I work in the fields of bioinformatics, epidemiology and in the use of large data for research. I have worked with electronic health record data since 1993 when I worked with, and then was Director of, the Center for Disease Control’s Vaccine Safety Datalink Project studying longitudinal health records covering 6% of all children in the U.S. <6 years of age. In November, 2000, at an FDA (CBER) meeting, ’Evaluation of New Vaccines: How Much Safety Data?’ arranged in response to the then-current findings of an increased risk for intussusception following RotaShield (rotavirus) vaccination and increased risk for myocardial infarction associated with rofecoxib, both observed shortly after licensure and widespread use, a group of us proposed the use of sequential analyses to rapidly address important public health questions related to drug safety and vaccine safety. Our team, comprised of myself, Martin Kulldoff PhD (Harvard Medical School), and Margaret Kolczak PhD (CDC) developed and then implemented ‘sequential probability ratio testing’ for both the Vaccine Safety Datalink and the mini-Sentinel’, and we led the work that demonstrated the feasibility of evaluating large population-based data on an ongoing basis, with daily, weekly or monthly data collection and analyses to be used for public health surveillance. This approach was critical to the evaluation of potential safety risks of two newly licensed vaccines, Menactra and MMRV, and the approach was adopted by the CDC for its ongoing Vaccine Safety Datalink Project, and the by the FDA for drug safety surveillance efforts as part of the Sentinel Initiative Project.
My research at UTHSC has focused on applications of artificial intelligence in medicine, and have published on the application of AI for early identification of Parkinson’s disease, early diagnosis of sepsis in hospitalized patients, early recognition of kidney failure and of organ failure in sickle cell disease, and prediction of cardiomyopathy among childhood cancer survivors. I am also PI of the UTHSC 100K Genomes Project, a fully consented DNA biorepository linked to longitudinal health data, spanning the state of Tennessee.
