Background:Necrotizing fasciitis (NF), a severe form of Necrotizing Soft Tissue Infection (NSTI), is a rapidly progressing, life-threatening condition characterized by extensive tissue destruction. Standard management involves early surgical debridement and systemic antibiotics; however, adjunctive wound care plays a crucial role in recovery. VELVERT®, a U.S.-patented dressing enriched with natural plant-based antimicrobials, has shown efficacy in chronic ulcers but has not yet been evaluated in NSTI. Objective:To assess the safety and efficacy of Velvert antimicrobial dressing as an adjunct to standard care in patients with NSTI. Methods:A single-center, open-label, non-randomized clinical study was conducted at M.L.B. Medical College, Jhansi, enrolling 20 patients (18-70 years) with clinically diagnosed NSTI. After debridement, Velvert dressings were applied every fourth day for up to 60 days. Primary endpoints included complete wound closure and rate of healing. Secondary assessments were ≥50% wound area reduction, pain relief, dressing frequency, microbial load, surgeon satisfaction, and adverse events. Both Intent-to-Treat (n=20) and Per-Protocol (n=18) analyses were performed. The study was registered with CTRI (CTRI/2024/02/063282). Results:Complete wound closure occurred in 61.11% (11/18) of Per-Protocol patients, with an additional 16.67% achieving ≥50% reduction. Mean wound size reduction was 20.73 cm2 (p=0.004). By Day 60, 60% reported no pain, and 77.78% showed ≥50% healing. Dressing usage ranged from 2-15 applications. No serious adverse events were observed, and surgeons reported high satisfaction. Microbial analysis indicated reduced bacterial load. Conclusion:Velvert demonstrated safety and efficacy as an adjunct therapy in NSTI, enhancing wound healing, reducing pain, and offering a promising plant-based alternative to conventional dressings.
Introduction:Central Serous Chorioretinopathy (CSCR) is characterized by serous detachment of the neurosensory retina and typically affects young to middle-aged males. It is rarely seen in young females and may follow systemic illnesses such as malaria. Early diagnosis can be difficult when classical fundus changes are absent. Material and Methodology: A 20 year old female presented to the out patient department of the ophthalmology department of Maharani Laxmi Bai Medical College, Jhansi with the complaint of mild blurring of vision and perception of distorted lines in both eyes since last 10 days.With history of Malaria positive,following the administration of chloroquine,the patient developed macular changes that were subsequently diagnosed as acute Central serous Chorioretinopathy based on optical coherence tomography findings. A detailed ocular and systemic examination was performed.
Abstract Objectives To evaluate the correlation between weight loss and quality of life as assessed by SF-36 (36 item shoer form survey) questionnaire in patients treated with Semaglutide injection for weight management Materials and Methods This phase III, multicentre, randomized, open-label, active-controlled clinical study (CTRI/2025/04/085487) was conducted across 22 study sites in India after receiving regulatory and ethics committee approvals. Total 267 adult patients with BMI ≥ 30 kg/m2 or ≥ 27 kg/m2 with the presence of at least one co-morbidity of hypertension, dyslipidaemia and/or type 2 diabetes mellitus were included. Patients were randomized to either test arm to receive synthetic semaglutide injection or reference arm to receive Wegovy® every week subcutaneously for 24 weeks. The primary endpoint was change in body-weight from baseline to week 24. Secondary endpoints included change in BMI, waist circumference, SF-36, and glycaemic measures in diabetic and non-diabetic patients. Safety assessments included incidence of TEAEs and anti-drug/neutralizing antibody. This post-hoc analysis was conducted to evaluate the correlation between weight loss and improvement in quality of life as assessed by SF-36 physical function score. Results The Pearson’s correlation coefficient between change in weight loss and SF-36 physical functioning score was −0.155 with P value = .0467 in test arm. The Pearson’s correlation coefficient between change in weight loss and SF-36 physical functioning score was -0.046 with P value = .6831 in reference arm. There is negative correlation between weight loss and change in SF-36 physical functioning score in both test and reference arm indicating greater reduction in weight loss is associated with more increase in SF-36 score signifying improvement is SF-36 total score. The correlation between weight loss and improvement in SF-36 physical functioning score was statistically significant in test arm. The statistical significance observed in the Pearson correlation analysis in test arm supports the robustness of this association. Conclusions This post-hoc analysis demonstrated that there is improvement in quality of life as measured by SF-36 physical function score with reduction in weight, in patients treated with semaglutide injection for weight management.
This review examines the bidirectional relationship between the gut microbiota and cardiovascular diseases (CVDs), aiming to understand how microbial dysbiosis contributes to CVDs, including atherosclerosis, hypertension, and heart failure. Recent research emphasizes the gut microbiota's role in modulating immunity via SCFAs and tryptophan metabolites, maintaining intestinal barrier integrity, and producing metabolites such as SCFAs (acetate, propionate, butyrate) and pro-atherogenic TMAO. Dietary patterns, particularly the Mediterranean versus Western diet, significantly influence gut microbiota composition and CVD risk. Polyphenols and exercise have shown positive effects on gut microbiota and cardiovascular outcomes. A significant interplay exists between gut microbiota and cardiovascular health. Dysbiosis and metabolites like TMAO and LPS are implicated in CVD, while SCFAs and a balanced microbiota offer protection. Future research should focus on precision medicine, next-gen probiotics, optimized FMT, and multiomics approaches to identify personalized CVD therapies.
Impaction of deciduous teeth is a rare finding, and its bilateral occurrence is further rare. Among the first generation of teeth, second molar is the most commonly impacted tooth. We hereby present a rare case of a 5-year-old child with bilateral impacted inverted dilacerated maxillary deciduous central incisors, which has not been previously reported in the literature. Management techniques of impacted primary teeth include observation, removal of obstruction and follow-up, surgical exposure or repositioning with or without orthodontic traction, and extraction of primary teeth followed by space maintainers. Surgical extraction of the impacted deciduous incisors was done in the present case, as there were bleak chances of spontaneous eruption.
