OBJECTIVE:To propose a comprehensive set of competencies and educational objectives for communication and social competencies in undergraduate medical education and to support the nationwide implementation of these issues in all medical schools. METHODS:Thirty experts from different medical and psychosocial disciplines participated in a 2-day workshop using the Nominal Group Technique (NGT) to develop an initial set of educational objectives. These were refined, structured, and rated according to their importance by means of a two-step Delphi Survey involving additional experts in medical education. RESULTS:The initial workshop resulted in 188 educational objectives assigned to 26 different topics. After the Delphi Survey, 131 objectives remained, assigned to 19 different topics. Some objectives that could be assigned to more than one topic were subsumed under a new more general category. CONCLUSION:The described consensus process proved successful as one method to develop a set of educational objectives. PRACTICAL IMPLICATIONS:The Basel consensus statement can be used to orientate curriculum reform and development in medical education.
[english] Aim: The aim of the project is to present and discuss a longitudinal model curriculum “Communication and social competencies” for undergraduate medical education. Procedure and results: In a two-day workshop, a multidisciplinary, cross-faculty group of medical educators developed a curriculum model based on the “Basel Consensus Statement”. It can now be used by German-speaking medical schools as a blueprint for curriculum planning and implementation processes. The modular structure enables it to be implemented either in whole or in part. The model can also be used to facilitate the conversion of medical education into Bachelor and Master degree programmes. The longitudinal model curriculum features 131 educational objectives and makes suggestions for didactic concepts and assessment tools. For various disciplines, it also recommends at what point in time specific topics should be taught. Conclusion: The longitudinal model curriculum “Communication and social competencies”, based on the educational objectives of the “Basel Consensus Statement”, is a new curricular instrument that can be used by German, Austrian and Swiss medical schools. It can help to simplify the realisation of the Bologna process, also across different faculties. Communication skills training is an accepted part of undergraduate medical programs nowadays. In addition to learning experiences its importance should be emphasised by performance-based assessment. As detailed checklists have been shown to be not well suited for the assessment of communication skills for different reasons, this study aimed to validate a global rating scale. A Canadian instrument was translated to German and adapted to assess students’ communication skills during an end-of-semester-OSCE. Subjects were second and third year medical students at the reformed track of the Charité-Universitaetsmedizin Berlin. Different groups of raters were trained to assess students’ communication skills using the global rating scale. Validity testing included concurrent validity and construct validity: Judgements of different groups of raters were compared to expert ratings as a defined gold standard. Furthermore, the amount of agreement between scores obtained with this global rating scale and a different instrument for assessing communication skills was determined. Results show that communication skills can be validly assessed by trained non-expert raters as well as standardised patients using this instrument.
[english] Purpose: The purpose of the position paper is to present a guideline about which communicative and social competencies medical students should have achieved at the end of their medical studies. This guideline is based on a consensus of experts from different disciplines and medical faculties. Referring to the English-speaking consensus statements developed in Toronto and Kalamazoo, the German speaking guideline has been called “Basel Consensus Statement”. Methods: The guideline was developed in three steps, an initial two-day workshop in Basel, followed by a two-step Delphi-survey. The aim of the Delphi-survey was to evaluate the competencies and educational objectives that were developed at the workshop in Basel according to their importance for medical education. 30 persons attended the workshop. They were also the target group for the first Delphi-round. Recipients of the second Delphi-round were the members of the Association for Medical Education in German-speaking countries. 77 persons participated in the survey. Altogether, over 100 persons from 30 different faculties could be involved into the development of the guideline. Results: Based on the survey results, five areas of competencies were defined divided in general and specific competencies. These areas were operationalised into topics and educational objectives. The areas of specific competencies are: doctor-patient-relationship, teamwork, personality and professionalism, reasoning and decision making. Altogether, 19 themes and 131 educational objectives were enclosed into the guideline. Conclusion: The position paper presents a first interdisciplinary German speaking guideline for communicative and social competencies in medical education that hopefully will strengthen the importance of these competencies in medicine. In addition, it will hopefully be a worthwhile theoretical foundation for improving teaching, learning, and assessment in this field. In Anlehnung an die bestehenden angloamerikanischen Consensus Statements, die in Toronto und Kalamazoo erarbeitet wurden, wird die vorliegende deutschsprachige Empfehlung „Basler Consensus Statement“ genannt. Methodik: Die Entwicklung erfolgte in drei Schritten, einem zweitätigen Workshop in Basel und einer zweistufigen Delphi-Befragung. Ziel der Delphi-Befragungen war es, die Kompetenzbereiche und Ausbildungsziele, die in Basel erarbeitet worden waren, nach ihrer Wichtigkeit für das Medizinstudium zu beurteilen. Insgesamt konnten damit über 100 Personen aus 30 Fakultäten im deutschsprachigen Raum in den Entwicklungsprozess involviert werden. Ergebnisse: Auf der Basis der Ergebnisse der ersten und zweiten Delphi-Runde ergaben sich fünf Kompetenzbereiche, die in allgemeine und spezifische Kompetenzen unterteilt wurden. Für beide wurden Themengebiete und Ausbildungsziele festgelegt. Die spezifischen Kompetenzen umfassen die Bereiche: Ärztin-Patientin-Beziehung, Teamarbeit, Persönlichkeit und Professionalität, Urteilsbildung und Entscheidungsfindung. Insgesamt wurden 19 Themengebiete und 131 Ausbildungsziele in die Empfehlung aufgenommen. Fazit: Mit dem Basler Consensus Statement liegt nun ein diskussionswürdiges deutschsprachiges Positionspapier vor, das die Wertigkeit kommunikativer und sozialer Kompetenzen als einen wichtigen curricularen Bestandteil stärken wird und das als Basis für die curriculare Entwicklung in diesem Bereich fungieren kann.
[english] Background: Inadequate teamwork often causes medical errors. However, this subject is not integrated in undergraduate medical education in Germany yet. To foster a preventive error culture and to close this gap in our curriculum we piloted a new course concept for one cohort of students in the reformed medical track at Charité Universitätsmedizin Berlin in 2006. The main topic of this course within the compulsory communication skills training was “team communication and error management”. Methods: Students studied different instruments for error prevention. Furthermore video material was used to analyse different aspects of error management such as team communication, fields of competency and management of fatal errors. The new concept was evaluated quantitatively and qualitatively by students and faculty. Results: Evaluation results show high acceptance of the new semester topic and the used teaching methods from both students and faculty. Conclusion: On basis of our evaluation results the course will be expanded and also integrated earlier within the curriculum. [german] Zielsetzung: Unzureichende Teamarbeit ist eine häufige Ursache für Behandlungsfehler. Trotz der Relevanz dieses Themas für die Patientensicherheit gehört es in Deutschland noch nicht zur Pflichtlehre im Medizinstudium. Um diese Lücke im Curriculum zu schließen und schon im Studium eine präventive Fehlerkultur zu fördern, wurde für eine Studierendenkohorte im Reformstudiengang an der Charité Universitätsmedizin Berlin (N = 67) ein neues Lehrkonzept zum Thema „Teamarbeit und Fehlermanagement“ entwickelt und im Sommersemester 2006 zum ersten Mal in der Pflichtveranstaltung Interaktion implementiert. Methodik: Die Studierenden lernten verschiedene Instrumente der Fehlerprävention kennen. Darüber hinaus wurden anhand von Filmsequenzen verschiedene Aspekte des Fehlerumgangs wie Team-Kommunikation, Einhalten und Übertreten von Kompetenzbereichen oder Umgang mit schwerwiegenden Fehlern diskutiert. Der Kurs wurde durch die Studierenden und die Lehrenden quantitativ sowie qualitativ evaluiert. Ergebnisse: Die Evaluationsergebnisse zeigen eine hohe Akzeptanz des neuen Semesterthemas und der didaktischen Umsetzung auf Seiten der Lehrenden und Studierenden. Schlussfolgerung: Auf dieser Basis soll der Kurs erweitert und im Sinne einer Lernspirale zusätzlich schon früher im Curriculum verankert werden.
