on pathology-specific clinical data, as provided by the ADNI, or INSIGHT initiatives. We propose a variational approximation of Bayesian Canonical Correlation Analysis (CCA). The proposed formulation is inspired by current advanced in variational learning, and offers the potential to scale to high-dimensional observations, such as medical images and arrays of biological data. We proved that the variational lower bound can be optimized through modern learning libraries such as Torch and TensorFlow. We currently benchmarked the method with respect to classical CCA on both synthetic data and on the classical benchmarking datasets in machine learning (IRIS dataset). With respect to the synthetic dataset (Fig. 1A), we observed a strong agreement between the score components computed with classical CCA and our method. Moreover, the classification results on IRIS showed that the two methods essentially provide the same latent representation (Fig. 1B). Our method shows promising results for the future application to medical data. The method is computationally efficient and scalable, hence able to process complex multivariate multidimensional datasets. We expect to highlight meaningful relationship among biomarkers that could be used to develop optimal strategies for disease classification, quantification, and prediction. In the future, the proposed approach will be tested in several experimental settings : – classification/stratification ; – prediction and imputation from a set of observed data (e.g., predict biological and clinical output from medical imaging information).

The morphological changes affecting the brain over time are related to several biological processes, governed either by healthy aging or by pathological factors. Since these processes are to date largely unknown, we need statistical approaches to automatically identify these latent morphological evolutions through the analysis of structural brain magnetic resonance images (MRIs). Such approaches require to scale to high-dimensional volumetric observations with application to the analysis of large-scale biomedical databases such as UK Biobank. We present a novel spatio-temporal analysis method, aiming at automatically estimating latent spatio-temporal patterns of brain changes from collections of brain MRIs over time. This approach extends standard methods (such as ICA) to encode priors on spatial and temporal properties of the signal measured in brain images. Our method considers the observed data as a matrix factorization of both temporal and spatial sources. The temporal sources are treated as independent Gaussian processes to promote smoothness in time and model a plausible aging evolution. The spatial sources are modeled as Gaussian random fields to encode the spatial continuity of the brain sub-structures. This particular structure allows to factorize the spatial covariance matrix as a Kronecker product over the three spatial dimensions, which greatly simplifies computations and reduces the dimensions of the matrices we work on. The overall model is efficiently optimized through stochastic variational inference. We tested our model on synthetic data. We generated statistically independent temporal sources and spatial sources as smooth heatmaps. Then we trained our model so that it disentangles the observed data in two matrices that best fit the generated mixed observations. Fig. 1 below shows on plot A the original temporal sources (in red) and their approximation by the independent Gaussian processes (in blue). Likewise, plot B shows on top the heatmaps manually generated and under them the maps generated by the algorithm. We can observe that the method is able to capture these raw sources from the noisy observations given as target. Eventually this method may provide an ideal exploratory tool for analyzing large-scale medical imaging datasets such as the UK Biobank. Indeed, it allows to efficiently scale to both high-dimensional data and large sample sizes, and also identifies hidden spatio-temporal processes in a completely unsupervised manner.
We present a clinical and therapeutic approach aiming to create new care for patients with neurocognitive disorders (NCD). Enriched environment including physical and cognitive stimulation improves cognitive functions 1. The aim of this paper is to describe a method for non-pharmacological management of NCD using serious exergames (SEG). SEG are serious video games integrating physical activity. We propose to use SEG as a tool to provide enriched environment associating physical activity and cognitive stimulation.
L'apathie est un des troubles les plus fréquents dans les troubles neurocognitifs (TNC) et a des répercussions importantes sur la qualité de vie des patients. Plusieurs outils existent pour évaluer l'apathie et ses différents domaines(comportements/cognitions, émotions, interactions sociales, Robert et al., 2018). Néanmoins, ces outils ne consistent actuellement qu'à l'utilisation d'échelles cliniques et des auto-questionnaires(comportant un caractère subjectif et hors contexte). Dans le cadre d'un projet visant à développer un outil diagnostique multidimensionnel (MoTAP), notre étude porte spécifiquement sur l'évaluation du domaine émotionnel. Nous nous sommes intéressés à l'utilisation d'un test informatisé validé (Clock'N test, Gros et al., 2015, 2016) pour l'évaluation des troubles émotionnels en temps réel et de manière implicite. Le paradigme de ce test consiste en un amorçage émotionnel à travers des vidéos avant une tâche d'estimation temporelle (durée d'un son neutre). Les normes ont été établies sur la base de la déformation temporelle, qui a été démontrée comme corrélée aux réponses physiologiques émotionnelles (Réponse électrodermale). 62 sujets du Centre Mémoire de Ressources et de Recherches de Nice ont été inclus dans cette étude (âge = 74,1 ans, sd = 8,2 ans) et répartis en deux groupes : groupe TNC Mineurs (n = 32, âge = 75,3, MMSE = 25,3) et groupe TNC Majeurs (n = 19, âge = 75,6, MMSE = 17,6). La passation du Clock'N test, effectuée sur une tablette tactile, durait une dizaine de minutes. L'apathie a été évaluée avec les Critères Diagnostic de l'apathie. Pour évaluer l'apathie émotionnelle, nous avons pris en compte le critère B3 Émotion. 4 sujets ont été classifiés comme apathiques au niveau émotionnel dans le TNC mineurs, et 8 dans le TNC majeurs. Le Clock'N test montre une bonne sensibilité pour détecter l'apathie émotionnelle dans les TNC mineurs (0.75) mais plus modérée pour les majeurs (0.63). La spécificité est modérée pour les mineurs (0.68) mais faible pour les majeurs (0.36). Ces résultats sont donc encourageants quant à l'utilisation du Clock'N test pour l'évaluation de la dimension émotionnelle de l'apathie dans les TNC, et montrent la pertinence de ce test surtout chez les sujets avec TNC mineur. Les études futures devront se concentrer sur la pertinence pour les TNC majeurs. L'inclusion d'un plus grand nombre de participants permettra l'établissement de mesures plus solides, et de normes pour les différents types de TNC.
Background: Apathy is a very common behavioural and psychological symptom across brain disorders. In the last decade, there have been considerable advances in research on apathy and motivation. It is thus important to revise the apathy diagnostic criteria published in 2009. The main objectives were to: a) revise the definition of apathy; b) update the list of apathy dimensions; c) operationalise the diagnostic criteria; and d) suggest appropriate assessment tools including new technologies. Methods: The expert panel (N = 23) included researchers and health care professionals working on brain disorders and apathy, a representative of a regulatory body, and a representative of the pharmaceutical industry. The revised diagnostic criteria for apathy were developed in a two-step process. First, following the standard Delphi methodology, the experts were asked to answer questions via web-survey in two rounds. Second, all the collected information was discussed on the occasion of the 26th European Congress of Psychiatry held in Nice (France). Results: Apathy was defined as a quantitative reduction of goal-directed activity in comparison to the patient's previous level of functioning (criterion A). Symptoms must persist for at least four weeks, and affect at least two of the three apathy dimensions (behaviour/cognition; emotion; social interaction; criterion B). Apathy should cause identifiable functional impairments (criterion C), and should not be fully explained by other factors, such as effects of a substance or major changes in the patient's environment (Criterion D). Conclusions: The new diagnostic criteria for apathy provide a clinical and scientific framework to increase the validity of apathy as a clinical construct. Irregularities in the language domain could be a strong predictor of disease progression. Automatic speech processing and machine learning techniques, which enable extraction of dementia, relevant information from the speech audio signal may be of great interest To evaluate the interest of using automatic speech analyses for the assessment of mild cognitive impairment (MCI) and early Azheimer disease. Participants n = 165 (Healthy elderly subjects (HC), MCI patients, Alzheimer’ disease (AD) and mixed dementia (MD) patients) were recorded with a mobile application while performing several short vocal cognitive tasks during a regular consultation. These tasks included verbal fluency, picture description, counting down and a free speech task. The voice recordings were processed in two steps: in the first step, vocal markers were extracted using speech signal processing techniques; in the second, the vocal markers were tested to assess their ‘power’ to distinguish between HC, MCI, AD and MD. The second step included training automatic classifiers for detecting MCI and AD, based on machine learning methods, and testing the detection accuracy. The fluency and free speech tasks obtain the highest accuracy rates of classifying AD vs. MD vs. MCI vs. HC. Using the data, we demonstrated classification accuracy as follows: HC vs AD = 92% accuracy; HC vs. MD = 92% accuracy; HC vs. MCI = 86% accuracy and MCI vs. AD = 86%. These results indicate the potential value of vocal analytics cognitive and the use of a mobile application for accurate automatic differentiation between HC, MCI and AD.
Apathy is an early and common neuropsychiatric syndrome in Alzheimer's disease (AD) patients. In clinical trials, apathy is associated with decreased motor activity that can be monitored by actigraphy. The triple transgenic mouse AD model (3xTgAD) has been shown to recapitulate the biochemical lesions as well as many of the synaptic and cognitive alterations associated with AD. In the present work we found that these mice also develop an early and consistent apathy-like behavior as evidenced by a drastic decrease in spontaneous activity measured by actimetry. We recently established that these mice also display an intraneuronal accumulation of the beta-secretase-derived beta APP fragment (C99) appearing early, in absence of A beta. Interestingly, we found that the apathy-like behavior observed in 3xTgAD mice was temporally associated with C99 accumulation and synaptic alterations. Since it is well known that the genetic background can strongly influence behavior and can induce transcriptional variability in animal models, we decided to determine the influence of genetic background on the above-described alterations. We backcrossed 3xTgAD mice to C57BL/6 and found that the genetic background had no influence on either C99 accumulation or synaptic plasticity alterations, but strongly affected the apathy-like behavior.

Cette revue présentera des solutions technologiques, comme les capteurs portatifs (actigraphie) et les capteurs environnementaux (analyse automatisée visuelle ou de la voix, smarthomes), ainsi que des solutions numériques (serious games, exergames, réalité virtuelle).

Avec le vieillissement de la population, la maladie d’Alzheimer et les pathologies associées représentent un défi majeur de santé publique. Parallèlement, les nouvelles technologies de l’information et de la communication prennent une part de plus en plus importante dans notre vie quotidienne et peuvent être un support tant pour l’évaluation que pour une aide directe des usagers. Les serious games sont des applications informatiques, dont l’intention est de combiner, avec cohérence, à la fois des aspects sérieux (Serious) comme l’enseignement, l’apprentissage, la communication, la rééducation, avec des ressorts ludiques issus du jeu vidéo (Game). Dans le cadre du projet AZ@GAME [2] lauréat AAP e-santé no 1 des Investissements d’Avenir, des jeux sont en cours de développement avec pour objectif de stimuler les capacités cognitives et physiques du patient. Un des problèmes rencontrés concerne l’engagement des sujets à comprendre puis à pratiquer ces jeux. En effet, engagement et motivation diminuent quand le patient rencontre des problèmes d’utilisation. Nous présentons ici un système d’assistance automatisé basé sur la technologie Kinect. Trois groupes de patients (Plaintes mnésique : n = 10 ; âge moyen, 76,6 ; MCI : n = 10 âge moyen, 77,9 ; et maladie d’Alzheimer : n = 10 ; âge moyen, 79,9) ont participés à une séance d’entraînement utilisant un jeu pour stimuler l’attention concentration. Le jeu était proposé avec ou sans l’aide du système. Les résultats indiquent que tous les groupes ont eu de meilleures performances (score et temps de jeu) avec l’aide du système. Ce résultat est tout particulièrement important dans le groupe de patients Alzheimer (score > de 31 % et temps de réalisation < de 10 % avec le système). Compte tenu de ces résultats la prochaine étape aura pour objectif de proposer un système encore plus motivant à une population plus importante de patients.
La prise en charge non-pharmacologique de la maladie d’Alzheimer et des maladies apparentées (MA) représente un enjeu de santé majeur chez les personnes âgées . L’environnement Enrichi (EE), combinaison de stimulations cognitive, physique et d’engagement social en contexte émotionnel positif, apparaît comme une méthode efficace pour lutter contre la progression d’une MA . La principale difficulté est de proposer aux patients un EE adapté et motivant. Les serious games peuvent aider dans ce sens . Xtorp est un Serious exerGame (à activité physique, SeG) d’action/aventure développé pour KinectTM. Le joueur pilote un sous-marin (Fig. 1). Il doit devenir Amiral 5 étoiles en collectant de l’expérience au cours de batailles et missions. Dix patients (MA stade léger) et 8 témoins ont suivi un programme d’entrainement avec le jeu durant 1 mois, réparties en 12 séances. Les performances au jeu, les émotions perçues (PANAS) et l’intensité d’effort physique induite par le jeu ont été étudiées. Tous les participants ont terminé au moins une fois Xtorp. Les patients ont une capacité de jeu inférieure aux témoins (temps total de jeu et vitesse de progression patents : 420 minutes et 185 points d’expérience/minute, témoins : 489 minutes et 287 points d’expérience/minute). Les patients et les témoins n’ont quasiment ressenti que des émotions positives, légèrement plus fortes pour les témoins (PANAS positifs patients : 27/50, témoins, 36/50 ; PANAS négatifs patients : 12/50, témoins 11/50). Enfin, le jeu a été stimulant physiquement mais à un moindre degré chez les patients (fréquence cardiaque de réserve moyenne et pic par séance patients : 33 % et 53 %, témoins : 44 % et 62 %). En conclusion Xtorp est un EE utilisable, motivant qui permet de réaliser une activité physique potentiellement modérée chez des patients présentant des troubles cognitifs.
