Цель. Определить прогностическое значение поражений магистральных артерий нижних конечностей и результатов их коррекции при синдроме диабетической стопы (СДС). Материал и методы. Обследованы 452 пациента с СДС. Гемодинамически значимые нарушения магистрального кровотока по артериям нижних конечностей выявлены в 53,3% случаев. Реконструктивно-восстановительные операции на артериях нижних конечностей выполнены у 22,6% пациентов. Оперативная активность в выборке составила 60,0%. Локальные вмешательства на стопе были выполнены у 48,2% пациентов, из которых у 14,7% потребовалось проведение серии дополнительных локальных вмешательств, а у 6,4% в итоге пришлось прибегнуть к высокой ампутации нижней конечности (ВАНК). Первичные ВАНК были использованы в 11,7% случаях, в 26% из них возникли осложнения со стороны культи, по поводу которых пациенты были оперированы. Результаты. Факторами риска выполнения дополнительных операций на стопе после локальных вмешательств при хирургическом лечении СДС являются поражения подвздошно-бедренного сегмента, сочетанные поражения бедренно-подколенного сегмента и артерий голени. С риском выполнения ВАНК после локальных вмешательств на стопе связана сочетанная патология подвздошно-бедренного сегмента и артерий голени. Риск выполнения первичной ВАНК ассоциирован с поражением подвздошно-бедренного сегмента, бедренно-подколенного сегмента, артерий голени, сочетанным поражением бедренно-подколенного сегмента и артерий голени. Риск выполнения ВАНК в процессе хирургического лечения соотносится с наличием поражений подвздошно-бедренного сегмента, бедренно-подколенного сегмента, артерий голени. После выполнения реконструктивно-восстановительных операций на магистральных артериях нижних конечностей в процессе дальнейшего лечения возрастает риск необходимости выполнения дополнительных операций на стопе после локальных вмешательств и риск выполнения ВАНК после локальных вмешательств на стопе, и снижается риск выполнения первичной ВАНК и ВАНК в процессе лечения. Исходные гемодинамически значимые поражения магистральных артерий нижних конечностей и выполнение по их поводу реконструктивно-восстановительных операций не оказывает влияние на частоту развития местных осложнений после первичной ВАНК. Выводы. Полученные сведения могут быть использованы для прогнозирования результатов и определения рациональной тактики хирургического лечения пациентов с СДС.
Введение. Психодерматология – это специальность, которая сочетает психиатрию и дерматологию. Кожа и центральная нервная система связаны через их общее эмбриональное происхождение – эктодерму. У них также есть общие нейромодуляторы, пептиды и биохимические системы внутренней информации. Психиатрия больше сосредоточена на внутренних невидимых симптомах, а дерматология – на внешних видимых признаках. Эта связь между кожными заболеваниями и психикой, к сожалению, недооценивается. Несмотря на распространенность различных психодерматологических заболеваний, многие пациенты нередко остаются недостаточно диагностированными или недолеченными по ряду причин, таких как отсутствие стандартизированных психодерматологических услуг, необходимость обучения и образования в этой области среди дерматологов и готовность пациента обращаться за психиатрической помощью. Цель. Оценка осведомленности белорусских и узбекских дерматологов, моделей их практики и отношения к психодерматологии. Материалы и методы. Проведено исследование осведомленности дерматологов в психодерматологии на основе анонимных анкет с участием 102 дерматологов из Беларуси и Узбекистана. Анкета состояла из 2 блоков: первый включал демографические данные, второй – вопросы, связанные с психодерматологией. Цифровые данные обрабатывали статистически с использованием пакета Microsoft Office на базе OC Windows 10. Для проверки распределения на нормальность использовали критерий χ2 Пирсона, для сравнения выборок – t-критерий Стьюдента для независимых выборок. Различия считались статистически значимыми при p<0,05. Результаты. Психодерматологию как двунаправленное взаимодействие психических и дерматологических состояний пациента понимали 53,9% респондентов. Самыми частыми заболеваниями, протекающими с вовлечением психоэмоциональной сферы, были названы акне, алопеция, витилиго, псориаз и атопический дерматит. 42,3% белорусских дерматологов отмечают, что чувствуют себя «немного некомфортно» при общении с пациентами, имеющими психодерматологические патологии, а 37,7% врачей только 1 раз в год отправляют таких пациентов к психотерапевту. Нейтральный уровень коммуникации и редкое направление к психотерапевту отметили 40% узбекских дерматологов. Самыми частыми диагнозами при направлении к психотерапевту были названы патомимия, трихотилломания, дерматозойный бред. Не смогли назвать ни одного ресурса (журнала, веб-сайта, общества) для пациентов и их семей, где была бы доступна информация о психодерматологии, 67,3% белорусских и 92% узбекских респондентов. Однако 76,9% белорусских и 50% узбекских дерматологов проявили интерес к посещению обучающих мероприятий по психодерматологии. Заключение. Результаты нашего исследования показывают, что белорусские и узбекские дерматологи хорошо осведомлены о термине «психодерматология», но только незначительная часть практикующих врачей чувствует себя достаточно подготовленной для лечения пациентов с психосоматическими аспектами. Относительно значительный разрыв между количеством пациентов с психодерматологическим компонентом и направлениями дерматологов к специалистам по психическому здоровью указывает на потенциальную важность более интенсивного сотрудничества между дерматологами, психиатрами, психологами. Introduction. Psychodermatology is a specialty that includes a combination of psychiatry and dermatology. The skin and the central nervous system are interconnected through their common embryonic origin, the ectoderm. They also share common neuromodulators, peptides, and biochemical internal information systems. Psychiatry focuses more on internal invisible symptoms, while dermatology focuses on external visible signs. Unfortunately, this interconnection between skin diseases and the psyche is underestimated. Despite the prevalence of various psychodermatological diseases, many patients often remain underdiagnosed or untreated for a number of reasons, such as the lack of standardized psychodermatology services, the need for training and education in this field among dermatologists, and the patients’ unwillingness to seek psychiatric help. Purpose. The purpose of this study is to assess the awareness of dermatologists from Belarus and Uzbekistan, their practice patterns and attitudes towards psychodermatology. Materials and methods. The study of dermatologists’ awareness of psychodermatology was carried out basing on unnamed questionnaires with the participation of 102 dermatologists from Belarus and Uzbekistan. The questionnaire consisted of two blocks: the first block included demographic data, the second block – questions related to psychodermatology. The digital data were statistically processed using the Microsoft Office suite based on OC Windows 10. Pearson’s X-squared criterion was employed to check the distribution for normality, and Student’s t-test for independent samples was applied to compare samples. The differences were considered statistically significant at p<0.05. Results. Psychodermatology was understood by 53.9% of the respondents as a bidirectional interaction of mental and dermatological conditions of the patient. The most common diseases involving the psychoemotional sphere were acne, alopecia, vitiligo, psoriasis and atopic dermatitis. 42.3% of Belarusian dermatologists say they feel "a little uncomfortable" when dealing with patients with psychodermatological pathologies, and only 37.7% of doctors send such patients to a psychotherapist only once a year. A neutral level of communication and a rare referral to a psychotherapist were noted by 40% of Uzbek dermatologists. The most common diagnoses when referring to a psychotherapist were pathomimia, trichotillomania, dermatozoal delirium. 67.3% of Belarusian and 92% of Uzbek respondents could not name a single resource (magazine, website, society) for patients and their families where information about psychodermatology was available. However, 76.9% of Belarusian and 50% of Uzbek dermatologists expressed interest in attending psychodermatology training events. Conclusion. The results of our study show that Belarusian and Uzbek dermatologists are well aware of the term "psychodermatology", but only a small part of practitioners feel sufficiently prepared to treat patients with psychosomatic aspects. The relatively significant gap between the number of patients with a psychodermatology component and the referrals of dermatologists to mental health specialists indicates the potential importance of more intensive cooperation between dermatologists, psychiatrists, and psychologists.
Introduction. Modern scientific data emphasize the need for an integrated approach to the diagnosis and treatment of infectious and inflammatory diseases of the maxillofacial region, taking into account the current microbiological profile, local monitoring of resistance and the patient’s comorbid background. The aim of the study is to analyze the main pathogens of infectious and inflammatory diseases of the maxillofacial region. Material and methods. A retrospective study was conducted at the dental purulent department from 2020 to 2024. The objects of the study were the results of microbiological cultures performed upon admission and / or during inpatient treatment of patients with clinically verified infectious and inflammatory processes. Results and discussion. The analysis of 2020–2024 showed the proportion of identified pathogens of 48–64 %, with a dominance of staphylococci (> 60 % of strains). A significant decrease in S. aureus was noted with an increase in coagulase-negative staphylococci (CNS, p = 0.01) and a twofold increase in MRSA. The proportion of streptococci is stable (p >0.05), enterococci are rare in cultures, but their proportion among pathogens increased in 2024. The frequency of Candida spp., non-fermenting gram-negative rods and enterobacteria is stable (p > 0.05), but a tendency towards an increase in enterobacteria was revealed, the emergence of carbapenem-resistant K. pneumoniae is especially worrisome (up to 28.87 % in 2024). It was revealed that odontogenic infection pathogens are capable of forming microbial biofilms to varying degrees. Comparison of 2014/2024 revealed a decrease in the proportion of staphylococci (mainly KOS), an increase in streptococci, opportunistic flora, Candida spp. and enterobacteria (especially resistant K. pneumoniae). Conclusions. Thus, the dominance of staphylococci with an increase in KOS and MRSA remains, the proportion of opportunistic microorganisms (enterococci, enterobacteria) and carbapenem resistance in K. pneumoniae increases. This requires constant microbiological monitoring and adaptation of antimicrobial therapy.
The aim of this review article is to systematize current data on materials and alloys used for the restoration of bone defects in the maxillofacial region, evaluate their advantages and limitations, and assess their prospects for clinical application. Titanium remains the “gold standard” in medical implants due to its high strength, biocompatibility, and durability. However, modern trends in medicine and materials science indicate a growing interest in bioresorbable materials and 3D technologies, which open new horizons for the development of more advanced and functional implants. These innovative approaches not only enhance implant integration with biological tissues but also minimize the risks of long-term complications, such as bone stress shielding or the need for secondary surgeries to remove the implant. One of the most crucial aspects of modern implantology is the necessity for a personalized approach to each patient. This includes considering not only anatomical features but also individual biological and physiological parameters. Interdisciplinary research that brings together the expertise of surgeons, material scientists, bioengineers, and biologists plays a key role in the successful development of new materials and technologies, which present immense opportunities for improving patients’ quality of life. However, to fully realize this potential, continued active research, strengthened interdisciplinary collaboration, and attention not only to technical aspects but also to ethical and economic considerations in the development and implementation of innovations are essential.
