Neonatal septicemia is the most important cause of neonatal mortality. A wide variety of bacteria both aerobic and anaerobic can cause neonatal sepsis. Genus Pantoea is a member of Enterobacteriaceae family that inhabits plants, soil and water and rarely causes human infections, however, Pantoea dispersa has not been reported as a causative organism for neonatal sepsis. We hereby report two neonates with early onset sepsis caused by Pantoea dispersa. Early detection and appropriate antibiotic therapy can improve overall outcome of this rare infection in neonates.
Background and purpose. - Long-term results of decompressive laminectomy in degenerative lumbar stenosis have been studied in only six prospective studies. The objective of our study was to evaluate the functional outcome at long term of patients after decompressive laminectomy in lumbar stenosis and to determine predictive factors of favorable outcome.Methods. - A prospective cohort data were collected by an independent observer five years after decompressive laminectomy for degenerative lumbar stenosis. The endpoint was the assessment of the Beaujon score for functional evaluation. The result was considered as favorable if the Beaujon score increased by at last five points' between the preoperative stage and at follow-up examination. Logistic regression was then performed with univariate and multivariate analysis to reveal predictive factors of good long-term outcome (P <= 0.05).Results. - The preoperative characteristic of our population (n = 98) was a mean age of 67.3 +/- 8.8 years, a low comorbidity (mean Charlson score = 2.8 +/- 1.5), overweight status (BMI = 29.4 +/- 6.3) and the mean Beaujon score was 9.3 +/- 3.1. At five years after surgery, the mean Beaujon score became 14.1 +/- 4.2. Favorable functional outcome concerned 45.9% of our series. The predictive factor of favorable outcome identified in the univariate analysis the neurological deficit (P = 0.05) and in the multivariate analysis the low comorbidity (P = 0.01).Conclusion. - The long-term results of surgical treatment of lumbar spinal stenosis were moderate with an improved outcome in 49.5% of cases in our study. Background. - For suprasellar meningioma, the fronto-basal exposure is considered the standard approach. The superior interhemispheric (IH) approach is less described in the literature.Objective. - To assess the surgical complications, functional outcome (visual, olfaction), morbidity and mortality rates and late recurrence, after resection by superior IH approach of midline skull base meningioma.Methods. - Between 1998 and 2008, 52 consecutive patients with midline meningioma on the anterior portion of the skull base (mean age: 63.8 +/- 13.1; sex ratio F/M: 3.7) were operated on via the superior IH approach. After a mean follow-up of 56.9 +/- 32.9 months, an independent neurosurgeon proposed a prospective examination of functional outcome to each patient, as well as a visual and olfactory function assessment.Results. - Fifty-two patients were divided into a group with olfactory groove meningioma (n=34) and another with tuberculum sellae meningioma (n=18). The outcome was characterized by postoperative complications in 13 patients (25%), mortality rate in two (3.8%) and long-term morbidity at in 17(37%) of 50 surviving patients. Based on multivariate analysis, no prognosis factor was significant as regards the favorable outcome. The mean postoperative KPS score (86.6 +/- 9.4) was significantly improved. However, dysexecutive syndrome was observed in four patients (8%), hyposmia-anosmia in 34 (68%) and visual acuity deteriorated in one (2%).Conclusion. - The superior IH approach could be considered a safe anteriorly orientated midline approach for removal OGM and TSM meningioma. Les infarctus cérébelleux peuvent être difficiles à diagnostiquer car le tableau clinique est souvent dominé par des symptômes peu spécifiques orientant plutôt vers une pathologie bénigne. Lorsque les infarctus du cervelet sont associés à une atteinte du tronc cérébral, la sémiologie est plus riche et relègue au second plan les signes cérébelleux purs. Quelle que soit la topographie de l’infarctus, une parfaite connaissance de l’organisation des territoires des artères cérébelleuses est nécessaire. Cette connaissance est indispensable pour établir avec précision le diagnostic, comprendre les mécanismes et organiser la prise en charge. Des algorithmes cliniques de prise en charge des vertiges, des céphalées ou des vomissements, permettraient de mieux sélectionner les candidats à l’imagerie cérébrale. Ainsi, l’identification précoce des patients qui portent un risque élevé de détérioration ultérieure permettrait d’améliorer le pronostic des infarctus des territoires des artères cérébelleuses.Cerebellar infarction can be difficult to diagnose because the clinical picture is often dominated by fairly non-specific symptoms, which are more indicative of a benign condition. When cerebellar infarction affects the brainstem, the semiology is richer, and pure cerebellar signs are rendered less important. A perfect knowledge of the organisation of the cerebellar artery territories is required, regardless of the infarct topography. This knowledge is essential for making an accurate diagnosis, understanding the mechanisms and organising a treatment plan. Clinical algorithms for the treatment of dizziness, headaches and vomiting would improve the selection of candidates for brain imaging. Thus, the early identification of patients with a high risk of subsequent deterioration would lead to a better prognosis in cases of cerebellar artery territory infarction.
Les patients pris en charge à la phase aiguë d’un accident vasculaire cérébral (AVC) bénéficient des progrès de l’imagerie. L’Imagerie par Résonance Magnétique (IRM) permet de réaliser précocement, dès les premières minutes, le diagnostic de lésions ischémiques. Celles-ci sont susceptibles de régresser si un traitement adapté, visant à lever l’occlusion artérielle, est mis en route dans les premières heures. L’IRM évalue les possibilités de mise en route de ce traitement, écartant notamment l’éventualité d’un AVC hémorragique, détermine le pronostic et, enfin, participe au diagnostic étiologique. Patients with brain damage in the ischemic, but not yet infarcted, phase have the greatest potential for recovery. Here we review the most commonly employed diagnostic tools that are currently used before stroke therapy. While computed tomography is pertinent to differentiate ischemic from hemorrhagic stroke, this technique cannot be used as an etiological screening too. The ischemic origin of symptoms can be confirmed with magnetic resonance imaging which also contributes to for therapeutic decision making, prognosis assessment and etiological screening. The authors report the results of a multicenter retrospective series with a long-term follow-up and the analysis of clinical, radiological, surgical data as well as the complementary treatments in patients with intramedullary astrocytomas (IA).We performed a retrospective analysis of all the patients with IA operated on between 1984 and 2011 at 7 French centers (Kremlin-Bicêtre, Lille, Lyon, Marseille, Montpellier, Nice, and Nîmes). The minimum follow-up was 12 months. The clinical evaluation was based on the McCormick scale (MCS) results from the pre- and postoperative period.Data from 95 patients with a pathologically confirmed diagnosis of IA were considered: 54 patients were treated at the Neurosurgical Department of Kremlin-Bicêtre Hospital, 8 were treated at Lille and 33 were treated in the south region of France. The epidemiological analysis was performed on the whole cohort of patients while follow-up considerations were made solely on the 54 patients managed at Kremlin-Bicêtre Hospital to obtain homogeneous data. The average age at diagnosis was 35.6 years without significant gender difference (47 % men for 53 % women). The age at first clinical manifestation was 33.7 years. The average duration of the symptoms before the diagnosis was 22.9 months. Neuropathic pain was the principal revealing symptom (76 % of cases). The localization of IA was thoracic in 40 %, purely cervical in 28.4 %. Complete removal was achieved in 29.5 % of cases when considering the whole cohort and in 38 % of cases treated at Bicêtre Hospital. The histological distribution recorded was: grade 1 in 35 %; grade 2 in 35 %; grade 3 in 22 % and grade 4 in 8 %. During the early postoperative period (3 months) a worsening of functional capacity was observed with an increase in the frequencies of ranks 3 and 4 of MCS in 18.4 %. At 5 years follow-up, the frequencies of ranks 1 and 2 were increased. The application of a Cox model for the determination of the relative risk of death for IA grade 1 and 2 (66 patients) showed a probability of survival at 5 years of 78.6 % (CI 95 %: 68.6 %–87.6 %). Survival at 10 years is to 76.8 % (CI 95 %: 62.3 %–84.2 %).Surgery is indicated if the patient is symptomatic or the tumor increases in size. A radical excision remains the mainstay of treatment, while searching to preserve the motor function. A total resection was however only possible in 38 % of cases. A regular postoperative follow-up is compulsory and the adjuvant treatment is based on chemotherapy and radiotherapy according to the histological type.Les auteurs rapportent les résultats chirurgicaux d’une série multicentrique avec un long suivi en analysant les données cliniques, radiologiques, chirurgicaux, les complications de la chirurgie et le traitement complémentaire des astrocytomes intramédullaires (AI).Nous avons analysé tous les dossiers de patients opérés entre 1984 et 2011 d’un AI confirmé à l’examen anatomopathologique, dans sept centres français (Kremlin-Bicêtre, Lille, Lyon, Marseille, Montpellier, Nice, Nîmes). Un suivi clinique et radiologique minimal était de 12 mois. L’évaluation clinique était basée sur la classification de McCormick (MC) en pré- et en postopératoire.Un total de 95 patients ayant un AI a été retenu : 54 patients opérés au Kremlin-Bicêtre, 8 à Lille et 33 de la région Sud. L’analyse épidémiologique a été réalisée sur l’ensemble de la série mais en ce qui concerne le suivi et les corrélations, par souci d’homogénéité nous nous sommes contentés de l’analyse de la série de 54 cas de Bicêtre. La moyenne d’âge au moment du diagnostic était de 35,6 ans sans prédominance significative de sexe (47 % d’hommes pour 53 % de femmes). L’âge de début des symptômes était de 33,7 ans. La durée moyenne des symptômes avant le diagnostic était de 22,9 mois. Les douleurs neuropathiques sont les principaux symptômes révélateurs (76 % des cas). Les AI sont de localisation thoracique dans 40 % et cervicale pure dans 28,4 %. Le taux d’exérèse complète était de 29,5 % dans l’ensemble de la série, et de 38 % pour les patients de Bicêtre. Sur le plan histologique, il s’agissait d’un grade 1 dans 35 % ; grade 2 dans 35 % ; grade 3 dans 22 % et d’un grade 4 dans 8 %. En postopératoire, on note une aggravation du handicap fonctionnel avec augmentation des fréquences des grades 3 et 4 de MC dans 18,4 %. À 5 ans du suivi, le taux des patients avec un score de MC 1 et 2 augmente. L’application du modèle de Cox pour la détermination du risque relatif de décès pour les IA grade 1 et 2 (66 patients) montre une probabilité de survie à 5 ans de 78,6 % (IC95 % : 68,6 %–87,6 %) et à 10 ans de 76,8 % (IC95 % : 62,3 %–84,2 %).Une prise en charge chirurgicale précoce doit être envisagée dès l’apparition des premiers signes cliniques. Une chirurgie doit également être proposée devant des signes radiologiques d’évolutivité. Le but est l’exérèse complète mais celle-ci ne doit pas dépendre de l’aggravation neurologique. L’exérèse complète est possible que dans 38 % des cas. Un suivi à long terme est obligatoire. Le traitement complémentaire est basé sur la radiothérapie et la chimiothérapie et va dépendre du type histopathologique.
Le rejet vasculaire entraîne une ischémie chronique de l'organe transplanté. Il se caractérise par un phénomène d'agression — réponse de la paroi artérielle qui aboutit à une oblitération des vaisseaux. L'hypothèse de notre travail était qu'une thérapie cellulaire autologue au receveur préviendrait la réponse oblitérative de la paroi artérielle. L'objectif de ce projet était de mettre en évidence la cinétique d'expression intra tissulaire de trois cytokines (SDF1, MCP1 et VEGF) ayant un rôle dans la mobilisation et la captation des cellules progénitrices afin de pouvoir proposer une thérapie cellulaire au moment le plus opportun. 1) Le modèle était une allogreffe aortique (n=35) chez la souris BALBC donneuse / C57BL6 receveuse. Trois souris étaient sacrifiées à J0, J1, J3, J6, J9, J12, J20. A chaque temps, 2 isogreffes (C57BL6 sur C57BL6) étaient réalisées. 2) Les concentrations des cytokines SDF-1, VEGF et MCP-1 étaient dosées pour chaque souris sacrifiée par méthode ELISA dans le sang d'une part et sur le greffon aortique prélevé d'autre part. Dans le groupe allogreffe, la concentration tissulaire en SDF-1 et en VEGF était plus importante à J12 qu'aux autres temps (SDF-1: 22,16 ng/mg, par exemple t = 2,75 pour J0; VEGF: 28,3 pg/mg, par exemple t = 3,27 pour J9). A J12 la concentration tissulaire en SDF-1 et en VEGF était plus importante en allogreffe qu'en isogreffe. SDF-1 (22,16 ng/mg vs 5,69 ng/mg, t = 3,38), VEGF (28,3 pg/mg vs 9,3 pg/mg, t = 3,06) La concentration tissulaire et sérique de MCP-1 était plus importante à J0 qu'aux autres temps en allogreffe (tissulaire 222 pg/mg vs 32,7 ng/mg à J9, t = 6,24). Il n'y avait pas de différence avec les isogreffes. Malgré l'absence de donnée objective sur les mécanismes de production des cytokines, ces résultats étaient en faveur d'une thérapie cellulaire centrée sur l'expression à J12 de SDF-1, VEGF, plus spécifiques que MCP-1.
Spinal ependymomas are predominantly slow-growing lesions constituting approximately 30–88% of primary spinal intramedullary tumors. They usually present as circumscribed lesions, with regular margins and a clear surgical plane. Gross-total resection is often feasible and potentially curative but neurosurgeons should keep in mind that the ultimate goal of surgery is the preservation of spinal cord function. We present the surgical technique to safely resect an intramedullary ependymoma using a posterior median sulcus approach. A brief description of current management of this pathology is also presented.Les épendymomes intramédullaires sont des tumeurs généralement à croissance lente, ils représentent environ 30 à 88 % des tumeurs intramédullaires primaires. Le plus souvent ce sont des lésions circonscrites avec un bon plan de clivage, ce qui facilite l’exérèse chirurgicale avec un bon taux d’exérèse complète. Le but du traitement chirurgical est d’obtenir une exérèse complète avec une préservation des fonctions neurologiques et un minimum de complication. Le but de ce travail est de décrire les différentes étapes microchirurgicales d’exérèse d’un épendymome intramédullaire à travers le sillon médullaire dorsal. Nous décrivons aussi brièvement les différentes étapes de prise en charge de cette pathologie tumorale.
Nous rapportons dans ce chapitre les résultats de l’activité d’un certain nombre de services de neurochirurgie, dédiée à la prise en charge des tumeurs de l’orbite et la comparons à celle d’un service neurochirurgical parisien dont les liens et l’implication avec un service d’ophtalmologie étaient très étroits. L’activité des différents centres intéressés est relativement faible et est constituée principalement par le traitement des méningiomes sphéno-orbitaires, les autres types tumoraux étant traités de façon inégale et sporadique.In this chapter, we report the results of orbital tumor management in a few neurosurgical departments and compare it to a Paris neurosurgical department that has developed a close relation with an ophthalmological department. These departments’ activity is quite low, treating mainly sphenoorbital meningiomas. Other tumor groups are unequally and sporadically managed.
Glial cysts of the pineal gland are usually benign and asymptomatic. They develop from the pineal parenchyma and contain liquid. The diagnosis is made by magnetic resonance imaging. In contrast large cysts can be symptomatic due to compression of the aqueduct of Sylvius, compression of the midbrain tectum or mass effect in the posterior fossa. We report the case of a symptomatic cyst treated by an endoscopic procedure.

The STICH (surgical treatment for intracerebral haemorrhage) study showed no overall benefit from early surgery when compared with initial conservative treatment. In contrast, the stereotactic aspiration technique can be safely performed and in a uniform manner. Despite the reduction of ICH volume, no improvement in mortality and functional result was obtained. Endoscopy is a new therapeutic option for ICH with good results for hematoma removal. Based on these feasibility studies, a randomized control trial regarding this procedure would be required to assess the efficacy of this procedure. Due to the lack of benefit observed in the recent STICH trial, emergency surgical evacuation should be reserved for patients with large lobar haemorrhage, mass effect and rapidly deteriorating clinical condition.

Le traitement neurochirurgical de la douleur chronique comprend actuellement deux domaines : 1) Celui des techniques dites lésionnelles qui consistent à interrompre sélectivement la transmission du message nociceptif et/ou à détruire les générateurs de la douleur. Elles sont indiquées dans les douleurs cancéreuses résistantes aux morphiniques et dans quelques rares cas de douleurs neuropathiques réfractaires. 2) Celui des techniques de neuromodulation qui cherchent à renforcer les mécanismes inhibiteurs et à limiter les mécanismes activateurs de la douleur soit par stimulation électrique (stimulation nerveuse périphérique, médullaire, cérébrale profonde ou stimulation corticale), soit par administration intrathécale de produits antalgiques (morphine, ziconotide) à l’aide de pompes ou sites implantables. Les techniques de neurostimulation sont indiquées en cas de douleurs neuropathiques chroniques. Les injections intrathécales, suivant la molécule antalgique choisie, peuvent être utilisées dans la douleur du cancer et également pour certaines douleurs neuropathiques. Ces différentes techniques chirurgicales peuvent apporter un soulagement, parfois spectaculaire, à quelques patients douloureux chroniques bien sélectionnés et résistants à tous les autres traitements antalgiques. La qualité du résultat est ainsi directement liée à la rigueur des indications.Neurosurgical treatment of pain used two kind of techniques: 1) Lesional techniques interrupt the transmission of nociceptive neural input by lesionning the nociceptive pathways (drezotomy, cordotomy, tractotomy…). They are indicated to treat morphine-resistant cancer pain and few cases of selected neuropathic pain. 2) Neuromodulation techniques try to decrease pain by reinforcing inhibitory and/or to limit activatory mechanisms. Chronic electrical stimulation of the nervous system (peripheral nerve stimulation, spinal cord stimulation, motor cortex stimulation…) is used to treat chronic neuropathic pain. Intrathecal infusion of analgesics (morphine, ziconotide…), using implantable pumps, allows to increase their efficacy and to reduce their side effects. These techniques can improve, sometimes dramatically, selected patients with severe and chronic pain, refractory to all other treatments. The quality of the analgesic outcome depends on the relevance of the indications.
