
Abstract: Background: Between 2021 and 2023, we launched three quality assurance (QA) projects to establish a secure, clinically useful, and easily implementable QA system for (1) adult psychotherapy patients in (a) outpatient training clinics and (b) private practice and (2) child and adolescent psychotherapy in outpatient care. These projects aim to strengthen psychotherapy training, support routine clinical decision-making, and enable systematic research on structural, process, and outcome quality. Objective: The overarching goal of the projects is to provide a lean, standardized, GDPR-compliant QA platform that delivers automated, clinically meaningful feedback to therapists and institutions for both treatment indication (baseline) and progress monitoring (follow-up), and to enable research on treatment indication, processes, and effectiveness in routine care. Methods: Patients contacting participating institutions receive automated online questionnaires that assess relevant clinical dimensions, including symptom severity, personality functioning, trauma-related symptoms, and emotion regulation. Follow-up assessments occur every 3 months for up to 8 year, regardless of therapy duration. Age-appropriate instruments and multi-informant perspectives are used for children and adolescents. Participating therapists and institutions receive individual, group, and institutional feedback reports to support internal QA. Results: The QA systems provide immediate clinical feedback, including reliable change indices and graphical trajectories. Institutions receive regular reports about several quality dimensions with comparisons to anonymized reference data. The systems have been implemented in 32 outpatient clinics and over 350 private practices, demonstrating high feasibility, technical stability, and acceptance. Conclusions: The QSP/QVA and QVA-KJ systems offer a feasible and easy-to-implement approach to integrating quality assurance into routine outpatient care across child, adolescent, and adult psychotherapy. The systems provide clinically useful feedback, support clinical decision-making, strengthen therapist training, and foster psychotherapy research. All systems are continuously maintained and refined in close collaboration with participating therapists and institutions.
Zusammenfassung: Theoretischer Hintergrund: Psychotherapie soll Sicherheit und faire Versorgung bieten, wird für minorisierte Patient_innen jedoch nicht selten zum Schauplatz subtiler Diskriminierungserfahrungen. Von Therapeut_innen getätigte Mikroaggressionen gefährden die therapeutische Allianz und den Behandlungserfolg, insbesondere wenn sie unerkannt bleiben. Fragestellung: In welcher Form treten typische Mikroaggressionen in deutschen Psychotherapien auf? Methode: Schilderungen von N = 278 minorisierten Patient_innen über ihre Erfahrungen mit Rassismus, Sexismus, Hetero- und Cis-Sexismus innerhalb von Psychotherapien wurden in online-Erhebungen gesammelt und inhaltsanalytisch ausgewertet. Ergebnis: Es konnten zehn gängige Typen psychotherapeutischer Mikroaggressionen identifiziert werden. Sie traten über verschiedene Patient_innengruppen sowie unabhängig vom Therapieverfahren auf und spiegeln ein strukturell verankertes Muster subtiler Diskriminierung wider. Schlussfolgerungen: Die vorliegende Taxonomie bietet eine Grundlage, um Sensibilität, Selbstreflexion und kulturelle Demut im psychotherapeutischen Handeln zu stärken.
Zusammenfassung: Theoretischer Hintergrund: Der neue Masterstudiengang „Klinische Psychologie und Psychotherapie“ („KliPPt-Master“) verfolgt das Ziel, Studierenden sowohl praktische als auch wissenschaftliche Kompetenzen zu vermitteln. Entscheidend dafür sind die Lehrenden, die die Anforderungen der Approbationsordnung in ihrer Lehre umsetzen. Fragestellung: Wie schätzen Lehrende die klinisch-praktische und die wissenschaftliche Kompetenz der KliPPt-Studierenden im Vergleich zu M.Sc.-Psychologie-Studierenden ein? Wie gut bereitet der KliPPt-Master aus ihrer Sicht auf verschiedene Karriereschritte vor? Worin werden seine Vor- und Nachteile gesehen? Methode: Es nahmen N = 251 Lehrende an einer querschnittlichen Online-Befragung teil. Ergebnisse: Die praktische Kompetenz der KliPPt-Studierenden wurde signifikant niedriger bewertet als ihre wissenschaftliche Kompetenz ( d = -0.26). Im Vergleich zu M.Sc.-Psychologie-Studierenden zeigte sich eine höher eingeschätzte praktische ( d = 0.67) und eine niedriger eingeschätzte wissenschaftliche Kompetenz ( d = -0.63). Eine Vernachlässigung wissenschaftlicher Inhalte wurde kritisiert. Schlussfolgerungen: Lehrende sehen eine positive Entwicklung der klinisch-praktischen Kompetenz, betonen jedoch die Notwendigkeit einer besseren Integration praktischer und wissenschaftlicher Inhalte.
Zusammenfassung: Open-Science-Praktiken gewinnen auch in der klinisch-psychologischen Forschung zunehmend an Bedeutung, stellen Forschende jedoch vor besondere rechtliche und ethische Herausforderungen. Dieser Überblicksartikel fasst zentrale gesetzliche Rahmenbedingungen im deutschsprachigen Raum zusammen – insbesondere Datenschutz-, Urheber- sowie weitere relevante Forschungsrechte – und zeigt ihre Bedeutung für Open Data, Open Materials und Open Access auf. Im Fokus stehen praxisorientierte Empfehlungen zu Einwilligungsprozessen, zur Datenaufbereitung und zu rechtskonformen Veröffentlichungswegen. Zudem werden Besonderheiten für vulnerable Gruppen wie Minderjährige, Menschen mit neuropsychologischen Störungen sowie nicht-einwilligungsfähige Personen diskutiert und Anforderungen an multizentrischen Studien oder klinischen Einrichtungen dargestellt. Der Artikel bietet konkrete Lösungsansätze zur Anonymisierung, Auswahl geeigneter Repositorien, abgestuften Zugriffskontrollen sowie zur frühzeitigen Abstimmung mit Datenschutzbeauftragten und Ethikkommissionen. Ziel ist es, Forschenden eine fundierte Orientierung zu geben, um Open Science in der klinisch-psychologischen Forschung verantwortungsvoll, rechtssicher und ethisch reflektiert umzusetzen.
Zusammenfassung: Theoretischer Hintergrund: Die COVID-19-Pandemie führte zu deutlichen Belastungen bei Kindern und Jugendlichen. Multiple Barrieren erschweren den Zugang zu professioneller Hilfe, weshalb niedrigschwellige Angebote im Sinne des Stepped Care-Ansatzes notwendig sind. Fragestellung: Die Akzeptanz einer psychotherapeutischen Beratung, die Beratungsanlässe und durchgeführte Interventionen wurden untersucht. Methode: 143 Kinder und Jugendliche ( M = 13.47 Jahre, SD = 2.83, Range: 6 – 19) nahmen an 322 Beratungssitzungen teil, die von fünf Beraterinnen durchgeführt wurden. Die Beratungsprotokolle wurden hinsichtlich des Vorstellungsgrunds und der darauffolgenden Interventionen untersucht. Ergebnisse: Die Beratung wurde überwiegend als hilfreich empfunden. Die Teilnehmenden fühlten sich verstanden und ernst genommen. Die häufigsten Belastungen waren Ängste (50 %, n = 72), depressive Symptome (43 %, n = 62) und familiäre Probleme (36 %, n = 51). Häufig wurden Mehrfachbelastungen genannt ( M = 3.59, SD = 2.21, Range: 1 – 11 Belastungen). Die eingesetzten Interventionen waren ressourcenorientiert und umfassten praktische Tipps zum Umgang mit Problemen sowie Verweise auf ergänzende Hilfemaßnahmen. Schlussfolgerung: Die Rückmeldung der Teilnehmenden, die zumeist wiederholte Inanspruchnahme und der Beratungsbedarf verdeutlichen die Bedeutung des niedrigschwelligen Beratungsangebotes.
Theoretical Background: The new psychotherapy master's program aims to teach students practical and scientific skills. Fundamental to this are the lecturers, who implement the licensing requirements in their courses. Research Questions: How do lecturers rate the clinical-practical and scientific competence of psychotherapy master's students compared to psychology master's students? How well does the psychotherapy master's degree prepare students for the various career steps, and what advantages and disadvantages do lecturers mention? Method: In total, 251 lecturers completed a cross-sectional online survey. Results: The practical skills of psychotherapy master's students were rated significantly lower than their scientific skills (d = -0.26). Compared to psychology master's students, psychotherapy master's students were rated higher in practical skills (d = 0.67) and lower in scientific skills (d = -0.63). Lecturers noted there was insufficient emphasis on scientific content. Conclusion: Lecturers perceive positive development in students' clinical-practical competence but highlight the need for stronger integration of practical and scientific training.
Open science practices are becoming increasingly important in clinical psychology research, but they pose particular legal and ethical challenges for researchers. This article provides an overview of the key legal frameworks in the German-speaking countries, focusing on data protection, copyright, and other relevant research rights, highlighting their importance for open data, open materials, and open access. It provides practical recommendations for consent processes, data preparation, and legally compliant publication channels. Additionally, it discusses special considerations for vulnerable groups, such as minors, individuals with neuropsychological disorders, and individuals unable to give consent. It presents requirements in the context of multicenter studies or clinical institutions. The article offers concrete solutions for anonymization, selecting suitable repositories, implementing tiered access controls, and coordinating with data-protection officers and ethics committees. The aim is to provide researchers with sound guidance on implementing open science in clinical psychology research in a responsible, legally compliant, and ethically reflective manner.
Background: The impact of the COVID-19 pandemic on young people has been profound. Low-threshold services based on the stepped care approach are needed to overcome multiple barriers to accessing professional help. Objective: We examined the acceptance of psychotherapeutic counseling, the reasons for seeking counseling, and the interventions carried out. Methods: 143 children and adolescents (M = 13.47 years, SD = 2.83, range: 6-19) participated in 322 counseling sessions, conducted by five trained counselors. We analyzed the reason for presentation and subsequent interventions. Results: Children and adolescents found counseling mostly helpful, felt understood, and taken seriously. The most prevalent stressors were anxiety (50 %), depressive symptoms (43 %), and family problems (36 %). The majority reported multiple stressors (M = 3.59, SD = 2.21, Range: 1-11 stressors). The counselors primarily adopted a resource-oriented approach, offered guidance on problem-solving, and made referrals for further support. Conclusions: This article highlights the importance of low-threshold psychotherapeutic counseling services based on the feedback from the children and adolescents as well as their repeated use of this service and the described demands.
: Background: Participatory research has become important for intervention development. Furthermore, there is a lack of training to improve communication on mental health issues in cancer care. Objective: To develop an online communication training tool for medical staff in a participatory process. Method: A participatory research team (PRT, n = 11 medical staff and cancer patients) discussed psychosocial stressors and challenging communicative situations in eight moderated focus group discussions. Based on the results and the empirical literature, training materials were created and evaluated by the PRT. All focus group discussions were audio-recorded, transcribed verbatim, and analyzed qualitatively. Results: Qualitative analysis identified "information exchange" and "responding to emotions" as the most crucial communication strategies. Training should focus on how to communicate with cancer patients with depression and anxiety. Conclusion: In the low-threshold online intervention, participants learn how to convey information appropriately and respond to patients' emotional stress through model conversations between patients and practitioners. Zusammenfassung: Theoretischer Hintergrund: Partizipative Forschung ist f & uuml;r die Entwicklung von Interventionen wichtig. Daneben gibt es nur wenige Kommunikationstrainings & uuml;ber psychische Probleme bei Krebspatient_innen. Fragestellung: Ziel war die partizipative Entwicklung eines Online-Kommunikationstrainings f & uuml;r medizinisches Personal. Methode: Ein partizipatives Forschungsteam (PRT, n = 11 medizinische Fachkr & auml;fte und Krebspatient_innen) diskutierte in acht moderierten Fokusgruppendiskussionen psychosoziale Stressoren und herausfordernde Kommunikationssituationen. Auf Grundlage der Ergebnisse und der empirischen Literatur wurden Schulungsmaterialien erstellt und anschlie ss end erneut vom PRT evaluiert. Alle Diskussionen wurden audiografiert, verbatim transkribiert und qualitativ analysiert. Ergebnisse: Der qualitativen Analyse zufolge waren die Kommunikationsstrategien & bdquo;Informationsaustausch" und & bdquo;auf Emotionen eingehen" am wichtigsten. Ein Training sollte sich darauf konzentrieren, wie man mit depressiven und & auml;ngstlichen Krebspatient_innen kommuniziert. Schlussfolgerung: In der niedrigschwelligen Online-Intervention lernen Teilnehmer_innen anhand von Modellgespr & auml;chen zwischen Patient_innen und Behandler_innen Informationen ad & auml;quat zu vermitteln und auf die emotionalen Belastungen der Patient_innen einzugehen.
: Background: Routine outcome monitoring (ROM) is a valuable tool for personalizing psychotherapeutic treatment and enables psychotherapy process research in children and adolescents. However, to the best of our knowledge, no child- and adolescent-specific ROM system has been widely implemented in the German healthcare system. Objective: We propose a consensus-based core outcome set that accommodates the specific needs of child and adolescent psychotherapy across diverse organizational settings. Method: Outcome domains and assessment time points were derived from existing ROM frameworks and theoretical considerations. Psychometrically validated German instruments were selected, prioritizing open-access tools. Results: The core outcome set includes measures for clinically relevant constructs (e. g., symptom burden, therapeutic process) and supports both periodic and session-by-session assessments. Age-specific requirements and multi-informant perspectives were considered. Conclusion: The core outcome set is currently in the implementation phase. It holds promise for enhancing existing quality assurance initiatives (e. g., KODAP) and facilitating multicenter ROM in Germany. Ziel: Wir schlagen ein konsensbasiertes Core Outcome Set vor, das die spezifischen Anforderungen der Kinder- und Jugendlichenpsychotherapie ber & uuml;cksichtigt und flexibel eingesetzt werden kann. : The Trier Treatment Navigator (TTN) is an adaptive feedback system for psychotherapy developed within ongoing research on measurement-based care and quality assurance. To accommodate different implementation environments, the TTN is available in two operational settings: TN-PRO for private outpatient practice and TN-HUB for research clinics and training centers. Both share the same core architecture but differ in their feature configurations and technical requirements. Key components include routine outcome monitoring, individualized treatment recommendations, early warning signals for patients at risk of unfavorable treatment response, and clinical support tools to guide treatment adaptation. The system is a non-commercial scientific project aimed at investigating the outcomes and processes of data-informed psychotherapy. Future work will focus on technological innovation and exploratory dissemination in private practice. This article aims to describe the conceptual framework, core components, and implementation settings of the TTN for clinicians, researchers, and training institutions interested in feedback-informed treatment and measurement-based care. Zusammenfassung: Der Trier Therapie Navigator (TTN) ist ein flexibles Feedbacksystem f & uuml;r Psychotherapie, das im Rahmen laufender Forschungen zu Measurement-Based Care und Qualit & auml;tssicherung entwickelt wurde. Um unterschiedlichen Implementierungskontexten gerecht zu werden, verf & uuml;gt der TTN & uuml;ber zwei kontext-spezifische Einstellungen: TN-PRO f & uuml;r niedergelassene Psychotherapeut_innen und TN-HUB f & uuml;r Forschungskliniken und Ausbildungsambulanzen. Beide haben die gleiche Kernarchitektur, unterscheiden sich jedoch in ihren Funktionskonfigurationen und technischen Anforderungen. Zentrale Komponenten umfassen das Routine Outcome Monitoring, individuelle Behandlungsempfehlungen, Fr & uuml;hwarnsignale f & uuml;r Patient_innen mit erh & ouml;htem Risiko eines ung & uuml;nstigen Behandlungsverlaufs sowie klinische Unterst & uuml;tzungstools zur Behandlungsanpassung. Das System ist ein nicht-kommerzielles wissenschaftliches Projekt, das darauf abzielt, die Ergebnisse und Prozesse datengest & uuml;tzter Psychotherapie zu erforschen. Zuk & uuml;nftige Arbeiten werden sich auf technologische Innovationen und eine explorative Erprobung in niedergelassenen Praxen konzentrieren. Background: Although exposure therapy is highly effective for anxiety disorders, it is still rarely used in clinical practice. Broader implementation requires assessing reasons for exclusion, delivery methods, and therapists' safety behaviors (e. g., proactively teaching strategies to reduce anxiety such as breathing exercises). Currently, there are no validated German-language questionnaires that measure these constructs. Objective: This study translated and validated the German versions of the Broken Leg Exception Scale (BLES), the Exposure Therapy Delivery Scale (ETDS), and the Exposure Implementation Beliefs Scale (EIBS). Method: N = 223 licensed psychotherapists and psychotherapists in training completed the questionnaires. Results: All questionnaires demonstrated satisfactory to good psychometric properties. More negative beliefs about exposure therapy were associated with greater exclusion tendencies, lower guideline adherence, and stronger endorsement of therapist safety behaviors. Conclusions: The German-language questionnaires proved to be reliable, valid, and efficient instruments for assessing clinically relevant aspects of exposure therapy implementation.
: Background: Social anxiety models posit increased self-focused attention; however, it remains unclear how attention is allocated to negative (socially relevant symptoms such as blushing) compared with positive (feeling relaxed) self-relevant stimuli. Aims: We examined whether individuals with speech anxiety exhibit a relative attentional bias toward negative versus positive self-relevant stimuli. Method: A total of 66 participants with high or low speech anxiety responded to stimuli during speech anticipation and performance. External stimuli were LEDs on audience members' foreheads; self-relevant stimuli were tones, purportedly indicating arousal or relaxation. Results: Participants responded slower to self-relevant than to external stimuli. No differential bias for negative self-relevant stimuli (arousal) was observed. However, speech-anxious individuals reacted faster to all stimuli during the speech. Conclusion: No evidence was found for a valence-specific attentional bias toward self-relevant stimuli in speech-anxious individuals. Instead, increased overall reactivity during speech performance suggests heightened vigilance or arousal. This points to a generalized attentional sensitivity in socially anxious individuals, rather than selective attention to negative self-relevant cues. Zusammenfassung: Hintergrund: Modelle der sozialen Angst postulieren eine erh & ouml;hte selbstfokussierte Aufmerksamkeit. Unklar ist, wie Aufmerksamkeit f & uuml;r negative (sozial-relevante Symptome wie Err & ouml;ten) im Vergleich zu positiven (z. B. entspannt sein) selbstrelevanter Reizen gesteuert wird. Zielsetzung: Hier wird gepr & uuml;ft, ob sprech & auml;ngstliche Personen eine relative Aufmerksamkeitsverzerrung zugunsten negativer vs. positiver selbstrelevanter Reize zeigen. Methode: 66 Proband_innen mit hoher oder niedriger Redeangst reagierten w & auml;hrend Redeantizipation und -durchf & uuml;hrung auf verschiedene Reize. Externe Reize waren LEDs auf den Stirnen von Zuh & ouml;rer_innen; selbstrelevante Reize waren T & ouml;ne, die angeblich Erregung oder Entspannung anzeigten. Ergebnisse: Reaktionen auf selbstrelevante Reize waren langsamer als auf externe. Ein spezifisches Bias f & uuml;r negative selbstrelevante Reize (Erregung) zeigte sich nicht. Hoch & auml;ngstliche Personen reagierten jedoch w & auml;hrend der Rede schneller auf alle Reize. Schlussfolgerung: Es fand sich kein valenzspezifisches Bias, sondern eine erh & ouml;hte generelle Reaktivit & auml;t, was auf gesteigerte Wachsamkeit bei sozial & auml;ngstlichen Personen hindeutet.
Background: Individuals with disabilities are underrepresented in psychotherapeutic care. However, it remains unclear to what extent German psychotherapy training institutes prepare future therapists for treating this target group. Objective: To determine how frequently German psychotherapy training institutes integrate content for treating intellectual (ID), motor (MD), and sensory disabilities (SD) into their curricula. Methods: We did a complete survey of all state-recognized institutes (N = 236); the response rate was 19.5 % (n = 46). We collected the data via a standardized questionnaire and open-text responses and analyzed them using descriptive statistics for integration frequency and content-analytical categorization. Results: Curricular integration follows a hierarchy (ID 39.1 %, MD 15.2 %, SD 4.3 %). CBT (52.2 %) and systemic institutes (66.7 %) integrate more frequently than psychodynamic institutes (11.8 %). Integration is mostly case-based, not systematic. Conclusions: The results show that disability-specific content has so far been included in the curriculum only to a limited extent, reflecting the limited scope of action under the old legislation. As an inventory of the transition between the old and new training regulations, the study provides orientation regarding the needs for implementing the new training structures.
Aim: To investigate whether the frequent consumption of junk food and soft drinks is associated with behavioral problems and/or prosocial behavior in adolescents. Method: A total of 22,578 children and adolescents, with a mean age of 13.2 years (SD = 1.74), participated in the study. Primary endpoints were "internalizing and externalizing problems" and "prosocial behaviour" of the Strengths and Difficulties Questionnaire. Frequent consumption of "junk food" and "soft drinks" was assumed if these foods were consumed on at least 5 days a week. Covariates were age, gender, type of school, subjective social status, and sensation-seeking. Results: On at least 5 days a week, 18.9 % of the sample drank soft drinks, 11.3 % consumed junk food. Adolescents who frequently consumed junk food were significantly more likely to exhibit internalizing (aOR = 1.17, 95 % KI 1.01 - 1.36) and externalizing (aOR = 2.16, 95 % KI 1.79 - 2.61) problem behavior and were less likely to exhibit prosocial behavior (aOR = 0.68, 95 % CI 0.60 - 0.76). The same associations were also found with frequent consumption of soft drinks. Discussion: Adolescents in Germany frequently consume junk food and soft drinks. Due to the cross-sectional nature of the study, the reported associations should not be interpreted as causal.
