Sensory chemotherapy-induced peripheral neuropathy (CIPN) is well-recognized, but motor CIPN remains understudied. This secondary analysis focused on the long-term severity and impact of motor disorders, their relation to sensory CIPN, neuropathic pain, psychological distress, and health-related quality of life (HRQoL) after oxaliplatin-based chemotherapy in colorectal cancer (CRC) survivors. Data from a multicenter, cross-sectional study were re-analyzed to explore motor CIPN among CRC survivors up to 5 years post-chemotherapy, with no longitudinal follow-up. OBJECTIVE:Executive impairments are frequent and may concern a large spectrum of health conditions throughout life. Given the complexity of the executive functions, their assessment requires the administration of multiple tests. There is only one source of French-language normative data for seven traditional executive tests for adults under age 50. The aim of the present study was to establish demographically adjusted normative data of four executive tests commonly used during the neuropsychological assessment in France for adults aged 18-65.METHOD:The following tools were administered to 518 community adults aged from 18 to 65: Digit Span, Trail Making Test, Color Word Interference Test, and Verbal Fluency Test. An Overall Test Battery Mean was computed. Multiple regressions were computed for normally distributed scores and percentiles were established for non-normally distributed scores.RESULTS:Multiple regression analyses indicated that younger age and higher education were both associated with better performance. Age did not predict the number of correct responses on the Verbal Fluency Test. Gender did not have any effect on executive performances. Regression equations to calculate Z-scores are presented. Percentiles are presented for the number of recorded errors on the Trail Making Test, Color Word Interference Test, and Verbal Fluency Test.CONCLUSIONS:We provide reliable and updated norms for four executive tests that are among the most used by clinical neuropsychologists in France. Our work represents a valuable addition to the limited norms currently available for the assessment of executive functions in French young and middle-aged adults.
L’hypertension intracrânienne (HTIC) idiopathique est plus fréquente chez les femmes, en cas de surpoids et d’hyperandrogénie. Nous rapportons un cas d’HTIC chez un patient transgenre F to M sous hormonothérapie masculinisante. Patient âgé de 20 ans, avec un IMC a 23 kg/m2, accompagné depuis 2020 par l’équipe transidentité. Il est sous ANDROTARDYL depuis mars 2022 : une demie ampoule toutes les 4 semaines puis majoré à 1 ampoule depuis novembre 2022. L’hormonothérapie est bien tolérée sur le plan clinique et biologique. Contexte de polyaddition sevré depuis juin 2022 et traitement en cours par METHADONE. En décembre 2022, le patient a présenté une diplopie, des maux de tête et des vomissements. Une HTIC a été confirmée par les examens et était à l’origine d’un œdème papillaire. Une HTIC idiopathique était retenue devant un bilan étiologique négatif. Une enquête de pharmacovigilance a évoqué le rôle probable de l’ANDROTARDYL. Bonne évolution clinique sous DIAMOX et arrêt de l’hormonothérapie. La revue de la littérature retrouve des cas d’HTIC idiopathique chez des patients sous testostérone, notamment des personnes dans des parcours de transition F to M [1]. Une des hypothèses est la présence de récepteurs aux androgènes sur les plexus choroïdes, d’où l’augmentation de production de liquide céphalo-rachidien en cas d’hyperandrogénie. Les patients concernés doivent bénéficier d’un accompagnement diététique et d’une adaptation de la posologie d’ANDROTARDYL.
Les hyper androgénies de la femme ménopausée sont rares, un taux de testostérone totale supérieur à 1,5 ng/mL évoque une cause tumorale surrénalienne ou ovarienne. Patiente de 59 ans, ménopausée à 46 ans ayant comme antécédents : colectomie totale puis amputation rectale pour maladie de Crohn, ovariectomie gauche pour kyste bénin. Les symptômes d’hyper-androgénie sont d’apparition progressive depuis trois ans et modérés, principalement une alopécie. Testostérone totale à 2,9 ng/mL, 17OHP à 9,4 ng/L, stimulée par le SYNACTHENE à 80 ng/L. Sulfate DHEA 0,8 μmol/L. FSH, LH de type ménopausique. Absence d’hypercorticisme au vu du test de freinage et du cycle du cortisol physiologique. ACTH 14 ng/L. Dépistage génétique de l’hyperplasie congénitale des surrénales : mutation homozygote mineure CYP21A2, habituellement très peu ou pas symptomatique. Test de suppression par DECAPEPTYL 11,5 mg une injection trimestrielle, avec freinage significatif de la testostérone qui chute à 0,14 ng/mL à 1 mois puis 0,6 ng/mL à deux et trois mois. Le scanner surrénalien écarte un adénome. L’IRM ovarienne est normale. Probable hyper-androgénie d’origine ovarienne, de type tumoral, diagnostiquée parallèlement à un bloc en 21-hydroxylase. Cette hyper androgénie est très sensible à l’action de freinage par DECAPEPTYL argumentant pour l’origine ovarienne. Probable tumeur à cellules de Leydig difficile à mettre en évidence en IRM. L’indication d’ovariectomie n’est pas retenue actuellement au vu des multiples interventions pelviennes subies par la patiente rendant le geste chirurgical complexe. Devant l’efficacité clinico-biologique du DECAPEPTYL, ce traitement est maintenu.
To investigate the prevalence of neuro-functional disability and its determinants 12 months after community-acquired bacterial meningitis (CABM) in adult patients. In a prospective multicenter cohort study (COMBAT), all consecutive cases of CABM were enrolled and followed up for 12 months. Neuro-functional disability at 12 months was evaluated using a combination of the Glasgow Outcome Scale (functional disability), and the modified Rankin Disability Scale (physical disability). Factors associated with neuro-functional disability were identified by multivariate logistic regression. Among 281 patients, 84 (29.9%) patients exhibited neuro-functional disability at 12 months: 79 (28.1%) with functional disability and 51 (18.1%) with physical disability. Overall, 6 patients (2.1%) died during the follow-up. The most common pathogen identified was Streptococcus pneumoniae (131/272, 48.2%); 77/268 patients (28.7%) had a physical disability at hospital discharge. Factors independently associated with 12-month neuro-functional disability were a pneumococcal meningitis (adjusted OR = 2.8; 95% confidence interval (CI) = [1.3; 6.7]), the presence of a physical disability at hospital discharge (aOR = 2.3; 95%CI = [1.2; 4.4]) and the presence of behavioral disorders at hospital-discharge (aOR = 5.9; 95%CI = [1.6; 28.4]). Dexamethasone use was not significantly associated with neuro-functional disability (OR = 0.2; 95%CI = [< 0.1;1.3]). Neuro-functional disability is frequently reported 12 months after CABM. Detailed neurological examination at discharge is needed to improve the follow-up. NCT01730690.
