Background: In sarcoidosis, an antigen specific immune response is a putative mechanism, resulting in granulomatous inflammation. Since the proteasome is involved in antigen presentation, alterations in the alveolar and parenchymal proteasomal system may be a feature of sarcoidosis.Objectives: To explore the role of proteasomes and immunoproteasomes in sarcoidosis.Methods: Total proteasome concentration and activity was assessed in bronchoalveolar lavage (BAL) supernatant obtained from sarcoidosis patients (n = 67) and healthy controls (n = 18) using ELISA and cleavage of specific fluorogenic substrates ( epoxomicin), respectively. Immunohistochemistry of lung tissue sections and immunocytochemistry of BAL macrophages for immunoproteasome was performed in sarcoidosis patients and controls.Results: Proteasome was present in BAL supernatants of all sarcoidosis patients. In sarcoidosis, abundant immunoproteasome staining was seen in pneumocytes type II and granulomas. Total proteasome concentration was greater in active sarcoidosis, stages II (101 ng/ml +/- 79; p = 0.009) and III (119 ng/ml +/- 66; p = 0.012), than in inactive sarcoidosis or in healthy controls (35 ng/ml +/- 34). In the absence of epoxomicin, all fluorogenic substrates were hydrolyzed by BAL supernatant of sarcoidosis patients and controls.Conclusions: Patients with active sarcoidosis but not healthy controls demonstrate immunoproteasome in the lung tissue and in granulomas. Thus, the putative immune response in sarcoidosis may be mediated or sustained by the proteasomal system. (C) 2014 Elsevier Ltd. All rights reserved.
Einleitung: Die Alveolarproteinose (AP) ist ein Füllungssyndrom mit Akkumulation von Phospholipiden und Lipoproteinen in den Alveolen. Das Proteasom ist ein Proteinkomplex, der als zentraler Regulator den intrazellulären Proteinabbau bewirkt und Oligopeptide für die Antigenpräsentation generiert. Das Proteasomsystem wurde bereits im Alveolarraum von Patienten mit ARDS, ALI und Sarkoidosis nachgewiesen. Darüber hinaus kann das Proteasom extrazellulär Albumin abbauen, um den intraalveolären onkotischen Druck zu reduzieren. Ziel dieser Studie war es, das Proteasom in der BAL bei AP nachzuweisen und seine eventuelle Rolle als Biomarker zu prüfen.

Die häufigste Form ist die primäre AP, assoziiert mit Autoantikörpern gegen GM-CSF. KL-6 ist ein Mucin welches von alveolären Typ II und Clara-Zellen produziert wird. Das Chemokin PARC/CCL-18 ist chemotaktisch für Lymphozyten, hat strukturelle Ähnlichkeit mit MIP-1, und wird von Alveolarmakrophagen produziert. Patienten mit interstitiellen Lungenerkrankungen weisen erhöhte KL-6 und PARC-Spiegel in der bronchoalveolären Lavage (BAL) und im Serum auf. Ziel dieser Studie war es, KL-6 und PARC als Serummarker bei AP zu vergleichen.
In early 2003 it was realized that mechanical changes in the Tevatron dipoles had led to a deterioration of the magnetic field quality that was hindering operation of the accelerator. After extensive study, a remediation program was started in late 2003 that will continue through 2005. The mechanical and magnetic effects are discussed. The readjustment process and experience are reported, along with other observations on aging magnets.
Nach der Bestrahlung eines Mammakarzinoms, eine Routinemassnahme bei brusterhaltender Operation, kann in seltenen Fällen eine Bronchiolitis obliterans mit organisierender Pneumonie (BOOP) auftreten. Das klinische Bild ist vergleichbar mit der idiopathischen Form einer BOOP, die vorherrschenden Symptome sind Fieber, nichtproduktiver Husten und Dyspnoe. Radiologisch fallen periphere, alveoläre z.T. wandernde Infiltrate auf. In vielen Fällen sind diese Infiltrate in nicht bestrahlten Lungenarealen zu finden. Im Gegensatz dazu ist die Strahlenpneumonitis auf das Bestrahlungsfeld begrenzt. Die BAL zeigt eine Zunahme der Lymphozyten, Neutrophilen, Eosinophilen und Mastzellen. Als Ursache wird eine durch die Bestrahlung induzierte Immunreaktion diskutiert. Da jedoch nur ein Teil der Patienten eine BOOP entwickelt, wird ein Trigger angenommen, der eine weitere Entzündungsreaktion in Gang setzt. Differenzialdiagnostisch muss an eine Pneumonie oder eine Lymphangiosis carcinomatosa gedacht werden.
