
PURPOSE:This study aims to investigate the histopathological and biochemical effects of the topical application of Hypericum perforatum on the healing of surgical wounds created in the oral mucosa of rats with experimentally induced diabetes mellitus.MATERIALS AND METHODS:The study was carried out on 48 adult male Wistar albino rats. The animals were divided into two main groups as control and study groups. Two main groups were divided into three subgroups according to the sacrification days. All rats were given streptozotocin 60 mg/kg, after 72 hours, and those having blood glucose levels above 200 mg/dL were included in the study. Mucosal defects were created in the palatal area of the rats. H. perforatum oil was applied topically twice a day to the wounds of the rats in the study group. Animal were sacrificed on the 3rd, 7th and 10th days and samples taken from the palatal wounds were examined histologically and biochemically.RESULTS:On the 7th day, ulceration, necrosis, epithelialization, polymorphonuclear leucocytes and hydroxyproline variables showed statistically significant differences (p<0.05). Ulceration, necrosis and polymorphonuclear leukocytes values were higher in the control group, whereas epithelialization and hydroxyproline values were found to be higher in the H. perforatum group. Among 10th-day groups, there was only a statistically significant difference between the values of hydroxyproline, whereas H. perforatum-treated group showed high hydroxyproline levels (p<0.05).CONCLUSION:Topically applied H. perforatum did not create any difference on the 3rd day, but it has positively affected the wound healing on the 7th and 10th days in diabetic rats.
PurposeThere is a lack of information regarding the dental patient's point of view of oral and maxillofacial surgeons (OMFSs). The aim of this study was to evaluate the perceptions and preferences of a group of university dental hospital patients for OMFSs.Materials and methodsThis study was based on patients' self-assessment using a questionnaire. A total of 530 patients were enrolled for the study. The patients' preferences regarding the surgeons' age, gender, religion, race and experience were determined and compared statistically.ResultsA total of 506 questionnaires were considered as complete and used in the analysis. Female patients preferred female practitioner more than male patients did (p=0.002), but no significant difference was found between male and female patients regarding preference for the age (p=0.464), ethnicity (p=0.926) and religion (p=0.261) of the OMFS. The educational status of the patients did not have an effect on the gender preference for the OMFS (p=0.114); however, educational status significantly affected the preferences for the ethnicity and religion of the practitioners (p=0.001).ConclusionToday patient expectations and perceptions take place in post graduate education programs. In this study we determined a wide range of different factors for choosing an OMFS. The diversity of these factors may affect the quality of the health service and thus must be considered in determining the content of oral and maxillofacial curriculum.
Purpose Debonding is expected as a frequent failure type in zirconia restorations. Therefore the aim of the current study is to evaluate the shear bond strength between colored zirconia and resin cement. Materials and methods There were 11 groups evaluated each containing 12 zirconia discs (15 mm x 12 mm x 1.6 mm). Groups were colored with the colors A3, B1, C4, D2, and D4 of the VITA classical shade scale. Coloring procedure was carried out for either 3 second or 60 seconds for the study groups and the control group was left untreated. Specimens were then bonded to translucent resin cement having a thickness of 3 mm and width of 3 mm. The shear bond strength of the samples was measured in a universal testing machine with a crosshead speed of 1 mm per minute. Two-way analysis of variance and Tukey’s HSD test were used for pairwise comparisons. Also paired t-test was used for comparing groups with the same color but having different shading times. Results Any significant difference was not found between the shear bond strengths of samples depending on whether color or shading times. Among the groups, B1 (60 seconds of coloring) had the highest bond strength (10.05 MPa), while A3 (60 seconds of coloring) showed the lowest bond strength (6.72 MPa). However, these differences were not statistically significant. Conclusion Coloring zirconia did not affect the shear bond strength between zirconia and resin cement.
Purpose The Child Perceptions Questionnaire 11-14 (CPQ 11-14) is a generic tool that was developed to measure oral health-related quality of life in early adolescents. The aim of this study was to prepare a Turkish version of the CPQ 11-14 and to test its psychometric properties in an adolescent orthodontic patient sample. Materials and Methods The questionnaire was adapted to Turkish using a forward backward translation method, and it was found to be understandable in a pilot study (n=15). The Turkish version of the CPQ 11-14 was administered to 200 orthodontic consultation patients (aged 11-14 years). Retests were conducted in 50 patients 2 weeks after the first tests. The ICON index was used to determine the orthodontic treatment need. Decayed, missing, and filled teeth were also recorded with the DMFT index. Spearman correlations and t-tests were used to assess validity. Internal consistency was assessed using Cronbach's alpha coefficient, and intraclass correlation coefficients were calculated to assess test-retest reliability. Results Significant positive correlations were found between CPQ 11-14 scores and the global ratings of oral health (r=0.381), global ratings of well-being (r=0.350), ICON scores (r=0.211), and DMFT scores (r=0.233), supporting construct validity. Children who needed orthodontic treatment had a worse quality of life than those who did not need orthodontic treatment (p=0.016). Cronbach's alpha and intraclass correlation coefficients were calculated as 0.917 and 0.817, respectively, demonstrating good internal consistency and acceptable test-retest reliability. Conclusion The Turkish version of the CPQ 11-14 was found to be valid and reliable in 11-14-year-old orthodontic patients.
Purpose: The objective of this systematic review was to compare the antero-posterior, vertical and angular changes of maxillary incisors with conventional anchorage control techniques and mini-implant based space closure methods. Materials and Methods: The electronic databases Pubmed, Scopus, ISI Web of knowledge, Cochrane Library and Open Grey were searched for potentially eligible studies using a set of predetermined keywords. Full texts meeting the inclusion criteria as well as their references were manually searched. The primary outcome data (linear, angular, and vertical maxillary incisor changes) and secondary outcome data (overbite changes, soft tissue changes, biomechanical factors, root resorption and treatment duration) were extracted from the selected articles and entered into spreadsheets based on the type of anchorage used. The methodological quality of each study was assessed. Results: Six studies met the inclusion criteria. The amount of incisor retraction was greater with buccally placed mini-implants than conventional anchorage techniques. The incisor retraction with indirect anchorage from palatal mini-implants was less when compared with buccally placed mini-implants. Incisor intrusion occurred with buccal mini-implants, whereas extrusion was seen with conventional anchorage. Limited data on the biomechanical variables or adverse effects such as root resorption were reported in these studies. Conclusion: More RCT’s that take in to account relevant biomechanical variables and employ three-dimensional quantification of tooth movements are required to provide information on incisor changes during space closure.
Purpose The aim of this study was to compare the depth of curve of Spee (COS) in Angle Class I, Angle Class II and Angle Class III malocclusions. Materials and methods The Samples were chosen among the diagnostic materials in İstanbul Medipol University Department of Orthodontics. Ninety plaster models were chosen, and were divided into 3 groups (n=30) according to Angle dental malocclusion classification. The depth of curve of Spee was measured on left and right sides of mandibular dental models and mean values were used as depth of curve of Spee. ANOVA test was used to evaluate normally distributed data. Comparison of the sides were performed by using paired sample t test. Significance level was set to p<0.05. Results The depth of COS was found as deepest in Class II malocclusion (2.9±0.8 mm) and was relative flat in Class III malocclusion (2.1±0.9 mm) and the difference was statistically significant (p<0.05). No significant difference was found between Angle Class I and Class III malocclusions. Conclusion Since the depth of curve of Spee is increased in Class II malocclusions, this factor should be considered in treatment planning.
PURPOSE:The aims of this study were to examine the accuracy of iPex II and to compare it with those of Raypex 5, Raypex 6 and iPex electronic apex locators (EALs).MATERIALS AND METHODS:Thirty fresh human mandibular premolar teeth were used in this study. Crown segments were cut and root canals were coronally flared. A #10 K-file was inserted until its tip can be seen within apical foramen to determine actual working length (AWL). Teeth were embedded in alginate and each multi-frequency EALs were randomly tested to determine the electronic working length (EWL). Differences between AWLs and EWLs were statistically compared.RESULTS:No significant differences were found between four EALs. EWL measurements by Raypex 5 were accurate in 64.29%, Raypex 6 in 53.58%, iPex in 64.29% and iPex II in 50% of the specimens, within the range of ±0.5 mm from the AWL.CONCLUSION:Within the limitations of this in vitro experiment, our findings indicate that the accuracy of working length measurements calculated with iPex II was similar to those of other multi-frequency EALs used in this study.
Purpose: The aim of this in vitro study was to assess the diagnostic performance of cone-beam computed tomography (CBCT) in the detection of secondary carious lesions under composite resin fillings applied to different types of cavities. Materials and Methods: Occlusal cavities (O) (n=18), occlusal cavities with mesial or distal component (MO/DO) (n=30), and mesial–occlusal–distal cavities (MOD) (n=30) were prepared in seventy eight extracted human posterior teeth. In half of the cavities in each group, artificial secondary caries lesions were simulated. All cavities were restored by using composite resin. All specimens were embedded in silicone and they were positioned to have approximal contacts. CBCT imaging was done and data were evaluated two times with two week interval by two observers, using a five-point confidence scale. Intra- and inter-observer agreements were calculated with Kappa statistics (κ). The area under (Az) the receiver operating characteristic (ROC) curve was used to evaluate the diagnostic accuracy. Results: Intra- (κ =0.89) and inter-observer (κ = 0.79) agreements were found to be excellent. Az values were highest for the O restorations which is followed by the MOD and DO/MO restorations. Az values for MOD and DO/MO restorations were very low and no statistically significant difference was found. Sensitivity for DO/MO restorations and specificity for MOD restorations were found to be the lowest values. Conclusion: Diagnostic performance of CBCT was higher in O composite restorations than MOD and DO/MO restorations for secondary caries detection. The use of alternative imaging methods rather than CBCT may be useful for evaluating secondary caries under composite MOD and DO/MO restorations.
Purpose The aims of this study were; to compare the shear bond strength values of resin-based luting cements using etch-and-rinse, self-etching or self-adhesive techniques and to evaluate the effects of pretreatment with 0.2%, and 2% chlorhexidine (CHX) solutions on the bonding stability to dentin. Materials and methods Ninety specimens were divided into 9 groups of equal sample size (0.2% CHX, 2% CHX and no CHX application groups). Variolink N (multi-step etch-and-rinse technique), Panavia F2.0 (self-etching technique), or RelyX U200 (self-adhesive technique) resin-based luting cement was applied. All specimens were subjected to shear bond strength test (SBS) after bonding procedure. The mode of failure was analyzed by using a stereomicroscope. Results There were no significant differences among study groups for the dentin treatment factor. However, for luting cement factor, significant differences were found and Variolink N showed the highest SBS values. Conclusion CHX application has no immediate effect on the SBS values of any resin cements. Despite the development of simplified cementation techniques, etch-and-rinse technique is still the most reliable technique because of its high bond strength to dentin.
Purpose Little has been reported how the addition of nanoparticles could affect the hardness of maxillofacial silicones. The purpose of this study was to evaluate the effects of different types of nanoparticle additives and dark storage for 2-years on the Shore A hardness of two types of maxillofacial silicone elastomers. Materials and Methods A-2000 and A-2006 Room Temperature Vulcanized (RTV) silicone elastomers were tested in this study. Silaned silica, fumed silica and titanium dioxide nanoparticles at concentrations of 10% by volume were used as fillers for the maxillofacial silicone elastomers. A total of 64 silicone specimens were fabricated, which includes 8 samples, 30 x 10 mm in size, from each silicone elastomers for each subgroup and also controls. After the initial Shore A hardness measurements, specimens were kept in dark conditions at room temperature for 2 years. The final measurements were then taken from the silicone specimens. All data were statistically analyzed. Results For both types of silicones, there was statistically significant differences among study groups (p<0.001). Fumed silica group showed the lowest hardness values in A-2000 after storage. However, no significant difference was observed between control and fumed silica groups. Control group showed the lowest hardness values in A-2006, while the highest hardness values were found in fumed silica group; there was no significant difference between silica and silane groups. Conclusion Shore A values of the specimens were within the acceptable range for the maxillofacial silicones after aging. Nanoparticle addition did not prevent hardening of the silicone elastomers with time.

Purpose Tooth color is one of the most important factors in dental and facial esthetics. The aim of this study was to determine the tooth color of the upper frontal teeth in subjects and to provide comparisons with different skin and eye colors. Material and methods Tooth color was determined in 255 patients in the intercanine sector of the maxilla using an intraoral spectrophotometer Vita Easyshade® (Vita Zahnfabrik. H Rauter GmbH & Co. KG. Bad Sackingen. Germany). Skin and eye color was determined by visual perception. The shades of the Vita Tooth Guide 3D Master (Vita Zahnfabrik, H Rauter GmbH & Co, KG, Bad Sackingen, Germany) were registered by the spectrophotometer. Pearson’s chi-squared test was used to examine the differences between tooth color and skin and eye color. Results The most frequent shade registered in the central incisors was 2M1 (62 subjects, 8.10%); in the lateral incisors, 1.5M1.5 (65 subjects, 8.50%); and in the canines, 2M3 (142 subjects, 18.56%). Pearson’s chi-squared test results showed a statistically significant difference in the relations between skin and eye color and central incisor color (p<0.01; p<0.001), lateral incisor color (p<0.001), and canine color (p<0.001; p=0.001). On the other hand, no significant difference was observed in the relation of the lateral incisors and canines with eye color. Conclusion Skin and eye color significantly correlate with tooth color; however, the color of lateral incisors and canines does not correlate significantly with eye color.
Melkersson-Rosenthal Syndrome (MRS) is a rare disorder consisting of a triad of persistent or recurrent orofacial edema, relapsing facial paralysis and fissured tongue. It is rarely possible to observe all aspects of the classical triad at the same time, since these symptoms may appear in different times of life cycle. The most common symptom is orofacial edema. Although etiology of MRS is unclear, various factors such as infections, genetic predisposition, immune deficiency, food intolerance and stress have been held responsible. MRS is diagnosed based on clinical features. This case report describes a 39 years old male patient with recurrent swelling of the upper lip. Clinical examinations showed classical triad of MRS. The diagnosis and treatment procedures were presented with special emphasis to the clinical features of this rare condition.
Lymphangioma is a benign hamartomatous lesion caused by congenital malformation of the lymphatic system. This benign tumor is detected most commonly at birth or in early childhood but rarely in adults. On clinical examination, most lymphangiomas contain clear lymph fluid, but some may present as transparent vesicles containing red blood cells due to hemorrhage. In addition, lymphangioma may occur in association with hemangioma. This tumor occurs most commonly in the head and neck area, but rarely in the oral cavity. The dorsum of the tongue is the most common location in the mouth, followed by the lips, buccal mucosa, soft palate, and floor of the mouth. There are various treatment approaches for lymphangioma, but surgical excision is the preferred method. We present a case of a 26-year-old man with lymphangioma on the anterior dorsal part of the tongue, not associated with any dysfunction in mastication or speech disorders.
PURPOSE:The aim of this study was to assess the association between the presence of maxillary incisor trauma (MIT) with age, gender, dentition type, the degree of overjet (OJ), lip form, respiratory type and dental arch form in patients with Class II division 1 dental malocclusion.SUBJECTS AND METHODS:256 patients (mean age: 15.80 ± 2.2) were included in this study. The patients' gender, dentition type, superior lip form, dental arch form and respiratory type were recorded. Participants were divided into four groups according to the severity of OJ: 3.5 mm<Group 1 (OJ I) ≤6 mm with competent lip, 3.5 mm<Group 2 (OJ II) ≤ 6 mm with incompetent lip, 6 mm<Group 3 (OJ III) ≤9mm, 9 mm<Group 4 (OJ IV). Mann Whitney-U test was used to examine the group differences for trauma and non-trauma groups. Logistic regression analysis was used to assess the factors for trauma and their risk indicators.RESULTS:3.5 mm<OJ II≤ 6 mm with incompetent lip had the highest odds of experiencing MIT among the OJ groups with an odds ratio (OR) of 3.143 and 95% confidence interval (CI) 1.125-2.779. The odds were 3.572 times higher in the group with short lip form than found in the group with normal lip form (OR 3.572, 95% CI 1.130-2.340).CONCLUSION:The age, gender, respiratory type and dental arch form were not significantly associated the risk of MIT. OJ between 3.5 mm and 6 mm (with incompetent lip) and short lips increased the risk of having maxillary incisor trauma in patients with Class II division 1 malocclusion.
PURPOSE:The aim of this study is to evaluate oral health status and oral hygiene practices of female adolescents under state protection.SUBJECTS AND METHODS:Fifty-five female participants between the age of 12 and 18 who are under the care of The Child Protection Institution were included in this study. Self-administered questionnaires were used to obtain information regarding knowledge of oral health and habits. Dental caries status was scored according to the criteria of the World Health Organization (WHO), using the indices of Decayed (D), Missing (M), Filled (F), Surfaces (S) (DMFS). In addition, Gingival Index (GI) and Plaque Index (PI) were recorded during periodontal assessment. Data was statistically analyzed by using Oneway analysis of variance, Tukey's HSD and Student's t tests.RESULTS:Fourteen children had dental fear and 52.7% of them were not satisfied with their esthetic appearance. Although 78.2% of the children knew that brushing prevents dental caries, only 18 of them were brushing regularly twice per day. Mean DMFS, GI and PI scores were 13.18±5.68, 1.35±0.37 and 1.33±0.45, respectively. Only 20% of the children were using dental floss. There was statistically no significant difference between the DMFS scores of the children in terms of flossing. However, the difference between the mean GI and PI of the same group was found to be statistically significant (p<0.05).CONCLUSION:Children under state protection were found to have a reasonable knowledge of the causes of dental caries and gum bleeding. Therefore, adequate management of this positive attitude can significantly improve the oral health of this population.
Purpose: Bisphosphonates are commonly used drugs in pediatric patients in the treatment of osteoporotic diseases and various types of cancers. The purpose of this study was to evaluate the effects of pamidronate administration on mandibular growth and tooth eruption in new born rats. Materials and Methods: Forty Sprague Dawley rats were included in the study and divided into four groups as; 14th day pamidronate group, 30th day pamidronate group, 14th day control group and 30th day control group. Pamidronate groups were daily injected with 1.25 µg/g pamidronate disodium subcutaneously whereas control groups were injected with sterile saline. Eruption levels of lower incisor and molar teeth were assessed macroscopically. Mandibular growth was assessed by measuring reference points in cone beam tomography. Histological and histomorphometric examinations were performed under light microscope to evaluate tooth morphology and number of osteoclasts. Results: Retardation in mandibular growth, decrease in number of osteoclasts, delay in tooth eruption, degeneration in both tooth morphology and structure were observed in the pamidronate groups compared to control groups. Conclusion: Pamidronate administration during growth and development stage may adversely affect tooth eruption and mandibular growth in new born rats.
Purpose The aim of this study was to conduct a retrospective evaluation of the volumetric, cross-sectional surface area and the linear airway changes in healthy subjects undergoing orthognathic surgery. Materials and methods A total of 10 patients were included in this study and categorized into two groups. The first group consisted of five patients who underwent maxillary and mandibular advancements (MMA) with genioplasty. The remaining five patients who underwent maxillary advancement with mandibular setback (MAMS) comprised the second group. The changes in airway volume, surface area, and linear values obtained from defined hard and soft tissue parameters were evaluated using preoperative and postoperative cone-beam computed tomography. A paired t-test was used to explore the statistical significance. Results A statistically significant increase in the airway volume (34.3%) was observed in the MMA group. The changes in the MAMS group were not statistically significant, although an average volumetric decrease of 8.8% was observed. The minimal axial surface area measurements in the MMA group at the levels of the soft palate and the tongue were significantly increased (56.8% and 44.9%, respectively). However, MAMS resulted in no significant changes at these levels (11.2% and 9.1% decrease, respectively). Linear changes showed a statistically significant increase in the airway in the MMA group, whereas the same measurements failed to produce significant changes in the MAMS group. Conclusion As there were no significant changes in the measured parameters, surgeons can have greater confidence that MAMS does not have any negative influence on the airway.
This review outlines the biological basis and clinical protocols currently used in regenerative endodontic procedures (REPs) and discuss future directions in pulp regeneration approaches. The treatment of immature teeth with REPs has been described as a 'paradigm shift' as there is the potential for further root maturation. Clinically, REPs involve disinfection of the root canal system without damaging the endogenous stem cell potential present in the apical papilla and other tissues. These stems cells are introduced into the root canal space by inducing a blood clot followed by placement of an intracanal barrier to prevent microleakage. The biological concept of REPs involves the triad of stem cells, scaffold and signalling molecules. Currently, repair rather than true regeneration of the 'pulp-dentine complex' is achieved and further root maturation is variable. However, may clinicians consider the treatment of teeth with REPs as the optimal treatment approach for immature teeth with pulp necrosis.
PURPOSE:The aim of this study was to evaluate the relationship between initial probing depth (IPD) and changes in clinical parameters following non-surgical periodontal treatment (NPT) in chronic periodontitis patients.SUBJECTS AND METHODS:A total of 1672 periodontal pockets having 3mm≤IPD≤9mm of depth in 15 chronic periodontitis patients were included. NPT consisting of oral hygiene instructions, scaling and root planing was applied in two sessions. Probing depth (PD), clinical attachment level, gingival recessions (GR) were measured before and eight weeks after treatment. Pocket sites were grouped according to their IPD and root number as single- or multi-rooted teeth.RESULTS:Other than the sites having 3 mm IPD, PD reduction and GR increase were significant in all groups (p<0.001). Attachment gains (AG) were significant in all single-rooted teeth (p<0.001) again except those having IPD=3mm. However, AG was significant in multi-rooted teeth having only 7mm≤IPD≤9mm (p<0.05). Positive correlations were observed between IPD and PD reduction, GR increase and AG in single-rooted teeth (p<0.001). Furthermore, positive correlations were found between IPD and PD reduction and GR increase in multi-rooted teeth (p<0.001), but there was no correlation between IPD and AG.CONCLUSION:NPT may lead to positive association between IPD and PD reduction as well as GR increase, which is independent from tooth root anatomy.
