Objective The objective of this revised official guideline, published by the German Society for Gynecology and Obstetrics (DGGG) and coordinated in the joint guidelines program of the DGGG, the Austrian Society for Gynecology and Obstetrics (OEGGG), and the Swiss Society for Gynecology and Obstetrics (SGGG), is to provide evidence-based and consensus-based recommendations for the diagnosis, treatment, care, and support of girls and women with confirmed or suspected endometriosis. Methods This S2k guideline was developed through a structured consensus process involving representative members of various professions (37 professional associations, organizations, and self-help groups) and includes 25 statements and 73 recommendations which are based on a systematic literature review (2019 - 2023) and expert consensus. Recommendations For the first time, the revised guideline has placed a greater focus on individualized, symptom-oriented diagnosis and treatment that combines hormonal, surgical, and multimodal approaches. A significant innovation is the use of transvaginal ultrasound as the central diagnostic procedure for detecting endometriosis. Therapeutically, primary hormone treatment is now recommended as the first choice approach, with surgical interventions and multimodal approaches supplemented on an individual and symptom-oriented basis.
Harvesting a strip from the distal rectus femoris tendon as an autograft for ligament reconstruction of the knee or other joints. Ligament reconstructions of the knee, both primary and revision procedures. Relative: athletes in jumping sports requiring rapid recovery of explosive strength. Palpation of the distal quadriceps tendon at the “fusion zone,” approximately 5 cm proximal to the superior patellar pole. A 3–4 cm longitudinal skin incision is made at the junction of the lateral and middle third or centrally. The quadriceps tendon is exposed and identified proximally. After identification, two parallel incisions create an 8–10 mm wide graft. The tendon strip is mobilized with a clamp just above the fusion zone and separated from the deeper layers. Distal detachment of the graft can be performed either (a) with a scalpel after whipstitching of the free end or (b) with a closed tendon stripper following proximal release under tension. The preparation is then extended proximally by 7–8 cm with scissors and bluntly separated from deeper layers with the index finger. An open or closed tendon stripper (8–9 mm) is advanced proximally with the knee in 20° flexion until complete graft harvest. Rotational movements should be avoided. Rehabilitation follows the protocol of the corresponding ligament reconstruction. No specific measures are required for the donor site. Radiological assessment demonstrated a mean distal rectus femoris tendon length of 39 cm (32–47 cm). In cadaveric studies, the technique was feasible and reproducible. Clinically, the graft was used in 103 patients, with a mean graft diameter of 8.3 mm. In a few cases, hamstring augmentation was required. Complications such as arthrofibrosis or donor-site hematoma were rare and successfully treated. External studies confirmed the suitability of the rectus femoris tendon, particularly for revision anterior cruciate ligament reconstruction.
Endometriose ist eine Erkrankung hoch komplexer Pathologie. Charakteristisch sind Infertilität und chronische Schmerzzustände. Darüber hinaus besteht ein 2‑ bis 4‑fach erhöhtes Risiko zur Entwicklung eines Ovarialkarzinoms. Die Molekularpathologie kennt inzwischen somatische Gen-Alterationen, die potenziell zu Endometriose und auch zu einer malignen Transformation führen können. Genomische Alterationen liegen nahe, sind aber zum jetzigen Zeitpunkt noch eher spekulativ.
Eine Verletzung im Bereich der meniskotibialen und -kapsulären Bänder in der Pars posterior des medialen Meniskus wird als Rampenläsion bezeichnet. Sie tritt in der Regel in Kombination mit einer Ruptur des vorderen Kreuzbandes (VKB) auf. Da die Meniskusrampe das VKB bei der Sicherung gegen die anteriore tibiale Translation und Rotation unterstützt, wird die Reparatur instabiler Rampenläsionen im Fall einer Kombinationsverletzung empfohlen. Bei jeder Kreuzbandrekonstruktion sollten das posteromediale Kompartiment dargestellt und mit einer Punktionskanüle die meniskotibialen und -kapsulären Bänder überprüft werden. Bei Zweifeln an der so gestellten Diagnose Rampenläsion sollte diese über ein posteromediales Portal direkt visualisiert werden. Therapeutisch stehen 1. Abrasion zur Heilungsstimulation, 2. Naht mit einem All-inside-Implantat über das anteromediale Portal und 3. Nahthakenreparatur über ein posteromediales Portal zur Verfügung.
Ziel dieser Operation ist die transossäre Refixation einer posterioren Wurzelläsion des medialen Meniskus in Kombination mit einer meniskotibialen Fixation im Bereich des Übergangs von Pars posterior zur Pars intermedia (Zentralisation) zur Prävention der Extrusion. Nach einer Kniegelenksarthroskopie über ein hohes anterolaterales Standardportal und Überprüfen der Indikation wird die Insertionszone am Tibiaplateau aufgesucht und bis auf den Knochen debridiert. Über ein transossäres Zielgerät wird ein Führungsdraht von anterolateral mittig in der Insertionszone des Innenmeniskus platziert und mit einem 4,5-mm-Bohrer überbohrt. Das Innenmeniskushinterhorn wird dann mit geflochtenem Nahtmaterial armiert, welches anschließend über eine Fadenschlaufe aus dem tibialen Knochentunnel ausgeleitet wird. Es folgt die Anlage der Zentralisationsnaht im Bereich der Pars intermedia. Diese kann entweder transossär über einen tibialen Knochentunnel (4,5 mm, angelegt über Führungsdraht) oder mit einem Nahtanker erfolgen. Als Nahtanker eignen sich weiche Fadenanker. Für die transtibiale Zentralisation wird die Meniskusbasis mit geflochtenem Nahtmaterial in „Outside-in-Technik“ armiert. Diese Naht wird durch den tibialen Knochentunnel gezogen und im Bereich der anterolateralen Tibia mit der Armierungsnaht des Hinterhorns verknotet. Bei der Fadenankermethode wird im medialen Rand des Tibiaplateaus ein Fadenanker platziert und die Armierung des Meniskus direkt mit einem Faden des Ankers durchgeführt. Für das Einbringen der Führungsdrähte und Bohrer sollte ein 1–2 cm langer medialer Zugang angelegt werden. Das Rehabilitationsprotokoll beinhaltet 6 Wochen Entlastung, danach schrittweise Belastungssteigerung. Die Beweglichkeit beträgt für 4 Wochen 0‑0-60° und 0‑0-90° für die 5.–6. Woche.
