To improve the knowledge on influenza and respiratory syncytial virus (RSV) infections burden on paediatric population, a cohort of 1600 Italian children was prospectively followed up during the 2001–2002 and 2002–2003 winters. Specimens from each influenza-like illness (ILI) were collected and tested for influenza viruses and RSV by PCR and cell cultures. Two hundred sixty-eight and 440 episodes of ILI were registered during 2001–2002 and 2002–2003, with a cumulative incidence of 16.8% and 27.5%, respectively. A comparison between the morbidity data of this well-studied cohort and those of the national network (Influnet) and the one between different swabs and different detection methods was performed. Considering the high variability of respiratory epidemics in different areas, the cumulative morbidity is not far from that observed in the Liguria region by the national network (21.1% and 24.6%). Fifty-nine (22%) and 22 (8.3%) out of 268 specimens collected during 2001/2002 season and 60 (13.6%) and 34 (7.2%) out of 440 during 2002/2003 were influenza and RSV-PCR positive, respectively. During the former winter, influenza B virus was prominent, while during the last season A/H3N2 predominated. RSV circulation preceded influenza detection in both seasons. PCR confirmed higher sensitivity than cell culture in influenza virus detection; the combination of a paediatric sentinel-based network with virological surveillance performed by reverse transcription polymerase chain reaction (RT-PCR) for influenza and RSV seem a good way for the control of respiratory tract diseases.
The total burden of acute respiratory diseases in children is still not completely understood. Clinical and epidemiological surveillance of influenza (FLU) and other Acute Respiratory Infections (ARI) are currently a major objective of Public Health in order to approach “ad hoc” prevention strategies in pediatric population. The principal aim of our study was to describe the epidemiology of these diseases using an appropriate surveillance system. During 2000–2001 and 2001–2002 seasons, respectively 49 and 55 sentinel paediatricians monitored 10 Italian Regions (Liguria, Lombardy, Friuli Venezia Giulia, Tuscany, Umbria, Marche, Abbruzzo, Puglia, Calabria and Sicily) weekly notifying FLU, monitored as Influenza Like Illness (ILI), and ARI clinical cases. We calculated cumulative weekly morbidity rate×1000 subjects between 0 and 14 years during winter season (44th to 17th week). The FLU vaccinal status and the number of hospitalisations were also looked into.
Obiettivi : valutatazione della sicurezza, tollerabilita e immunogenicita di un nuovo vaccino influenzale virosomale, adiuvato con la frazione termolabile della tossina di E. Coli, somministato per via intranasale a soggetti ex tossicodipendenti, HIV positivi e negativi. Materiali e metodi : 349 soggetti HIV negativi ed 88 HIV positivi hanno ricevuto 2 dosi di vaccino a distanza di 8 1giorni. I soggetti hanno registrato su un apposito diario clinico la comparsa di reazioni locali e sistemiche. L’immunogenicita e stata valutata mediante HAI test al giorno della vaccinazione (T0) e a distanza di 29+3 giorni dalla stessa (T1). Risultati : non si sono verificati seri eventi avversi. Cefalea e naso chiuso sono state le reazioni postimmunizzazione, sistemiche e locali, piu frequenti. Nel gruppo di soggetti HIV negativi, le percentuali di sieroconversione sono state 41,26% per il ceppo virale A/H3N2, 43,55% per A/H1N1 e 41,26% per il ceppo B. Le percentuali di sieroprotezione sono state le seguenti: 68,48% per A/H3N2, 67,9% per A/H1N1 e 85,95% per B. L’incremento del Titolo Geometrico Medio (GMT T1/T0) e stato di 2,68 per A/H3N2, 3,41 per A/H1N1 e 2,58 per B, in ottemperanza ai criteri E.M.E.A. Il gruppo di soggetti HIV positivi ha mostrato una risposta immunologica minore (percentuali di sieroconversione: 33% per A/H3N2, 28,4% per A/H1N1, 39,8% per B; percentuali di sieroprotezione: 50% per A/H3N2, 36,4% per A/H1N1, 63,6% per B; GMT T1/T0: 2,1% per A/H3N2, 1,9% per A/H1N1e 2,3% per B). La regressione logistica ha dimostrato come una conta di CD4+ ›350/ml e un titolo anticorpale prevaccinale ›5 siano parametri correlati positivamente con il livello di protezione nei soggetti HIV positivi. Conclusioni : il vaccino si e dimostrato sicuro e ben tollerato in entrambi i gruppi. L’immunogenicita e stata maggiore nei soggetti HIV negativi. Nei soggetti HIV positivi, comunque, l’immunizzazione con il nuovo vaccino virosomale, ripetuta negli anni con antigeni tra loro simili, potrebbe elicitare una risposta immunologica migliore di quella ottenibile con i vaccini convenzionali.
