The two far ends of the age at the diagnosis of breast cancer are the age of younger than 35, and that of older than 70. Most probably, these two groups of patients differ in many ways. The aim of our present study was to underline the fact that age at the diagnosis of breast cancer is indeed a prognostic factor. Between October 1995 and March 2009, 80 old and 51 young breast cancer patients were treated at the Department of Diagnostic Radiology and Oncotherapy, Semmelweis University, Budapest. The prognostic and predictive factors of the tumors were analysed together with the disease-free and overall survival data. There were statistically significant differences between the two groups concerning the menstrual and reproductive factors, histological characteristics and immunophenotype of the tumors. Tumor size, nodal status and the Nottingham Prognostic Index did not show statistically significant differences. A trend to a shorter disease-free survival, higher rate of distant metastases and disease-specific death was seen in the group of young patients, but it was not significant. Overall survival was significantly shorter in the group of young patients. Therefore, we can state that young patients have a more aggressive disease and worse outcome. There is an increased importance of self examination in these groups, since both age groups are beyond the age limits of the screening population in Hungary. The media and primary school education as well should be involved in educating women concerning this aspect. The individual follow-up of young patients with positive family history should also be established.
Absztrakt Az emlőrák diagnózisakor betöltött életkor alapján a két szélsőséges, egymástól várhatóan legjobban különböző csoportot a 35 évnél fiatalabb és a 70 évnél idősebb betegek csoportja képezi. A jelen vizsgálat célja e két csoport összehasonlításán keresztül annak alátámasztása, hogy az életkor prognosztikai faktornak tekinthető a vizsgált betegek esetében. A Semmelweis Egyetem Radiológiai és Onkoterápiás Klinikáján 1995. október és 2009. március között kezelt 131 nőbeteg (80 idős és 51 fiatal) adatai kerültek feldolgozásra a diagnózis idején vizsgált prediktív és prognosztikus faktorok felhasználásával. Emellett vizsgáltuk a betegek teljes- (OS) és betegségmentes túlélését (DFS) is, valamint az ezeket befolyásoló faktorokat. Statisztikailag szignifikáns eltérés adódott a két csoport között a menarche időpontja, a reproduktív faktorok, a tumor hisztológiai jellemzői és immunfenotípusa tekintetében. A tumorméret, a nyirokcsomóstátusz és a Nottingham Prognosztikai Index (NPI) értékek tekintetében nem mutatkozott szignifikáns eltérés a két csoport között. Annak ellenére, hogy a fiatal betegek között több volt a vizsgálat végéig kialakult áttétes betegség, a betegségmentes túlélés tekintetében nem tapasztaltunk szignifikáns különbséget. A teljes túlélés azonban szignifikánsan hosszabbnak bizonyult az idős páciensek csoportjában. A fenti eredmények az irodalmi adatokkal egybehangzóan arra utalnak, hogy a fiatal nőknél a betegség rosszabb prognózisú, agresszívebb és rapidabb a klinikai lefolyás. Mivel egyik csoport sem tartozik a hazánkban szervezetten szűrt populációba, igen nagy jelentősége lenne az önvizsgálat (már iskolás korban történő) oktatásának, a tömegkommunikáció adta lehetőségek szélesebb körű kihasználásának, a pozitív familiáris anamnézissel bíró nők (különös tekintettel a fiatalok) esetében az egyéni követés megoldásának.
BACKGROUND:PST of locally advanced breast carcinomas causes tumour shrinkage and down-staging, therefore, optimal circumstances for surgery.PATIENTS AND METHODS:We treated 25 stage IIIA-IIIB breast cancer patients with PST (Doxorubicin/Docetaxel). Histological diagnosis (core biopsy) was available in each case. Tumour regression and cardiac function were recorded regularly.RESULTS:Five patients showed complete pathological remission. Instead of 19 mastectomies, only nine were performed and 16 patients underwent breast-conserving therapy.CONCLUSION:Using PST breast conservation rate is improved.
