Introduction the identification and management of frailty occurs mostly in primary care. Several different models of care exist. This study aimed to assess the impact of a new General Practitioner (GP)-led modified Comprehensive Geriatric Assessment (CGA) on service delivery, healthcare utilisation and patient outcomes. Method patients with moderate-severe frailty (electronic Frailty Index score > 0.24) in Newbattle Medical Practice, Scotland, were eligible for a novel intervention (MidMed) in which an additional GP performed a modified CGA and was directly accessible for appointments. The recruits to the intervention (MidMed) group were compared with those waiting to be enrolled (non-MidMed). Outcomes included unscheduled hospital admissions, primary care consultations, continuity of care (Usual Provider of Care (UPC) index), outpatient attendances and mortality. Adjusted rate ratios (aRR), for MidMed compared to non-MidMed, were estimated using regression models adjusting for demographics and healthcare utilisation histories. Results 510 patients were included: 290 MidMed (mean(SD) age 80.1(7.6)years; 59.6% female) and 220 non-MidMed (75.4(8.6)years; 57.7% female). Median follow-up was 396 days. aRR(95%CI) was 0.46(0.30-0.71) for >1 admission, 0.62(0.41-0.95) >1 Emergency Department (ED) attendance and 1.52(1.30-1.75) for use of primary care, with no difference in outpatient appointments or mortality. Continuity of care was better for the MidMed group (MidMed UPC 0.77(SD 0.19), non-MidMed 0.41(0.18), P < 0.001). Conclusion this GP-led service for frail patients was associated with lower risk of hospital readmission/ED reattendance, greater use of primary care and improved continuity of care. More detailed evaluation of novel primary care frailty services, over longer time-periods, including robust randomised controlled trials, are needed.
AimTo assess associations between statin intensity and adherence, persistence and discontinuation of statin therapy in Scotland.MethodRetrospective cohort study, using linked electronic health records covering a period from January 2009 to December 2016. The study cohort included adult patients (≥18 years) newly initiating statins within Greater Glasgow and Clyde, Scotland. Study outcomes comprised adherence, discontinuation and persistence to treatment, stratified by three exposure groups (high, moderate and low intensity). Discontinuation and persistence were calculated using the refill‐gap and anniversary methods, respectively. Proportion of days covered (PDC) was used as a proxy for adherence. Kaplan‐Meier survival curves and Cox proportional hazard models were used to evaluate discontinuation, and associations between adherence/persistence and statin intensity were assessed using logistic regression.ResultsA total of 73 716 patients with a mean age of 61.4 ± 12.6 years were included; the majority (88.3%) received moderate intensity statins. Discontinuation rates differed between intensity levels, with high‐intensity patients less likely to discontinue treatment compared to those on moderate intensity (prior cardiovascular disease [CVD]: HR 0.43 [95% CI 0.34‐0.55]; no prior CVD: 0.80 [0.74‐0.86]). Persistence declined over time, and high‐intensity patients had the highest persistence rates. Overall, 52.6% of patients were adherent to treatment (PDC ≥ 80%), but adherence was considerably higher among high‐intensity patients (63.7%).ConclusionHigh‐intensity statins were associated with better persistence and adherence to treatment, but overall long‐term persistence and adherence remain a challenge, particularly among patients without prior CVD. This needs addressing.
Background with rationale National Therapeutic Indicators (NTI) in Scotland use national administrative data to identify variation and drive quality improvement in GP practice prescribing. Twelve indicators were developed in 2011 and in 2012 practices were offered financial incentives to review their prescribing in two where there was greatest potential for improvement. Main AimTo quantify the impact of 2012 NTI alone and in combination with financial incentives. Methods/ApproachSix NTI were selected for analysis, aiming for heterogeneity in terms of whether the focus was quality of care (clinical) or cost effectiveness (cost). Impact was evaluated using segmented linear regression of time-series prescribing data, and changes in prescribing at 12 and 24 months after NTI introduction were estimated. Other national interventions expected to influence GP prescribing were also modelled. ResultsOne selected NTI could not be modelled because of non-linear trends in the baseline period. In practices not financially incentivised to review prescribing, improvements were variable. At 12 months relative changes were dipyridamole −31.9% (95%CI; −43.1 to −20.7), quinine −3.6% (95%CI; −7.2 to −0.1), non-preferred statins 6.2% (95%CI; 1.0 to 11.5), hypnotics and anxiolytics −2.9% (95%CI; −7.3 to 1.6) and high strength inhaled corticosteroids −1.0% (95%CI; −1.7 to −0.3). In comparison financially incentivising practices resulted in larger improvements and improvements in all NTIs modelled. At 12 months relative changes were dipyridamole −65.3% (95%CI; −68.4 to −62.2), quinine −52.3% (95%CI; −54.7 to −49.9), non-preferred statins −17.8% (95%CI; −21.9 to −13.6), hypnotics and anxiolytics −9.6% (95%CI; −12.6 to −6.7) and high strength inhaled corticosteroids −6.0% (95%CI; −7.3 to −4.8). ConclusionGP prescribing can be improved through feedback of national administrative data prescribing indicators. Impact on prescribing is larger when financial incentives to focus on particular areas of prescribing are additionally used.

Materials & methods: The prescription costs analysis database was used to document utilization and expenditure on SSRIs between 2001 and 2017 alongside documenting the initiatives. Results: Multiple interventions over the years increased international nonproprietary name prescribing up to 99.9% lowering overall costs. This, coupled with initiatives to limit escitalopram prescribing due to concerns with its value, resulted in a 73.7% reduction in SSRI expenditure between 2001 and 2017 despite a 2.34-fold increase in utilization. Safety warnings resulted in a significant reduction in the prescribing of paroxetine, citalopram and escitalopram alongside a significant increase in sertraline Conclusion: Multiple initiatives have increased the quality and efficiency of SSRI prescribing in Scotland providing direction to others.

Increasing prevalence of asthma and COPD has increased use of long acting inhalers. Different inhalers with patented delivery systems have kept costs high and inhalers among the highest expenditure items in ambulatory care in Scotland. IntroductionThe growing prevalence of non-communicable diseases, combined with greater recognition of the effectiveness of lipid lowering agents (LLAs), has fuelled their increasing use in recent years. Similarly, increasing recognition of mental health and, arguably, societal expectations and pressures, has driven appreciable growth in antidepressant prescribing in recent years. Concurrent with this, growing resource pressures enhanced by the continual launch of new premium priced medicines necessitates reforms and initiatives within finite budgets. Scotland has introduced multiple measures in recent years to improve both the quality and efficiency of prescribing. There is a need to document these initiatives and outcomes to provide future direction.MethodsAssessment of the utilization (items dispensed) and expenditure of key LLAs (mainly statins) and SSRIs between 2001 and 2017 in Scotland alongside initiatives.ResultsMultiple interventions have increased international non-proprietary name (INN) prescribing (99% for statins and up to 99.9% for SSRIs). They have also increased preferential prescribing of generic versus patented statins with low costs for generics, reduced inappropriate prescribing of ezetimibe due to effectiveness concerns, and increased the prescribing of higher dose statins (71% in 2015). These measures have resulted in a 50% reduction in LLA expenditure between 2001 and 2015 despite a 412% increase in utilization. Initiatives to reduce the prescribing of escitalopram as lack of evidence demonstrating cost-benefits over generic citalopram, along with high INN prescribing, achieved a 73.7% reduction in SSRI expenditure between 2001 and 2017 despite a 2.34-fold increase in utilisation. Concerns with paroxetine, and more recently citalopram and escitalopram following safety warnings, resulted in a considerable reduction in their use alongside a significant increase in sertraline.ConclusionsGeneric availability coupled with multiple measures has resulted in appreciable shifts in statin and SSRI prescribing behavior and reduced ezetimibe prescribing, resulting in improvements in both the quality and efficiency of prescribing to provide future direction.
Background: There is an increasing prevalence of asthma and chronic obstructive pulmonary disease (COPD) worldwide, leading to increased inhaler use. However, there are concerns with inhaler compliance resulting in different patented inhalers and longer-acting formulations. As a result, inhalers are now among the highest expenditure items in ambulatory care in Scotland leading to multiple initiatives to keep within budget without compromising care. Method: This study assesses inhaler utilization and expenditure between 2001 and 2017 alongside health authority initiatives. Results: There was an increase by 137% in inhaler utilization between 2001 and 2017, and a two-fold increase in expenditure, driven by the increasing use of patented combination inhalers to address concerns. This is very different to the oral markets where expenditure on proton pump inhibitors, statins, and antihypertensives have fallen considerably recently despite increased volumes due to the increasing use of low-cost generics. However, inhaler expenditure has started to fall with an increasing use of lower cost combinations and initiatives to reduce the steroid burden alongside monitoring patient care. Conclusion: Challenges with using and changing inhalers has meant this market has not followed other high-volume drug classes following patent loss. This is starting to change, with the situation monitored to enhance efficient prescribing alongside continued good quality care.
Background: Changing demographics with an increase in chronic disease prevalence coupled with more aggressive management targets, and the continued launch of new premium priced products, has put considerable strain on healthcare systems to continue providing universal, high quality healthcare. High appropriate use of generics in a class where care is not compromised by prescribing generics versus patented products, combined with quality measures, can help with providing quality healthcare. Objective: Assess the utilisation and costs of lipid lowering therapies (C10), Proton-Pump Inhibitors (PPIs) (A02BC), and Selective Serotonin Reuptake Inhibitors (SSRIs) (N06AB) in ambulatory care in Scotland between 2001 and 2015 alongside reforms to provide future guidance. Methods: Utilisation measured in the number of items dispensed to assess prescriptions and reimbursed expenditure captured in GB pounds from NHS Scotland national database. National and regional initiatives to improve the quality and efficiency of prescribing were captured using standard methodologies (4Es - education, engineering, economics and enforcement). No time series analyses analysis was conducted as multiple interventions over time. Results: Lipid lowering therapies (statins predominantly): multiple initiatives including identifying patients with CHD, encouraging preferential prescribing of generics, reducing ezetimibe prescribing as little evidence of benefit, and encouraging the prescribing of higher dose statins in line with recommendations, resulted in statin expenditure falling by 56% in 2015 vs. 2001 despite a 4.03 fold increase in utilisation. High dose statins now account for 71.3% of total prescriptions, up from 17.3% in 2001. PPIs: similar multiple initiatives resulted in a 68% reduction in expenditure despite a 2.91 fold increase in utilisation. High dose prescribing reduced to 20.3% of all PPIs in 2015, down from 25.8% in 2009, following concerns. SSRIs: similar initiatives and encouraging citalopram vs. escitalopram resulted in expenditure falling by 60.1% despite a 2.12 fold increase in utilisation. Concerns with citalopram in 2012 saw its prescribing fall in recent years whilst the prescribing of sertraline has grown. The prescribing of paroxetine continually fell from 2002 onwards as a result of concerns. Conclusion: Multiple strategies have been successful with improving the quality and efficiency of prescribing in Scotland. These will continue.

This resulted in a 50% reduction in expenditure of LLAs between 2001 and 2015 despite a 412% increase in utilization, increased prescribing of higher dose statins (71% in 2015) especially atorvastatin following generic availability, and reduced prescribing of ezetimibe (reduced by 72% between 2010 and 2015). As a result, the quality of prescribing has improved.CONCLUSION:Generic availability coupled with multiple measures has resulted in appreciable shifts in statin prescribing behavior and reduced ezetimibe prescribing, resulting in improvements in both the quality and efficiency of prescribing.
Introduction:There has been a considerable increase in the use of proton pump inhibitors (PPIs) in recent years due to their effectiveness versus H2 antagonists. This includes reducing GI bleeds in patients at risk. However, there are concerns with their long-term use and potential costs. Costs can be reduced with increased prescribing of low-cost generic PPIs. Aims: To analyse the influence of multiple demand-side measures in Scotland in recent years to increase the prescribing of low-cost generic PPIs as well as encourage the prescribing of lower strength PPIs. Methods: Documenting utilization (mainly items dispensed) and expenditure in Scotland from 2001 to 2017 using health authority databases combined with documenting the multiple initiatives and measures both nationally and regionally. Results: The multiple measures in Scotland ensured high International Nonproprietary Name (INN) prescribing (up to 100% for some PPIs) as well as the prescribing of generic versus patented PPIs, with costs of generic PPIs as low as 8.5% of their pre-patent loss prices. Overall, total expenditure on PPIs in Scotland was 66.7% lower in 2017 at GBP 18.83 million compared to 2001 levels. This was despite a three-fold increase in PPI utilization during this period. The savings were driven by the increasing use of generic omeprazole and lansoprazole versus patent protected PPIs. There was also a reduction in the prescribing of high strength PPIs during this period. Conclusion: Multiple initiatives in Scotland in recent years have reduced expenditure on PPIs despite appreciably increased utilization. Multiple initiatives have also helped to reduce the prescribing of higher strength PPIs. This is an exemplar to other countries seeking to enhance their prescribing efficiency.

In 2008, the Scottish Management of Antimicrobial Resistance Action Plan (ScotMARAP) was published by the Scottish Government. One of the key actions was initiation of the Scottish Antimicrobial Prescribing Group (SAPG), hosted within the Scottish Medicines Consortium, to take forward national implementation of the key recommendations of this action plan. The primary objective of SAPG is to co-ordinate and deliver a national framework or programme of work for antimicrobial stewardship. This programme, led by SAPG, is delivered by NHS National Services Scotland (Health Protection Scotland and Information Services Division), NHS Quality Improvement Scotland, and NHS National Education Scotland as well as NHS board Antimicrobial Management Teams. Between 2008 and 2010, SAPG has achieved a number of early successes, which are the subject of this review: (i) through measures to optimise prescribing in hospital and primary care, combined with infection prevention measures, SAPG has contributed significantly to reducing Clostridium difficile infection rates in Scotland; (ii) there has been engagement of all key stakeholders at local and national levels to ensure an integrated approach to antimicrobial stewardship within the wider healthcare-associated infection agenda; (iii) development and implementation of data management systems to support quality improvement; (iv) development of training materials on antimicrobial stewardship for healthcare professionals; and (v) improving clinical management of infections (e.g. community-acquired pneumonia) through quality improvement methodology. The early successes achieved by SAPG demonstrate that this delivery model is effective and provides the leadership and focus required to implement antimicrobial stewardship to improve antimicrobial prescribing and infection management across NHS Scotland.
