Psoriatic arthritis is a complex immune–mediated disease in which patients with psoriasis have inflammatory lesions of the musculoskeletal system. A clinical case of curation of a patient with psoriatic arthritis and pustular psoriasis is described. The interaction of the rheumatologist and the dermatologist made it possible to clarify the diagnosis and choose the appropriate treatment tactics for the patient.
The purpose of the study is to develop and implement health-improving and restorative programs for students of special medical groups in order to maintain the body's defense mechanisms, prevent diseases and improve the health of students, and increase students' motivation to engage in physical exercises and sports. Research methods: surveying (self-assessment of students' health status), medical methods for checking students' health, analysis of scientific literature on the research problem, pedagogical experiment, methods of mathematical statistics and generalization of the obtained experimental data. Research results and conclusions. A health-improving and restorative program has been developed and tested for first to third-year students of Kemerovo State Medical University, which has demonstrated its effectiveness in improving physical qualities of students such as general endurance, flexibility, agility, and core strength. Furthermore, the motivation of students to engage in physical exercises and sports has increased.
Social protection of certain groups of citizens occupies a key position among the state priorities. Special attention is paid to providing disabled people with necessary medicines on preferential terms, which is enshrined in federal laws, regulatory acts of the country and federal subjects, including the Kemerovo Region — Kuzbass. The basic document regulating this sphere is the following Federal Law No. 323-FZ of November 21, 2011, which establishes the basis of health care in the country. Drug provision of citizens is carried out differentiated, based on the level of disability of each person. Expenses for the purchase of medicines are compensated from the budgets of various levels of government according to the approved lists of medicines. In this regard, socially-oriented technologies used in the provision of medicines for the disabled are of particular relevance.
On November 08, 2024 in Moscow, under the auspices of the Russian Society for the Study of Pain (RSSP), a Council of Experts was held, during which leading specialists in the field of neurology, who have a high level of expertise in the treatment of back pain, discussed modern approaches to the therapy of discogenic lumbosacral radiculopathy (DLSR). The Council actualized the leading pathogenetic mechanisms of the disease onset and progression; discussed the mechanisms of pathogenesis, which are currently still insufficiently affected by pharmacotherapeutic intervention. Conservative therapy of DLSR is effective and allows to avoid surgical treatment in the majority (more than 90%) of cases. The leading role in conservative therapy of DLSR is played by informing the patient about the benign nature of DLSR, the possibility of spontaneous regression of disc herniation, methods of kinesiotherapy under the supervision of a specialist, epidural administration of local anesthetics and glucocorticoids in the acute period of the disease. In real practice, non-steroidal anti-inflammatory drugs, anticonvulsants are widely used, but their effectiveness remains controversial, so the search for new methods of conservative therapy is relevant. The experts noted the importance of the approach to the treatment of the problem under consideration from the position of its pathogenesis and discussed the introduction of new groups of drugs, emphasizing the need to expand clinical recommendations in favor of drugs with pathogenetically justified and complex mechanisms of action in back pain. The results of RCTs and the experience of clinical application of parenteral preparation of triterpene saponin escin — L-lysine escinate in different groups of patients with DLSR were discussed in detail. The expediency of conducting a multicenter placebo-controlled study of the efficacy of escin — L-lysine escinate in acute DLSR was noted.
Highlights The article summarizes modern ideas about the classification, pathogenesis, main clinical symptoms, diagnosis and approaches to the treatment of systemic amyloidosis. An important postulate of today's diagnosis of amyloidosis is the morphological identification of amyloid in biopsies of affected tissues, including by typing. The goal of therapy for any type of amyloidosis remains the removal of precursor proteins, which justifies the use of certain aggressive drug regimens or other drastic measures. Recent achievements in studying the problem of amyloidosis have made it possible to formulate classification criteria for amyloidosis and treatment approaches, as well as significantly improve the prognosis of patients with various types of amyloidosis. Abstract Amyloidosis is a rare disease that can be diagnosed during life. More often, amyloidosis is asymptomatic or has non-specific symptoms. For this reason, diagnosis and treatment is carried out untimely, the results of therapy are ineffective. The purpose of this analytical review is to identify the most effective methods for early diagnosis of amyloidosis. To characterize a promising method of amyloidosis therapy for today. Thus, timely diagnosis of systemic amyloidosis and modern approaches to the treatment of this disease will increase the overall survival of patients.
