ResumenAntecedentes:La selectividad clínica de los antidepresivos con especificidad farmacológica se sigue debatiendo todavía.Método:En el ensayo abierto presentado más adelante, se compararon los efectos de la fluoxetina, un inhibidor selectivo de la recaptación de la serotonina (SSRI), a través de dos grupos de pacientes internos depresivos que contrastaban en su expresión de síntomas (agitados/ansiosos frente a pacientes con retardo/embotamiento afectivo). Dieciséis pacientes (8 en cada grupo) fueron incluidos en el período de tratamiento de 4 semanas y sometidos a una valoración semanal. Se evaluó la depresión global, el retardo, el embotamiento emocional, la agitación, la ansiedad y el perfil de estado de ánimo.Resultados:Se observaron mejorías significativas de las puntuaciones en la HDRS y la MADRS en ambos grupos. Aunque no se encontró interacción grupo tratamiento en las puntuaciones globales de depresión, se observó un efecto diferencial sobre la ansiedad, la agitación, la irritabilidad y la labilidad emocional según el grupo.Discusión:Estos resultados preliminares apoyan la hipótesis de que el efecto de la fluoxetina sobre las dimensiones clínicas positivas podría llevar a un efecto diferencial en los pacientes con agitación/ansiedad, comparados con los pacientes con retardo/embotamiento afectivo.
A new method to solve linear dynamics problems using an asymptotic method is presented. Asymptotic methods have been efficiently used for many decades to solve non-linear quasistatic structural problems. Generally, structural dynamics problems are solved using finite elements for the discretization of the space domain of the differential equations, and explicit or implicit schemes for the time domain. With the asymptotic method, time schemes are not necessary to solve the discretized (space) equations. Using the analytical solution of a single degree of freedom (DOF) problem, it is demonstrated, that the Dynamic Asymptotic Method (DAM) converges to the exact solution when an infinite series expansion is used. The stability of the method has been studied. DAM is conditionally stable for a finite series expansion and unconditionally stable for an infinite series expansion. This method is similar to the analytical method of undetermined coefficients or to power series method being used to solve ordinary differential equations. For a multi-degree-of-freedom (MDOF) problem with a lumped mass matrix, no factorization or explicit inversion of global matrices is necessary. It is shown that this conditionally stable method is more efficient than other conditionally stable explicit central difference integration techniques. The solution is continuous irrespective of the time segment (step) and the derivatives are continuous up to orderN-1 whereNis the order of the series expansion.
Background: Clinical selectivity of antidepressants with pharmacological specificity still remains under debate. Method: In the open trial presented below, the effects of fluoxetine, a selective serotonin re-uptake inhibitor (SSRI), were compared across two groups of depressive inpatients contrasted on their symptomatological expression (agitated/anxious versus retarded/blunted affect). Sixteen patients (8 in each groups) were included in the 4-weeks treatment period and submitted to a weekly-based evaluation. Global depression, retardation, emotional blunting, agitation, anxiety and mood profile were assessed. Results: Significant improvements of the HDRS and MADRS scores were observed in both groups. Although no group x treatment interaction was found on the global scores of depression, a differential effect according to the group was observed on anxiety, agitation, irritability and emotional lability. Discussion: These preliminary results support the hypothesis that the effect of fluoxetine on positive clinical dimensions could lead to a differential effect in patients with agitation/anxiety when compared with patients with retardation/blunted affect.
The buckling loads are, in general, those for which ¦K¦ = 0, where K is the stiffness matrix. In this case, K is a transcendental function of the buckling load λ. The Wittrick and Williams' algorithm consists of finding how many buckling loads have been exceeded at each iteration. Coupled with the bisection method, this algorithm is infallible, but sometimes the convergence rate is slow. To accelerate the convergence, a new method for computing the buckling loads is presented. This method can be considered as an improvement on the bisection method of Wittrick and Williams' algorithm. An exact geometric matrix which is a function of the buckling load λ is derived. The stiffness and geometric matrices are used to perform a linearized eigenproblem combined with the Wittrick and Williams method.
The aim of this paper is to focus on methodological problems related to clinical studies on the onset of action of antidepressants, especially moclobemide. The methodological discussion proposed focuses on: global efficacy as a function of time, proposals for a specific approach to the study of the onset of action; quality of the response and onset of action. the dimensional level and the onset of action.
