À propos d’une série de 44 patients qui ont bénéficié d’une procédure d’ablation de la grande veine saphène par radiofréquence en ambulatoire au CHU d’Amiens, nous décrivons la mise en place, le déroulement et les résultats de cette technique, et discutons les contraintes et difficultés rencontrées. Entre octobre 2012 et mai 2014, 44 patients ont bénéficié dans notre service du traitement d’une incontinence ostio-tronculaire symptomatique de la grande veine saphène. Dans 44 cas, les caractéristiques anatomiques et hémodynamiques des grandes veines saphènes responsables de la symptomatologie, validées lors d’une consultation « de contact », autorisaient une ablation endoveineuse par radiofréquence. Cette consultation préalable est également indispensable à l’information du patient. La procédure est réalisée selon la technique « Closure Fast » (Covidien), au cours d’une vacation hebdomadaire, par un médecin du service dûment formé, assisté d’une infirmière, en environnement stérile, dans une salle dédiée définie par l’arrêté du 7 janvier 1993. Chaque patient bénéficie d’une injection de fondaparinux 2,5 mg avant le geste, poursuivi les 7 jours suivants, sauf s’il est déjà sous traitement anticoagulant oral. Hormis l’anesthésie par tumescence de la grande veine saphène par xylocaïne 1 %, le patient ne reçoit pas d’autre drogue, ce qui permet de le libérer après une surveillance courte d’une heure après la procédure. L’intervention ne nécessite pas d’arrêt de travail. Le patient est revu à j8 pour vérifier l’oblitération du segment de veine traité, le résultat fonctionnel de la procédure, ainsi que l’absence de complication du geste endoveineux. Des 44 procédures réalisées, une seule n’a pas abouti par échec technique. Deux procédures se sont révélées inefficaces à j8 (reperméabilisation du segment veineux traité). Les 41 patients qui avaient une oblitération de la grande veine saphène à j8 constataient une amélioration des symptômes de l’insuffisance veineuse très significative, qui persistait dans tous les cas à 3 mois. L’évaluation de la douleur per-procédurale sur l’échelle analogique n’a jamais dépassé 4, et a toujours été nulle en post-procédure. En dehors d’une thrombose veineuse superficielle d’une tributaire postérieure de cuisse, non traitée, aucune complication n’a été objectivée lors du suivi à 3 mois. L’ablation de la grande veine saphène par radiofréquence en milieu médical hospitalier, en ambulatoire, est une procédure simple, sûre et efficace. Son coût est moindre qu’en milieu chirurgical, mais reste néanmoins sa principale limitation en l’absence de remboursement des soins pour le moment.
Isolated distal deep vein thrombosis (iDDVT) (i.e. without proximal DVT or pulmonary embolism) represents half of all cases of lower limb DVT. Its clinical significance and management are controversial. Data on long‐term follow‐up are scarce, especially concerning risk and predictors of venous thromboembolism (VTE) recurrence.
Accepter une demande de filtration cave : sur quels arguments ? Choisir un type de filtre et informer le(a) patient(e) de la filtration et du suivi. Savoir assurer la traçabilité du patient filtré et programmer un retrait par consensus multidisciplinaire omnipraticien-spécialiste-anesthésiste-radiologue. Connaître l’efficacité et les limites des filtres caves. En présence d’une situation clinique à haut risque embolique primaire ou par récidive : On distingue les contextes péri-chirurgicaux mobilisateurs de thrombus, et les contextes médicaux où l’ anticoagulation est inappropriée. Si le pronostic vital est normal, le filtre peut être temporaire ou optionnel. S’il est court, un filtre permanent est adapté. La transversalité du service d’imagerie est idéale pour consigner la traçabilité d’une cohorte de patients. La filtration abaisse le taux de récidive embolique à moins de 5 %. L’angiographie 3D rotationnelle informe sur l’expansion et l’inclinaison du filtre, la géométrie de la veine cave et son contenu. Un filter permanent à stabilisateurs a une variante, l’optionnel à conversion par ouverture. Un temporaire est extrait dans 98 % des cas, un optionnel à retrait dans 70 % des cas. L’embolie pulmonaire (EP) est létale par récidive dans plus de 35 % des cas. Un contexte péri-chirurgical mobilisateur de thrombus périphérique ou médical avec anti-coagulation impossible, peut motiver la mise en place percutanée échoguidée d’un filtre cave. Le choix d’un filtre permanent est justifié en cas de pronostic vital court. Sinon, un filtre temporaire implanté sous la peau du cou pendant au maximum 3 mois convient aux patients d’unités chirurgicales. Un optionnel– amovible ou convertible – est adapté si l’espérance de vie du patient est normale. Tous les accès veineux sont possibles si le diamètre de la gaine est inférieur ou égal à 9-Fr. L’accès le mieux vécu par le patient est brachial. L’arthrose cervicale gêne l’accès jugulaire, l’incontinence urinaire ou fécale fera éviter l’accès fémoral. L’évaluation du couple veine cave – filtre après largage et avant retrait se fait avantageusement par angiographie 3D rotationnelle. Les étapes et les difficultés éventuelles des procédures y sont commentées, incluant la traçabilité et la date de convocation pour retrait. Sous filtration, l’EP récidive dans moins de 5 %. Le pourcentage de success à l’extraction d’un filtre temporaire est 98 %, d’un retirable 70 %.
Background: Engrailed-land Engrailed-2 are homeoproteins - transcription factors implicated in the morphogenesis of discrete structures. Engrailed proteins have a role in patterning the midbrain-hindbrain region and are expressed in the nuclei of rat embryo midbrain-hindbrain cells. We have previously found that both endogenous and exogenously expressed Engrailed proteins also associate with membrane regions implicated in signal transduction and secretion. Within total membrane fractions, a small proportion of Engrailed - about 5% - is protected against proteinase K proteolysis, suggesting that Engrailed has access to a luminal compartment. Together with our finding that homeodomains and homeoproteins can be internalized by live cells, these observations suggest that Engrailed might act as a polypeptidic messenger. In order to investigate this possibility, we looked to see if Engrailed could be secreted.Results: Engrailed expressed in COS cells can be recovered in abutting primary neurons and this is dependent on a short sequence in its homeodomain distinct from 'classical' secretion signals. This sequence, which overlaps with the sequence necessary for Engrailed internalization and which is highly conserved among homeoproteins, is the first example of an 'unconventional' sequence necessary for secretion. Less than 5% of total intracellular Engrailed is secreted and there is a correlation between secretion and access to the membrane compartment where the protein is protected against proteinase K,Conclusions: Our results lend weight to the proposal that Engrailed, and possibly other homeoproteins, might act as intercellular polypeptidic messengers.
Monitoring of trace levels of phenylurea and chlorotriazine herbicides and their metabolites deispropylatrazine (DIA) and deethylatrazine (DEA) was performed on river, estuarine and coastal seawater by off-line (n-octadecyl silica, C18) and on-line (styrene-divinylbenzene copolymer-based, PRP1) preconcentration techniques using reversed-phase chromatography and UV and electrochemical detection. It is shown that off-line and on-line technology fit monitoring of trace-level phenylurea and triazine analysis (detection limit 10–50 ng/l) in freshwater as well as seawater and in natural water with high dissolved organic carbon (DOC) content. Clean-up was unnecessary for most of the surface water sample for both off-line and on-line techniques. Samples of surface water originating from various rivers (Seine, Charente, Garonne), canals (Marennes-Oléron) and estuaries (Charente, Seudre salinity gradient up to 30‰) were analysed. Dissolved organic carbon (DOC) contents varied from <1 to 15 mg/1 and provided numerous interferents and a major ‘hump’ whose intensity is correlated with DOC content, suggesting that a major part of the organic matter, namely humic substances, is retained on the solid phase and eluted as polar compounds.
An automated system using on-line trace-enrichment, separation by liquid chromatography with a gradient elution and diode array detection is described for monitoring pesticides in ground water. Two systems were used for covering the wide range of polarities of the targeted pesticides: the first one consisted of a PLRP-S precolumn and a C-18 analytical column for moderately polar to non polar compounds and the second one, specially designed for the determination of polar pesticides and degradation products, consisted of a precolumn and analytical column both packed with porous graphitic carbon. The detection limits that can be reached by the two systems are in the low 0.1 mu g/l for the ground water samples of interest with the handling of 100-150 mi of water. An application is presented with the first objective of monitoring the quality of a ground water reservoir which comes from a small basin where the amount and nature of pesticides applied are known and where atrazine has been banned for the last five years and replaced by terbutylazine. The second objective was to have a better knowledge of the behaviour and persistence of pesticides from the results of a long-term survey of 18 months. Obviously, the water solubility is an important parameter for the leachability of pesticides. Although banned on this site for the last five years, atrazine and deethylatrazine are found at constant concentrations, 0.5 and 0.7 mu g/l respectively all the year round. The on-line system with the PGC precolumn and analytical column allowed to confirm the deethylatrazine concentration as well as the very low concentration (< 0.05 mu g/l) of both hydroxyatrazine and deisopropylatrazine. Although applied in replacement of atrazine, neither terbutylazine nor its major dealkylated metabolite were detected until now. Other more soluble compounds such as isoproturon or metamitron are detected during the period following their application.
Phenylureas and triazines were analysed by reversed-phase liquid chromatography using UV detection at 254 nm after an on-line preconcentration step on a PRP-1 copolymer in order to determine pesticides in river water at mu g/l level or lower. Running water was sampled in the Garonne from a station located at La Reole, upstream from Bordeaux and from a small tributary, the Dropt. Water samples were collected mainly at several periods between December 1989 and September 1990 at a low water time (December 1989) and during two high flood periods (February 1990 and May 1990). Atrazine, simazine, de-ethylatrazine, diuron, chlortoluron and isoproturon were detected and quantified. In the Garonne river, atrazine, simazine, de-ethylatrazine and diuron were usually present at the sampling time, whereas chlortoluron and isoproturon maximised during the winter flood (February). In the Dropt river, triazine concentrations were normally between 1.0 and 0.1 mu g/l and maximised at 2.2 mu g/l during the spring flood (June 1990). De-ethylatrazine/(Atrazine + Simazine) ratio seems to be significantly higher than in the Mississippi river and may be in relation to the use of simazine in the drainage basin. These data are in agreement with seasonal applications of phenylurea and triazine herbicides and hydrologic and pluviometric conditions.
