    1Nummular Dermatitis Following an Insect Bite in a Patient with Activated Phosphoinositide-3-Kinase Delta Syndrome (APDS): A Case Report and Literature Review.

    Activated phosphoinositide-3-kinase delta syndrome (APDS) is a rare primary immunodeficiency caused by gain-of-function mutations in the PIK3CD gene, resulting in the hyperactivation of the PI3K-Akt-mTOR pathway and subsequent immune dysregulation. Although recurrent sinopulmonary infections are typical, cutaneous manifestations remain poorly characterized. Nummular dermatitis is an uncommon presentation in this context and may reflect the underlying inflammatory imbalance. A 17-year-old female patient with genetically confirmed APDS (heterozygous PIK3CD c.3061G>A; p.Glu1021Lys) presented with erythematous papules that evolved into tense bullae on the lower limbs following an insect bite. Over subsequent weeks, multiple well-defined, coin-shaped eczematous plaques developed on the extremities and trunk, consistent with nummular dermatitis. The patient had a history of recurrent respiratory infections, chronic sinusitis, and otitis media, managed with intravenous immunoglobulin and prophylactic antibiotics. Topical betamethasone with fusidic acid and oral antihistamines produced partial improvement, though intermittent relapses occurred during follow-up. No systemic symptoms or new infectious complications were identified. This case represents an unusual dermatologic manifestation of APDS, where a minor external trigger led to persistent eczema-like lesions. Dysregulated PI3K-Akt-mTOR signaling may amplify local inflammation and impair epidermal barrier recovery. Recognition of atypical or chronic dermatitis in immunodeficient patients is crucial for early diagnosis, targeted treatment, and multidisciplinary management. Reporting such cases expands the clinical spectrum of APDS and underscores the importance of considering immune dysregulation in persistent cutaneous disorders.

    2Ergonomía En Endoscopia Gastrointestinal En México: Resultados De Una Encuesta Nacional

    Introducción: Los endoscopistas dedican la mayor parte de su tiempo en procedimientos, teniendo que realizar a diario movimientos repetitivos y constantes, lo que da como resultado una prevalencia alta de lesiones musculoesqueléticas. El objetivo de este estudio es conocer las características físicas, condiciones de trabajo y la prevalencia de lesiones asociadas a procedimientos en las y los endoscopistas de México. Método: Se realizó una encuesta electrónica, la cual se envió por correo electrónico y código QR a todos los socios de la Asociación Mexicana de Endoscopia Gastrointestinal. Resultados: Contestaron la encuesta 153 socios, el 31.4% fueron mujeres, la prevalencia de dolor fue del 81.7%, los sitios más frecuentes de dolor fueron dedos de las manos 39.9%, manos 32%, hombros 28.1% y espalda baja 35.3%; el 92.8% no recibió información sobre ergonomía y el 83% consideró que la sala no se adaptó a sus características físicas o de género. Conclusiones: Este estudio demostró la alta prevalencia de dolor musculoesquelético en las y los endoscopistas de México invitándonos a planear las estrategias laborales y académicas que favorezcan el correcto desarrollo de los procedimientos y de nuestra salud.

    3Effectiveness and Treatment Patterns of Isavuconazole in Real-World Use in Mexico: A Retrospective Study of 25 Patients

    Background Isavuconazole is a broad-spectrum antifungal used for the treatment of critical fungal infections like invasive aspergillosis and mucormycosis. In 2019, isavuconazole was approved as an orphan drug in Mexico. Aims Médica Sur Hospital is a tertiary-level hospital in Mexico City where we have had access to isavuconazole since its approval. Five years after approval, we decided to study its clinical outcomes in real-world settings. Methods We conducted a retrospective observational study of a cohort of adult patients treated with isavuconazole in our hospital from 2019 to 2024. Results We included data from 25 patients meeting the inclusion criteria, with a mean age of 57 years. Eight patients had hematologic disease, eleven presented with COVID-19-associated pulmonary aspergillosis (CAPA), and six patients had an invasive fungal infection in the intensive care unit (according to FUNDICU). The mean duration of isavuconazole use was 12.3 days (± 10.3 days). The main reasons for using isavuconazole were the ease of prescription (ten patients), as primary therapy in aspergillosis (six patients), after withdrawal of other triazole because of adverse events (three patients), a need to change to oral antifungal after an intravenous antifungal therapy (four patients), and as prophylaxis in stem cell transplantation (two patients). Conclusions Results on the use of isavuconazole in real-world clinical practice have shown that it is both well-tolerated and effective in a hospital setting in Mexico.

    4#187 - ALTERED INTESTINAL G PROTEIN-COUPLED RECEPTORS MEDIATED FATTY ACIDS SENSING IN METABOLIC DYSFUNCTION-ASSOCIATED STEATOTIC LIVER DISEASE AND OBESITY

    Introduction and Objectives Metabolic dysfunction–associated steatotic liver disease (MASLD) is characterized by increased free fatty acid flux and alterations in the gut microbiota. An underexplored component of this axis is the intestinal epithelium, which functions as a metabolic interface sensing luminal nutrients and microbial metabolites. Fatty acids act as signaling molecules through G protein–coupled receptors (GPCRs), which play complementary roles in metabolic regulation.Aim. This cross-sectional study aimed to determine the protein expression of GPCRs in intestinal mucosa from patients with and without MASLD and/or obesity. Patients and Methods Forty participants were stratified into four groups based on body mass index < or 25kg/m2 and MASLD diagnosed through transient elastography and Control Attenuation Parameter (CAP 0). Endoscopic colonic biopsies were processed for immunohistochemistry, and analysis assessed using H-Score and Cliff's delta. Results GPR40 showed an abrupt increase in the Lean-MASLD group (= 1.0) and an increase in the Obese/Obese-MASLD groups (= 1.0). GPR41 shows decreased expression in the Obese-MASLD group compared with the other groups (= 1.0). No significant change in GPR43 was observed in the Lean-MASLD group, but an increase was observed in the Obese/Obese-MASLD groups (= 0.6). All groups exhibited increased expression of GPR120 (Lean-MASLD = -0.9; Obese = -1.0; Obese-MASLD = -0.6). Conclusions Intestinal epithelial fatty acid-sensing GPCRs exhibit ligand-specific alterations in MASLD and obesity. Lean MASLD mainly associated with increased epithelial receptor expression, whereas obesity is associated with a broader reduction in fatty acid-sensing capacity.

    5Drenaje Quirúrgico De Absceso Anorrectal Y Fistulotomía Como Manejo Primario Se Asocia a Baja Tasa De Recurrencia

    Introducción. Los abscesos anorrectales se han convertido en un problema quirúrgico común, aproximadamente un tercio de ellos llegan a presentar fístula anal; el manejo puede ser conservador con antibióticos o aspiración con aguja y drenaje simple con anestesia local, considerándolos suficientes para remitir el cuadro agudo de esta patología; sin embargo, se ha asociado con una alta tasa de recurrencia y desarrollo de fístula, así como la necesidad de intervenir quirúrgicamente en varias ocasiones; aunque existe poca literatura con relación al drenaje quirúrgico, otros cirujanos prefieren la identificación del orificio primario y fistulotomía como manejo inicial, observando una disminución en el riesgo de formación de fístula y obteniendo resultados favorables para los pacientes. En este trabajo, se presenta el seguimiento de los pacientes con absceso anorrectal tratados con drenaje y fistulotomía. Objetivo. Analizar una serie retrospectiva de 235 casos de abscesos anorrectales en términos de resultados quirúrgicos, recurrencia y el desarrollo de la fístula. Material y métodos. Estudio observacional, retrospectivo de 235 casos de abscesos anorrectales tratados en tres hospitales de tercer nivel, durante el periodo comprendido entre junio 2011 a agosto 2023, con edad media de 42.6 años rango (19 a 73), con predominio del sexo masculino n = 167 (71%), se analizaron datos demográficos, comorbilidad, recuperación, seguimiento y recurrencia. Resultados. La edad promedio fue de 42.6 años (rango 19-73) con un predominio del sexo masculino (71%); según la localización 67% fueron isquiorrectales, 20% posteriores, 11.9% anteriores y 0.85% en herradura; el tiempo de recuperación fue de 18 días (rango 14-21), recurrencia del 3.8% (9 pacientes) con un seguimiento de 10 meses, el dolor se controló con paracetamol 750 mg tres veces al día alternando con ketoralaco 10 mg tres veces al día vía oral y antibióticos amoxicilinaclavulánico ácido 875 mg dos veces/día durante 10 días. Conclusiones. Este estudio muestra que la realización de ambos procedimientos de drenaje y fistulotomía, la incidencia de la fístula es muy baja, por lo que recomendamos en abscesos con origen criptoglandular este enfoque que potencialmente disminuye la necesidad de otra cirugía en el futuro.
