Le défaut de la sécrétion d'insuline, suite à la fibrose pancréatique, joue un rôle majeur dans la survenue des anomalies de la tolérance au glucose chez les patients atteints de M V. Le rôle de la sensibilité à l'insuline demeure débattu. L'objectif est d'investiguer les rôles respectifs de la diminution de la sécrétion d'insuline et de la sensibilité à l'insuline, dans la survenue des anomalies de la tolérance au glucose, chez les patients atteints de M V. 152 patients ont participé à deux tests d'hyperglycémie provoquée par voie orale (HGPO) à 21,2 ± 5,5 mois d'intervalle. La fonction pulmonaire et les mesures anthropométriques étaient mesurées le même jour. Les patients ont été classifiés selon leur tolérance au glucose : tolérance normale au glucose, intolérance au glucose ou diabète associé à la M V. La transition d'un statut de tolérance au glucose à un autre, de l'inclusion au suivi, a été étudiée. Au suivi, 55,5 % des patients ont maintenu leur tolérance au glucose stable (groupe A), 15,3 % ont amélioré leur tolérance au glucose (groupe B), alors que 30,3 % l'ont détériorée (groupe C). Pour les trois groupes, la sécrétion d'insuline est demeurée stable au suivi. La sensibilité à l'insuline est demeurée stable chez les patients du groupe A, elle s'est améliorée chez les patients du groupe B et s'est détériorée chez les patients du groupe C (p = 0,079, p = 0,003 et p < 0,001, respectivement). Les patients du groupe C présentaient également une diminution de la fonction pulmonaire (p = 0,001). Les patients atteints de MV présentent une grande variabilité de la tolérance au glucose. Dans un contexte de défaut de sécrétion d'insuline, les variations de la sensibilité à l'insuline sont associées aux fluctuations de la tolérance au glucose.
Malnutrition in cystic fibrosis (CF) is associated with increased mortality and can lead to fat-free (FFM) and fat mass (FM) loss. Dual-energy X-ray absorptiometry (DXA) is used and validated to measure FFM and FM. DXA's high cost has led to the utilization of less costly techniques such as bioelectrical impedance analysis (BIA). The aim of this study was to determine the agreement of FFM, FM and %FM measurements taken with DXA and BIA in adults with CF. We measured FFM, FM and %FM in 34 adults with CF with a leg-to-leg BIA and an iDXA and determined agreement using Bland-Altman analysis. While DXA and BIA measurements were well correlated (r > 0.8), mean biases between both methods were between 8 and 11%. BIA underestimated FM and %FM and overestimated FFM. In a clinical research setting where these measurements are used to phenotype patients, BIA cannot replace DXA.
Cystic fibrosis-related diabetes (CFRD) is the most frequent complication of cystic fibrosis (CF) and associated with increased mortality. Why patients have an accelerated loss of lung function before the diagnosis of CFRD remains poorly understood. We reported that patients with or without CFRD had increased glucose excursions when compared to healthy peers. Studies have demonstrated that patients with CF have increased glucose fluctuations and hyperglycemia and that this may affect the clinical course of CF and lead to lymphocyte dysfunction. T-helper 17 (Th17) lymphocytes produce and secrete the pro-inflammatory cytokine IL-17. The Th17 pathway is involved in CF lung inflammation, β-cell destruction in type 1 diabetes (T1D) and Th17 cells of patients with type 2 diabetes have increased production of IL-17 when compared to healthy peers. Also, regulatory T-cells (Tregs) have been shown to be dysfunctional and produce IL-17 in T1D. Furthermore, vitamin D can affect inflammation in CF, diabetes and the differentiation of lymphocytes. In this review, we discuss the potential roles of hyperglycemia on Th17 cells, Tregs and IL-17 as a potential cause for accelerated lung function decline before CFRD and how this could be modulated by vitamin D or by directly intervening in the IL-17A pathway.
Studies have demonstrated overproduction of prostaglandin E2 (PGE 2 ) in CF.In vitro study show that alterations in CFTR are involved in overproduction of PGE 2 .Findings that suggest that measurement of PGE 2 production may help to assess the severity of the altered regulation of CFTR and intensity of inflammatory process in the airways of CF patients.Tetranor PGEM (PGE-M) is a metabolite present in urine that accurately reflects the biosynthesis of the PGE 2 .In this study we assessed the relationship between levels of PGE-M with CF severity.Methods: 36 patients with stable CF and 24 healthy controls were recruited.All CF patients had undergone evaluation of pancreatic function, lung function by spirometry and genotype severity.PGE-M levels were measured by a liquid chromatographic/mass spectrometric assay.Results: PGE-M concentrations were markedly and significantly elevated (p < 0.0001) in the urine of CF patients (34.5±45 ng/mgCr) compared with healthy controls (7.4±4.25 ng/mgCr).There was no correlation between PGE-M urinary levels and spirometric values.In patients with pancreatic insufficiency there was a higher PGE-M urinary level (38±7 ng/mgCr) than in patients with conserved pancreatic function (10±4.6 ng/mgCr) (p = 0.05).There was an association of the severity of genotypes with levels of PGE-M: mild: 8.6±4.1 ng/mgCr; moderate: 19.7±11 ng/mgCr; severe: 48±53.3ng/mgCr (p < 0.0027).Conclusions: Measurement of urinary PGE-M may be a useful tool to investigate severity of CF and to assess the efficacy of any therapy aimed to improve the function of a defective CFTR.175 The onset of CFRD does not impact the association of IL-6 with worse clinical status
Introduction: Cystic fibrosis (CF) is the most common lethal autosomal recessive disease among Caucasians, affecting one in 3600 births in Canada. CF is known as a disease of thinness due to malabsorption and malnutrition, and respiratory failure is the principal cause of CF mortality. The main objectives were to assess CF body composition and to investigate its association with pulmonary function.
Cystic fibrosis (CF)-related diabetes is an important complication of CF caused by a decrease in insulin secretion that is associated with weight loss, poor nutritional status and increased mortality. Leptin, a hormone secreted from white adipose tissue, has an important role in energy homoeostasis by inhibiting food intake and increasing energy expenditure. Leptin secretion can be increased by nutrient signals such as insulin.Aims. - Considering that leptin plays a role in energy homoeostasis and that CF is associated to poor weight gain and decreased insulin secretion, leptin levels in CF patients with different glucose tolerances were investigated and compared with those of healthy control subjects.Methods. Two-hour oral glucose tolerance tests (OGTT) were performed in 82 patients with CF and various glucose tolerances as well as in 17 healthy control subjects during which blood was withdrawn every 30 min to measure glucose and insulin. Fasting leptin, fibrinogen and fat mass were also measured, and body mass index (kg/m(2)) calculated for all participants. Early and late insulin secretion was separated by calculating the area under the curve from time 0 to 30 min and 30 to 120 min of the OGTT (AUC(0-30) and AUC(30-120)).Results. - Leptin levels were comparable between CF patients and healthy control subjects. Interestingly, correlations were observed between leptin levels and insulin (AUC(0-120) and AUC(30-120)) after adjusting for gender and fat mass (P < 0.05).Conclusion. - This study suggests a potential role of insulin in regulating leptin levels in adults with stable CF. Cystic fibrosis related diabetes (CFRD) is an important complication of CF that increases mortality. Adiponectin, an adipokine secreted from adipose tissue, plays an important role in fatty acid and glucose metabolism. Lower total adiponectin (TA) levels have been linked to the risk of developing type 2 diabetes. However, studies show that the high molecular weight isoform (HMW), thought to be more active than TA, might be a better indicator of insulin sensitivity. Our aim was to determine the association between HMW and insulin sensitivity in CF subjects and determine if other factors might modulate its levels. Thirteen control subjects and 47 CF adults (16 with normal glucose tolerance, 16 prediabetic and 15 with CFRD) underwent an oral glucose tolerance test. Blood samples were taken at time 0, 30, 60, 90 and 120min. Body mass index, fibrinogen, glucose and insulin, TA and HMW were measured in every subject. Regression analysis was used to determine the association between TA, HMW and glucose (fasting glucose, 2h glucose and glucose AUC) as well as insulin (fasting insulin, insulin AUC, and Stumvoll insulin sensitivity index) parameters. TA and HMW levels were similar between CF patients and controls and were not associated to insulin sensitivity. TA was negatively associated to insulin AUC (p=0.0108) and 2h glucose (p=0.0116) in controls while these relationships were either weakly negative (p=0.0208) or weakly positive (p=0.0105) in CF patients. Also, HMW was negatively associated to insulin (p=0.00301) and glucose AUC (p=0.0546) in controls whereas these associations were positive in CF patients (p=0.0388, p=0.0232 respectively). In conclusion, our exploratory study on HMW adiponectin demonstrated similar levels of TA and HMW between CF patients and controls and different relationships between forms of adiponectin to glucose metabolism and insulin in CF.

Introduction Contrairement a la population generale, les personnes atteintes de mucoviscidose (MV) ont, depuis leur plus jeune âge, pour principale recommandation nutritionnelle une diete riche en gras et en calories. Ceci est principalement du au fait que, malgre la prise importante d’enzymes pancreatiques (pres de 80 %), une malabsorption intestinale persiste. Les principaux objectifs de cette etude sont de caracteriser les profils lipidiques, d’analyser les causes et les complications eventuelles de ces anomalies chez des patients MV ainsi que mettre en evidence une eventuelle association avec les anomalies de la tolerance au glucose. Patients et methodes 204 sujets MV et 15 sujets non-MV ont ete inclus dans cette etude. Tous les sujets ont subi une hyperglycemie provoquee oralement (HGPO) afin de determiner leur tolerance au glucose. Les profils lipidiques incluant entre autres cholesterol total, HDL, LDL et triglycerides ont ete mesures a jeun. Une balance electronique a ete utilisee pour determiner la masse grasse, la masse maigre ainsi que l’indice de masse corporelle (IMC). Une hypertriglyceridemie (HPTG) a ete retrouvee chez 20 patients MV qui ont par la suite ete apparies avec 20 patients MV non-HPTG. Les criteres d’appariement etaient : sexe, âge, insuffisance pancreatique, genotype et statut de tolerance au glucose. Resultats La comparaison des trois groupes (MV-HPTG, MV-non HPTG et non-MV) a mis en evidence des differences sur la secretion et la resistance a l’insuline. Pour un statut glycemique comparable, les patients MV-HPTG avaient une sensibilite a l’insuline moindre entrainant une plus forte secretion d’insuline. L’IMC, la tension arterielle, l’inflammation et la capacite pulmonaire ne semblent pas differer entre les groupes MV-HPTG et MV-non HPTG. Les femmes ont une tendance a etre plus souvent atteintes d’HPTG que les hommes. Discussion L’HPTG chez les patients MV semble etre un phenomene isole et ne semble pas avoir les complications rencontrees dans la population generale.

This measure correlates significantly with early insulin secretion quantified with the IVGTT (AUC0–10) (r=0.59, p=0.021 for healthy controls subjects, r=0.50, p=0.041 for CF subjects). Therefore, this easily calculated value (AUC0–30) obtained during the OGTT can be used as a proxy of insulin secretion in CF patients. Its accuracy at various stages of the disease should however be confirmed in futures studies.
La mucoviscidose (MV) est la plus fréquente maladie létale autosomique récessive chez les Caucasiens et est due à une mutation du gène CFTR. La mutation principale est une délétion d'un acide aminé en position 508 (F508). L'espérance de vie des patients MV s'est beaucoup améliorée et conséquememment de nouvelles complications telles que le diabète lié à la MV ont vu le jour (CFRD). Des études ont montré que les femmes et les sujets homozygotes pour la mutation ΔF508 ont un plus grand risque de développer le CFRD. Cependant, très peu d'études ont examiné l'effet du genre et du génotype sur la sécrétion d'insuline. Le but principal de cette étude était donc de comparer la sécrétion d'insuline entre les hommes et les femmes avec des génotypes différents. 196 sujets adultes non diabétiques ont été recrutés dans la clinique de MV du CHUM. Leurs génotypes ont été extraits de leurs dossiers médicaux. Tous les sujets ont subi une hyperglycémie provoquée oralement (HGPO) afin de déterminer leur tolérance au glucose : tolérance normale (NGT), intolérance (IGT) ou diabète. Des échantillons de sang ont été prélevés toutes les 30 min pendant 2 h pour mesurer les niveaux d'insuline. La sécrétion d'insuline et la sensibilité ont été évaluées à l'aide d'indices connus tels que le Stumvoll. À excursion glycémique comparable, les valeurs d'insuline varie entre les génotypes et entre les genres : les hétérozygotes et les femmes ont une sécrétion d'insuline plus élevée que les homozygotes et les hommes. Le suivi prospectif sur 2 ans indique que cette insulinémie élevée semble protéger de la dégradation de l'état glycémique des patients. Dans un groupe important de patients MV, sans diagnostic antérieur de diabète, on observe une association entre une sécrétion d'insuline plus élevée et l'état hétérozygote pour la mutation F508 et/ou le genre.
The aim of the study was to examine the association between total adiponectin and high molecular weight (HMW) adiponectin levels with cardio-metabolic risk factors in a population of sedentary, overweight, and obese postmenopausal women. Cross-sectional study was carried out on 55 nondiabetic sedentary overweight and obese postmenopausal women aged between 50 and 70 years. Insulin sensitivity was assessed by euglycemic-hyperinsulinemic clamp technique. Body composition and visceral fat were measured using dual X-ray absorptiometry and computed tomography, respectively. Other cardio-metabolic risk factors included: plasma lipids, hsC-reactive protein, energy expenditure (doubly labeled water), peak oxygen consumption, muscle strength (using weight training equipment) as well as total and HMW adiponectin. Correlations of total and HMW adiponectin with various cardio-metabolic risk factors were comparable. In addition, regression analysis results showed similar independent predictors of total and HMW adiponectin. Finally, the receiver operator characteristic (ROC) curves for total and HMW adiponectin to predict insulin sensitivity showed no difference between the areas under curve (AUC) (AUC total adiponectin=0.80 [95% CI: 0.66-0.95] versus AUC HMW adiponectin=0.76 [95% CI: 0.60-0.91], p=0.36). The present study indicates that HMW adiponectin does not seem to provide additional information than total adiponectin in relation to cardio-metabolic risk factors in overweight/obese postmenopausal women.
La mucoviscidose est la maladie autosomique récessive la plus fréquemment retrouvée chez la population caucasienne. Le diabète secondaire à la mucoviscidose est une complication importante de cette maladie et augmente le taux de mortalité des patients. L'adiponectine, un polypeptide sécrété par les adipocytes, joue un rôle important dans le métabolisme des glucides et des lipides, dans le diabète de type 2 et augmente la sensibilité à l'insuline. L'adiponectine totale peut être clivée pour devenir une forme de haut poids moléculaire et des études suggèrent que le rapport de l'adiponectine de haut poids moléculaire et la forme non-clivée est un indicateur de la sensibilité à l'insuline chez les humains. L'objectif de cette étude est d'examiner la relation entre les concentrations plasmatiques d'adiponectine totale et de haut poids moléculaire ainsi que le rapport et la tolérance au glucose des adultes atteints de mucoviscidose afin d'explorer le rôle de l'adiponectine au niveau de la sensibilité à l'insuline. Une hyperglycémie provoquée par voie orale a été réalisée chez 44 patients atteints de mucoviscidose avec différentes tolérances au glucose (normotolérants, intolérants ou diabétiques) et 13 sujets témoins appariés pour le sexe, l'âge et l'indice de masse corporelle. Nous avons ensuite dosés les niveaux de glucose, d'insuline et d'adiponectine avec les échantillons plasmatiques. La sensibilité à l'insuline a été vérifiée à l'aide d'indices validés tels que le Stumvoll et le « homeostasis model assessment (HOMA) ». D'autres marqueurs biologiques tels que le fibrinogène et le cholestérol HDL ont aussi été examinés. Dans les analyses préliminaires, on observe une corrélation entre l'adiponectine de haut poids moléculaire et le métabolisme des glucides seulement chez les femmes atteintes de mucoviscidose. CES données suggèrent que les niveaux d'adiponectine totale et de haut poids moléculaire pourraient être régulées différemment selon les sexes.
