    1The German Version of the Sleep Inertia Questionnaire in Central Disorders of Hypersomnolence: a Validation Study

    Background Sleep inertia, or sleep drunkenness, is a transitional state between sleep and wakefulness characterized by cognitive and physical impairment. It is particularly relevant in central disorders of hypersomnolence (CDH), but validated instruments to assess sleep inertia in German-speaking populations are lacking. This study translated and validated the German version of the Sleep Inertia Questionnaire (SIQ) in patients with CDH and healthy controls. Methods We included 109 participants from the international Swiss Primary Hypersomnolence and Narcolepsy Cohort Study (iSPHYNCS): idiopathic hypersomnia (IH, n = 40), narcolepsy type 1 (NT1, n = 21), other CDH/narcolepsy borderland (NBL, n = 34), and healthy controls (HC, n = 14). Psychometric evaluation included confirmatory factor analysis (CFA), internal consistency, test-retest reliability, construct validity, discriminative validity, and gender-based analyses. Results A modified four-factor second-order CFA model showed acceptable overall fit, supporting the factorial validity of the German SIQ while suggesting that its structure is not identical to the original English version. The SIQ demonstrated excellent internal consistency (Cronbach's alpha = 0.96) and test-retest reliability (intraclass correlation = 0.94). SIQ scores correlated strongly with fatigue (r = 0.79) and depression (r = 0.67), and moderately with sleepiness (r = 0.47) and sleep quality (r = 0.38). SIQ scores separated CDH from HC (AUC = 0.886) and IH from NT1 (AUC = 0.721). Females reported higher SIQ scores, particularly within IH. Conclusion The German SIQ is a reliable and valid instrument for assessing sleep inertia severity in CDH. Its factorial structure, group-level discriminative validity, and potential gender differences warrant further investigation in larger cohorts.

    3Understanding Generation Z Purchase Intention in the Digital Era: Combined Effect of Social Media Influence, Digital Identity and Consumer Values

    Accelerated digitization has fundamentally reshaped how younger generations engage with brands and make purchasing decisions. As the first generation to grow up in a fully digital environment, Generation Z increasingly relies on social media platforms for product discovery, knowledge exchange, and identity creation. While literature on Gen Z consumer behavior is expanding, studies often examine drivers of purchase intention such as social media influence (Parker et al. 2019), identity-based consumption (Ogle 2020), or consumer values (Kumar & Prakash 2021) in isolation. This study aims to develop a holistic conceptual framework for Gen Z purchase intention by integrating existing research on social media impact, digital selfconstrual, and consumer value orientations. Through a conceptual review, this paper synthesizes literature on digital marketing communication, symbolic consumption, and value-driven behavior. The findings indicate that Gen Z purchase intention cannot be fully explained by isolated factors. Instead, purchase motivations emerge from the interplay between digital communication environments, identity construction processes, and consumer value orientations (specifically materialism and consumer ethnocentrism). Consequently, this research proposes an alternative conceptualization of social media as a digital ecosystem that facilitates identity construction and consumption discourse, alongside consumer values that dynamically inform how individuals evaluate their choices. By linking digital marketing impact, identity-based consumption, and value-oriented decision making, this research contributes a more integrative perspective to the body of knowledge in consumer behavior. Ultimately, the framework extends the theoretical understanding of Generation Z purchase intention, particularly within fast-growing digital economies such as Indonesia.

    4Immunologischer Phänotyp Und Hautmodelle Des Netherton‐Syndroms

    Das Netherton‐Syndrom (NTS) ist eine syndromale Ichthyose, die durch Schuppenbildung, Erythrodermie und Neigung zu Allergien und Infektionen gekennzeichnet ist. Es wird durch Mutationen im SPINK5 ‐Gen verursacht, das für den lymphoepithelialen Kazal‐Typ‐5‐Inhibitor (LEKTI), einen Proteaseinhibitor, kodiert, der die Aktivität von Kallikreinen in der Haut reguliert. Die Behandlungsmöglichkeiten sind begrenzt, unzureichend, zeitaufwendig und finanziell belastend für die Patienten. In den letzten Jahren sind Genotyp‐Phänotyp‐Korrelationen beim NTS und immunologische Phänotypen beschrieben worden. Es wurden psoriasiforme Muster beschrieben. Außerdem gibt es in der Literatur Berichte über den Einsatz von Biologika als neue therapeutische Optionen für das NTS. Zuvor wurden die rekombinante humane LEKTI‐Domäne 8+9 (rhLEKTI 8+9) als potenter Kallikrein‐Inhibitor identifiziert, was darauf hindeutet, dass diese Untereinheiten für eine topische Proteinsubstitutionstherapie geeignet sind. Unser Ziel war es, das NTS genauer zu charakterisieren, um weitere personalisierte Therapieoptionen zu erörtern, indem wir eine große Kohorte von NTS‐Patienten phänotypisieren und Serumanalysen durchführen. Darüber hinaus arbeiteten wir an ersten Schritten für eine Proteinersatztherapie, indem wir rekombinant exprimiertes rhLEKTI8+9 untersuchten und 3D‐Epidermisäquivalente für die therapeutische Bewertung herstellten.

    5Was Tun Bei Sprachentwicklungsverzögerungen Und -Störungen? Die Neue S3-Leitlinie Fordert Umdenken

    Zusammenfassung Sprachentwicklungsstörungen (SES) gehören mit einer Prävalenz von 9,9 % zu den meistbehandelten Störungen des Kindesalters. Bei 7,6 % bestehen keine gravierenden sprachrelevanten Erkrankungen – (umschriebene) SES ((U)SES). Bei weiteren 2,3 % liegen Komorbiditäten wie Hörstörungen oder neurologische Störungen vor. SES beeinträchtigen oft die soziale, emotionale, kognitive, schulische und berufliche Entwicklung der Betroffenen. Zwischen dem 2. und 3. Geburtstag beobachtbare Sprachentwicklungsverzögerungen (SEV) gehen (U)SES regelmäßig voraus. Evidenzbasierte Interventionen bei SES und SEV zeigt eine kürzlich publizierte S3-Leitlinie auf. Diese empfiehlt erstmals Frühinterventionen bei SEV im 3. Lebensjahr – als strukturierte, angeleitete Elterntrainings oder frühe Sprachtherapie. Weitere Empfehlungen betreffen u. a. den Einbezug der Eltern und – wenn möglich – sprachgesunder Gleichaltriger in SES-Interventionen und die frühe Verordnung stationärer Sprachtherapie bei gravierenden SES. Das Leitlinienwissen muss nun in die Verordnungs- und Therapiepraxis eingehen. Neuere Studien legen zudem die häufigere Nutzung von Online- und Kleingruppentherapien mit intensiven Therapiephasen nahe.
