Évaluer le contrôle des facteurs de risque chez les patients coronariens à distance d’une réadaptation cardiaque. Les données de 2005 à 2015 de réadaptation cardiaque ambulatoire de 4 semaines avec un suivi à 1 an ont été analysées. La morbidité, la mortalité cardiovasculaire, les facteurs de risque ont été comparés aux études Reach et Euroaspire. Sur 1091 patients coronariens, 746 (68 %) ont été suivis à 1 an : 60 ans (20–85 ans) 15 % de femmes, 44 % hypertendus, 36 % fumeurs, 17 % diabétiques et 74 % dyslipidémiques. À la fin du stage, il existe une nette amélioration (PA ≤ 140/90 mmHg et IMC > 30 : 80 et 20 % vs 65 et 25 % en début de stage ; LDL < 0,7 g/L : 42 % et tabagisme en cours : 4 % vs 18 % lors de leur hospitalisation initiale). Malheureusement, ce bénéfice diminue nettement à 1 an (PA ≤ 140/90 mmHg : 63 %, LDL < 0,7 g/L : 27 % ; tabagisme actif : 6 %) tout en restant meilleur que dans Euroaspire IV. Alors que 54 % réalisent une activité physique insuffisante avant le stage, ils ne sont plus que 23 % à 1 an vs 60 % dans Euroaspire. La mortalité totale à 1 an est de 0,6 % vs 2,9 % dans Reach avec 12 % d’hospitalisation cardiaque (53 % pour évolutivité coronarienne). L’éducation des patients coronariens lors d’un stage de réadaptation cardiaque améliore significativement la prise en charge des facteurs de risque, la morbidité et la mortalité. From 2015 to 2015, cardiac morbidity, total mortality and risk factors data were collected from a 4-week ambulatory cardiac rehabilitation program with a control one year later and were compared with the results of Euroaspire and Reach studies. Out of 1091 (87%) coronary patients, 746 (68%) were re-examined 12 months later (aged 60 years (ranging from 20 to 85 years), 15% women, 44% hypertensive, 36% smokers, 17% diabetics, 74% with dyslipidemia). At the end of the program, management of CV risk factors was improved (BP ≤ 140/90 mmHg and BMI > 30: 80 and 20% vs 65 and 25% at the beginning of the program; LDL < 0.7 g/L: 42% and smoking: 4% vs 18% when hospitalized for their cardiac event). Unfortunately, this benefit decreased significantly at one year (BP ≤ 140/90 mmHg: 63%, LDL < 0.7 g/L: 27%; smoking: 6%) but it remains better than in Euroaspire IV study: 57, 20 et 16% respectively). While 54% had insufficient physical activity before cardiac rehabilitation, they were only 23% at 1-year vs 60% in Euroaspire study. We reported a total mortality of 0.6% vs 2.9% in Reach study and 12% cardiac hospitalization (53% for a new coronary event). Participation of coronary patients in a cardiac rehabilitation program significantly improves management of CV risk factors, morbidity and mortality. However, the control obtained at the end of the program decreases at one year.
Los linfedemas son la consecuencia de una estasis linfática que provoca un aumento de volumen del miembro afectado. Se clasifican en linfedemas primarios y secundarios. Los linfedemas secundarios del miembro superior tras el tratamiento de un cáncer de mama son los más frecuentes en países de nuestro entorno. Los linfedemas primarios son casi siempre esporádicos, en ocasiones familiares, o pueden formar parte de síndromes malformativos y/o genéticos más complejos. El diagnóstico de linfedema es clínico pero, en las formas primarias, la linfogammagrafía permite valorar con precisión la función linfática. La erisipela es la principal complicación de los linfedemas, aparte de la repercusión funcional y psicológica, en ocasiones importante. El principal diagnóstico diferencial de los linfedemas de los miembros inferiores es el lipedema definido por una distribución anormal de las grasas desde las caderas hasta los tobillos. El tratamiento se basa en la fisioterapia descongestiva completa (vendajes poco elásticos con bandas de alargamiento corto, drenajes linfáticos manuales, cuidados de la piel, ejercicios), cuya primera fase, intensiva, permite disminuir el volumen, y la segunda, estabilizar el volumen mediante la aplicación de una compresión elástica. La educación terapéutica, incluido el aprendizaje de los autovendajes, es indispensable para facilitar la autonomía del paciente.
Crystallization from solution is a crucial process used in the manufacture of a wide variety of materials. The first step in the crystallization process is the birth of a new crystalline phase, which is known as nucleation. Nucleation plays a key role in determining the results of any crystallization process with respect to the size, shape and crystal form obtained. Classical nucleation theory does not adequately explain the crystal nucleation process. Work described in the literature and at this Faraday Discussion describe more complex nucleation mechanisms which are generally known as two-step nucleation models. In addition, as most nucleation is influenced by dust, dirt and container surfaces, the importance of heterogeneous nucleation and the use of templates to accelerate nucleation and influence crystal form are promising methods for the study and control of nucleation. It is also clear from this Faraday Discussion that interest in this topic has grown, and new and novel experimental and modeling approaches are being used for the study of crystal nucleation from solution.
