BackgroundAdvancing the development of centers for disease control and prevention (CDCs) has become a priority within global public health governance. However, public health governance capacity varies significantly among CDCs across different countries and regions, grassroots CDCs face particular disadvantages. Establishing stable, efficient collaborative development mechanisms among CDCs across diverse regions to maximize overall effectiveness and ensure sustainable development represents a critical public health science issue.ObjectiveThis study aims to provide scientific references and a theoretical foundation for the coordinated development of grassroots CDCs within the Chengdu-Chongqing Economic Circle (CCEC) and the construction of public health systems.MethodsA questionnaire for collaborative development needs indicators in grassroots CDCs, comprising 4 primary needs and 13 secondary needs, was developed through literature review, the Delphi expert consultation method, and the Kano model. Analysis focused on questionnaires collected from eight grassroots CDCs within the CCEC. The importance of needs was ranked using the better-worse coefficient and satisfaction sensitivity analysis.ResultsAnalysis of the 110 valid questionnaires showed that for the must-be attribute, satisfaction sensitivity ranked as follows: performance compensation (0.883)> talent exchange and scientific research and innovation cooperation (0.824)> public health emergency rescue mechanism (emergency material reserve and cross-regional material mobilization; 0.817)> cross-regional case monitoring, investigation, and tracking (0.775). Regarding the one-dimensional attribute, the satisfaction sensitivity ranking was joint risk assessment and emergency command (0.937)> business archive co-construction and sharing mechanism (emergency response plan, and technical scheme) (0.909)> regional co-construction and sharing between the university and the local area (0.832). For the attractive attribute, the satisfaction sensitivity ranking was regional monitoring and early-warning information management system (0.922)> community chronic disease prevention and service (0.804)> coordinated transfer and diversion diagnosis and treatment of patient with infectious diseases within the region (0.734). However, the collaborative release and interaction mechanism of social integrated media information, public health collaborative governance entities, and the construction of a cross-regional expert database constitute indifferent attributes.ConclusionsThis study provides preliminary scientific evidence for the precise allocation of public health resources and the establishment of localized collaborative development mechanisms. Simultaneously, the research methodology and analytical framework offer new theoretical references for similar studies in other regions globally.
Fetal growth restriction (FGR) severely affects the health outcome of newborns and represents a major cause of perinatal morbidity. The precise involvement of circCULT1 in the progression of FGR remains unclear. We performed next-generation sequencing and RT-qPCR to identify differentially expressed circRNAs in placental tissues affected by FGR by comparing them with unaffected counterparts. Edu, flow cytometry, and transwell assay were conducted to detect HTR8/SVneo cell's function in regard to cell proliferation, migration, and invasion. The interaction between circCUL1 and hsa-miR-30e-3p was assessed through dual-luciferase reporter assays, validation of the interaction between circCUL1 and ANXA1 was performed using RNA pulldown and immunoprecipitation assays. Western blot analysis was performed to evaluate protein levels of autophagy markers and components of the PI3K/AKT signaling pathway. A knockout (KO) mouse model was established for homologous mmu-circ-0001469 to assess fetal mouse growth and development indicators. Our findings revealed an upregulation of circCUL1 expression in placental tissues from patients with FGR. We found that suppression of circCUL1 increased the trophoblast cell proliferation, migration, and invasion, circCUL1 could interact with hsa-miR-30e-3p. Further, circCUL1 stimulated autophagy, modulating trophoblast cell autophagy via the ANXA1/PI3K/AKT pathway, and a notable disparity was observed, with KO mice displaying accelerated embryo development and exhibiting heavier placentas in comparison to wild-type C57BL/6 mice. By modulating the ANXA1/PI3K/AKT signaling pathway through the interaction with hsa-miR-30e-3p, circCUL1 promotes autophagy while concurrently suppressing trophoblast cell proliferation, migration, and invasion. These findings offer novel insights into potential diagnostic markers and therapeutic targets for FGR research.
This study used a self-administered questionnaire to investigate the relationship between HIV/AIDS knowledge, social discrimination, HIV self-efficacy, sexual behavioral characteristics and HIV testing behavior among young students with a history of sexual activity in Chongqing. Propensity score matching was used to construct a control group with similar demographic characteristics to the HIV testing group, only without HIV testing behaviors. Structural equation modeling was used to conduct chain-mediated effect analysis. The HIV testing rate among young students with a history of sexual behavior was 17.16%. Young students with HIV testing behavior had higher levels of HIV/AIDS knowledge, lower social discrimination, better self-efficacy, and no difference in sexual behavior characteristic scores between the two groups (P = 0.062). The mediated effect values of social discrimination and self-efficacy were 0.022 and 0.063, respectively, while their chain mediated effect value was 0.007. There was no mediated effect of sexual behavior characteristics. The level of young students' HIV/AIDS knowledge does not directly influence their HIV testing behavior but plays a complete chain mediating effect on HIV testing behavior through social discrimination and self-efficacy. HIV testing behavior should be promoted through the perspectives of enhancing HIV/AIDS knowledge level, reducing social discrimination, and improving self-efficacy.
Lifestyle is an important factor affecting people's health. Evaluating and promoting healthy lifestyles among occupational population can not only improve work ability and efficiency, but also contribute to the prevention a variety of potential diseases. This study developed a preliminary index system based on literature review and group discussion. Two rounds of expert consultation were conducted on 12 experts using Delphi method. The questionnaire recovery rate of the two rounds of Delphi survey was over 85%, the expert authority coefficient was over 0.70, and Kendall W was 0.450 and 0.446, which were significant (P < 0.001). The weighting coefficient of each indicator was calculated using the hierarchical analysis method. Among them, the weighting coefficients of physical health lifestyle, mental health lifestyle, social adaptation lifestyle and occupational health lifestyle were 0.4133, 0.2922, 0.1078 and 0.1867, respectively. The consistency index CI = 0.024 and the consistency ratio CR = 0.027 of the first-level index judgment matrix. The weighting coefficient is acceptable and the indicators do not cause logical confusion. The healthy lifestyle assessment tool of the occupational population constructed in this study consists of 4 primary indicators, 13 secondary indicators and 45 tertiary indicators, which can provide a standardized and operable assessment tool for monitoring and evaluating the healthy lifestyle of the Chinese occupational population. At the same time, the weight analysis of various indicators through the analytic hierarchy process can also provide reference for the key areas of occupational health intervention.

Chongqing is one of the focuses of leprosy control in China. Although leprosy control in Chongqing has achieved remarkable results over the years, there are also some problems, such as recurrent epidemics and insufficient early detection in some areas. The aim of this study was to analyze the epidemiological characteristics of leprosy in Chongqing, from 1949 to 2019 and explore the potential factors sociated with cure of leprosy to provide a basis for improving leprosy prevention and treatment strategies in Chongqing. Epidemiological indicators such as incidence and prevalence rates were used to evaluate the prevalence of leprosy. The epidemiological characteristics and control situation of leprosy in patients were analyzed using demographic characteristics, diagnosis, and treatment. Survival analysis was conducted to explore factors associated with the cure of leprosy. From 1949 to 2019, 3,703 cases of leprosy were registered in Chongqing. The incidence of leprosy in the city peaked at 0.853/105 in 1960 and remained below 0.100/105 after 2003. The number of high incidence areas decreased significantly, but they were mainly concentrated in the northeast and southeast regions. The early detection rate increased yearly from 1949 to 2019, and the rate of grade 2 disability ranged from 38.2% to 21.7%, with a fluctuating downward trend after 1960. Male, young age, employment as a farmer, delayed diagnosis, and multibacillary leprosy were risk factors for leprosy cure. Chongqing should continue to strengthen leprosy monitoring to improve the early detection of leprosy and focus on sociated risk factors to carry out multiple strategies.
ObjectivesThis study aimed to describe the HIV/AIDS knowledge, sexual attitudes, and sexual behaviors of young students (14–25 years) in Chongqing, China, and to examine their use of preventive services and related factors.MethodA cross-sectional study was conducted with students from 67 schools from December 2019 to June 2021. The chi-square test, non-parametric test to assess the differences between respondents with different characteristics in the above aspects. Additionally, univariate and multivariate logistic regression analyses were conducted to examine factors associated with the awareness of HIV/AIDS knowledge. Confidence intervals without crossover or P < 0.05 was considered significant.ResultsA total of 31,782 participants were included in the study. A total of 62.62% of the respondents were considered to be aware of basic HIV/AIDS knowledge, including 78.44%, 62.15%, and 52.08% in undergraduate colleges, junior colleges and secondary vocational schools, respectively. In total, 60.96% of males and 29.42% of females accepted premarital sex, 15.49% of males and 6.18% of females reported being sexually experienced, and only 57.18% of the students used condoms every time they had sex. The percentage of condom use among students who were aware of HIV/AIDS knowledge was higher than that among students who were not aware. A total of 85.60% of the students thought they needed HIV/AIDS prevention and control knowledge, and 94.60% thought more health education activities on HIV/AIDS prevention needed to be conducted in schools. Entrance education, lectures or consultation hotlines, WeChat official accounts and other media platforms were rated as the most acceptable HIV/AIDS prevention education methods.ConclusionThese respondents lacked knowledge about HIV/AIDS, and an increasing number of students show an open attitude toward premarital sex and engage in risky sexual behaviors. It is urgent to enhance and widely disseminate comprehensive HIV/AIDS and sexual health education in multiple formats.
Abstract Background Fetal growth restriction seriously affects the health of newborns. However, the cause of FGR is unclear. Circular RNAs (CircRNAs) are a novel type of endogenous noncoding RNAs that regulate target gene expression by interacting with microRNA (miRNA) .The present research was major in investigating the regulation association among circCUL1, miR-30e, ATG7/14 and MAPK1 in FGR, and we explored the mechanism underlying pathogenesis of FGR Results circCUL1 promotes autophagy in placental trophoblast cells. Mechanism studies have found that circCUL1 regulates trophoblast autophagy through the miR-30e-ATG7/14 and ANXA1/PI3K/AKT pathways. We also observed that knocking out circCUL1 promoted mouse development. Conclusions our research suggested that circCUL1 may be involved in the pathogenesis of FGR. circCUL1 may be a potential diagnostic marker and therapeutic target of FGR.
Background The relationship between retirement and health is important to the formulation of retirement related policies but is a controversial topic, perhaps because selection bias has not been well-addressed in previous studies through traditional analysis methods. Using data from the China Health and Retirement Longitudinal Study (CHARLS), this study explored the potential impact of retirement on the health of elderly Chinese individuals, adjusting for selection bias. Methods We balanced the baseline differences between retirement groups and working groups based on nearest neighbor matching and genetic matching with a generalized boosted model (GBM), and regression analysis was used to evaluate the impact of retirement on the health of elderly individuals. Results No significant difference was found in any of the covariates between the two groups after matching. Genetic matching performed better than nearest neighbor matching in balancing the covariates. Compared to the working group, the retirement group had a 0.78 (95% CI: 0.65–0.94, P = 0.026) times higher probability of self-reported physical pain, a 0.76 (95% CI: 0.62–0.93, P = 0.023) times higher probability of depression, and a 0.57-point (95% CI: 0.37–0.78, P < 0.001) improvement in cognitive status score. Among male, the retirement group had a 0.89-point (95% CI: 0.45–1.33, P < 0.001) improvement in cognitive status score for low education, a 0.65 (95% CI: 0.46–0.92, P = 0.042) times higher probability of self-reported physical pain for middle education. For female with low education, the cognitive status of the retirement group was significantly higher by 0.99 points (95% CI: 0.42–1.55, P = 0.004), the probability of depression was 0.56 (95% CI: 0.36–0.87, P = 0.031) times higher in the retirement group than in the working group. There was no difference for the middle and high education. Conclusion Retirement can exert a beneficial effect on the health of elderly individuals. Therefore, the government and relevant departments should consider this potential effect when instituting policies that delay retirement.
Objective To investigate the expression, function and potential mechanism of circular RNA circVCAN(also known as hsa-circ-0073239)in fetal growth restriction (FGR) placenta. Methods The placenta tissue samples were collected from 24 pregnant women (12 with FGR and 12 normal pregnancy) admitted in our hospital from January to December 2018. Real-time quantitative PCR (RT-qPCR) was performed to detect the expression of circVCAN in these placenta samples. Human placental trophoblast cell line HTR8/SVneo was cultured and then transfected with circVCAN interference and overexpression plasmid, respectively. EdU assay, flow cytometer and Transwell assay were carried out respectively to verify the variations in the proliferation, apoptosis and invasion abilities of the cells. And the expression of autophagy related proteins LC3Ⅱ/LC3Ⅰ, ATG5 and ATG7 was measured by Western blotting. Results The expression levels of circVCAN, ATG5, ATG7 and LC3Ⅱ/LC3Ⅰwere significantly higher in FGR placenta tissue than in the normal group (P < 0.01). The termination of pregnancy was earlier in FGR patients than in normal group (P < 0.05), with newborns from the FGR group having lower birth body weight (P < 0.01) and placenta weight (P < 0.05) as well as shorter birth body length (P < 0.01) when compared with those from the normal group. Silencing of circVCAN resulted in enhanced proliferation and invasion abilities in HTR8/SVneo cells (P < 0.01), while circVCAN overexpression led to decreased proliferation and invasion abilities in the cells (P < 0.01), but no statistical difference was seen in the apoptotic rate (P>0.05). The expression levels of autophagy related proteins LC3Ⅱ/LC3Ⅰ, ATG5 and ATG7 were significantly lower in circVCAN interfered cells (P < 0.05), while higher in circVCAN overexpressed cells (P < 0.05). Conclusion CircVCAN is highly expressed in FGR placenta tissue, though it showing no effect on apoptosis but inhibiting the proliferation and invasion of HTR8/SVneo trophoblast cells, and may enhance autophagy by promoting the expression of ATG5 and ATG7.
