A one -stage population study was conducted in the working population in south Yakutia. The 174 people of non -indigenous nationality were examined. Increased uric acid (UA) levels were found in 27% of the individuals. The association of UA level with BMI, OT, lipid spectrum was revealed mainly in men, systolic blood pressure and blood glucose in women. Abdominal obesity was equally frequently recorded in both men and women, regardless of the presence or absence of hyperuricemia, Logistic regression showed satisfactory information content of the prognostic significance of the level of UA with hypertension only in the female population. Hyperuricemia was not an independent risk factor for the development of cardiovascular pathology.
A one-time medical and biological examination of the health status of 161 residents of Yakutsk aged 20 to 72 years who recovered from COVID-19 3-12 months ago during the first wave of the pandemic was carried out. Changes in lung function were detected in 29.5% of 139 patients who underwent spirometry. Among them, 7.2% of the surveyed revealed extremely severe changes. The appearance of symptoms and disturbances in the function of external respiration in the post-covid period did not have significant differences in hospitalized and outpatients, and were not associated with the post-covid period, the severity of lung damage in the acute period of the disease, smoking, and the presence of chronic diseases of the respiratory system. The obstructive and restrictive type of disorder was more often observed in women treated on an outpatient basis, which indicates a protracted recovery that requires the same rehabilitation measures that are carried out for hospitalized patients in order to eliminate multiple organ post-COVID complications.
Sources indicate that being infected with COVID-19 leaves patients with undesirable symptoms that form a new disease - post-covid syndrome. We examined the health status of 155 people who suffered a coronavirus infection. Informed consent to the study was obtained from all participants. They were interviewed, asked to fill out a series of questionnaires to assess symptoms and quality of life, and underwent a medical examination. Our results showed that after 8 months, people who have had COVID-19 complain about the symptoms of post-covid syndrome. At the same time, the most common clinical manifestations were sleep disorders (50.9%), hypertension (47.7%), weakness (33.5%), dizziness (32.9%), shortness of breath (27.7%), headaches (24.5%), gastrointestinal disorders (diarrhea, abdominal pain) (18.7%). Our statistical analysis showed a significant dependence of increased blood pressure (1.7 times higher) with the degree of lung damage.
Tuberculosis is one of the most common infectious diseases worldwide. According to the approved clinical recommendations, combined anti-tuberculosis therapy with isoniazid, rifampicin, pyrazinamide and ethambutol. Isoniazid is one of the most effective anti-tuberculosis drugs for the treatment of drug-sensitive tuberculosis, however, it has a wide range of undesirable side effects, including drug-induced liver injury. The enzyme glutathione S-transferase is involved in the detoxification of toxic metabolites of isoniazid. There are a number of studies in which the role of deletion genotypes GSTM1 and GSTT1 of the enzyme glutathione S-transferase has been investigated and established, both individually and in combination to increase the frequency of undesirable adverse reactions when using drugs. However, the data obtained are ambiguous and contradictory. In this regard, we have presented an article aimed at studying the effect of polymorphic genes GSTM1 and GSTT1 on the activity of alanine aminotransferase and aspartate aminotransferase in blood serum in patients with newly diagnosed tuberculosis of the respiratory system. Preliminary results of our study showed that carrying a combination of deletion genotypes in the GSTM1 and GSTT1 genes statistically significantly increases the activity of ALT and AST in tuberculosis therapy in patients of Yakut nationality. An increase in ALT and AST levels in the blood indicates the likelihood of hepatocellular liver damage during anti-tuberculosis therapy in carriers of a combination of deletion genotypes (GSTM1(del)/GSTT1(del)) of the enzyme glutathione-S transferase.
An analysis of the main biochemical parameters of the population living in the territory with unfavorable natural radioactivity of 222Rn revealed changes in the biochemical spectrum of blood serum in the population of the Aldan region, indicating the presence of signs of disadaptation. Lipid profile changes are associated with gender and smoking. The shift of lipid metabolism towards atherogenicity is more pronounced in men due to an increase in the atherogenic fractions of lipids in the blood that are susceptible to peroxidation and a decrease in the level of the antiatherogenic fraction of lipids. The shift of metabolic flows towards catabolism, activation of glycolysis, dyslipidemia indicate the tension of carbohydrate and lipid metabolism, which is a sign of disadaptation of the body and the risk of developing environmentally conditioned diseases, including neoplasms.
On the basis of age-standardized and calculated per 100,000 population cervical cancer (CC) incidence and mortality rates, an analysis of the prevailing over 5 years, from 2016 to 2020, is presented. CC data analysis was conducted among nine state entities of Russia, whose territories are wholly or partially located in the Arctic. The calculations carried out using the Friedman's and Wilcoxon's tests, as well as the T-test showed that in these territories there is an unfavorable situation in relation to CC. In most state entities, the CC incidence and its dynamics, as well as the dynamics of CC mortality over five years from 2016 to 2020 show worse results than in Russia as a whole. The most vulnerable to cervical cancer at the present time is the population of Krasnoyarski Krai, Nenetski and Chukotski Autonomous Okrugs. Therefore, these territories especially require the use of new and strengthening of existing measures aimed at the CC prevention, early detection and improvement of treatment.
A pilot single-stage study of residents of Yakutsk, attached to the YSC CMP Clinic, with a history of new coronavirus infection COVID-19 in the period from March to December 2020 (1st and 2nd waves of the pandemic) was conducted. 2 groups of 80 people from 30 to 59 years old were formed, which were divided according to the time of infection with COVID-19. The 1st group included patients with a postcovid period from 3 to 6 months, in the 2nd - from 7 to 11 months. The analysis revealed a high frequency of cardiometabolic disorders, in particular hypertension, obesity and metabolic syndrome. Men had a more severe course of infection, both in the first and in the second wave of the pandemic. The relationship between blood pressure, body mass index, waist circumference and blood triglyceride levels with the severity of the disease is presented.
The article presents the dynamics of the main medical and demographic indicators and features of mortality in the Arctic zone of the Republic of Sakha (Yakutia). For the period 2000-2019 there is a tendency to reduce overall mortality, including from diseases of the circulatory system and external causes, against the background of continuing birth rates. The mortality rate from external causes was 1.9 times higher than the average for the Russian Federation, and 1.6 times higher than the average for the Republic of Sakha (Yakutia). The infant mortality rate in the Arctic zone was also higher than the average for the republic and the Russian Federation as a whole. High rates of infant mortality and mortality from external causes in the Arctic zone testify to the unfavorable socio-economic situation in the region.
A comprehensive survey of the working population of the Aldan district of the Republic of Sakha (Yakutia) was conducted. A total of 175 people of working age of non-indigenous nationality were examined. A high frequency of pathology on the part of the digestive, cardiovascular and endocrine systems, including the first identified, was revealed. Overweight and obesity had a high incidence, being a risk factor for the development of cardiovascular diseases. Every 10 respondents have registered oncopathology, 3 participants of the study were sent for further examination with suspicion of malignant neoplasms. The role of primary health care, including shop doctors, is of great importance in the early detection and prevention of risk factors for the most common chronic non-communicable diseases and timely effective medical care, as well as health schools for patients with hypertension, coronary heart disease, diabetes mellitus, gastritis, cancer, etc.
A study was conducted on 161 citizens of Yakutsk who had a new coronavirus infection in the period from March to December 2020. A high incidence of arterial hypertension, obesity, and lipid-metabolic disorders was revealed. The relationship of lipids, glucose and uric acid in blood serum, arterial hypertension, waist circumference with the severity of the new coronavirus infection was shown. The high prevalence of cardiovascular pathology and its risk factors among study participants requires long-term follow-up of patients with post-COVID syndrome, which prevents premature death from diseases of the circulatory system.
Tuberculosis is one of the most common diseases worldwide. The pharmacokinetics of isoniazid are unpredictable and vary widely in patients taking standard doses of the drug. In order to find the optimal dose of isoniazid, therapeutic drug monitoring should be carried out, determining the equilibrium concentration of the drug in blood plasma by HPLC. The aim of this study is to determine the concentration of isoniazid in blood plasma by HPLC and calculate the pharmacokinetic parameters of isoniazid in patients with newly diagnosed pulmonary tuberculosis in the Republic of Sakha (Yakutia). We determined the equilibrium concentration of isoniazid in blood plasma by high-performance liquid chromatography in 33 patients with pulmonary tuberculosis, using a Milichrome a-02 device manufactured by CI "Econova" LLC (Russia) using a ProntoSIL 120-3-C18 AQ column. The results obtained confirm the high individual variability of pharmacokinetic parameters: the maximum equilibrium concentration of the drug, the area under the pharmacokinetic curve, the average stationary concentration of the drug, the degree of fluctuation in the concentration of isoniazid in the blood plasma of patients with tuberculosis. Individual differences in the pharmacokinetic parameters of isoniazid indicate the need for therapeutic drug monitoring when prescribing isoniazid.
The cytological study of the cervix was carried out in women aged 18 to 44 years. It was found that the higher rate of background and precancerous diseases of the cervix is noted in women aged 30-44 years. Early diagnosis and treatment of underlying and precancerous diseases of the cervix will contribute to contribute to improving the early diagnosis, monitoring, and prognosis of CU cancer.
To identify the territories of the Arctic in which urgent measures are needed for the cervical cancer prevention, including vaccination, a comparative analysis of cervical cancer incidence (CCI) was carried out among Arkhangelskaya and Murmanskaya Oblasts, republics Karelia, Komi, Sakha (Yakutia), Krasnoyarski Krai, Chukotski, Yamalo-Nenetski and Nenetski Autonomous Okrugs and Russia as a whole. The annual rates of CCI per 100 thousand populations, standardized by age, were analyzed. Multiple and paired rank analyzes, as well as the calculation of annual changes from baseline in CCI, showed that cervical cancer prevention is an important task in these state entities, and the population of the Nenetski Autonomous Okrug, which is characterized by a significant proportion of indigenous people and ethnic minorities, needs it most urgently.
The aim of the study was to study the ethnic and age characteristics of emotional disorders in the elderly and senile people of Yakutia suffering from chronic cerebral ischemia. The study included 522 patients with chronic cerebral ischemia aged 60 to 89 years, who were divided into three groups by ethnicity (Evens, Yakuts and Russians) and into two age groups (elderly and senile age). Ethnic and age-related features of emotional disorders in persons suffering from chronic cerebral ischemia were revealed, while many patients had a combination of both anxiety and depressive symptoms. With the aggravation of the stage of chronic cerebral ischemia, more pronounced symptoms of anxiety disorders were noted. Emotional disorders prevailed in elderly and senile Russians, less pronounced in elderly Evens. There were no gender differences among them.
The aim of case-control study was to assess the possible association between angiotensin II type 1 receptor (A1166C) gene polymorphism and essential hypertension in the indigenous population of Yakutia (Yakuts-56, Chukchi-34, Yukagirs-77, Evens-184). 168 subjects with the essential hypertension (cases) were compared to 179 normotensive subjects (controls). The frequency of the C allele was 17.9 and 19.4% among cases and controls, respectively (p = 0.600). No association was found between the angiotensin II type 1 receptor (A1166C) gene polymorphism and essential hypertension in the study population.
Every day, researchers are learning more and more about the SARS-CoV-2 virus, the epidemiology of coronavirus infection and its pathogenesis. The search for medicines for the prevention and treatment of this disease is constantly underway. However, to date, there are no specific antiviral drugs against SARS-CoV-2. Nevertheless, recent studies have identified many weaknesses in the life cycle of the virus, allowing the creation of effective targeted drugs. In the review, we have summarized the accumulated scientific experience of many researchers engaged in the search for plant-derived metabolites with potential action against SARS-CoV-2. Unlike synthetic drugs, herbal antivirals do not require labor-intensive pharmaceutical synthesis and are more affordable and relatively safe. In this paper, a search for promising natural compounds as possible means of prevention and treatment of the novel coronavirus infection caused by the SARS-CoV-2 virus was conducted.
The results of the analysis of statistical connection between annual (from 2007 to 2018) values of age- standardized death rates (ASD) from cervical cancer (CC) and demographic indicators - number and density of population, location of population on the "city-rural" principle, among the population of the republics -Altai, Buryatia, Tyva, Khakassia and Sakha (Yakutia) are presented. Positive correlation between the ASDs due to the CC and the indicator of the share of rural settlements in the republics has been revealed (correlation coefficient Spearman (r) = 0.7, p <= 0.05). With the other demographic indicators selected for the study, the correlation has not reached the required strength and reliability (r<0.7, p>0.05).
A case-control study was conducted in Kolymskoye and Andryushkino of Nizhnekolymsky district of Yakutia to determine the association of IL-6 gene polymorphism with arterial hypertension (AH) and risk factors. The relationship with AH and its risk factors for IL-6 gene polymorphism was obtained, namely, the influence of the G allele of the polymorphic locus C-174G and the C allele of the polymorphic locus C-572G on metabolic factors of atherogenesis. Hypertensive patients-carriers of the G allele of the C-174G polymorphic locus of the IL-6 gene, both in the homozygous and heterozygous versions, showed the highest average systolic blood pressure, a high incidence of abdominal obesity, lipid and carbohydrate metabolism disorders. The study of the polymorphic locus C-572G of the IL-6 gene in patients with hypertension showed the highest incidence of abdominal obesity, lipid and carbohydrate metabolism disorders in carriers of the C allele.
This paper presents the results of a cytological study of patients with pathology of the cervix uteri (CU) based on traditional cytology (TC) and on liquid-based cytology (LBC). It was found that the combined use of liquid-based cytology and conventional cytology can improve the effectiveness of CU pathology diagnostics. It is recommended to complete the cytological study with testing for human papillomavirus (HPV). The use of these screening methods (cytological and HPV tests) contribute to improving the early diagnosis, monitoring, and prognosis of CU cancer.
A study of the association of certain biochemical and immunological parameters with abdominal obesity and arterial hypertension was carried in indigenous peoples of the North of the Arctic territory of Yakutia. The data obtained show that hypertensive patients have a high frequency of abdominal obesity, high cholesterol due to the atherogenic fraction. A reliable relationship was obtained between the levels of glucose and lipids with waist size and systolic blood pressure. Significant correlation of pro-inflammatory cytokine IL-6 with abdominal obesity in hypertensive patients was revealed.
