During the past five years, 15 patients, aged between 17 years to 35 years of age (mean 26 years), presented for cosmetic improvement of scars which were induced by treatment with SolcoDerm, either in the childhood or in their early teens. It was an homogenic group of patients who were treated at least 10 years before the present examination. Two representative cases are presented, and the literature is reviewed.
Forty morbidly obese patients had plastic surgery to remove redundant tissues after gastric bypass surgery (GBP). The average weight reduction was 62 kg (range 29–95 kg) and the average age at operation was 36.5 years. Abdominoplasty was the most common procedure and no mortalities were recorded. The postoperative complications were related mostly to wound healing. The average hospitalization time was 8 days (range 5–11 days) and the patient's new body image was satisfactory.
The deforming forces of the scar contracture associated with burns of the head and neck region involve primarily the skin and secondarily the facial musculoskeletal structures. A case of severe face and neck burn accompanied by extreme facial skeletal deformity is reported. Best results are obtained in patients treated properly and promptly by a team including plastic and maxillofacial surgeons as well as orthodontists.
A series of 20 patients (15 men and 5 women) suffering from malignant melanoma of the scalp is reported. Their epidemiologic data and outcome are described. The ages ranged from 48 to 78 years (mean 63 years). Analysis of the cases demonstrated that lesions occurring posterior to the tragal line (in hair-bearing area) have the worst prognosis. The 5-year survival rate was poor; 12 patients died within this period. Two representative cases are described, and the preventable aspects of the disease are emphasized.
Seven women with recurrent breast carcino- ma, which were treated by the combination of wide exci- sion, immediately reconstruction with myocutaneous flap (TRAM or latissimus dorsi) and adjuvant brachy- therapy, are reported. The mean interval between the mastectomy and the recurrence was 36 months (range 24-48) and mean age was 46.5 years (range 35-52 years). The patients received a brachytherapy dose of 4600 rad 7 days post-operation. The follow-up ranges from 26-37 months, and there has been no local recur- rence or distant metastasis. The results, and advantages of the brachytherapy treatment are reported and a typical case is described.
A small series of melanomas of the external ear is presented. The presentation in general was late and they varied from 2.6 mm to 4.8 mm in thickness. The outcome in most of the patients (9 patients) was bad and within 3 years only 4 survived. The fact that 9 patients remembered having a nevus that suddenly changed to melanoma emphasizes the need for early removal of such nevi.
In summary, a case of macrocephaly and lipomatosis of the scalp and forehead has been presented. The phenotypic and clinical features found in our patient are in accordance with previous reports. The differential diagnosis with other hamartoneoplastic syndromes has been evaluated and discarded, concluding that the present report is in accordance with Bannayan-Zonana syndrome. However, as in other inherited traits, finding a genetic test to diagnose this entity remains a challenge.
Coverage of wounds after deep chemical peeling varies from exposure treatment with antibiotic ointment to coverage with mask tape and gauzes. We report here our experience with 20 patients who were treated by chemical peeling either as part of a rejuvenation procedure or because of recurrent superficial tumors in sun-damaged skin. The skin substitute Omiderm was used as an occlusive transparent tape mask. Omiderm is described and its advantages are emphasized. The relevant literature is reviewed.
This article describes the case of a patient who suffered an electrical full thickness burn of the chest wall and a concomitant osteomyelitic complication of two ribs. A review of the existing literature on bone and joint changes after burns is presented. Osteomyelitis of ribs must be kept in mind while treating patients for chest wall burns.
Prolongation of skin allograft survival by immunosuppression of burn casualties has been reported sporadically during the past two decades Recently cyclosporin A (CycA) has been used effectively for such an indication. We report here two paediatric patients with extensive burns (85-95 percent BSA) treated with fresh, family-related skin allografts that were rejected during CycA treatment after 14-18 days. One of these children survived while the other died with candida sepsis.
The 'Molotov cocktail' terrorist weapon which is thrown into a travelling car has given a new type of injury to people who sustain massive smoke inhalation together with disfiguring burns of face, thighs, hands and chest, and post-traumatic psychological disorder The combination of petrol ignition with the synthetic fumes inside the care is a unique occurrence with a high morbidity and mortality which is difficult to treat and to manage. We propose to show that the combination of all the above components can be defined as the 'Molotov cocktail' burn syndrome.
The junction between the inframammary and vertical suture lines after reduction mammaplasty is a weak point well known to plastic surgeons. This junction is prone to either necrosis or disruption, or both, in some cases. A small triangle of skin left in that location may prevent this problem. This is done routinely in all the cases of McKissock reduction mammaplasty, but it could be employed in any other techniques as well. The cosmetic appearance is good. The method, which is not new and has been used in the past, has been found very helpful and is described here along with its advantages.
Vinegar intake is considered a food item that improves blood glucose in humans. This review aimed to discuss studies that investigated the impact of vinegar intake on the glycemic profile in humans and the putative mechanistic cellular pathways in both human and animal models. A search of literature was performed on the Cochrane, MEDLINE and Web of Science databases for articles published between 1995 and 2018. There is considerable support for vinegar having a positive acute effect on blood glucose levels when combined with carbohydrate-rich meals. Conversely, there are few chronic interventions analyzing the impact of vinegar intake on blood glucose. Based on available evidence, we hypothesize three pathways by which vinegar may improve blood glucose: The inhibition of α-amylase action; increased glucose uptake; and mediation by transcription factors. When evaluating the current body of literature, daily vinegar intake in amounts of ∼10–30 mL (∼2–6 tablespoons) appear to improve the glycemic response to carbohydrate-rich meals; however, there is a paucity of studies investigating chronic effects of vinegar intake.
A large tongue is uncommon. When present, it is nearly always related to acromegaly or hypothyroidism and is resolved after treatment of the hormonal disturbances. Macroglossia related syndromes, although more rare, have been described before - the most common being Down's syndrome or Trisomy 21. In this article, a series of sixteen patients with various types of macroglossia not related to Down's syndrome are described; the surgical treatment and its results are presented, as is a brief review of the literature.
A case of malignant fibrous histiocytoma of the left forearm is described. The clinicopathological findings are reviewed and the salvage treatment, which included excision and immediate reconstruction with a free rectus abdominis muscle flap followed by brachytherapy is reported. The literature is reviewed and the advantages of this particular treatment are emphasized.
Reconstruction of the columella is difficult and there is not a universally accepted technique for performing it. We report a series of eleven cases during the last twelve years for columellar reconstruction. The diverse techniques employed are described and the relevant literature reviewed. A special mention is made about the better way to reconstruct the columella in cases of median cleft of the face.
