    El monitoreo hemodinámico en recién nacidos sometidos a cirugía cardíaca constituye un componente esencial para la detección precoz de complicaciones y la optimización del tratamiento. La presión auricular izquierda (PAI) representa un parámetro clave para la evaluación de la función ventricular izquierda, la precarga y la interacción cardiopulmonar. Su monitoreo se reserva principalmente para neonatos con cardiopatías congénitas complejas, particularmente en el período postoperatorio inmediato de cirugía cardiovascular, donde existe riesgo de alteraciones en el retorno venoso pulmonar, disfunción ventricular izquierda o inestabilidad hemodinámica. Permite una evaluación precisa de la precarga y facilita la detección precoz de complicaciones como obstrucciones residuales o bajo gasto cardíaco. El objetivo de este artículo es contemplar las características principales de la PAI desde una perspectiva fisiopatológica y resaltar el rol crucial del profesional de enfermería en el cuidado del recién nacido crítico. La evidencia disponible indica que la adecuada vigilancia de la PAI permite identificar precozmente alteraciones hemodinámicas, prevenir complicaciones como el edema pulmonar y el bajo gasto cardíaco, y guiar intervenciones terapéuticas oportunas. El cuidado de enfermería especializado es determinante para la seguridad del paciente y la evolución clínica favorable.

    Hypertension is the main risk factor for cardiovascular disease (CVD), affecting 20-40% of Latin American (LATAM) adults, and responsible for more than two million deaths annually due to CVD. The different ethnic, economic, geographic, and cultural characteristics of the LATAM population influence the high prevalence of all cardiovascular risk factors (CVRF), particularly metabolic disturbances such as type 2 diabetes (DM2), obesity and the metabolic syndrome. Their main determinants in LATAM includes environment, food quality, social inequity, low education, political aspects, contextual behaviour, and genetics. The prevalence of overweight and obesity in LATAM increased during the last four decades reaching figures of 10-20% in childhood, 30-40% in adolescence, and 60-70% in adults. Many studies in the region have reported the extremely low rates of awareness, treatment, and control of CVRF in the general population of LATAM, particularly in patients with metabolic disorders, and the consequent high cardiovascular morbidity and mortality. This 2026 LATAM consensus is developed by a large group of experts from different LATAM countries, the USA and Europe, representing areas of internal medicine, cardiology, nephrology, endocrinology, geriatrics, paediatrics, pharmacology, and epidemiology. A careful search for novel studies in LATAM, together with new evidence that has emerged since the 2019 LATAM consensus, support the statements and recommendations in the current report. This update aims to provide clear and useful recommendations for health professionals to improve awareness, treatment, and control of hypertension and associated CVRF in the region.

    El cerebro es el órgano más afectado por la hipertensión arterial, y esta es el principal factor de riesgo vascular potencialmente modificable para desarrollar deterioro cognitivo, enfermedad de Alzheimer y otras demencias. El daño vascular del cerebro permanece silente durante al menos una década y su expresión clínica dependerá de la condición del paciente hipertenso frente al tratamiento antihipertensivo, del control de los factores de riesgo asociados y la reserva cognitiva. Por tal motivo, la evaluación cognitiva temprana del paciente hipertenso –como un “biomarcador” (subrogado) del daño vascular del cerebro mediado por la hipertensión–, mejora la estratificación del riesgo vascular global y previene la enfermedad cognitiva. Este documento proporciona un algoritmo práctico, basado en la evidencia científica, y herramientas sencillas, como el test del reloj, plausible de ser utilizado en la práctica médica asistencial y detectar en forma temprana el compromiso del cerebro como órgano blanco de la hipertensión arterial.
