Ultrasound-guided regional anesthesia (UGRA) has changed how we manage pain by allowing doctors to visualize nerves and other structures in real time, making the process more accurate, effective, and safe while also reducing risk. This narrative review examines the latest advancements in UGRA techniques, evaluating their efficacy and safety, particularly for peripheral nerve and truncal plane blocks, their potential to diminish opioid consumption, and their clinical applications in perioperative care. This review followed the Scale for the Assessment of Narrative Review Articles (SANRA) quality assessment guidelines and searched six databases for studies published from January 2015 to October 2025, including trials, meta-analyses, cohort studies, reviews, and technical reports. This review aims to give doctors, anesthesia experts, and researchers the newest information and methods about UGRA, focusing on better ways to see needles and use additives like dexamethasone and dexmedetomidine to make regional anesthesia more effective and last longer, which can help lower opioid use and speed up recovery after surgery, while ultrasound guidance makes these techniques safer and more accurate. Another important finding is that real-time imaging and protocols help reduce safety concerns like toxicity, nerve damage, and infection. Artificial intelligence (AI) and robotics in UGRA enhance precision, safety, and recovery, paving the way for further standardization and accessibility.
Background . Antimicrobial resistance (AMR) has high medical and social significance, as it is shaped by clinical, veterinary, agricultural, and environmental processes, as well as horizontal transfer of mobile genetic elements. Its analysis requires the assessment of individual resistance genes, experimental models, diagnostic approaches, as well as surveillance systems that track antibiotic resistance genes (ARGs) across human, animal, and environmental reservoirs. The aim of the study was to summarize data on experimental and computational models of ARG dissemination, characterize selected AMR markers, and evaluate current methods for their detection. Material and methods . A narrative review with a transparent search strategy and thematic synthesis was conducted. The search included PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar; the last search was performed on May 2, 2026. After full-text assessment, 46 sources were included in the review. Results. The review discusses blaCTX-M-15, blaNDM, blaKPC, blaOXA-23, floR and intI1 markers, as well as mobile genetic elements involved in their dissemination. It summarizes the strengths and limitations of in vitro models, gut models, mouse models, biofilm systems, mathematical modelling, AST, PCR/qPCR, WGS, mNGS, machine learning, and CRISPR-Cas12a diagnostics. Conclusion . CRISPR-Cas12a platforms may support rapid targeted screening of predefined ARGs but do not replace phenotypic susceptibility testing or genome-scale analysis. The most robust strategy is the integration of AST, WGS/mNGS, targeted molecular diagnostics, machine learning, and One Health surveillance.
X-ray examinations remain one of the most common and accessible methods of visual diagnosis, widely used in clinical practice. The quality of X-ray interpretation directly affects the accuracy of diagnosis and the effectiveness of subsequent stages of treatment, but for medical students, this process is often quite difficult. In this regard, it is necessary to study the causes of errors in the interpretation of X-ray images at the stage of professional training. The study aimed to identify the main causes of errors in the interpretation of X-ray images and the characteristics associated with the training profile of students. The study involved 164 medical university students studying paediatrics (n = 62), general medicine (n = 88) and preventive medicine (n = 14). Data were collected through an anonymous online questionnaire that included closed and open-ended questions aimed at assessing the difficulties of interpreting radiographic images, the level of confidence and the amount of practical experience. Descriptive statistics, one-way analysis of variance, Kruskal-Wallis test, χ² and Pearson’s correlation analysis were used for statistical analysis. The study determined that the most common errors were missing pathological changes, incorrect localisation of foci and misinterpretation of artefacts. The greatest difficulties were caused by chest X-rays, lateral projections, and images of the musculoskeletal system. The average level of confidence of students in interpreting X-rays was low, at 4.1 ± 1.7 points. Medical students demonstrated a higher level of confidence and a lower error rate compared to students from other disciplines. A moderate negative correlation was found between the level of confidence and the number of errors made. The results obtained indicate the need to expand the scope of practice-oriented training, introduce a systematic approach to the interpretation of X-ray images, and use modern digital educational tools to improve the quality of training for future doctors. Качество интерпретации рентгенограмм напрямую влияет на точность диагностики и эффективность последующих этапов лечения, однако для студентов медицинских вузов данный процесс нередко представляет значительную сложность. В связи с этим актуальным является изучение причин ошибок при интерпретации рентгенологических изображений на этапе профессионального обучения. Цель – выявить основные причины ошибок при интерпретации рентгенологических изображений и особенности, связанные с профилем подготовки студентов. В исследовании приняли участие 164 студента медицинского университета, обучающиеся по педиатрическому (n = 62), лечебному (n = 88) и медико-профилактическому (n = 14) профилям. Сбор данных осуществлялся посредством анонимного онлайн-анкетирования, включавшего закрытые и открытые вопросы, направленные на оценку трудностей интерпретации рентгенологических изображений, уровня уверенности и объёма практического опыта. Для статистического анализа использовались методы описательной статистики, однофакторный дисперсионный анализ, критерий Краскела-Уоллиса, χ² и корреляционный анализ Пирсона. Установлено, что наиболее распространёнными ошибками являлись пропуск патологических изменений, неверная локализация очагов и неправильная трактовка артефактов. Наибольшие затруднения вызывали рентгенограммы грудной клетки, латеральные проекции и изображения опорно-двигательного аппарата. Средний уровень уверенности студентов в интерпретации рентгенограмм был низким и составил 4,1 ± 1,7 балла. Студенты лечебного профиля демонстрировали более высокий уровень уверенности и меньшую частоту ошибок по сравнению с обучающимися других профилей. Выявлена умеренная отрицательная корреляция между уровнем уверенности и количеством допущенных ошибок. Полученные результаты свидетельствуют о необходимости расширения объёма практикоориентированного обучения, внедрения системного подхода к интерпретации рентгенологических изображений и использования современных цифровых образовательных инструментов для повышения качества подготовки будущих врачей. Компьютерная томография (КТ) является важным инструментом в диагностике и лечении черепно-мозговой травмы (ЧМТ). Классификация КТ по Маршаллу, Роттердамская и Хельсинкская шкалы оценки КТ были последовательно разработаны как шкалы прогнозирования исхода на основе КТ-аномалий при ЧМТ. Ни одна из них не классифицирует патологические результаты КТ одинаково. Мы стремились определить, какая из них является наиболее точной и обладает наилучшей дискриминационной способностью при оценке раннего исхода. AUC показали, что все системы подсчёта очков обладали одинаково высокой дискриминантной способностью при прогнозировании раннего неблагоприятного исхода: AUC Маршалла — 0,85, Роттердамской шкалы — 0,81, Хельсинкской шкалы — 0,83. Была выявлена высокая корреляция между шкалами Маршалла и Роттердама (r=0,76), а также умеренная корреляция между шкалой Маршалла и Хельсинкской (r=0,60) и между Роттердамской и Хельсинкской (r=0,49). Кроме того, низкая оценка по шкале комы Глазго и высокая оценка тяжести травмы могут служить надёжными предикторами ранней смерти и неблагоприятного исхода Computed tomography (CT) is an important tool in the diagnosis and treatment of traumatic brain injury (TBI). The Marshall CT classification, the Rotterdam and Helsinki CT assessment scales have been consistently developed as outcome prediction scales based on CT anomalies in TBI. None of them classifies pathological CT results in the same way. We sought to determine which of them is the most accurate and has the best discriminative ability when assessing an early outcome. The AUC showed that all scoring systems had an equally high discriminating ability in predicting an early adverse outcome: Marshall's AUC was 0.85, the Rotterdam scale was 0.81, and the Helsinki scale was 0.83. A high correlation was found between the Marshall and Rotterdam scales (r=0.76), as well as a moderate correlation between the Marshall scale and the Helsinki scale (r=0.60) and between Rotterdam and Helsinki (r=0.49). In addition, a low score on the Glasgow Coma Scale and a high assessment of the severity of injury can serve as reliable predictors of early death and an unfavorable outcome
ABSTRACT Purpose: Trauma remains a leading cause of hospital admissions worldwide, particularly in low- and middle-income countries. Brazil’s dual healthcare system reflects disparities in resource availability and clinical outcomes. This study compared epidemiological profiles of trauma patients treated at a public versus a private hospital in Salvador, Bahia, Brazil. Methods: We retrospectively analyzed 6,472 trauma-related hospitalizations from August 2021 to August 2023. Data were extracted from Hospital Information System/Department of Informatics of the Unified Health System and a private hospital database. Patients admitted under the 10th revision of the International Classification of Diseases, categories S00–T88, were included, excluding burns and orthopedic injuries. A χ2 test assessed associations between trauma mechanisms and hospitals. Results: Among the 6,472 cases, 6,342 occurred in the public hospital and 130 in the private hospital. In the public hospital, males accounted for 82.1% of the cases, and the leading causes were motorcycle crashes (21.7%) and gunshot wounds (16.2%). At the private hospital 58.4% of patients were males, with falls (50.8%) predominating among older adults. Violence-related injuries predominated in the public hospital, while falls predominated in the private hospital. Conclusion: This study revealed distinct trauma patterns in the public versus the private hospital in one Brazilian city. The public hospital treated younger male victims of violence, while the private hospital treated older patients with falls. These findings underscore the need for policies addressing trauma care disparities.
