Medication preparation and dose calculation are critical components of safe medication administration, requiring nursing students to integrate knowledge, skills, and clinical decision-making competencies. Assessing students’ self-efficacy in these processes is important for identifying their educational requirements. Therefore, measurement tools that can comprehensively assess this multidimensional construct are required. This study aimed to develop the Medication Preparation and Calculation Self-Efficacy Scale and examine its psychometric properties to assess nursing students’ self-efficacy regarding the medication preparation and calculation process. This methodological study included 539 nursing students from two universities, with independent samples used for EFA and CFA. The scale items were developed based on a literature review and Bandura’s self-efficacy theory. Content validity was evaluated using the Delphi technique with input from 15 experts in the field. Construct validity was examined using Exploratory Factor Analysis (EFA) and Confirmatory Factor Analysis (CFA). Convergent and discriminant validity were assessed using Composite Reliability (CR), Average Variance Extracted (AVE), and the Fornell–Larcker criterion. Cronbach’s alpha and intraclass correlation coefficients (ICC) were calculated for reliability analyses. Criterion-related validity was evaluated using a five-item medication calculation test. The content validity index values for the scale ranged from 0.87 to 1.00 at the item level and were 0.97 at the scale level. The EFA revealed a three-factor structure that explained 63.72

BACKGROUND:This study investigates the effects of lean management practices on nurses' direct patient care activities and the interruptions they encounter in healthcare settings. The literature indicates that lean management enhances efficiency and improves patient care. Increased nursing time per patient correlates with better outcomes; however, rising patient loads and frequent interruptions hinder nurses' ability to deliver effective care, jeopardising patient safety. Addressing these inefficiencies is essential, given nurses' critical role in ensuring quality care. METHOD:Using a single-centre, observational, pre-post time and motion design alongside participatory action research from August 2019 to July 2022, the study included three phases: a survey assessing nurses' views on problem-solving and lean management; observations identifying nurses' activities and interruptions; and focus group discussions. The results presented here are from the observation phase, with 34 nurses from surgical, internal medicine, and mixed services participating. Quantitative variables were expressed as means, standard deviations, and significance was assessed at a 95% confidence interval (p < 0.05). RESULTS:Observations in 2019 (324 h) and 2022 (314 h) revealed that nurses dedicated a significant portion of their time to direct patient care, which increased post-lean implementation (surgical: 50.67%, internal: 50.09%, mixed: 44.38%). Waste rates decreased by 35.81%, and documentation time decreased by 23.55%. Overall interruptions also decreased significantly (p < 0.05). CONCLUSION:Lean management effectively reduces waste and improves direct patient care time, enhancing patient safety and care quality. Continuous improvement initiatives in nursing practices are essential for success.
PURPOSE:Inefficient patient flow in chemotherapy units is often caused by improper resource allocation, delays, and excessive workloads. Lean healthcare practices offer process visualization and waste reduction, addressing these issues. This project aims to optimize chemotherapy patient flow by improving care coordination, reducing waiting times, and shortening hospital stays. METHODS:The project employed Value Stream Mapping methodology from May 2022 to May 2023. The team mapped the chemotherapy administration process, identified inefficiencies, and evaluated factors influencing patient flow, including delays and resource utilization. RESULTS:Prior to the intervention, the percentage of value-added time from patient admission to discharge was 47.35%. After applying lean principles, the total hospital stay was reduced by 56.8%, treatment duration decreased by 31.57%, and unscheduled patient visits decreased by 22.93%. The evaluation of test results, chemotherapy drug transfer, and drug preparation processes were identified as the least efficient, with value-added time percentages of 12.87%, 17.12%, and 28.89%, respectively. The longest waiting time was recorded before the start of chemotherapy, averaging 72 minutes. CONCLUSIONS:Lean practices were effective in reducing waiting times and hospital stays, improving patient flow. Key improvements included enhanced coordination, streamlined drug preparation, and reduced delays in patient assessments. The project is reinforced with field data, which enhances the applicability of the findings in clinical practice. The success and sustainability of these improvements are dependent on ongoing management support and the adoption of lean practices across the hospital. Implementing lean practices in high-volume units is recommended to further enhance operational efficiency.
Brain fog was first described in the 1850s as a form of mental fatigue, particularly observed among individuals engaged in occupations requiring intensive cognitive effort. The present study aims to examine the occurrence of brain fog among healthcare professionals—who are required to perform multiple tasks simultaneously and make critical decisions—and to identify contributing factors through a review of the current literature. Accordingly, a traditional literature review was conducted by retrieving articles published between 2019 and 2025 from the PubMed database using the keywords “brain fog”, “cognitive impairment” and “mental fatigue”. Brain fog is characterized by forgetfulness, difficulty concentrating, impaired multitasking ability, and diminished cognitive functioning. Healthcare professionals constitute one of the occupational groups most frequently exposed to brain fog. Physicians, nurses, and other healthcare personnel are subjected to various stressors and adverse working conditions that may precipitate brain fog, including time pressure, uncertainty in patient treatment and care processes, extended working hours and insufficient nutrition. Given that healthcare professionals are responsible for improving and maintaining individual health and quality of life, the experience of brain fog within this group poses risks to patient safety due to the cognitive symptoms it entails. Examining brain fog and its associated factors among healthcare professionals is essential for implementing preventive strategies, enhancing the quality of patient care, minimizing errors during clinical practice, and improving job satisfaction and quality of life among healthcare personnel. Furthermore, investigations may provide a foundation for developing evidence-based strategies to mitigate brain fog in healthcare settings.
Objective: For a safe intramuscular injection, it is essential to provide nurses with knowledge and skills for using the ventrogluteal site. Background: Treatment and care of patients in intensive care units require the use of many medical and technological instruments. Pressure injuries occur when medical devices, which are used more in intensive care patients and are in direct or indirect contact with the skin, cause focal and localized forces on the superficial or deep tissues. Objective: In this study, it was aimed to examine the risk factors, incidence and characteristics of medical device-related pressure injuries in intensive care patients. Methods: This study has a prospective and descriptive design. The study was carried out in the adult intensive care unit of a healthcare institution located in the western Turkey. 138 intensive care patients treated in the level 3 adult intensive care unit were enrolled in the study. The first observations and evaluations of intensive care patients in terms of pressure injuries were made within the first 24 h after admission to the clinic. Observations continued daily during the hospitalization period of the patient. Data were collected with the Intensive Care Patient Information Form, Glasgow Coma Scale, Braden Pressure Ulcer Risk Assessment Scale and Identification Form for Medical device-related Pressure Ulcers. Analysis of data was performed with descriptive statistical methods, Shapiro-Wilk Test, Mann-Whitney U Test and Chi-Square analysis. Results: Medical device-related pressure injury developed in 11.6% (n = 16) of intensive care patients. Anatomically, pressure injury occurred most frequently on the lip (37.5%) and most frequently due to the intubation tube (37.5%). Most of the developed wounds (75.0%) were found to be stage 2. Multinominal logistic regression analysis, which was performed to determine the effect of independent variables on medical device-related pressure injuries in intensive care patients, was found to be statistically significant (X-2 = 37.098, p < 0.001). When the regression coefficients were examined, it was found that total hospitalization time in the intensive care unit (beta = 0.948, p < 0.01) and PaCO2 level (beta = 0.923, p < 0.01) had a positive, and duration of aerobic respiration with nasal cannula or mask (beta = -0.920, p < 0.01) and Braden score (beta = -0.948, p < 0.01) had a negative and significant effect on medical device-related pressure injuries. Conclusions: In this study found that the MDRPIs development rate was lower than other studies. It was observed that pressure injuries due to medical devices developed more frequently in patients with longer hospitalization days, higher PaCO2 levels, shorter duration of oxygenated breathing with nasal cannula or mask, and lower Braden scores.
Background: Due to the equipment used in the health system, drugs, and the need for specialized health personnel, the operating rooms are the most costly, and time is the most valuable resource. From this point of view, the study was aimed to raise awareness about Value Stream Mapping, which is one of the problem-solving practices in nurses, and to practice with a sample VSM study (eliminating non-value-creating activities, creating flow in processes, and avoiding delays due to the preparation of the surgery in the pre-op period with continuous improvement). Methods: Observational action research. The research, Value Stream Mapping, which is one of the lean methods, was carried out in a private hospital between December 2019 and March 2022. A total of 59 patient flows were observed in three stages. First, the current situation map was drawn. Value-added and non-value-added time was determined. The problems experienced in the current situation were determined and suggestions were developed for their solution, kaizen studies were carried out. Onset time and delays in minutes were monitored for all first cases on a daily basis. Result: Non-value-added activities were reduced from 102 min to 52 min on average. In particular, the quality increased from 0 % to 36 %. "If patients were not in the operating room within 10 min of the scheduled start time, this was considered a late start". While the rate of starting the surgery on time at the first observation was 0 %, this rate increased to 70 % at the end of the study. Conclusions: Improving the pre-op preparation process was effective in increasing the percentage of surgery that started on time in the operating room. It is thought that value stream mapping can be a useful method for improving on-time starts in the operating room.

AIM:The aim of this study is to explore the extent of missed nursing care in Turkey and identify its predictors.DESIGN:This was a descriptive, cross-sectional, multicentre study.METHODS:A total of 477 nurses working in seven public hospitals participated in this study from March to July 2019. The survey included two components: a personal and professional characteristics data form and the MISSCARE survey.RESULTS:The study revealed that emotional support, patient bathing and ambulation were the most frequently missed nursing care activities. An inadequate number of assistive personnel and staff, along with an unexpected increase in patient volume, were identified as the primary reasons for missed nursing care. Of the 21 missed nursing care activities, nine predictive models showed statistical significance (p < 0.05). Factors such as the type of unit, years of work experience, working hours, number of patients cared for in a shift and intention to leave the unit were found to be significant predictors of seven missed nursing care activities (p < 0.05).CONCLUSION:This study found that numerous variables influence each care activity, which suggests the need to devise more targeted and specific strategies to minimize missed nursing care. Thorough investigation into the impact of these strategies on each care activity is essential.

ABSTRACT Objectives: Nurses are required to make ethical decisions with the ethical principles of the nursing profession while making decisions in clinical practice. Therefore, it is very important to recognize and solve ethical problems. The purpose of this research is to learn in-depth the views of nurses about ethical practices and nurses' behavior towards individuals when giving care. Methods: A hermeneutic-phenomenological qualitative study was performed. Qualitative data were gathered through semi-structured in-depth interviews from March to November 2020, four hospital in Kocaeli. This qualitative study was carried out with the data collected in face-to-face in-depth interviews with the semi-structured questionnaire. Interviews were conducted with 40 nurses and each interview took 30–60 min. The obtained data were evaluated with the inductive content analysis method. Results: Content analysis of the interview based on nurses' descriptions revealed four major themes. Themes: The meaning given to care ethics, difficulties encountered in nursing care, ethical values in nursing care and ethical approachs of hospital management. The nurses indicated level of care ethics knowledge was insufficient and they needed to receive comprehensive ethical care training from a qualified specialist. In addition, nurses' personal characteristics, religious and cultural values, and professional experiences were found to be factors affecting nursing care. It was also find that the support and motivation of nurses by their managers positively affected their care. Conclusion: In the interviews, nurses consider it necessary to provide care by paying attention to ethical behaviors in nursing care and practices for the development and professionalism of the profession, but they state that they do not have an idea about how this will be provided. It was determined that nurses thought that more attention should be paid to the importance and value of ethics in nursing interventions, and ethical practices in nursing care should be included in in-service training programs.
Background: Making the employee feel valued can significantly affect the employee's commitment to the organization. The most important criteria of lean health services are to develop a culture in which employees are empowered to be actively involved in the decision-making process. Objective: This study aims to examine whether lean management practices have an effect on increasing nurses' “organizational commitment” and “job satisfaction”. Method: The research was conducted in a private hospital in 2019-2021 in Turkey. In the study, the "Organizational Commitment and Job Satisfaction Questionnaire" was applied to staff nurses. Then the hospital management decided to implement “Lean Hospital” practices. In this process, lean training was given to the nurse management, and lean methods and techniques were applied in the field. After approximately 1.5 years, the questionnaires were re-issued to the participants again to compare and contrast results. Results: In the study, the mean score of the nurses from the "Job Satisfaction Questionnaire" before lean was 3.79±0.29, while it was 3.82±0.28 after lean. The average score from the "Organizational Commitment Questionnaire" before implementing the lean practices was 3.74±0.32. It was 4.18±0.21 after implementing the lean practices. While there was a low level of positive correlation between the nurses' mean scores of organizational commitment and job satisfaction questionnaires before implementing lean practices, it was found that the result was moderately positive after lean practices. Conclusions/Implications for Practice: Ensuring the participation of employees in the decision-making process and listening to their suggestions in lean management were positively associated with “organizational commitment” and “job satisfaction”. However, it should not be forgotten that lean practices are a long-term and continuous methodology.
Background: Hand hygiene (HH) is the most effective way to prevent health care–associated infections; however, HH compliance rates continue to be suboptimal.
