As a physician with a strong clinical background in radiation oncology, I have concentrated my research on gynecology and genitourinary cancer, areas in which I received specialized training during my clinical research fellowship at Princess Margaret Hospital.
My primary goal has been to advance the field of radiation therapy through translational research and technology transfer, effectively bridging the gap between laboratory concepts and phase I/II clinical trials. Through my clinical research in gynecological and genito-urinary cancers, I have identified areas that required improvement (1-4), particularly in pelvis organ tissue preservation (2, 4) and radiation dose escalation for specific tumors (1). One notable example is my study on prostate cancer, where I demonstrated the safety and efficacy of dose escalation to the dominant intra-prostatic tumor (5), key to this project was the interdisciplinarity with urologists, radiologists, nuclear medicine physicians, physicists, and specialized nurses. Similarly, in my gynecological cancer patients, it was crucial to develop innovative methods for sparing organs-at-risk (2), such as the active bone marrow, as some patients suffer from chronic anemia (3). By implementing these techniques, I contributed to enhancing the quality of care for my patients.
My expertise in these advancements was acknowledged by the European Organisation for Research and Treatment of Cancer (EORTC), resulting in my appointment as the chairman of the Gynecology Cancer Group.
During my doctoral studies, a significant breakthrough emerged when I implemented low-dose irradiation (radiation below the threshold for generating substantial DNA damage, as low as 1 Gy) in models of peritoneal disseminated ovarian cancer. Remarkably, these very low doses transformed immunologically "cold" tumors, characterized by the absence or exclusion of T cells, into "hot-inflamed" tumors, which are indicative of a favorable response to immunotherapy. The culmination of this pioneering work performed in the context of the Ludwig Institute for Cancer Research – Prof. Coukos Lab was a successful first-in-human clinical study with paired tumor biopsies taken from the irradiated areas. This groundbreaking research was published in the prestigious journal Cancer Discovery (6).
This initial clinical translation's success prompted me to establish my own research group. My laboratory has recently provided evidence of the potential of FLASH radiotherapy to mitigate radiation-induced damage in healthy tissues, specifically the bowel (7). The implications of these findings are profound and extend beyond gynecological tumors, as we could improve the feasibility and efficacy of radiation treatments for numerous other tumor types. Compared to conventional dose-rate radiotherapy, the combination of the tissue-sparing effect and the reduced duration of FLASH radiotherapy provides patients with a remarkable advantage (8). However, additional characterization is required to better comprehend this benefit and facilitate its clinical implementation by combining it with surgery, chemotherapy, targeted therapy, and immunotherapy. My objective is to unlock the full potential of FLASH radiation and optimize its integration into early phase clinical trials with relevant translational biology in order to offer patients new hope and better outcomes.
