1Obstructive Sleep Apnea: A Bibliometric Analysis of the 50 Most-cited Articles in Dentistry from 2002 to 2022
Kaleem Fatima, Pinky Singh, Ganesh Chaudhary

Background Obstructive sleep apnea (OSA) is a condition in which the upper airway narrows or closes repeatedly during sleep, interrupting normal breathing. To evaluate its trends and advancements, a bibliometric study examining the 50 most frequently referenced articles in dentistry published between 2002 and 2022 was conducted. Methods The Scopus online database was searched for relevant literature on OSA published in the last two decades. VOSviewer software was used for data analyzes, science mapping, and network analysis. A country-based correlation analysis and keywords based on bibliographic data were used to create a co-occurrence network. Results The top 50 most highly cited articles appeared across 17 separate journals, comprising 39 original research articles (78%) and 11 reviews (22%). The top three cited journals were the European Journal of Orthodontics (EJO), Journal of Oral and Maxillofacial Surgery (JOMS), and American Journal of Orthodontics and Dentofacial Orthopedics (AJODO). The bibliometric analysis revealed that the top 50 articles were authored by individuals from 20 different countries, with the USA contributing the greatest number of publications and receiving the highest citation count. Conclusions The 50 most-cited articles primarily focused on the diagnosis and management of OSA, with the highest citations from the EJO, JOMS, and AJODO. Practical Implications This research emphasizes the importance of diagnostic techniques, such as 3D airway imaging and sleep studies (polysomnography), for detecting associated morphological and physiological variations. Orthodontics, oral and maxillofacial surgery, and dental sleep medicine are all addressed in the most-referenced studies, highlighting the impact of craniofacial structures on airway patency and the need for individualized diagnostic and interdisciplinary treatment approaches.

2Effect of Pulpal Haemostasis Time on Partial Pulpotomy Outcome in Cariously Exposed Mature Permanent Teeth with Symptomatic Irreversible Pulpitis.
Saada Sulaiman,Ruchika Roongta Nawal,Sudha Yadav,Sangeeta Talwar,Seema Yadav, Henry Fergus Duncan

AIM:To correlate the outcome of partial pulpotomy (PP) with the time taken to arrest pulpal bleeding in cariously-exposed mature permanent teeth with a clinical diagnosis of symptomatic irreversible pulpitis (SIP). METHODOLOGY:This single centre, one-arm clinical trial was registered in the Clinical Trials Registry-India (CTRI/2023/06/054485). One-hundred and thirty-five permanent molar teeth with extremely deep caries presenting with a clinical diagnosis of SIP were recruited. PP was performed following a standardized protocol, by first amputating 2-3 mm of exposed coronal pulp, prior to haemostasis being achieved using 2.5% sodium hypochlorite with pressure up to 15 min. The time taken to arrest bleeding was recorded and categorized into three groups: 1-5 min, 6-10 min and 11-15 min. Biodentine was used as the pulp capping material and the tooth restored. Pain scores were recorded pre-operatively, at 24 h, 48 h and 7 days post-operatively using a visual analogue scale (VAS). Clinical and radiographic evaluations were carried out at 6 and 12 months. Statistical analysis included: Friedman test, Kruskal-Wallis, t-test, Mann Whitney U test, Cramer's V coefficient (V) and Spearman correlation (rho). The significance level was p < 0.05. RESULTS:Of the 135 cases, 123 cases were evaluated at 12-months reflecting a recall rate of 91.1% (123/135). The overall success rate of PP at 12 months was 91.9%. No statistically significant association was shown between the treatment outcome and time taken to achieve haemostasis across the three categories. However, a weak correlation was demonstrated between bleeding time and pre-operative pain intensity (rho = 0.262; p = 0.002). A moderate association was evident between patient age and treatment outcome (V = 0.38; p = 0.001). Other variables, such as gender, tooth type, site of caries, pre-operative VAS score, pain on percussion and periapical index score did not significantly influence the outcome (p > 0.05). CONCLUSION:Within the limitations of this study, time required to achieve pulpal haemostasis (up to 15 min) did not significantly influence treatment outcome at 12 months, indicating that haemostasis time may not be a reliable predictor of success. PP demonstrated a high overall success rate, suggesting that favourable outcomes can be achieved even in cases with prolonged bleeding, provided adequate haemostasis is attained. TRIAL REGISTRATION:The study was registered prospectively with the Clinical Trials Registry of India with Registration No. (CTRI/2023/06/054485).

3Molecular Insight into Proliferative Verrucous Leukoplakia: Are TERT Promoter Mutations a Predictive Marker?
    Nikita Garg,Priya Kumar,Aadithya B Urs,Seema Kapoor, Somesh Kumar

Proliferative verrucous leukoplakia (PVL) is a rare yet aggressive oral potentially malignant disease (OPMD) with the greatest rate of malignant transformation. Its pathophysiology is poorly understood despite much histopathological and molecular research. Particularly, Telomerase reverse transcriptase (TERT) promoter mutations at C228T and C250T have been linked to many epithelial malignancies; however, their significance in PVL is yet unknown in the Indian population. The aim of this study was to assess frequency of TERT promoter mutations (C228T, C250T) and rs2853669 single nucleotide polymorphism (SNP) in PVL, oral leukoplakia (OL), oral squamous cell carcinoma (OSCC) and healthy controls. 120 fresh frozen tissue specimens (30 each of OL, PVL, OSCC and controls) were tested for presence of C228T and C250T mutation in TERT promoter gene region and SNP at rs2853669, using Sanger sequencing on genomic DNA. TERT C228T mutation was found in 6.7

4Reverse Method for Hollowing a Definitive Obturator: A Simplistic Approach
Kumari Deepika, Preeti Mishra, Rekha Gupta

Acquired palatal defects may arise from trauma, infection, pathology, radiation therapy, or surgical resection, resulting in oro-nasal communication that adversely affects speech, swallowing, mastication, and patient morale. Obturator prostheses are commonly used to restore these functions; however, in extensive maxillectomy defects, increased prosthesis weight and the absence of adequate retentive undercuts may compromise retention, stability, and patient comfort. Hollowing of the obturator is an effective method for reducing prosthesis weight and improving function. This clinical report describes the rehabilitation of a patient with a total maxillectomy defect secondary to COVID-19-associated mucormycosis using a simplified reverse hollowing technique for fabrication of a definitive obturator. Reverse hollowing was performed from the palatal aspect following conventional processing, and an acrylic shim was incorporated to re-establish palatal contour, thereby improving speech resonance and swallowing efficiency. A heat-cured resilient liner (Molloplast B) was used to engage soft-tissue undercuts and enhance retention. The described technique is simple, efficient, economical, and reproducible, and may be particularly useful in extensive defects where conventional hollowing procedures are difficult to perform.

5Comparative Evaluation of Herbal and Conventional Endodontic Irrigants on Postoperative Pain and Substance P Levels in Patients with Symptomatic Irreversible Pulpitis and Apical Periodontitis.
Lubna Hassan,Sonali Taneja, Vidhi Bhalla, Setu Katyal

Introduction Postoperative pain is a common complication following root canal treatment (RCT) in teeth with symptomatic irreversible pulpitis and apical periodontitis. This study aimed to compare the effects of ketorolac tromethamine, 100% Moringa oleifera leaf extract, 50% Azadirachta indica leaf extract, and 5.25% sodium hypochlorite, as final root canal irrigants, on postoperative pain intensity and Substance P expression in patients with symptomatic irreversible pulpitis and apical periodontitis. Materials and methods In this prospective observational study, 84 patients with maxillary or mandibular molars diagnosed with symptomatic irreversible pulpitis and apical periodontitis were divided into four groups (21 patients each) based on the final irrigant used during single-visit RCT. Preoperative and postoperative Substance P levels were measured in pulp and periapical blood/exudate samples using enzyme-linked immunosorbent assay (ELISA). Pain intensity was assessed preoperatively and 24 hours postoperatively using a visual analog scale (VAS). Statistical analyses included paired t-tests, Wilcoxon signed-rank tests, one-way analysis of variance (ANOVA), Kruskal-Wallis tests, post-hoc comparisons, and Pearson correlation. Results All irrigants were associated with significant intragroup reductions in both Substance P levels and pain intensity at 24 hours (p = 0.001). Preoperative Substance P levels showed significant intergroup differences, which largely resolved postoperatively. Postoperative pain scores differed significantly between groups, with A. indica being associated with higher pain than the other three irrigants, which demonstrated comparable outcomes. No consistent correlation was found between Substance P levels and pain scores. Conclusion All tested irrigants effectively reduced postoperative pain and Substance P expression. Sodium hypochlorite, ketorolac tromethamine, and M. oleifera leaf extract provided similar analgesic efficacy, which was superior to that of A. indica. Alternative irrigants may serve as viable adjuncts in clinical practice, particularly when enhanced biocompatibility or anti-inflammatory effects are desired.
