Unresectable locally advanced pancreatic cancer (LAPC) is managed with first-line chemotherapy or induction chemotherapy with consolidative chemoradiotherapy. Phosphorus-32 (32P) microparticles serve as a brachytherapy device, approved for use in LAPC, that has the advantage of delivering radiation directly to the tumour while reducing the risk of damage to nearby tissue or organs. The case of a 72-year-old woman with an unresectable pancreatic adenocarcinoma in the head of the pancreas with long-term survival following downstaging to resection with 32P microparticles is presented here. The patient received first-line gemcitabine+nab-paclitaxel chemotherapy, with a partial response occurring during the first three cycles of chemotherapy; however, the tumour remained unresectable and subsequently increased in size after a justified delay in chemotherapy. 32P microparticle implantation was conducted as planned at the end of cycle 4, and no device- or procedure-related adverse events were observed. Gemcitabine+nab-paclitaxel chemotherapy was continued, and computed tomography (CT) imaging during cycle 6 (6.8 months after diagnosis) revealed stabilisation of tumour size and a slight decrease in the density of the pancreatic mass. The surgical team determined that surgery could be feasible at this stage. Surgical resection by an open Whipple procedure was successfully completed 7.9 months after diagnosis. As of January 2026, the patient remains alive without evidence of disease 50 months after diagnosis and 42 months post-resection. This case study shows that the addition of 32P microparticles to first-line gemcitabine+nab-paclitaxel chemotherapy, in a patient with unresectable LAPC, is associated with not only disease stabilisation but also conversion to surgical resection, with no associated radiation-related adverse events.
Recent oncologic guidelines for metastatic colorectal cancer have incorporated peritoneal disease into treatment frameworks, emphasizing patient selection and multidisciplinary decision-making. The coexistence of liver and peritoneal metastases (PM) is a marker of biologically aggressive disease with a dismal prognosis. As a rule, simultaneous PM had excluded patients from hepatic resection, leading to reliance on systemic therapy alone. However, recent advances in systemic treatment, cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have challenged this paradigm. Contemporary systematic reviews have shown that combined CRS/HIPEC and liver-directed therapy can provide significant survival benefits in selected patients. The selection of patients for surgery with concurrent liver and peritoneal metastases should be done with even greater caution. This article synthesizes the available evidence and highlights evolving strategies for patients with liver disease and extrahepatic peritoneal involvement.
Este manuscrito presenta una revisión sistemática y un consenso de de expertos en oncología y cirugía en el manejo del cáncer gástrico metastásico (CGm). Se revisó la literatura disponible en PubMed y Google Scholar, seleccionando 28 estudios relevantes (21 ensayos clínicos y siete revisiones sistemáticas), además de guías internacionales. El objetivo fue evaluar el papel de la cirugía y de las terapias locorregionales en pacientes con metástasis peritoneales, hepáticas, pulmonares o ganglionares, así como establecer recomendaciones multidisciplinares. La cirugía citorreductora asociada a quimioterapia intraperitoneal hipertérmica (HIPEC) podría ofrecer beneficios en pacientes seleccionados con carcinomatosis peritoneal limitada (índice de carcinomatosis peritoneal [ICP] ≤ 6), ausencia de metástasis a distancia, adecuada respuesta a quimioterapia sistémica y estado funcional ECOG 0-1. La resección de metástasis no peritoneales también podría ser considerada en contextos específicos. Se requieren ensayos clínicos prospectivos para confirmar estos hallazgos y definir los criterios óptimos de selección.
BACKGROUND:The optimal limb lengths for Roux-en-Y gastric bypass (RYGB) to maximize weight loss and address obesity-related diseases remain debated. OBJECTIVE:To compare the effect of a longer versus shorter biliopancreatic limb (BPL) in RYGB on weight loss and remission of associated diseases. SETTING:Multicenter randomized controlled trial in three university hospitals in Spain. METHODS:Ninety-eight adults with a body mass index (BMI) of 35-50 kg/m2 were randomized to receive either a 150 cm BPL with a 70 cm alimentary limb or the reverse configuration. Primary endpoint was weight loss at 2 years. Secondary endpoints included remission of obesity-related diseases, postoperative morbidity, and nutrients' status. RESULTS:Mean age and BMI were 47 years (standard deviation [SD] 9) and 45 kg/m2 (SD 4), respectively. Baseline demographic and clinical characteristics were comparable between groups. Mean operative time was 136 min (SD 30). Minimum median follow-up was 2 years. At 24 months, excess weight loss was 89% versus 78% (P = .02) and total weight loss was 39% versus 33% (P = .0002), favoring the long BPL group. Complete remission of type 2 diabetes mellitus (T2D) at 12 months was higher with the long BPL group (90% vs. 64%, P = .045). This finding was time-limited, with no significant difference at 24 months. No other comorbidities reached statistical significance. CONCLUSIONS:A longer BPL in RYGB is associated with greater weight loss and higher T2D remission at 1 year, with favorable trends for other associated diseases, supporting individualized limb-length tailoring. Definitive conclusions regarding metabolic outcomes cannot be drawn without larger, adequately powered studies.
This manuscript presents a systematic review and expert consensus from oncology and surgery on the management of metastatic gastric cancer. A literature search was conducted in PubMed and Google Scholar, selecting 28 relevant studies (21 clinical trials and 7 systematic reviews), along with international guidelines. The objective was to assess the role of surgery and locoregional therapies in patients with peritoneal, hepatic, pulmonary, or nodal metastases, and to establish multidisciplinary recommendations. Cytoreductive surgery associated to HIPEC may offer benefits in selected patients with limited peritoneal carcinomatosis (Peritoneal Carcinomatosis Index ≤ 6), an absence of distant metastasis, an adequate response to systemic chemotherapy and an ECOG performance status of 0-1. Resection of non-peritoneal metastases may also be considered in specific contexts. Prospective clinical trials are required to confirm these findings and define optimal selection criteria.
Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy has emerged as a transformative approach to treating peritoneal surface malignancies. Two main HIPEC delivery techniques exist: the open technique (OT) and the closed technique (CT), including variations such as CO₂ recirculation systems. Comparative studies are limited and mostly retrospective.In this exploratory clinical trial, consecutive patients with colorectal peritoneal metastases between March and September 2015 were randomized to OT or CT with CO₂ infusion (CT-CO₂). Intraoperative physiological parameters, postoperative outcomes, and survival were compared. Twenty patients were randomized (9 OT and 10 CT-CO₂ included in the final analysis).Both techniques maintained safe hemodynamic, metabolic, and temperature profiles without statistically significant differences. Intraabdominal pressure increased moderately in CT-CO₂ but remained within safe limits. Minor complications were more frequent in the CT-CO₂ group, though major complications were similar.Both OT and CT-CO₂ are safe options for HIPEC administration. The study’s exploratory nature and limited sample size represent important limitations. Further trials including a conventional closed HIPEC arm are warranted.
Background and objectives: Recurrent isolated pancreatic metastasis from Renal Cell Carcinoma (RCC) after pancreatic resection is rare. The purpose of our study is to describe a series of cases of relapse of pancreatic metastasis from renal cancer in the pancreatic remnant and its surgical treatment with a repeated pancreatic resection, and to analyse the results of both overall and disease -free survival. Methods: Multicenter retrospective study of patients undergoing pancreatic resection for RCC pancreatic metastases, from January 2010 to May 2020. Patients were grouped into two groups depending on whether they received a single pancreatic resection (SPS) or iterative pancreatic resection. Data on short and long-term outcome after pancreatic resection were collected. Results: The study included 131 pancreatic resections performed in 116 patients. Thus, iterative pancreatic surgery (IPS) was performed in 15 patients. The mean length of time between the first pancreatic surgery and the second was 48.9 months (95 % CI: 22.2-56.9). There were no differences in the rate of postoperative complications. The DFS rates at 1, 3 and 5 years were 86 %, 78 % and 78 % vs 75 %, 50 % and 37 % in the IPS and SPS group respectively (p = 0.179). OS rates at 1, 3, 5 and 7 years were 100 %, 100 %, 100 % and 75 % in the IPS group vs 95 %, 85 %, 80 % and 68 % in the SPS group (p = 0.895). Conclusion: Repeated pancreatic resection in case of relapse of pancreatic metastasis of RCC in the pancreatic remnant is justified, since it achieves OS results similar to those obtained after the first resection.
Introduction: Treatment of Peritoneal Surface Malignancies (PSM) with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) has achieved results never seen before in these patients, which classically have a poor prognosis. The possibility of conducting clinical trials in these diseases is complicated, since some of them are rare, so the analysis of large databases provides very valuable scientific information. The aim of this study is to analyze the global results of the National Registry of the Spanish Group of Peritoneal Oncologic Surgery (REGECOP), whose objective is to register all patients scheduled for HIPEC nationwide. Methods: This is a retrospective analysis of the data recorded in the REGECOP from 36 Spanish hospitals from 2001 to 2021. There were 4159 surgical interventions in 3980 patients. Results: 66% are women and 34% are men with a median age of 59 years (range 17–86). 41.5% of the patients were treated for Peritoneal Metastases (PM) of colorectal cancer (CRC); 32.4% were women with ovarian cancer (OC) with PM; 12.8% were treated for pseudomyxoma peritonei (PMP); 6.2% had PM from gastric cancer (GC); 4.9% had PM of non-conventional origin; and, finally, 2.1% of cases were patients diagnosed with peritoneal mesothelioma. The median Peritoneal Cancer Index (PCI) was 9 (0–39), and complete cytoreduction was achieved in 81.7% of the procedures. Severe morbidity (Dindo–Clavien grade III–IV) was observed in 17.7% of surgeries, with 2.1% mortality. Median hospital stay was 11 days (0–259). Median overall survival (OS) was 41 months for CRC patients, 55 months for women with OC, was not reached in PMP patients, was 14 months for GC patients, and 66 months in mesothelioma patients. Conclusions: large databases provide extremely useful data. CRS with HIPEC in referral centers is a safe treatment with encouraging oncologic results in PSM.
Gastric cancer (GC) has a poor prognostic and only one in four patients will have survived by 5 years after diagnosis. These poor results are due to the fact that most patients are diagnosed in advanced stages; peritoneal metastases (PM) are especially frequent and are difficult to treat. Currently, PM are considered a terminal stage of GC with a poor survival rate and are treated with palliative systemic chemotherapy. Since the beginning of the century, the treatment of PM from different origins has evolved; cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have become the treatment of choice for many malignant diseases that affect the peritoneum. CRS and HIPEC have also been used for patients with GC and PM, achieving survival results that have never been achieved when using systemic chemotherapy alone. The use of HIPEC can even prevent the development of peritoneal recurrences in patients with locally advanced GC as adjuvant therapy, can reduce the volume of peritoneal disease as neoadjuvant therapy, and can control symptoms in a palliative setting. The aim of this review is to collate the current scientific evidence regarding the treatment of PM of GC origin with surgery and intraperitoneal chemotherapy.
Diffuse malignant peritoneal mesothelioma (PM) is a rare neoplasm, traditionally associated with a poor prognosis. There are other varieties of PM that are even less frequent and of uncertain malignancy. Cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) has achieved prolonged survival in selected patients. The aim of this study is to analyze the patients with PM assessed in our center. Clinicopathological characteristics, diagnostic procedures and survival results from patients with PM appraised at our unit, according to the applied treatment, were analyzed. Seventeen patients were assessed between 2007 and 2019. Three cases had multicystic PM that were treated with complete CRS + HIPEC; all patients are alive and free of disease after a long follow-up. Three other cases had biphasic PM; a curative treatment could be performed in none of them, and their survival was minimal (<6 moths). Lastly, 11 cases with epithelioid PM were treated. Two cases were considered unresectable at laparoscopy (PCI 39); one of them had a long survival (67 months) with three iterative laparoscopic palliatives HIPECs for refractory ascites. The other nine cases were treated with curative CRS + HIPEC, with a median PCI of 14 (range 4-25), and a median overall survival (OS) of 58 months, with a 5-year OS of 47.4%. In conclusion, CRS + HIPEC, when possible, appears to be the optimal treatment for patients with PM. Knowledge of this therapeutic option is crucial, both to offer it to patients and to avoid delays in their referral to appropriate centers for treatment.
Background: Prognostic factors have been extensively reported after resection of colorectal liver metastases (CLM); however, specific analyses of the impact of preoperative systemic anticancer therapy (PO-SACT) features on outcomes is lacking. Methods: For this real-world evidence study, we used prospectively collected data within the international surgical LiverMetSurvey database from all patients with initially-irresectable CLM. The main outcome was Overall Survival (OS) after surgery. Disease-free (DFS) and hepatic-specific relapse-free survival (HS-RFS) were secondary outcomes. PO-SACT features included duration (cumulative number of cycles), choice of the cytotoxic backbone (oxaliplatin- or irinotecan-based), fluoropyrimidine (infusional or oral) and addition or not of targeted monoclonal antibodies (anti-EGFR or anti-VEGF). Results: A total of 2793 patients in the database had received PO-SACT for initially irresectable diseases. Short (<7 or <13 cycles in 1st or 2nd line) PO-SACT duration was independently associated with longer OS (HR: 0.85 p = 0.046), DFS (HR: 0.81; p = 0.016) and HS-RFS (HR: 0.80; p = 0.05). All other PO-SACT features yielded basically comparable results. Conclusions: In this international cohort, provided that PO-SACT allowed conversion to resectability in initially irresectable CLM, surgery performed as soon as technically feasible resulted in the best outcomes. When resection was achieved, our findings indicate that the choice of PO-SACT regimen had a marginal if any, impact on outcomes.
Background The French PRODIGE 7 trial, published on January 2021, has raised doubts about the specific survival benefit provided by HIPEC with oxaliplatin 460 mg/m 2 (30 minutes) for the treatment of peritoneal metastases from colorectal cancer. However, several methodological flaws have been identified in PRODIGE 7, specially the HIPEC protocol or the choice of overall survival as the main endpoint, so its results have not been assumed as definitive, emphasizing the need for further research on HIPEC. It seems that the HIPEC protocol with high-dose mytomicin-C (35 mg/m 2 ) is the preferred regime to evaluate in future clinical studies. Methods GECOP-MMC is a prospective, open-label, randomized, multicenter phase IV clinical trial that aims to evaluate the effectiveness of HIPEC with high-dose mytomicin-C in preventing the development of peritoneal recurrence in patients with limited peritoneal metastasis from colon cancer (not rectal), after complete surgical cytoreduction. This study will be performed in 31 Spanish HIPEC centres, starting in March 2022. Additional international recruiting centres are under consideration. Two hundred sixteen patients with PCI ≤ 20, in which complete cytoreduction (CCS 0) has been obtained, will be randomized intraoperatively to arm 1 (with HIPEC) or arm 2 (without HIPEC). We will stratified randomization by surgical PCI (1–10; 11–15; 16–20). Patients in both arms will be treated with personalized systemic chemotherapy. Primary endpoint is peritoneal recurrence-free survival at 3 years. An ancillary study will evaluate the correlation between surgical and pathological PCI, comparing their respective prognostic values. Discussion HIPEC with high-dose mytomicin-C, in patients with limited (PCI ≤ 20) and completely resected (CCS 0) peritoneal metastases, is assumed to reduce the expected risk of peritoneal recurrence from 50 to 30% at 3 years. Background: Renal Cell Carcinoma (RCC) occasionally spreads to the pancreas. The purpose of our study is to evaluate the short and long-term results of a multicenter series in order to determine the effect of surgical treatment on the prognosis of these patients. Methods: Multicenter retrospective study of patients undergoing surgery for RCC pancreatic metastases, from January 2010 to May 2020. Variables related to the primary tumor, demographics, clinical characteristics of metastasis, location in the pancreas, type of pancreatic resection performed and data on short and long-term evolution after pancreatic resection were collected. Results: The study included 116 patients. The mean time between nephrectomy and pancreatic metastases' resection was 87.35 months (ICR: 1.51-332.55). Distal pancreatectomy was the most performed technique employed (50 %). Postoperative morbidity was observed in 60.9 % of cases (Clavien-Dindo greater than IIIa in 14 %). The median follow-up time was 43 months (13-78). Overall survival (OS) rates at 1, 3, and 5 years were 96 %, 88 %, and 83 %, respectively. The disease-free survival (DFS) rate at 1, 3, and 5 years was 73 %, 49 %, and 35 %, respectively. Significant prognostic factors of relapse were a disease free interval of less than 10 years (2.05 [1.13-3.72], p 0.02) and a history of previous extrapancreatic metastasis (2.44 [1.22-4.86], p 0.01). Conclusions: Pancreatic resection if metastatic RCC is found in the pancreas is warranted to achieve higher overall survival and disease-free survival, even if extrapancreatic metastases were previously removed. The existence of intrapancreatic multifocal compromise does not always warrant the performance of a total pancreatectomy in order to improve survival. (C) 2021 The Authors. Published by Elsevier Ltd.
Background We aimed to identify clinicopathological and molecular features associated with progression-free survival (PFS) and overall survival (OS) after pulmonary metastasectomy for metastatic colorectal cancer in a retrospective cohort in Brazil. Materials and Methods We did a retrospective review of thoracic surgeries performed in a single large academic hospital in Brazil from January 1985 to September 2019. Demographics, previously described prognostic factors, and clinicopathological and molecular characteristics were abstracted. Univariate Cox regression was performed for each variable, and, when significant, data were dichotomized to provide clinically meaningful thresholds. Results Records from 698 patients were reviewed. Fifty-eight patients underwent pulmonary metastasectomy with curative intent. Of those, 53.4% had a single metastatic lesion. The median size of the largest lesion was 1.5 cm. Results of RAS, RAF, and mismatch repair testing and of cytokeratin 20 (CK20) and CDX2 testing were available for 13.8% and 58.6% of the sample, respectively. Median PFS was 14 months, median OS was 58 months, and 5-year survival was 49.8%. Unfavorable prognostic factors for OS included disease-free interval (DFI) <24 months, synchronous presentation, size of the largest lesion ≥2 cm, and loss of CK20 expression. Presenting with more than one lesion was prognostic for PFS but not for OS. Conclusion In this Brazilian cohort, our findings corroborate existing data supporting DFI, synchronous presentation, and number and size of lesions as prognostic factors. Furthermore, we found that loss of CK20 expression may be associated with more aggressive disease and shorter OS. Additional molecular prognostic factors after pulmonary metastasectomy for colorectal cancer should be further explored. Implications for Practice This study consolidates disease-free interval, synchronous presentation, and number and size of lesions as clinically relevant data that may help guide therapy for patients with colorectal cancer and lung metastases who are candidates for curative-intent metastasectomy. Additionally, in this sample, lack of cytokeratin 20 expression in metastases was associated with shorter progression-free survival and overall survival, suggesting that biomarkers also may have a role in guiding therapy in this setting and that additional biomarkers should be further explored.

Background Pancreatectomy plus celiac axis resection (CAR) is performed in patients with locally advanced pancreatic cancer. The morbidity rates are high, and no survival benefit has been confirmed. It is not known at present whether it is the type of pancreatectomy, or CAR itself, that is the reason for the high complication rates. Methods Observational retrospective multicenter study. Inclusion criteria: patient undergoing TP, PD or DP plus CAR for a pancreatic cancer. Results Sixty-two patients who had undergone pancreatic cancer surgery (PD,TP or DP) plus CAR were studied. Group 1: 17 patients who underwent PD/TP-CAR (13TP/4PD); group 2: 45 patients who underwent DP-CAR. Groups were mostly homogeneous. Operating time was longer in the PD/TP group, while operative complications did not differ statistically in the two groups. The number of lymph nodes removed was higher in the PD/TP group (26.5 vs 17.3), and this group also had a higher positive node ratio (17.9% vs 7.6%). There were no statistical differences in total or disease-free survival between the two groups. Conclusion It seems that CAR, and not the type of pancreatectomy, influences morbidity and mortality in this type of surgery. International multicenter studies with larger numbers of patients are now needed to validate the data presented here.
Background: There are two ways of carrying out Hyperthermic IntraPEritoneal Chemotherapy: the open (or “Coliseum") technique and the closed technique. There is no study that demonstrates a greater long-term clinical benefit of either technique.
