RÉSUMÉIntroduction. Le cancer et ses traitements peuvent altérer la sexualité, dimension essentielle de la qualité de vie souvent négligée en pratique clinique. En Afrique subsaharienne, les données sur l'impact du cancer sur la fonction sexuelle restent limitées. Cette étude avait pour but de décrire les troubles de la sexualité et leur évolution avant et après le diagnostic de cancer chez des patients suivis à Abidjan. Méthodes. Nous avons mené une étude transversale descriptive à recueil comparatif intra-individuel, du 1er décembre 2023 au 31 janvier 2024, au CHU de Treichville. Les patients adultes, en couple et sous traitement anticancéreux, ayant donné leur consentement éclairé, ont été inclus. Les paramètres de sexualité avant le diagnostic (recueil rétrospectif) ont été comparés à ceux observés après le diagnostic. Les données ont été analysées avec SPSS 21 ; les proportions ont été comparées par le test du Chi² ou de Fisher, avec un seuil de significativité à p<0,05. Résultats. Cent patients ont été inclus (âge moyen : 48,5 ans ; 66% de femmes). Les cancers gynéco-mammaires prédominaient (46%). Après le diagnostic, une diminution significative du désir sexuel était observée (86% vs 32%, p=0,033), ainsi qu'une augmentation de l'abstinence (17% vs 62%, p<0,001). La qualité des rapports (74% vs 31%, p<0,001) et la satisfaction du partenaire (90% vs 36%, p<0,001) étaient altérées. Chez les hommes, les troubles érectiles augmentaient (11,8% vs 41,2%, p=0,033). Aucune différence significative n'était observée pour la sécheresse vaginale et la dyspareunie. Conclusion. Le cancer et ses traitements altèrent significativement la sexualité des patients, affectant le désir, la fréquence et la qualité des rapports, ainsi que la satisfaction du partenaire. L'intégration systématique de la santé sexuelle dans la prise en charge oncologique s'impose. ABSTRACT Introduction. This study aimed to describe sexual dysfunction and its evolution before and after a cancer diagnosis among patients followed in Abidjan. Method. This was a descriptive cross-sectional study with comparative data collection, conducted at Treichville University Hospital from December 1, 2023, to January 31, 2024. Adult patients in a relationship and undergoing anticancer treatment, who provided informed consent, were included. Sexuality parameters before diagnosis (retrospectively collected) were compared with those observed after diagnosis. Results. A total of 100 patients were included (mean age: 48.5 ± 12.5 years; 66% female). Gynecological and breast cancers accounted for 46% of cases. After diagnosis, a significant decrease in sexual desire was observed (86% vs 32%, p = 0.033), along with an increase in abstinence (17% vs 62%, p < 0.001). The quality of sexual intercourse (74% vs 31%, p < 0.001) and partner satisfaction (90% vs 36%, p < 0.001) were impaired. Among men, erectile dysfunction increased (11.8% vs 41.2%, p = 0.033). No significant differences were observed for vaginal dryness and dyspareunia. Conclusion. Cancer and its treatments significantly impair patients’ sexuality. Integrating sexual health into oncological care is essential.
Introduction. Le syndrome d'Eagle, lié à l'élongation anormale de l'apophyse styloïde ou à la calcification du ligament stylo-hyoïdien, est une cause fréquente mais sous-diagnostiquée de douleurs cervico-faciales. Bien que plusieurs voies chirurgicales aient été décrites, leur comparaison en contexte africain reste peu documentée. Cette étude avait pour objectif de décrire la principale technique opératoire et rapporter les résultats à Abidjan. Matériels et méthodes. Nous avons mené une étude rétro-prospective descriptive du 1er janvier 2007 au 31 décembre 2023 dans les services ORL de deux centres hospitaliers universitaires et des structures privées à Abidjan. Les patients adultes présentant un syndrome d'Eagle symptomatique avec une apophyse styloïde >3 cm confirmée par imagerie et ayant bénéficié d’une styloïdectomie par voie endobuccale avec amygdalectomie préalable ont été inclus. Les paramètres principaux étaient la technique opératoire, les complications postopératoires et la disparition des symptômes à 90 jours. Résultats. Sur 250 patients inclus (âge moyen 40 ans, ratio H/F 0,4), la symptomatologie était unilatérale dans 60% des cas. La longueur moyenne des apophyses était de 4,36 cm. Les suites ont été simples dans 96,4% des cas. Une rhinolalie a été observée chez 3,6% des patients. À 90 jours, 88,6% des patients présentaient une disparition complète des symptômes. Conclusion. La styloïdectomie par voie endobuccale constitue une approche efficace et sûre pour la prise en charge du syndrome d'Eagle symptomatique en Afrique subsaharienne, avec un taux élevé de succès symptomatique et une morbidité faible.
BACKGROUND:Extraforaminal lumbar disc herniations and foraminal stenosis are challenging due to nerve root proximity. Conventional lateral transforaminal endoscopy requires a long oblique trajectory and is obstructed by the iliac crest at L5-S1. We describe a full-endoscopic monoportal Wiltse paraspinal technique with inferior foraminal docking and its preliminary outcomes. METHODS:Ten consecutive patients (4 women, 6 men; mean age 61.1 years, range 50-76) were prospectively analyzed between September and December 2025. Pathologies comprised 7 extraforaminal disc herniations and 3 foraminal stenoses at L3-L4, L4-L5, and L5-S1 (2, 3, and 5 cases, respectively). Through a paraspinal incision 4-6 cm from midline, dilatation of the multifidus-longissimus interval allows inferior bony docking on the caudal transverse process before foraminal recalibration. VAS leg/back pain, ODI, and satisfaction were assessed preoperatively and at 3 months. RESULTS:No intraoperative complication, conversion, or postoperative instability occurred. Mean VAS leg pain decreased from 7.9 to 2.8 (-65%); VAS back pain from 7.4 to 4.1; ODI from 55.2% to 28.7% (-48%). All 4 patients with preoperative neurological deficits improved. Satisfaction was very satisfied (4, 40%), satisfied (5, 50%), and undecided (1, 10%), yielding 90% satisfactory outcomes. At L5-S1, the iliac crest never obstructed the trajectory. CONCLUSIONS:The full-endoscopic monoportal Wiltse approach with inferior foraminal docking is a feasible, safe, and coherent technique for lateral lumbar disc pathology and foraminal stenosis. Its principal advantage is that it avoids the risk of exiting nerve root injury inherent to the percutaneous targeting of the transforaminal approach. These preliminary results support further prospective comparative studies.
Background:Central nervous system tumors present significant diagnostic and therapeutic challenges, as WHO histomolecular prognostic criteria are not always optimal. Decision-making increasingly relies on multidisciplinary team meetings (MDTs), which require highly qualified specialists. We aimed to assess available human resources and propose a framework for subregional collaboration through a West African neuro-oncology MDT. Methodology:A multicenter descriptive study was conducted via an online questionnaire from April 13 to June 15, 2025, involving 35 healthcare professionals from nine West African countries. Results:Participants included 45.7% neurosurgeons, over 57% with <3 years' experience. Most managed <100 patients/year, and 90% performed <100 tumor excisions. Awake surgery and stereotactic biopsy were rare. Access to chemotherapy, pathology, immunohistochemistry, and molecular biology was reported by 83%, 80%, 51%, and 14% of respondents, respectively. Median turnaround time was about 3 weeks for histopathology and about 1 month for immunohistochemistry. Clinical trial access was available to 26%. Neurosurgeon density was <1 per million inhabitants, peaking in Senegal (1 per 700,000). Only 34% reported functional MDTs. Nearly all participants (95%) supported establishing a subregional network with regular online MDTs (monthly or bimonthly) and a specialized interuniversity degree. Conclusion:MDTs are essential in neuro-oncology, enabling individualized therapeutic planning. Given critical workforce shortages in West Africa, a subregional collaborative MDT network could optimize patient care, enhance resource sharing, and standardize management across the region.
In sub-Saharan African countries like Côte d’Ivoire, most individuals with Type 2 diabetes mellitus (T2DM) are undiagnosed, and therefore at risk of experiencing late T2DM diagnosis. Within this region, little is known about late T2DM diagnosis, which is defined as the onset of complications at, or around the time of, laboratory diagnosis. Our objective was to assess the prevalence of late T2DM diagnosis in Abidjan (Côte d’Ivoire) and identify biological and social factors associated with late T2DM diagnosis. A retrospective chart review was carried out between January and May 2022 using patient records in four diabetes centres of Abidjan. Late diagnosis was defined as the onset of at least one T2DM complication within two years following T2DM laboratory diagnosis. Descriptive and multiple correspondence analyses were conducted. Patients (n = 217) had a mean age of 49.0 ± 11.4 years at T2DM laboratory diagnosis, and about half were female (male-female ratio: 0.9). One quarter (25.3
