Interventional endoscopies entail a risk of infection secondary to perforation of the luminal wall. Thereby, bacteria may be introduced into the sterile environment of the peritoneal cavity (PC). Limited data are available regarding the efficacy of prophylactic anti-infective treatments. The aim of the study was to examine the efficacy/safety of anti-infective means in the prevention of infection by interventional endoscopies in a randomized controlled animal trial.
BACKGROUND AND STUDY AIMS:Radiofrequency ablation (RFA) is an accepted method of tissue destruction for solid organ tumors. Endoscopic ultrasound (EUS)-guided RFA has been used for lesions in the pancreas and liver, but there is limited experience of lymph node ablation using EUS-guided RFA. The aim of this study was to determine feasibility and safety of prototype EUS-guided RFA of mediastinal lymph nodes. METHODS:This was an endoscopic experimental feasibility study in a porcine model. After EUS-guided puncture of targeted lymph nodes, the stylet of a 19-G needle was replaced by a prototype RFA probe. RFA was performed by ERBE generator (bipolar settings: 10 watts, effect 2, 2 minutes). The animals were euthanized, and the targeted lymph nodes were identified and removed for histology and measurement of the effect achieved. RESULTS:A total of 18 mediastinal lymph nodes were ablated (mean size 20.8 ± 6.6 mm in the long axis). The average length of exposed probe was 10.0 ± 3.0 mm. The mean length and diameter of necrosis was 9.8 ± 3.6 mm and 5.5 ± 1.6 mm, respectively. Linear regression comparing needle length with necrosis diameter revealed a coefficient gradient of r = 0.92 (P = 0.0001). With EUS-RFA a mean of 17.6 ± 10.3 % (range 8.0 % - 53.2 %) of the respective lymph node area was ablated. No complications (i. e. hemodynamic instability, local bleeding, tissue damage) occurred during the procedure. Technical problems included stripping of the probe by the EUS needle and bending of the tip of the probe. CONCLUSIONS:EUS-RFA of lymph nodes was performed safely and successfully using a prototype EUS-compatible probe. This method may have the potential for future use in patient care.
Einleitung: Die endoskopische Ablation maligner Lymphknoten (LK) spielt bisher in der Behandlung maligner Erkrankungen des Thorax und Abdomens keine Rolle. Insgesamt ist die Radiofrequenzablation (RFA) eine etablierte Methode zur lokalen Destruktion solider Organtumoren, insbesondere hepatozellularer Karzinome. Untersuchungen zur Ablation von LK existieren bisher nicht. Ziel dieser Studie war es, die endosonographische (EUS) Ablation von LK mit einer neuartigen RFA-Sonde zu prufen. Methodik: Bei 6 weiblichen Schweine wurdes das Vorliegen von LK (mediastinal und paragastral) durch EUS gepruft. Bei LK-Nachweis erfolgte eine EUS-gesteuerte Punktion des Ziel-LK mit einer 19G-EUS-Nadel (Wilson Cook). Uber die liegende EUS-Nadel wurde die RFA-Sonde (EMcision) bis in das Zentrum des LK vorgebracht. Die RFA erfolgt mit einem Erbe-Generator (Erbe) in bipolarer Einstellung mit 10 Watt, Effekt 2 uber 2 Minuten. In der Autopsie wurden die entsprechenden LK aufgesucht und fur die weitere Aufarbeitung entfernt. Ergebnisse: Eine EUS-Darstellung adaquater Lymphknoten war bei 6/6 Tieren moglich, 8 LK abladiert, N=6 mediastinal; N=2 gastral. Die Gesamtlange der LK lag bei 21,1±6,5mm bei einer Lange der in die LK eingebrachten RFA-Sonde von 8,8±4,4mm. Innerhalb der LK zeigte sich nach Ablation eine Nekrosezone von 9,3±5,2mm Lange und 2,8±0,8mm Breite. Eine positive Korrelation bestand fur die Sondenlange und Nekroseausdehnung (Lange r=0,98; Breite r=0,93). Eine EUS-Darstellung von Teilen der RFA-Sonde war in allen Fallen moglich. Gewebeeffekte liesen sich bei 3/6 Tieren nachweisen. Unmittelbare Komplikationen durch Punktion oder Ablation traten nicht auf. Technische Probleme zeigten sich durch ein Verbiegen der Sonde innerhalb des Lymphknotens und Schwierigkeiten beim Entfernen der Sonde uber die Nadel. Diskussion: Eine EUS-gesteuerte RFA von LK mittels einer Prototyp RFA-Sonde uber eine 19G-Punktionsnadel ist am Tiermodell ein sicheres und zuverlassiges Verfahren. Es besteht eine direkte Korrelation zwischen Sondenlange und der erreichten Gewebedestruktion. Im Rahmen weiterer Studien sollte die Effektivitat des Verfahrens und die Einstellung der Sonde weiter gepruft werden. Eine langfristige Sicherheit muss in Uberlebensstudien gezeigt werden.
Einleitung: Bei vielen NOTES-Eingriffen wird die Peritonealhöhle (PH) über einen transgastralen (tg) Zugang erreicht. Durch die Passage des Endoskops über die Mundhöhle kann eine Vielzahl von Erregern in die sterile PH verschleppt werden. Daher stellen Infektionen einen wesentlichen Risikofaktor dieser Prozeduren dar. Die Datenlage zur Effektivität prophylaktischer Maßnahmen ist widersprüchlich. Ziel dieser Studie war es, die Effektivität verschiedener, antiinfektiver Maßnahmen bei tg NOTES-Eingriffen zu ermitteln.
Ablation of malignant lymphadenopathy does not currently play a role in the treatment algorithms of mediastinal, abdominal or pelvic malignancies. There is no data available regarding the benefit of ablation of malignant lymph nodes. Radiofrequency ablation (RFA) is a widely accepted method of tissue destruction for solid organ tumors. EUS-guided RFA of organs such as the pancreas and liver have been described. Data regarding RFA of lymph nodes has not been demonstrated. In many natural orifice translumenal endoscopic surgery (NOTES) interventions the peritoneal cavity (PC) is accessed through the gastrointestinal wall. While the endoscopes pass through the oral cavity and stomach they may carry some natural bacteria with them into the sterile envoironment of the PC. Thus, infections may represent a major risk of such procedures. Only limited and conflicting data are available regarding the efficacy of prophylactic anti-infective treatments.
