目的 对深圳市某区电子行业进行调查,分析该行业噪声职业健康风险和劳动者职业健康检查情况,为电子行业噪声危害的防控提供参考依据.方法 2015年5-9月,以深圳市某区8家中等规模的电子企业作为调查对象,检测8家企业共102个噪声岗位的噪声强度,以GBZ/T 229.4-2012《工作场所职业病危害作业分级第4部分:噪声》对噪声进行危害作业分级,并对2 772名劳动者职业健康检查和疑似职业病发病情况进行分析.结果 8家企业102个噪声岗位的噪声强度在73.4~96.1 dB(A)之间,噪声超标岗位有52个,超标率为51.0%.噪声危害作业分级在重度危害(Ⅲ)以下.劳动者职业健康检查检出疑似职业病6例(0.2%),职业禁忌证45例(1.7%),纯音听阈测试结果异常702人(25.8%),多家企业纯音听阈测试结果异常人数接近50%.结论 深圳市某区电子行业噪声职业病危害风险等级较高,危害形势严峻,需要高度关注.
目的 比较定性风险评估法、 接触限值比值法和综合指数法对蓄电池生产企业职业健康风险评估的结果,探索适合该类企业的职业健康风险评估方法.方法 以某中等规模铅酸蓄电池生产企业为研究现场,采用定性风险评估法、接触限值比值法和综合指数法进行职业健康风险评估并进行比较.结果 三种评估方法对包板/封盖岗位的铅尘危害评估结果均为3级(中等风险);定性风险评估法对极组/端子焊接岗位铅烟危害的评估结果为3级,接触限值比值法和综合指数法评估结果均为4级(高风险);定性风险评估法对加酸岗位的硫酸危害评估结果为2级(低风险),接触限值比值法和综合指数法评估结果均为3级;定性风险评估法和接触限值比值法对清洗岗位的氢氧化钠危害评估结果均为2级,综合指数法评估结果为3级;定性风险评估法对封胶岗位的乙酸丁酯危害评估结果为2级(低风险),接触限值比值法和综合指数法评估结果均为1级(可忽略风险).结论 与接触比值法和综合指数法相比,定性风险评估方法对无机化合物和铅烟的评估结果偏低,对有机溶剂类化学毒物的评估结果偏高;接触比值法和综合指数法评估结果相近,更适用于蓄电池生产企业的职业健康风险评估.
目的 探讨镉对职业接触者肾功能、氧化应激以及转录因子NF-E2相关因子2(nuclear factor E2-related fac-tor 2,Nrf2)的影响,为职业镉中毒的防治提供理论依据.方法 选取34名镉职业接触者作为研究对象,检测尿β2微球蛋白(β2-MG)、尿白蛋白(ALB)、尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、血尿酸和尿素氮等指标评价职业接触者肾功能损伤情况,测定超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)活力、丙二醛(MDA)与8-羟基脱氧鸟苷(8-OHdG)含量反映职业接触者氧化应激水平,检测蛋白激酶样内质网激酶(PERK)和Nrf2的蛋白含量分别反映职业接触者内质网应激状态与Nrf2转录因子的表达水平.结果 在测得的各尿、血镉浓度条件下,职业接触者尿β2-MG、ALB含量、NAG活力与血尿酸、尿素氮含量差异均无统计学意义(均P>0.05),但在3~5μg/L血镉水平条件下,职业接触者体内抗氧化酶SOD与GSH-Px活性显著增高(均P<0.05),而脂质过氧化产物MDA与DNA氧化修饰产物8-OHdG的含量明显降低(均P<0.05),而且在该条件下,PERK和Nrf2蛋白含量显著升高(均P<0.05).结论 在没有慢性轻度镉中毒的前提下,镉未引起职业接触者肾功能的损伤.但较小于3μg/L和大于5μg/L的血镉浓度,在3~5μg/L血镉水平条件下,镉触发了职业接触者内质网应激反应,并通过激活PERK介导的N rf2转录因子,诱导其下游抗氧化酶活性的升高,从而抑制了氧化损伤产物的生成,最终起到抗氧化作用.
Objective To investigate the effects of cadmium on PERK-Nrf2 signaling pathway in kidney of rats in cadmium-induced nephrotoxicity.Methods Twenty-four SD rats were randomly divided into four groups by weight:control group,intrap-eritoneally injected with 0.9% saline solution;the low-dose,medium-dose and high-dose CdCl2administration groups,intraper-itoneally injected with 5,10 and 20 μmol/kg CdCl2solution,respectively.After 48 hours,the rats were sacrificed to get the kid-ney,and then the mRNA expression of binding immunoglobulin protein(Bip),PKR-like endoplasmic reticulum kinase(PERK), nuclear factor E2-related factor 2(Nrf2),glutathione-s-transferase subunit-P1(GST-P1),glutamate cysteine ligase catalytic sub-unit(GCLC),heme-oxygenase(HO-1),NAD(P)H:quinone oxidoreductase(NQO1)were measured with real-time PCR and the protein expression of Bip,PERK,Nrf2 detected with Western blotting.Results Compared with the control group,the expres-sion of Bip,PERK,Nrf2 mRNA and Bip,PERK,Nrf2 protein in the 10 μmol/kg dose group were significantly increased(all P<0.05).There was a strong positive correlation between the expression of PERK protein and the expression of Nrf 2 mRNA at the CdCl2dose of 10 μmol/kg(P<0.05).The mRNA expression levels of GST-P1,GCLC,HO-1 and NQO1 in the 10 and 20 μmol/kg dose group were significantly higher than those in the control group(all P<0.05).There was a strong positive corre-lation between the mRNA expression of GCLC and HO-1 and the mRNA expression of Nrf2,while the expression of GST-P1 mRNA and NQO1 mRNA had a strong positive correlation with Nrf2 protein.Conclusion With a certain exposure dose,Cad-mium can induce the endoplasmic reticulum stress and activate the PERK-Nrf2 signaling pathway in kidney of rats,and then ini-tiate the Nrf2-mediated antioxidation to up-regulate the expression of the downstream phase Ⅱ enzymes.
目的 探讨企业在应用《正己烷职业危害防护导则》前后,工人对正己烷职业危害防护认知状况的改变,为用人单位做好正己烷危害防治工作提供科学依据.方法 基线调查后,采用现场知识培训以及微信公众号的方式干预,2周后再次进行相同的问卷调查.结果 干预后的工人对正己烷职业危害防护相关知识的知晓率明显提高,其中,“工作岗位禁忌证”、“含有正己烷溶剂”和“正己烷中毒环境”的正确认识方面,分别提高了64.3%、25.4%和24.4%;“接触途径”的知晓率提高了20.7%;“正己烷慢性中毒的典型症状”、“接触岗位”、“职业健康检查时间”、“个人防护措施”以及“应急救援”几个知识点的知晓率均有提高.在“个人防护用品”方面,知识知晓与态度形成、行为改变两两之间的干预前、后差异有统计学意义(P<0.05).结论 《导则》在职业接触正己烷的用人单位中具有一定的适用性,对预防正己烷职业危害发生有重要的指导作用.
目的 采用3种职业健康风险评估模型对深圳市某变压器生产企业职业病危害进行风险评估,探索适合变压器生产企业的职业健康风险评估方法.方法 调查深圳市某变压器生产企业职业卫生现状和主要化学有害因素种类,采用定性风险评估和半定量风险评估(接触限值比值法和综合指数法)进行风险评估,并对结果进行比较分析.结果 定性风险评估方法与半定量风险评估方法中的接触比值法和综合指数法相比,对化学有害因素中铅烟的风险评估结果偏低,但是对有机溶剂类化学毒物的风险评估结果总体偏高.接触限值比值法和综合指数法评估结果相近.结论 3种风险评估方法各有优缺点,评估结果也有差异.在变压器生产企业职业健康风险评估实践中,半定量风险评估方法更加适用.
为了解决原有的检测工作场所空气中金属及其化合物方法只能逐一分析,操作繁琐,分析周期长,建立了电感耦合等离子体质谱法(ICP-MS)同时测定工作场所空气中27种金属元素的方法.结果显示,待测元素在实验选定的范围内有良好的线性关系,r均>0.999;以采集75 L空气计,最低检出限均<1/100接触限值;各元素的平均回收率为95.2%~104.9% ,RSD为 1.32%~3.70% ,均符合GBZ 210.4-2008 的相关要求;测定多种有证质量控制标准物质(滤膜)的结果均在标准参考值±不确定度范围内.提示,ICP-MS法可作为工作场所空气金属元素检测的新方法.
目的 掌握宝安区疾病预防控制机构人力资源现状.方法 采用构成比的方式对宝安区疾控机构的编制数、常住人口人均拥有疾病防控人员数等人员数量信息以及年龄、性别、学历、职称、编制情况、岗位类别等人员结构信息进行描述性统计分析;对不同年份、不同机构级别等层次间的相应数据进行比较.结果 宝安区实际在岗疾病防控工作人员693人,其中54.5%属于临聘人员,常住人口的人均拥有在编人员数仅为1.1;区级机构"36岁至45岁"年龄段所占比例最高,为32.1%,35岁以及下人员占43.5%;高中初级职称人数比例总体为1.6:2.9:5.6,区级机构为2.9:3.1:4,街道级机构为1.2:2.8:6;以本科学历为主,本科及以上学历人员占65.7%,职称以初级为主,占46.5%;宝安区总体的专业技术人员比例为80.9%,低于深圳市的考核标准要求,管理人员占6.5%,工勤人员占12.6%;结论 宝安区疾病预防控制机构人员编制数不足,聘用人员所占比例过大,人员稳定性差,防控工作压力巨大,应控制总量,增加基层疾控机构人员编制实际用于人才引进;工勤人员比例偏高,不利于中心专业化发展,应大力加强专业技术人才队伍建设;区级疾控机构人员年龄偏大,应按照工作缺口及时补充年轻的专业技术人员形成合理的人才梯队;职称结构不尽合理,应确定科学的人才引进策略.
Cadmium,lead,arsenic and other heavy metals and metalloids are important environmental pollutants.Oxidative stress caused by metal pollutants is an important toxicological mechanism.The studies demonstrate that Nrf2 signal pathway is a central regulator of cellular anti-oxidative stress,which plays an important role in the defense response of cell mediated antioxidative stress.This paper mainly introduced the effects and mechanisms of cadmium,lead and arsenic on Nrf2 signal pathway,and briefly summarized the effects of zinc,chromium and copper on the Nrf2 signaling pathway in the process of toxic effects.
Objective To explore the effects of nuclear factor E2-related factor 2(Nrf2)on the oxidative stress induced by cadmium chloride(CdCl2)in human embryonic kidney epithelial(HEK)cells.Methods HEK cells in the exponential phase were incubated with a final concentration of 0,2.5,5,10,or 20 μmol/L CdCl2 for 24,48 or 72 hours in vitro.After cells were harvested,the relative expression level of Nrf2 mRNA was detected by real-time polymerase chain reaction(real-time PCR),and Western blotting was applied to measure the relative translation level of Nrf2 nuclear protein.Results The SOD activity and GSH-Px activity were significantly decreased in HEK cells after 48 h exposure to 20 μmol/L CdCl2 (P<0.05),and the GSH-Px activity tended to increase at 72 h.The MDA content was significantly higher after 72 h exposure to 10 or 20 μmol/L CdCl2 than in the control group at same time point and in 24 h CdCl2 treatment group(P<0.05).There was a significant change in the relative expression level of Nrf2 mRNA at 10 or 20 μmol/L CdCl2 for 48 or 72 h(P<0.05).The relative translocation level of Nrf2 nuclear protein was significantly increased in HEK cells treated with 20 μmol/L CdCl2 for 48 or 72 h(P<0.05).After 72 h exposure to 20 μmol/L CdCl2,a positive correlation was found between the GSH-Px activity or MDA content and the relative translation level of Nrf2 nuclear protein in HEK cells(P<0.05).Conclusion Cadmium can inhibit the oxidative stress damage by inducing the activation of Nrf2 signaling factor while causing oxidative stress in HEK cells.The mechanism may involve the promotion of the expression of antioxidant enzymes.
Objective To establish a methodology for determining indium in human whole blood , serum and urine by inductively coupled plasma-mass spectrometry ( ICP-MS ) .Methods The whole blood , serum and urine samples were diluted 10 times in 0.01%(mass fraction) Triton X-100 plus 0.50%(mass fraction) nitric acid solution, and the indium level was determined by ICP-MS.Rhodium standard solution was used as the internal standard control .Results The working curve obtained from measurement of whole blood , serum and urine of normal individuals was compared to the standard curve and showed no significant difference in quantitative analysis ( P>0.05 ) .The linearity range of indium concentration in whole blood , serum and urine was 0.000-20.000 μg/L, and all the correlation coefficients were greater than 0.999 with a detection limit of 0.144 μg/L.The recovery rates of whole blood , serum and urine were 87.90%-95.92%, 91.50%-94.20%and 90.40%-96.57%, respectively .The relative standard deviations ( RSDs) of within-run precision were 3.81%-7.05%, 3.75%-5.90% and 4.31%-6.62%, respectively .The RSDs of between-run precision were 2.90%-7.10%, 3.80%-5.92% and 4.16%-5.94%, respectively .Samples could be stored for at least 14 days under the temperature of -20 ℃.The indium in whole blood , serum and urine of workers occupationally exposed to indium (exposure group, 135 person-time) and control group workers (120 person-time) were examined.Indium was detected for 17 person-time in whole blood and serum in the exposure group with a detection rate of 1.26%.Indium was not detected in urine samples in exposure group .It was not detected in all samples in control group .Conclusion This methodology has features of simple operation , high accuracy and good precision , which is suitable for the accurate quantitative analysis of indium in biological samples .
通过系统培训提升健康教育工作人员专业素质是保障健康教育工作顺利进行的根本手段,而建立一套能够客观反映培训问题、评测健康教育人员专业化培训效果的培训评估体系,是加强健康教育人才队伍建设的关键。本文借鉴柯氏四级评估模型和CIPP模型从培训前、培训中、培训后三个阶段构建科学、完善、综合的健康教育人员专业化培训效果评估模型,为公共卫生机构建立科学、有效的健康教育人员培训策略提供参考。
目的 按照职业卫生标准制定指南建立合适的工作场所中氨的采样方法,并探讨采样和分析方法能很好对接的工作场所空气中氨的测定方法. 方法 选用不同的采样介质,考察其采样效率,解吸效率、分析溶液对后续测定的影响;并考察建立的方法技术指标与标准要求的符合性. 结果 通过筛选发现,用酸性硅胶管、甲烷磺酸吸收液的短时间采样和长时间采样效率均大于95%,用甲烷磺酸解吸液解吸,平均解吸效率为91.1%;解吸液/吸收液采用分光光度法和离子色谱法分析的RSD均<4%.用2种方法对标样的测定结果都在给定的不确定度范围内.采用分光光度法和离子色谱法测定某电镀厂采集的吸收液样品,两种方法的测定结果经统计学检验,差异无统计学意义(P>0.05).硅胶管采集的样品可以保存14d,采样后的吸收液可以保存7d. 结论 采用装有5 ml甲烷磺酸吸收液的大型多孔玻板吸收管和酸性硅胶管都可以采集工作场所空气中氨,纳氏试剂分光光度法和离子色谱法都可以用来对溶液中的氨进行定量.但考虑到对仪器的保护,采用离子色谱法检测时宜采用吸收液采样,而分光光度法检测时则两种采样方法均可使用.
本文针对目前健康教育工作领域培训存在的问题,借鉴其他领域有益的培训策略和先进的培训模式,提出从纵向和横向结构上构建起立体式、多角度、联动式的培训组织体系,实现培训资源利用效率的最大化;厘清健康教育工作体系的4类培训对象,实现培训的全覆盖;根据区域或机构的公共卫生任务、不同学历层次、专业水平的培训对象的需求确定具体的培训内容,选择恰当的培训组织形式和教学方式以保证培训效果的最优。加强培训制度化建设,以建立起科学、系统、有效的健康教育人员培训模式。
目的 对深圳市某蓄电池生产企业化学毒物职业危害进行风险评估,为蓄电池生产企业职业病危害关键控制点的风险管控提供依据.方法 调查某蓄电池生产企业职业卫生现状和主要化学毒物种类,采用新加坡有害化学物质职业暴露半定量风险评估方法进行风险评估.结果 该蓄电池生产企业中包板、装外壳、封盖、极组焊接、极组串联和端子焊接等岗位的铅尘、铅烟为高风险水平,加酸、清洗、封胶等岗位的硫酸、氢氧化钠和乙酸乙酯属于低风险水平.结论 该蓄电池生产企业在目前的工作条件下,所存在的化学毒物可能对劳动者身体健康产生不良影响,其中需重点关注产生铅尘、铅烟危害的工作岗位,应根据不同风险水平采取不同的控制措施.
目的 建立电感耦合等离子体质谱(ICP-MS)半定量技术快速筛查工作场所空气中多种金属元素的方法.方法 用微孔滤膜采集工作场所空气中金属元素及其化合物,滤膜经酸消解定容,通过蠕动泵和三通混合器在线加入内标溶液后,使用ICP-MS半定量技术测定.结果 待测元素在实验选定的范围内(0~1 000 μg/L)具有良好的线性关系,r均大于0.999;以采集75 L空气计最低检出限均<1/100接触限值;各元素的平均回收率在83.9%~ 110.6%之间,RSD在1.32%~3.70%之间;测定多种有证质量控制标准物质(滤膜)的结果均在标准参考值±2个不确定度范围内;同时应用该法和原子吸收法测定某工厂工作场所空气中金属元素,该法的测定值为原子吸收法测定值的70%~120%.结论 可以为工作场所空气中金属元素监测和突发化学中毒事故中金属毒物及其相关化合物的快速筛查提供新的检测技术手段.
目的 建立职业接触者尿汞的直接测汞仪方法.方法 尿液直接采用热分解齐化法原子吸收法进行检测.结果 方法的线性范围为1.6 μg/L~100.0 μg/L,回归方程为y=0.005 08x +0.002 8(r =0.999 6);批内相对标准偏差为0.53%~ 1.56%,批间相对标准偏差为1.51% ~3.57%;3种浓度的尿样的加标回收率为91.6% ~ 100.0%;方法的检出限为0.48 μg/L,定量限为1.59 μg/L.最佳仪器条件:尿样在200℃干燥2 min,1.5 min内干燥温度升至650 ℃,最后灰化2 min.本研究优化的常见金属对本研究结果无干扰.结论 该方法操作简便快速,各项指标可满足职业卫生尿汞监测的相关要求.
目的 识别、评价和预测某公司印刷品生产建设项目在生产过程中可能产生的职业病危害因素的种类、危害程度及职业卫生防护设施防护效果,论证该项目职业卫生措施的可行性.方法 依据国家相关法规和标准,采用类比法、检查表法和工程分析法进行定性与定量评价.结果 对类比工程正己烷、二氯甲烷、异丙醇、甲醇、甲苯、二甲苯、噪声、紫外辐射等危害因素进行检测,各化学毒物检测结果均符合国家职业接触限值要求,合格率为100%;紫外辐射测量结果合格率为25%,噪声测量结果合格率为100%.拟建项目可能产生的职业病危害有正己烷、正戊烷、异戊烷、正庚烷、二氯甲烷、甲醇、异丙醇、甲苯、二甲苯、乙苯、异佛尔酮、环己酮、乙醛、臭氧、紫外辐射及X射线.预计该项目除紫外辐射强度外,其他职业病危害因素水平符合国家职业卫生接触限值的要求.结论 该项目为职业病危害较重的建设项目,项目拟采取的职业病危害防护设施和控制措施符合国家职业卫生要求.
目的 对某拉链生产项目产生的职业病危害因素的种类和程度进行识别和判断,分析和评价职业病危害防护措施及效果.方法 根据现行有效法律法规和标准规范的要求,采用职业卫生现场调查、职业卫生检测、职业健康检查、检查表分析法等方法,对该项目进行评价.结果 该企业在生产过程中存在的职业病危害因素有化学毒物(苯、甲苯、二甲苯、环己酮、正己烷、异丙醇、氨、氰化物、氢氧化钠)、粉尘(其他粉尘)、噪声;检测结果:化学毒物、粉尘浓度均符合国家职业接触限值要求,噪声存在超标现象,合格率为58%.结论 该建设项目职业病危害风险定性为较重项目,在确保各类职业病防护设施正常有效运行情况下,并落实控评报告补充措施和建议情况下,该建设项目职业病危害控制措施是可行的.
Objective To explore an improved method for determination of cyclohexane and methylcyclohexane in workplace air by solvent desorption-gas chromatography.Methods Cyclohexane and methylcyclohexane in workplace air were collected by activated carbon tubes, desorbed with carbon disulfide, separated by DB-1 capillary chromatography column, detected by flame ionization detector and quantified using the standard calibration curves.Results The linear range of the concentration of cyclohexane and methylcyclohexane were 1.0-1 402.2 and 0.8-1 999.4 mg/L respectively. Both the correlation coefficients were 0.999 9.Both the detection limits were 0.3 mg/L.The limits of quantification were 1.0 and 0.8 mg/L respectively.Both the minimum detectable concentrations were 0.2 mg/m3 .The minimum quantitative mass concentrations of cyclohexane and methylcyclohexane were 0.7 and 0.6 mg/m3 respectively ( sample volume was 1.5 L) .The average desorption efficiencies were 98.5%-99.3% and 97.6%-99.0% respectively.The relative standard deviations ( RSD) of within-run precision were 0.36%-0.59%and 0.34%-0.50%respectively.The RSD of between-run precision were 0.89%-2.04% and 0.87%-2.22% respectively.The samples could be stored for up to 7 days at room temperature.Conclusion This method has features of simple operation, high sensitivity and good precision, which is suitable for simultaneous determination of cyclohexane and methylcyclohexane in workplace air.
