Interactions between extracellular signals with the extracellular matrix (ECM) influence cellular phenotype and molecular functions, affecting proliferation, differentiation, adhesion, apoptosis, and migration. Deregulation of ECM remodeling contributes to the development of diseases, including breast cancer. The study aimed was to identify microRNAs (miRNAs) that may regulate the activity of genes involved in ECM remodeling and focal adhesion across five breast cancer subtypes in Polish women. The study enrolled patients representing five breast cancer subtypes: 130 luminal A, 100 HER2-negative luminal B, 96 HER2-positive luminal B, 36 non-luminal HER2-positive, 43 triple-negative breast cancer (TNBC) cases. Cancer tissue samples were collected during surgery along with adjacent healthy tissue margins (control group). The expression profiles of genes associated with ECM remodeling and focal adhesion were evaluated with mRNA microarrays and reverse transcription quantitative polymerase chain reaction (RT-qPCR). Protein expression was assessed using enzyme-linked immunosorbent assay (ELISA). miRNA detection and target prediction were performed using miRNA microarrays. Overexpression of COL1A1, FN1, ITGB1, and THBS1 may be associated with reduced levels of miR-129, miR-432, miR-124, miR-384, respectively. Decreased COL6A6 expression may result from increased activity of miR-1246. Additionally, the study revealed increased levels of COL1A2, COMP, SPP1 with reduced activity of RELN, across all five breast cancer subtypes. This is the first study to comprehensively analyze miRNA-mediated regulation of ECM-related genes across five breast cancer subtypes in a Polish cohort. Overexpression of COL1A1, FN1, and ITGB1 is linked to reduced levels of specific miRNAs, while decreased COL6A6 expression is associated with increased miR-1246 activity.
Purpose To evaluate efficacy results of the GEC-ESTRO phase III non-inferiority trial in subgroups of patients defined by the ASTRO criteria for patient selection for APBI. Methods Patients aged ≥40 years with low risk invasive breast cancer or ductal carcinoma in-situ after breast conserving surgery were randomized to receive either 50 Gy WBI with tumor bed boost of 10 Gy or APBI using multicatheter brachytherapy. 633/1184 patients were randomized to APBI using multicatheter brachytherapy. In the context of the current available complete results after 10 years follow-up, we report results of an additional sub-analysis of these latter 633 patients regarding 10-year local control rates, disease-free and overall survival results comparing sub-groups categorized according to ASTRO criteria for patient selection for APBI. The trial is registered with ClinicalTrials.gov, NCT00402519. Results We recorded the 10-year local recurrence rate in the APBI arm in the value of 3.5%, which is one of lowest 10y.-local recurrence rates among all other published phase 3 trials. Among 633 patients treated with sole multicatheter brachytherapy for APBI, according to ASTRO criteria altogether 423 patients (66.8%) were classified as "suitable", 195 (30.8%) as "cautionary" and 11 (1.7%) as "unsuitable"; in 4 cases no ASTRO categorization was possible. In the "suitable" and "cautionary" groups, resp., the cumulative incidence (95% CI) of local recurrence at 10 years was 3.29% (1.44-5.13%) and 5.03% (1.8-8.27%), respectively (p=0.612). 10-year disease-free survival was 81.91% (78.1-85.9%) and 83.63% (78.31-89.32%), respectively (p=0.63). 10-year overall survival was 88.26% (85.07-91.57%) and 95.04% (91.92-98.27%), respectively (p=0.02). Conclusion The 10-year results are comparable in patients treated with breast conserving surgery followed by APBI using multicatheter brachytherapy in "suitable" and "cautionary" risk groups' according to ASTRO criteria. As a consequence we believe that in due time, a careful retrial of current ASTRO selection criteria for APBI should be considered. Na tle innych zaburzeń mowy jąkanie nie wykazuje wysokiej frekwencji. Dotyczy jednak osób w różnym wieku z różnych środowisk, żyjących w różnych czasach i miejscach. Dla osób z nim się zmagających często stanowi źródło frustracji i stresu. Niestety – jak wykazują badania – w całym społeczeństwie poziom wiedzy na temat jąkania nie jest zbyt wysoki, a osoby mówiące niepłynnie często doświadczają negatywnych reakcji społecznych (wyśmiewanie, drwina, niska ocena ich potencjału). Potwierdzają to badania z wykorzystaniem wystandaryzowanych narzędzi: POSHA-S/C oraz POSHA-S-Child. Do przyczyn upowszechniania się stereotypowego obrazu jąkania i mitów na ten temat zaliczyć można m.in. portretowanie tego zjawiska w filmach fabularnych. Należy mieć jednak nadzieję, że powstające w ostatnich latach filmy dokumentalne mogą przyczynić się do zmiany tego obrazu.W niniejszym artykule podjęto próbę oceny, czy skonfrontowanie studentów z treścią pełnometrażowego filmu dokumentalnego o jąkaniu (When I stutter – „Czasami się jąkam”) może wpłynąć na zmianę percepcji tego zjawiska wśród respondentów, a przede wszystkim na zmianę postaw społecznych wobec jąkania i na zmianę stereotypowego postrzegania osób jąkających się. Badania metodą sondażu diagnostycznego przeprowadzono na grupie 158 studentów z dwóch polskich uczelni (Uniwersytet Rzeszowski i Państwowa Uczelnia im. Stefana Batorego w Skierniewicach). Ankietowani wypełnili kwestionariusze przed projekcją filmu i po niej. Wyniki przeprowadzonych analiz jednoznacznie pokazały zmianę postaw wobec osób jąkających się. Respondenci przed wydarzeniem uznawali, że istnieją liczne ograniczenia w wykonywaniu wielu zawodów (głównie logopedy, nauczyciela i aktora), po spotkaniu zmienili zdanie na ten temat – większość nie widziała już żadnych ograniczeń. Przed projekcją 2/3 ankietowanych uznało jąkających się za nerwowych, nieśmiałych i zalęknionych, po obejrzeniu filmu dominowały opinie, że są oni przede wszystkim odważni i wrażliwi. W ankiecie PRE respondenci sugerowali, że jąkający się powinni wkładać więcej wysiłku w pracę nad swoją mową, po projekcji takich sugestii było zdecydowanie mniej, a dominowały stwierdzenia o akceptacji i pochwałach.Wszyscy uczestnicy spotkań podkreślali potrzebę upowszechniania filmów oraz organizowania wydarzeń przybliżających problemy osób zmagających się z jakimikolwiek ograniczeniami. Wyniki badań (zarówno tych zaprezentowanych w artykule, jak i innych dostępnych w literaturze przedmiotu) pozwalają wysnuć tezę, że film – jako medium ogólnodostępne i stosunkowo łatwe w odbiorze – może odgrywać znaczącą rolę dla zmiany postaw wobec osób zmagających się z rozmaitymi zaburzeniami. Nie tylko zatem można, ale wręcz trzeba go wykorzystywać dla podniesienia wiedzy społeczeństwa o różnych zaburzeniach, chorobach, niedyspozycjach.
