
Objective To explore the application of outcomes-based education(OBE)and case based learning(CBL)in the course of General Surgery.Methods The undergraduate students,majoring in clinical medicine in grade 2020 who participated in the study of"General Surgery"in the second semester of the 2022-2023 academic year,were selected as the research objects.Two classes were randomly selected to receive traditional teaching methods(control group)and OBE combined with CBL teaching methods(observation group)respectively.The results of theoretical test scores,experimental test scores,comprehensive test scores of the two groups were analyzed by t test,and Chi-square test(Continuous correction chi-square test)was used to analyze the satisfaction of the two groups.Results The observation group scored 55.09±7.486 points in theoretical assessment,72.34±7.769 points in experimental skill operation,72.08±4.446 points in comprehensive assessment.All of them were higher than the control group in theoretical examination score 46.50±10.004,experimental skill operation score 68.33±7.902 and comprehensive evaluation score 67.20±5.507,and the differences were statistically significant(t=4.692,2.557,4.706,P<0.05).The satisfaction survey re-sults showed that the total satisfaction of the observation group(97.83%)was better than that of the control group(82.98%),and the difference was statistically significant(χ2=4.287,P<0.05).Conclusion OBE concept combined with CBL teaching method has certain advantages in the teaching reform of General Surgery,and can be further promoted in the future.
Lumbar disc herniation is a common degenerative disease of the spine,and intervertebral disc degenera-tion(IDD)is the main cause of its occurrence;the main causes of disc degeneration include inflammation,mechanical loading,ageing,genetics,nutritional deficiencies and other factors.Statins are competitive inhibitors of 3-hydroxy-3-meth-ylglutaryl coenzyme A reductase,which have various effects such as lowering blood lipids and inhibiting inflammation.Studies in a rat model of disc degeneration have shown that exogenous administration of statins can maintain the structural integrity of the disc and thereby inhibit the development of IDD by inhibiting inflammation and apoptosis and promoting the synthesis of extracellular mechanisms.Statins play an important role in the process of IDD.This article focuses on the application of statins in the treatment of IDD and their role,and explores their potential therapeutic mechanisms,which can help further explore the feasibility of statins in the treatment of IDD.
Objective To observe the efficacy of one-stage open reduction and internal fixation for Gustilo Ⅱ and ⅢA open ulna and radius fractures.Methods Retrospective analysis was performed on the data of patients with open fractures of Gustilo Ⅱ Gustilo ⅢA of ulna and radius treated by open reduction and internal fixation surgery in our hospital from January 2017 to January 2022,and follow-up was conducted to observe whether there were superficial infec-tion,deep infection,fracture healing and functional recovery of forearm after surgery.Results Follow-up was 3-13 months,with an average of 4.3 months.A total of 3 stage I unstitched wounds showed superficial infection,and wound healing was finally achieved after repeated debridement and infection control.There were 0 case of deep infection.Callus growth,contrapuntal,line satisfaction and angle-less deformity were seen in all fractures.The functional evaluation of frac-ture healing was excellent in 33 cases,satisfactory in 7 cases,unsatisfactory in 2 cases and poor in 0 case.Conclusion The characteristics of open fractures were carefully evaluated before Gustilo Ⅱ and ⅢA open ulnar and ra-dius fractures.After thorough debridement and one-stage open reduction and internal fixation,satisfactory clinical results could be obtained.Its exact fixation effect is helpful for wound healing,early functional exercise,and is an alternative treatment for open ulnar and radius fractures.
Objective To explore the relationship between fear of negative evaluation and interpersonal sensitivity,and focus on examining the roles of emotional resilience and self-concept clarity in this relationship.Methods A ques-tionnaire survey was conducted on 952 college students using the Negative Evaluation Fear Scale,Interpersonal Sensitivity Scale,Emotional Resilience Scale,and Self Concept Clarity Scale.Results The fear of negative evaluation can signifi-cantly positively predict interpersonal sensitivity(β = 0.38,P<0.001);Emotional resilience plays a mediating role between negative evaluation fear and interpersonal sensitivity(indirect effect was 0.16 accounting for 30%of totul effect);The self-concept clarity plays a significant moderating role in the mediating model of negative evaluation fear affecting interpersonal sensitivity through emotional resilience in the first and second half(β=0.05,P<0.05;β=0.10,P<0.001).Conclusion The fear of negative evaluation not only directly affects the interpersonal sensitivity of college students,but also indirectly affects their interpersonal sensitivity through the mediating effect of emotional resilience.At the same time,self-concept clarity plays a moderating role in this mediating pathway,that is,compared to high levels of self-concept clarity,low levels of self-concept clarity have a stronger indirect effect.
Objective To investigate the correlation between depression and internet use among adolescents with depression,and to examine the mediating effect of sleep disorders.Methods Adolescents who met the ICD-10 diagnos-tic criteria for depression were selected from the inpatient and outpatient departments of the Shandong Mental Health Center.The IAT,CDI,PSQI,and other scales were used to survey them,and a total of 397 questionnaires were collected.Statistical analysis,including demographic information,single-factor correlation analysis,Pearson correlation analysis,multiple linear regression,and mediation effect testing were carried out.Results Single-factor correlation analysis found that age was a common influencing factor for internet use and depression among adolescents.Pearson correlation analysis found a significant positive correlation between depression level and internet use and sleep problems(r=0.48,r=0.68,P<0.001).There was also a significant positive correlation between sleep problems and internet use(r = 0.35,P<0.001).Multiple linear regression analysis found that negative emotions(β=0.14,P=0.030),low efficacy(β=0.21,P<0.001),interpersonal problems(β=0.13,P=0.018),sleep disorders(β=0.16,P=0.002),and internet use were positively correlated.Mediation effect analysis found that sleep disorders played a partial mediating role between negative emotions,low efficacy,interpersonal problems,internet usage,and depression,with mediation effect values of 0.52,0.88,and 1.20,and relative mediating ratios of 25.07%,24.62%,and 29.78%,respectively.Conclusion In adolescents with depressive disorder,negative emotions,inefficiency and interpersonal problems affect the degree of Internet use through the mediating effect of sleep disorders.
Objective To explore the effect of clinical pathway(CP)and team based learning(TBL)in the prac-tical teaching of joint sports medicine.Methods 80 students of clinical medicine major in Jining Medical University were selected as the research objects.According to different teaching methods,the students were divided into the control group and the observation group,40 students each group.The control group adopted traditional teaching method(LBL)combined with multimedia assisted teaching method,and the observation group adopted CP combined with TBL teaching method basis of the control group.After the internship,the scores of the two groups were compared,and the teaching effect was evaluated through satisfaction survey.Results The theoretical score 38.70±3.32,clinical skill score 36.90±2.89,and total score of the entrance examination 75.60±3.75 in the control group were lower than those in the observa-tion group 43.53±3.08,clinical skill score 40.83±3.59,and total score of the entrance examination 84.35±4.76,with statistically significant differences(t=6.736,P<0.05;t=5.390,P<0.05;t=9.124,P<0.05).And the satisfaction sur-vey found that the total recognition rate of CP combined with TBL teaching method(92.5%)is better than LBL com-bined with multimedia assisted teaching method(72.5%)(χ2 = 6.556,P<0.05),with statistically significant differ-ences.Conclusion CP combined with TBL teaching method can improve the learning efficiency and recognition,and has an ideal application effect in teaching activities.
Nasal inverted papilloma is a common tumor of the nasal cavity and paranasal sinus,which is classified as borderline tumor due to its characteristics of postoperative recurrence and malignant transformation.The etiology of nasal inverted papilloma is complex,and the influence of virus infection has been paid more and more attention.Surgical treat-ment is the most effective treatment method,but how to give timely intervention measures to reduce postoperative recur-rence and malignant transformation is an urgent clinical problem.Therefore,this paper focuses on the etiology of nasal in-verted papilloma,the influence of human papilloma virus(HPV)infection,clinical staging system,and treatment plan were reviewed and analyzed,hoping to provide new ideas for diagnosis and treatment of this disease.
Objective To establish an ultra performance liquid chromatography tandem high resolution mass spec-trometry for the determination of sodium glutamate in sodium glutamate injection.Methods Kinetex C18(2.0×100mm,1.7μm)was used as the chromatographic column,and the mobile phase was 0.1%formic acid water and methanol solution.In the sample,glutamic acid was derivatized with 3-bromopropyl triphenylphosphorus(3-BMP)at a flow rate of 0.40L/min,and the parent ions and characteristic fragments of glutamic acid derivatives were 450.18→320.16,respectively.Draw standard curves using data-independent scanning mode.Results The linear relationship of sodium glutamate was good in the range of 0.024~1000pmol(R2 = 0.9999),the minimum detection limit was 4.00fmol,the minimum quantitative limit was 24.25fmol,the average recovery rate was 91.71%,and the RSD of intra-day and inter-day precision were 1.67%~2.64%and 1.66%~4.55%respectively.Conclusion The method is sim-ple,sensitive and accurate,and the sample processing is simple.It is suitable for product quality inspection and labora-tory determination of sodium glutamate in sodium glutamate injection.
Objective To investigate the status quo of self-efficacy in young and middle-aged patients with coro-nary heart disease after PCI,and analyze its influencing factors and its relationship with social support,sleep quality and anxiety.Methods A cross-sectional survey was conducted among 200 patients who were hospitalized or outpa-tients in a Grade-A hospital in Anhui province from January to December 2022.The general data questionnaire,Return to Work Self-efficacy Questionnaire,Social Support Rating Scale(SSRS),Pittsburgh Sleep Quality Questionnaire(PSQI),and Self-rating Anxiety Scale(SAS)were used to investigate.Results The return to work self-efficacy score of young and middle-aged patients after PCI was 40.87±11.87,which was at a low level;the social support score was 33.09±6.99 at a low level;the score of sleep quality 9.68±4.80 was at a poor level;The anxiety score was 47.64±12.359,which was mild to moderate anxiety level.The results of multi-factor analysis showed that anxiety and the num-ber of coronary artery lesions were the main factors affecting self-efficacy of returning to work.Correlation analysis showed that the return to work self-efficacy score was correlated with the total score of social support,anxiety and sleep quality(P<0.05).The social support score was positively correlated with the self-efficacy of returning to work,and the correlation coefficient was 0.223.The return to work self-efficacy was negatively correlated with the total score of anxiety and sleep quality,with correlation coefficients of-0.888 and-0.782.Conclusion The level of self-efficacy of young and middle-aged patients with coronary heart disease returning to work after PCI needs to be improved.Strengthening patients' social support,reducing patients' negative emotions and improving their sleep quality may be ef-fective measures to improve patients' self-efficacy returning to work.
Objective To explore the relationship between mindfulness and bedtime procrastination of medical college students,and analyze the role of boredom tendency between them.Methods 763 medical college students were selected and tested with Child and Adolescent Mindfulness Measure,Short Boredom Proneness Scale,and Bedtime Pro-crastination Scale.Results Mindfulness was negatively correlated with boredom tendency and bedtime procrastination(r1=-0.55,r2=-0.25,P<0.001),while boredom tendency was positively correlated with bedtime procrastination(r= 0.36,P<0.001);After controlling gender and age,boredom tendency plays a completely mediating role between mindful-ness and bedtime procrastination,with a mediating effect value of-0.158.Conclusion Boredom tendency is an impor-tant factor that mindfulness affects bedtime procrastination.In the process of trying to intervene bedtime procrastination with mindfulness,we should pay attention to broadening students' horizons,establishing learning and life goals,reducing boredom,and reducing bad sleep hygiene behaviors.
Objective To explore the relationship between childhood trauma and suicidal ideation of college students,and the chain mediating role of perceived social support and psychological resilience.Methods A sample of 509 undergraduates were investigated with Childhood Trauma Questionnaire(CTQ),Perceived Social Support Scale(PSSS),Connor-Davidson Resilience Scale(CD-RISC)and Self-rating Idea of Suicide Scale(SIOSS).Results Childhood abuse and childhood neglect were significantly positively correlated with suicidal ideation(r=0.42,0.40,P<0.001),while significantly negatively associated with perceived social support(r=-0.35,-0.52,P<0.001)and psychological resilience(r=-0.17,-0.39,P<0.001).Childhood abuse indirectly affected suicidal ideation through the mediating role of perceived social support and the chain mediating role of perceived social support and psychological resilience(indirect effect was 0.120 accounting for 28.64%of total effect).Childhood neglect indirectly affected suicidal ideation through the mediating role of perceived social support and psychological resilience,and the chain mediating role of perceived social support and psychological resilience(indirect effect was 0.241 accounting for 58.78%of total effect).Conclusion Childhood trauma could not only directly affect suicidal ideation,but also indirectly affect suicidal ideation through the independent mediating role of perceived social support and psychological resilience,and the chain mediating role of perceived social support and psychological resilience.
Neck,shoulder,waist and leg pain is a common clinical disease,and neuropathic pain caused by the compression of the nucleus pulposus and stimulation of the spinal nerve root after intervertebral disc degeneration(IDD)is a common cause of neck,shoulder,waist and leg pain.With the accelerated pace of life,neck,shoulder,waist and leg pain is becoming increasingly younger,and in severe cases,it can lead to loss of labor ability or even disability,posing a huge medical and economic burden to individuals,families,and society.How to delay and treat IDD has become a hot and difficult topic of global research.Platelet rich plasma(PRP)is a new non-invasive non-surgical treatment for IDD.It can not only improve the microenvironment of degenerative intervertebral discs,but also promote the proliferation,differentia-tion,matrix synthesis and anti-apoptosis of nucleus pulposus cells.Simultaneously the combined application of PRP and mesenchymal stem cells may bring better therapeutic effects.However,due to the inconsistent preparation standards of PRP,it poses a serious challenge to the research of standardized treatment.This article summarizes the latest research re-sults of PRP in the treatment of intervertebral disc degeneration,with the aim of better clinical application of PRP.
Objective The purpose of this study is to explore the risk and protective factors of resilience of female family caregivers with severe mental illness(SMI).Methods 193 female family members of inpatients with SMI in Wutaishan Hospital were included.The scale.Zarit Caregiver Burden Scale(ZBI),Cognitive Emotion Regulation Ques-tionnaire(CERQ)and Resilience Scale(CD-RISC)were used to assess family members'burden,positive cognitive emotions and resilience,and mediation analysis were used to actively identify the impact of positive cognitive emotions on resilience.Results There was a significant correlation between caregiver burden,positive cognitive emotions,resili-ence,and optimism(P<0.01);Positive cognitive emotions partially mediate the impact of caregiver burden on optim-ism and resilience(P<0.05).In addition,positive cognitive emotion regulates the relationship between caregiver burden and optimism(P<0.05).Conclusion Positive cognitive emotion is a protective factor for resilience mediating the rela-tionship between Caregiver Burden and resilience,suggesting the need to develop interventions to encourage positive thinking and regulate cognitive emotions among women caregivers of adults with SMI.
Objective To explore the relationship between childhood neglect and adolescent depression and the effect of resilience in it.Methods A total of 1 590 students from 4 middle schools in Bijie of Guizhou province were surveyed by Neglect Subscale,Brief Resilience Scale(BRS),and eleven-item Kutcher Adolescent Depression Scale(KADS 11).Results The score of childhood neglect 2.52±3.40 was positively correlated with the score of adolescent depression(5.06±5.77,r=0.44,P<0.001),and they were negatively correlated with the score of resilience(19.83±3.89,r=-0.20,-0.31,all P<0.001).The mediating effect of resilience was significant and the indirect effect was 0.044,accounting for 10.66 of the total variance.Moderating effect of resilience between childhood neglect and adolescent depression was significant(P<0.001).Conclusion Resilience plays both mediating and moderating role between childhood neglect and adolescent depression.
Objective To explore the mediating effect of smartphone addiction between professional attitude and academic burnout.Methods 1345 undergraduate nursing students were investigated with the Professional Attitude Scale,the Smartphone Addiction Scale-short Version and the Academic Burnout Scale.Results The total scores were 103.11±14.07 for professional attitude,33.24±7.91 for smartphone addiction,and 57.52±8.97 for academic burnout.The results showed that professional attitude was obviously negatively correlated with smartphone addiction and academic burnout(r=-0.146,-0.386,P<0.05).Besides,smartphone addiction was significantly positively correlated with aca-demic burnout(r=0.497,P<0.05).Smartphone addiction played a mediating effect between professional attitude and academic burnout.Conclusions Undergraduate students' professional attitude can directly affect the academic burnout,and it can also affect academic burnout through smartphone addiction.
Chronic diseases seriously threaten the health of Chinese residents,effective chronic disease management can greatly reduce the harm of diseases,the development of Internet medical treatment provides a good opportunity for chronic disease management.Through literature research,the experience and model of Internet chronic disease manage-ment at home and abroad are reviewed,and based on the innovative care for chronic conditions framework(ICCC)framework proposed by the World Health Organization(WHO),the problems of Internet chronic disease management are analyzed from the macro,meso and micro levels.Based on the reality of Internet chronic disease management in China,countermeasures and suggestions for Internet chronic disease management are put forward.At the macro level,the govern-ment should combine the pilot experience of Internet chronic disease management to improve the national supporting poli-cy system;at the meso level,community health institutions should increase community resource input and strengthen the ability of community institutions in Internet chronic disease management.At the micro level,it is necessary to improve the self-management ability of patients(and families)of chronic diseases,mobilize their own participation and initiative,and promote patients' self-management.

我校在留学生医学免疫学的教学过程中开展了线上线下混合教学模式,同时将流行性传染病与医学免疫学相关的一系列知识点融入医学免疫学的教学,注重对来华留学生展开流行性传染病的预防、治疗和疫苗接种的科普工作.混合式教学模式为留学生提供了更多的学习自由度和灵活度,增强留学生学习的能动性,提高医学免疫学的学习效率.
目的 针对六芳基二咪唑(HABI)类光致变色化合物在可见光区和近红外光区吸收较弱甚至无吸收的问题,以HABI类光致变色化合物为骨架,结合激子耦合效用,设计具有红光响应的HABI类负向光致变色化合物.方法 通过密度泛函理论计算,完成理论吸收光谱探究;通过核磁共振波谱仪和高分辨质谱对目标化合物进行结构表征;利用分光光度计对目标化合物进行光化学性质测试,进而阐明其应用前景.结果 目标化合物BisBN-ImD-1具有明显的激子耦合效应,并在 580nm处有一个较强的吸收峰.在光照(λ=730nm)的条件下,目标化合物的吸收光谱在 525~700nm的吸收强度明显降低.而在 470nm的光照射下,其在可见光区的吸收则完全消失.停止光照后,较快的热致逆反应使其吸收光谱在室温下可快速恢复至初始状态.结论 通过有机合成获得的目标化合物BisBN-ImD-1具有红光激发的良好光化学性质,为其在生物医学领域的应用奠定基础.
