Electronic medical records were used to calculate the Homeostatic Model Assessment of Insulin Resistance (HOMA-IR), Triglyceride–Glucose (TyG) index, Atherogenic Index of Plasma (AIP), and Metabolic Score for Insulin Resistance (METS-IR). Receiver operating characteristic (ROC) curve analysis was used to assess discriminatory performance, and optimal cut-off values were determined using the Youden index. Combined analyses were performed according to concordant classification of the component indices relative to their respective cut-off values.Results: Fasting glucose, fasting insulin, triglyceride levels, HOMA-IR, TyG, AIP, and METS-IR were significantly higher in women with PMOS (p< 0.05). Among individual indices, TyG showed the highest discriminatory performance (AUC=0.775; 95% CI: 0.706– 0.843). HOMA-IR and AIP demonstrated moderate discriminatory performance, whereas METS-IR showed lower performance. Among the combined assessments, HOMA-IR and TyG yielded an AUC of 0.830 (95% CI: 0.747– 0.913), with 80.8% sensitivity and 85.2% specificity.Conclusion: Metabolic indices, particularly TyG, may be useful for identifying metabolic abnormalities in normal-weight women with PMOS. Combined assessment of complementary metabolic indices may provide additional information for metabolic risk evaluation. We investigated the presence of asprosin (ASP), betatrophin, elabela (ELA), glucagon and subfatin (SUB) in the milk of mothers with gestational diabetes mellitus (GDM) and compared their levels with blood levels. We also investigated whether these peptides are synthesized by the breast. We investigated 12 volunteer mothers with GDM and 14 pregnant non-GDM control mothers. The peptides were measured using ELISA and their tissue localization was determined using immunohistochemistry. Breast milk contains ASP, betatrophin, ELA, glucagon and SUB. The amount of the peptides ranged from highest to the lowest in colostrum, transitional milk and mature milk. The amount of peptides in the milk was greater than for blood. The peptides, except for ELA, were increased in milk and blood by GDM. Betatrophin and ELA are synthesized in the connective tissue of the breast. ASP, glucagon and SUB are synthesized in the alveolar tissue of the breast. These peptides in breast milk may contribute to the development of the gastrointestinal tract of newborns and infants.
The efficacy of residential interventions to reduce cockroach allergens in public housing developments was evaluated over months of follow-up. Repeated measurements were collected from 39 apartments, with longitudinal analyses used to evaluate changes over time. Bla g 1 kitchen concentrations were reduced 71% and bed concentrations 53% (86% and 70% for Bla g 2, respectively) by 6 months post-intervention, after which concentrations began to increase. Apartments with higher concentrations were usually in poorer condition and benefited most from pest management efforts. Intensive interventions can significantly reduce the allergen burden in public housing apartments, but intervention efforts must be sustained.
Aim: There are many toxic substances in cigarette smoke and cigarette.These toxic substances affect the health of smokers and people around.Smoking and passive smoking exposure adverse effects on the health of many children.One of the adverse effects of smoking on health directly influences children in the growing period and especially on infants.The free radicals found in cigarette smoke can damage all the tissues in our body and cellular structures.Oxidative stress due to the increased free radicals cause a decrease in the levels of A, C, E vitamins which are part of antioxidant system.In this study, we aimed to evaluate serum antioxidant vitamin (A, C, E) levels of passive smokers. Material and Method:The study includes 78 babies at 3 months.The same infants were re-evaluated in the at 6 months.Feeding of babies' exposure to the cigarette were asked to the mothers.Serum vitamin A, C and E levels and urinary cotinine/creatinine ratios were measured at third and sixth months.Breastmilk vitamin A, C, E levels were measured at third and sixth months.Results: Infants with smoking mothers at 3 and 6 months, serum vitamin A, C, E levels were lower than other babies with activepassive non-smoking mothers.Infants with smoking mothers have urinary cotinine/creatinine ratios are higher than that of activepassive non-smoking mothers.There was no difference in terms of nutrition with respect to smoke exposure at 3 and 6 months.Infants with smoking mothers have vitamins A, C, E levels lower than that of active-passive non-smoking mothers. Conclusion:Smoke exposure of infants affects antioxidant vitamin levels of the babies.Urinary cotinine/creatinine ratio, an indicator of exposure to smoke, are higher in infants of passive smokers.In addition, mothers' exposure to smoke affects the vitamins A, C, E levels of mother's milk.
Background/Aim: Vitamin D exerts its effects in the body through its receptors. Polymorphisms in vitamin D Receptor (VDR) gene are known to cause certain diseases and affect vitamin D levels. In this study, we planned to examine the relationship between vitamin D levels and Vitamin D gene polymorphisms among children. Methods: The study group included 124 healthy children living in the same region. Vitamin D (VitD), Parathyroid Hormone (PTH), Alkaline phosphatase (ALP), Calcium (Ca), Phosphorus (P) and Magnesium (Mg) were examined in the blood samples taken. In terms of measured Vitamin D levels, children were divided into group 1 (Vitamin D 0.05 for all). VDR polymorphisms were found to have no effect on Vitamin D levels. Conclusion: In our study, no relationship was found between the genotypes of ApaI and TaqI polymorphisms and Vitamin D levels. This study is important in terms of remarking the fact that Vitamin D deficiency is still a public health problem and its contributions to VDR gene polymorphism research.
Aim: Indirect hyperbilirubinemia, a widespread problem in the newborn period, may need emergency treatment for prevention of neurological sequelae and mortality in some cases. We aimed to report the incidence, etiological factors, clinical findings, and the treatment of neonates with indirect hyperbilirubinemia. Methods: Ninety-six cases of non-physiological indirect hyperbilirubinemia and prolonged jaundice which were followed-up in the Neonatology Unit of Kafkas University Hospital between January 2018-October 2019 were evaluated. The therapeutic approach was determined according to the recommendations of American Academy of Pediatrics in 2004.Results: The incidence of IHB was 24.8% (n=96) among 387 hospitalized neonates. The mean gestational age, birth weight (BW), and bilirubin level on admission were 36.2 (2.5) weeks, 2628.9 (820) g, and 12.1 (5.29) mg/dL, respectively. Among all, vaginal delivery ratio was 38.5%, and cesarean delivery rate was 61.5%. About 34.4% were first-time mothers. The rates of breastfeeding and formula feeding were 39.6% and 1%, respectively. Around 59.4% were both breast- and formula-fed. The etiological factors of IHB were as follows: Prematurity and/or low birth weight (LBW) (20.9%), breast feeding jaundice (8.3%), ABO incompatibility (17.7%), Rh incompatibility (7.3%), ABO+Rh incompatibility (3.1%), cephal hematoma (2.1%), urinary infection (4.2%), sepsis (4.2%), pneumonia (2.1%), omphalitis (1%), subgroup incompatibility (1%), Glucose 6 phosphate dehydrogenase deficiency (1%) and unknown etiology (7.3%). Exchange transfusion rate was 1% (n=1), and 5 neonates (5.2%) were administered immunoglobulin therapy among 27 (28.1%) with hemolytic hyperbilirubinemia. Conclusion: Indirect hyperbilirubinemia is an important risk factor for mortality and morbidity in newborn period. Defining the risk factors for non-physiologic indirect hyperbilirubinemia, adequate follow up and prompt treatment would reduce neurological sequelae and mortality rates.
Objetivo: O objetivo do presente estudo foi investigar o impacto bioquímico e histopatológico do tratamento de ozônio em modelo experimental de osteomielite em ratos. Métodos: Vinte e quatro ratos Sprague-Dawley machos adultos (3 meses de idade, pesando de 300 a 400 g) foram alocados randomicamente em três grupos. O grupo I (n = 8) serviu como controle. No Grupo II (n = 8), o modelo de osteomielite experimental foi induzido no fêmur e não foi aplicado nenhum tratamento. O grupo III (n = 8) recebeu tratamento com ozônio intraperitoneal por 3 semanas depois da formação de osteomielite no fêmur. Foram coletadas amostras de sangue para avaliar a capacidade antioxidante total (CAT), a concentração da proteína carbonil (PCO) e da lactato desidrogenase (LDH) no soro. As amostras do fêmur foram avaliadas por histopatologia quanto a inflamação, necrose, osteomielite e formação de abscesso. Resultados: Os níveis séricos de TAC foram notavelmente maiores (p < 0,001), enquanto os níveis de LDH foram menores (p = 0,002) no Grupo III em comparação com o Grupo II. Nenhuma diferença significativa foi detectada entre os grupos com relação ao nível de PCO. Do mesmo modo, o Grupo III apresentou resultados histopatológicos mais favoráveis para osteomielite (p = 0,008), inflamação (p = 0,001), necrose (p = 0,022) e formação de abscesso (p = 0,022). Conclusão: O ozônio pode ser um tratamento adjuvante útil na osteomielite. Mais estudos com animais e com seres humanos são necessários para esclarecer e confirmar esses efeitos preventivos, compreender a fisiopatologia subjacente e estabelecer diretrizes. Aim: Viruses are important causes of acute gastroenteritis for children.Statistics related to the epidemiology of gastroenteritis caused by rotavirus and adenovirus vary according to regions.This study aims to specify the frequency of adenovirus and rotavirus for pediatric patients with acute gastroenteritis who apply the Medical Faculty of Kafkas University. Material and Method:The records of the feces samples of 3763 patients with the preliminary diagnosis of acute gastroenteritis between October 10th, 2013 and October 30th, 2018 were examined retrospectively.The presence of adenovirus and rotavirus antigens was investigated with a qualitative immunochromatographic test.The laboratory data of the cases were analyzed by using SPSS (Statistical Package for Social Sciences) 16.0 program.For statistical significance, p<0.05 was considered. Results:The viral antigens were positive for 744 out of 3763 (20%) patients.It has been identified that 549 of them (74%) are rotavirus, 53 of them (7%) are adenovirus and 142 of them (19%) are both rotavirus and adenovirus positive.Among those viral antigen positive patients, 355 (48%) of them were female, and 389 (52%) of them were male.It has been found that the antigen positiveness (% 84) is most frequent in the 0-2 age group and when being analyzed in terms of seasons, it is more frequent in winter and spring. Conclusion:In addition to parasitological and bacteriological examinations of stool samples, controlling the viral antigens is essential determining the etiological agent and treatment for childhood acute gastroenteritis infections.In the symptomatic treatment of viral gastroenteritis, we aimed to draw attention to the widespread use of rotavirus vaccine in the community to prevent the use of empirical antibiotics and to reduce rotavirus gastroenteritis.
Our aim was to investigate the protective role of ozone treatment against gentamicin-induced nephrotoxicity in an experimental rat model. In this study, a total of 30 rats were allocated in 5 groups (n=6 in each group). The control group (Group 1) received isotonic saline only, while Groups 2 and 3 received gentamicin at doses of 15 mg/kg/day and 50 mg/kg/day, respectively. In Group 4, intraperitoneal ozone treatment (1 mg/kg, 5% O3-95% O2) was performed after administration of gentamicin at a dose of 15 mg/kg/day. Group 5 underwent ozone treatment intraperitoneally following the application of gentamicin (50 mg/kg/day). Nephrotoxicity was formed by administration of glycerol. Serum levels of urea, creatinine, neutrophil-gelatinase-associated lipocalin (NGAL), lactate dehydrogenase (LDH), total antioxidant capacity (TAC) and protein carbonyl were measured, and kidneys were histopathologically examined after the sacrifice of animals on the 5th day. Group 4 displayed more favorable outcomes regarding biochemical markers of oxidative stress such as NGAL, LDH, creatinine, urea, TAC and protein carbonyl. Similarly, histopathological alterations indicating gentamicin-induced nephrotoxicity such as hemorrhage, the presence of protein casts and epithelial injury in renal tubules were less evident in Groups 4 and 5 which received ozone treatment. To conclude, results of this experimental study demonstrated that ozone treatment might ameliorate biochemical disturbances and histopathological alterations linked with gentamicininduced nephrotoxicity. However, further trials are warranted to document the actual therapeutic potential of ozone treatment in the clinical setting.
Abstract Background Renalase (RNL) is a controversial enzyme as to whether it oxidizes catecholamines (CAs) (as is generally accepted) in the blood or not. CAs (dopamine [DPMN], epinephrine [EPI] and norepinephrine [NEPI]) are associated with hypertension, including pregnancy-induced hypertension, which occurs in 8–10% of all pregnancies. Therefore, the aim of the study was to compare CAs and renalase concentration in (i) normotensive controls (C), (ii) patients with preeclampsia (PE) and (iii) patients with severe preeclampsia (SPE), which is one of the well-known symptoms of hypertension. Methods This case-control study involved 90 women divided into three groups – 30 C, 30 PE and 30 SPE – whose age and body mass indexes (BMIs) were similar. A total of 270 blood samples (90 maternal samples, 90 umbilical cord artery samples and 90 umbilical cord vein samples) were obtained. CAs and RNL concentrations of the biological samples were measured by enzyme-linked immunosorbent assay (ELISA). Results Comparing the amounts of CAs, RNL and systolic blood pressure (SBP)/diastolic blood pressure (DBP) between healthy control pregnant women and pregnant women with PE and SPE (SBP/DBP was 120/80 mm Hg for C, above 140/90 mm Hg for PE and above 160/110 mm Hg for SPE), the levels of CAs were significantly increased whereas RNL was reduced. The correlation between SBP/DBP and the amount of RNL in pregnant women with PE and SPE was negative. Conclusions These novel results are evidence that hypertension seen in PE and SPE is directly related to increased levels of CAs and reduced RNL concentrations. The use of RNL preparations may be preferred in future to prevent maternal and perinatal morbidity and mortality due to pregnancy-induced hypertension.
Pre-eclampsia is multisystem metabolic diseases, commonly accompanied by hypertension and proteinuria, which are among the important causes of maternal and perinatal mortality and morbidity worldwide. In a pre-eclampsia animal model study in the last year, Elabela (ELA) infusion was reported to correct hypertension and proteinuria and to normalise the birth weights of the offspring. Therefore, our main goal in this human study is to compare ELA, apelin (APLN) and nitric oxide (NO) levels in the maternal blood of pregnant women with pre-eclampsia and severe pre-eclampsia and in their newborns' venous-arterial cord blood with maternal blood of healthy pregnant women and their newborns' venous-arterial cord blood. Thirty controls, 28 pre-eclampsia and 24 severe pre-eclampsia cases and their newborns participated in this study. Maternal blood and newborn venous-arterial cord blood samples were collected from these patients. ELA, APLN and NO levels in these samples were measured by ELISA method. When the maternal blood ELA, APLN and NO amounts were compared with control groups, there was a significant decrease in both pre-eclamptic and severe pre-eclamptic women and this was more prominent in the women with severe pre-eclampsia. When ELA, APLN and NO levels in the newborn venous-arterial cord blood of control group was compared with that of severe pre-eclamptic and pre-eclamptic women; it was parallel with maternal findings. ELA, APLN and NO levels appear to play a role in the pathophysiology of pre-eclampsia. It is predicted that if these molecules, which are reduced due to pre-eclampsia and severe pre-eclampsia, are brought to physiological limits in the future; pre-eclampsia related maternal and perinatal mortality and morbidity can be reduced.Impact StatementWhat is already known on this subject? There are two studies (one human and one animal) in the literature evaluating only maternal elabela (ELA) levels in pre-eclamptic pregnancies. The animal study demonstrated decreased blood ELA levels in pre-eclamptic animals and the human study found increased blood ELA levels in pre-eclamptic patients. There are no studies evaluating maternal ELA levels in severe pre-eclampsia patients and also there are no studies evaluating maternal ELA levels in pre-eclampsia and severe pre-eclampsia patients. There are no studies evaluating newborns' venous-arterial blood APLN and NO levels. Apelin (APLN) and nitric oxide (NO) results were controversial in pre-eclampsia and severe pre-eclampsia patients.What the results of this study add? The present study, for the first time, demonstrates that decreased blood ELA, APLN and NO levels in maternal blood of pregnant women with pre-eclampsia and severe pre-eclampsia and in their newborns' venous-arterial blood. Furthermore, we have also demonstrated for the first time that decreased ELA, APLN and NO are also related with low birth weights.What the implications are of these findings for clinical practice and/or further research? The low levels of ELA, APLN and NO in maternal blood and newborns' venous-arterial blood may be the result or the cause of pathologic changes in pre-eclampsia and severe pre-eclampsia. Also, ELA, APLN and NO may be new indicator parameters of systemic endothelial dysfunction together. More studies are needed to evaluate the relationship between of ELA, APLN and NO and pre-eclampsia and severe pre-eclampsia and in newborns' venous-arterial blood.
Pathological pregnancies, such as gestational diabetes, preeclampsia, severe preeclampsia, intrauterine growth retardation and macrosomic fetuses, are among the most fundamental problems of obstetrics clinics that are risk factors for both mother and child. Our main goal here is to compare maternal blood and newborn venous-arterial cord blood asprosin levels in pathological and healthy pregnancies. The study included 30 pregnant women with gestational diabetes, 30 with preeclampsia, 30 with severe preeclampsia, 30 with intrauterine growth retardation, 29 with macrosomic fetuses and 30 healthy pregnant women. All mothers were voluntary participants. Arteries and venous blood samples from both mothers and newborns were taken, in which asprosin levels were measured by ELISA. There was a statistically significant increase in asprosin levels in pregnant women with gestational diabetes, preeclampsia, severe preeclampsia and macrosemic fetuses compared with the control group, whereas in those with intrauterine growth retardation a significant decrease was observed. Venous and arterial cord blood asprosin levels were also close to maternal asprosin levels. Regarding the asprosin levels in venous and arterial cord blood in all newborns, the former was higher, but was not statistically significant.
Background: Chemerin and dermcidin, which have antimicrobial properties, are molecules that are also related to insulin resistance and inflammation. Research aims: The aims were to determine the amounts of chemerin and dermcidin in the milk and blood of mothers with gestational diabetes, and to compare the amounts of chemerin and dermcidin in the milk and blood of mothers with and without diabetes. Methods: This was a two-group nonrandomized longitudinal study with a convenience sampling of mothers without gestational diabetes (n = 27) and mothers with gestational diabetes (n = 26). Human milk and blood samples were obtained from these mothers during colostrum, transitional, and mature milk periods. The amount of chemerin and dermcidin in these samples was measured by enzyme-linked immunosorbent assay. Results: The presence of chemerin and dermcidin was first detected in human milk. The amounts of chemerin and dermcidin in the blood of all the mothers were greater in the colostrum period and lowest in the mature period. The amount of chemerin and dermcidin in the milk of all the mothers was greater than that in the blood. The amounts of chemerin and dermcidin were significantly increased in both blood and human milk within the gestational age samples. Conclusions: Chemerin and dermcidin may contribute to the protection of infants from infections during infancy. Increased amounts of these molecules found within the gestational diabetes group may also prevent adverse maternal and fetal outcomes.
ABSTRACT Objective: The aim of the present study was to investigate the biochemical and histopathological impact of ozone treatment in an experimental model of osteomyelitis in rats. Methods: A total of 24 adult male Sprague-Dawley rats (3 months old, each weighing 300 to 400 g) were randomly allocated into three groups. Group I (n=8) served as a control and received no interventions or medications. In Group II (n=8), osteomyelitis was induced in the femur and no treatment was applied. Group III (n=8) received intraperitoneal ozone treatment for 3 weeks after the formation of osteomyelitis in the femur. Serum samples were taken to assess total antioxidant capacity (TAC), protein carbonyl content (PCO), and lactate dehydrogenase (LDH). Bone specimens obtained from the femur were histopathologically evaluated for inflammation, necrosis, osteomyelitis, and abscess formation. Results: Serum TAC levels were notably higher (p<0.001), while LDH levels were lower (p=0.002) in Group III than Group II. No significant difference was detected between groups with respect to PCO level. Similarly, Group III displayed more favorable histopathological outcomes with respect to osteomyelitis (p=0.008), inflammation (p=0.001), necrosis (p=0.022), and abscess formation (p=0.022). Conclusion: Ozone may be a useful adjunct treatment for osteomyelitis. Further studies in animals and humans are needed to clarify and confirm these preventive effects, understand the underlying pathophysiology, and establish guidelines. Diaper dermatitis is a common condition in childhood and topical steroid cream is used in the treatment.Iatrogenic Cushing's syndrome can be seen as a result of hypothalamic-pituitary-adrenal axis suppression due to long-term use and misuse of such steroids.We present a case of iatrogenic Cushing's syndrome in an eightmonth-old infant after the use of clobetasol 17-propionate 0.05% cream for about six months for diaper dermatitis.Steroid cream use should be queried in the history of the patients with Cushing's syndrome symptoms in infancy in particular and the family should be informed on the possible side effects of these creams and the month-old infant after the use of clobetasol 17-propionate 0.05% cream for about six months for diaper dermatitis.Steroid cream use should be queried in the history of the patients with Cushing's syndrome symptoms in infancy in particular and the family should be informed on the possible side effects of these creams and the necessary form and duration of use in detail.
Background: Intoxication with Atropa belladonna is endemic in Eastern Anatolia especially in springtime. However, in literature, there are no much studies about Atropa belladonna intoxication in childhood. Objectives: This study examines clinical and laboratory findings of pediatric Atropa belladonna intoxication cases. Methods: Hospital official database was searched for international classification of diseases code of intoxication. The inclusion criteria included individuals under the age of 15 and who had a history of Atropa belladonna ingestion. Results: The most frequent clinical findings were mydriasis, flushing, and meaningless speech. There was a significant rise in the white blood cell count, glucose, aspartate aminotransferase, and C - reactive protein levels. Conclusions: Atropa belladonna intoxication clinically causes anticholinergic syndrome and can result in deteriorations in some laboratory tests.
PURPOSE To compare the preventive effects of N-acetyl cysteine (NAC), ozone preconditioning and ozone treatment against contrast-induced nephropathy (CIN) in an experimental rat model. METHODS Thirty adult male Wistar rats were randomly distributed into five groups (n=6 for each group). Group I served as control and Group II had only contrast agent, while Group III received NAC and Group IV received intraperitoneal ozone 6 hours before and 6 hours after introduction of contrast agent. Ozone treatment was applied for 5 days after the contrast agent was introduced in Group V. After induction of CIN, groups were compared in terms of serum levels of urea, creatinine, neutrophil gelatinase associated lipocalin, protein carbonyl, total antioxidant capacity (TAC) as well as degree of renal injury at histopathologic level. RESULTS Groups II-V displayed more obvious histopathological alterations such as hemorrhage and renal tubular injury compared with Group I. TAC (p=0.043) and creatinine (p=0.046) levels increased significantly in Group II after the intervention. In Group III, protein carbonyl level diminished remarkably (p=0.046), while creatinine level was increased (p=0.046) following the intervention. TAC level was higher in Group IV (p=0.028) and Group V (p=0.026) following the procedure. CONCLUSION The N-acetyl cysteine and ozone treatment may alleviate the biochemical and histopathological deleterious effects of contrast-induced nephropathy via enhancement of total antioxidant capacity and decreasing oxidative stress.
