
Objective: To investigate the influence of continuity of care on self- care agency and sleeping quality in patients with type 2 diabetes. Methods: 50 cases of type 2 diabetes from January 2012 to July 2013were divided into control group and observation group. The control group received routine hospital care,the observation group was given continuity of care after discharge. Self- care agency and sleeping quality were observed in two groups. Results: After 3 moths,the Measurement Scale scores of observation were higher than the control group,and sleeping self- assessment was lower than the control group( P 0. 05). Conclusion: The continuity of care significantly improved self- care agency and sleeping quality in patients with type 2 diabetes.
脂质代谢重编程已成为肿瘤研究领域的热点,异常脂质代谢与恶性肿瘤发生、发展密切相关,通过调节异常脂质代谢途径可有效防治恶性肿瘤,但目前相应药物尚处于研发阶段.从中医理论来看,异常脂质代谢相当于"浊邪"(痰、湿、瘀等邪气聚集),既是致病因素,又是病理产物,其贯穿于恶性肿瘤的发生、发展始终,且"浊邪"的产生也与恶性肿瘤病机"本虚标实"相吻合,"扶正祛邪"法,可有效抑制恶性肿瘤生长,又可调节异常脂质代谢.基于此,本研究以脂质代谢重编程为着眼点,探讨"扶正祛邪"法在治疗恶性肿瘤和调节异常脂质代谢(祛除"浊邪")中的应用和潜在机制.
帕金森病(PD)是常见于中老年的神经系统变性疾病,因其症状复杂,病情进展缓慢,平均期望寿命长等特点,有必要制定长期系统的治疗策略.近期临床研究已经表明中西医结合治疗PD具有明确优势.马羽教授基于中西医结合的思维与方法,通过医工结合、多学科联合的方式,以PD患者临床症状及个体化问题为导向,以提高患者生命质量为主线,建立"帕金森病中西医整合诊疗体系",以期为医生临床诊疗PD提供参考.
Objective:To determine the histological effect of Panax notoginseng saponin(PNS) on heterotopic ossification(HO) of Achilles tendon in rats and identify the underlying mechanism based on transforming growth factor-β 1 (TGF-β 1 )/Sma-and Mad-related protein(Smad) 2/3 signaling pathways.Methods:A total of 126 Sprague-Dawley(SD) rats were randomly divided into seven groups:a normal control group,a sham operation group,a model group,a western medicine treatment group,a high dose group,a medium dose group,and a low dose group,with 18 rats in each group.After receiving HO modeling surgeries,rats were treated with PNS in different concentrations,western medicine,or normal saline by gavage for two weeks.The incidence of HO was evaluated by X-ray and CT scan at 2,5,and 10 weeks postoperatively.A series of general histological staining methods,including hematoxylin-eosin(HE) staining,Masson’s trichrome staining,and Safranin O-fast green staining,were used to evaluate the histological changes in HO lesions.The expression of TGF-β 1 ,phosphorylated Smad 2(p-Smad2),and p-Smad3 in the local lesion was compared among groups by immunohistochemistry.Results:At 10 weeks postoperatively,results of the X-ray examination showed that there was no significant difference in the rate of bone formation among all groups receiving HO modeling surgeries(P>0.05).At two weeks after surgery,there was a significant increase in Achilles tendon cells in rats(P <0.05) and a disturbed arrangement of Achilles tendon fibers.A large number of proliferating hypertrophic chondrocytes were observed at five weeks and a large amount of mature ectopic bone tissues was formed at 10 weeks.The relative bone mass at 10 weeks in the medium dose group was significantly lower than that in the model group(P <0.05).The results of immunohistochemistry showed that the postoperative expression of TGF-β 1 ,p-Smad2,and p-Smad3 in the sham operation group was significantly higher than that in the normal control group(P <0.01).Compared with the model group,the medium dose group exhibited significantly decreased TGF-β 1 expression two weeks after surgery(P <0.05) with delayed peak expression.Conclusion:The expression of TGF-β 1 increased significantly after the HO modeling surgery of the Achilles tendon in rats.A certain concentration of PNS can reduce the relative bone mass of ectopic bone tissues,which may be related to the possibility that PNS delayed the peak expression of TGF-β 1 and down-regulated its expression.
Objective:To explore the clinical efficacy of modified Dahuang Mudan decoction in the treatment of hyperuricemia in patients with chronic kidney disease(CKD).Methods:Retrospective analysis was performed,42 cases of hyperuricemia with CKD treated with allopurinol tablets in Qinhuangdao Traditional Chinese Medicine Hospital from October 2019 to June 2022 were includ-ed as the control group;46 cases of hyperuricemia with CKD who received the treatment of modified Dahuang Mudan Decoction combined with allopurinol tablets in the same period were included as the observation group,and they were both treated for 8 weeks.The 24-hour urine protein quantification,24-hour urine uric acid level,clinical efficacy,renal function indicators such as se-rum creatinine(SCr),blood urea nitrogen(BUN),glomerular filtration rate(GFR),uric acid(UA),traditional Chinese medicine syndrome score,and incidence of adverse reactions between the two groups were compared.Results:The total effective rates of the observation group and the control group were 91.30%(42 cases/46 cases)and 66.66%(28 cases/42 cases),respectively.The re-nal function indicator SCr were(98.21±14.01)μmol/L and(121.52±29.14)μmol/L,respectively;BUN were(7.56±0.63)mmol/L and(9.24±0.98)mmol/L,respectively;serum UA were(681.43±106.73)μmol/L and(549.63±87.49)μmol/L,respectively;GFR were(28.97±9.23)mL/(min·1.732)and(24.56±8.44)mL/(min·1.732),respectively,and the differ-ence was statistically significant(P<0.05).The uric acid metabolism indicator 24-hour urine protein quantifications were(1.01± 0.87)g and(1.94±1.12)g,respectively;24-hour uric acid levels were(398.31±32.47)μmol/L,(456.32±40.58)μmol/L,respectively,and the difference was statistically significant(P<0.05).TNF-α levels were(45.27±5.93)μg/L and(88.62± 8.71)μg/L,respectively;IL-6 levels were(41.73±6.21)ng/L and(82.56±8.98)ng/L,respectively;CRP levels were(7.97±0.38)mg/L and(14.22±1.89)mg/L,respectively,and the difference was statistically significant(P<0.05).The tradi-tional Chinese medicine(TCM)symptom scores of the two groups were both improved,and that of the observation group was lower than that of the control group;the differences were both statistically significant(P<0.05).There were no adverse drug reactions in the observation group and the control group.Conclusion:The treatment of hyperuricemia in patients with CKD with modified Dahuang Mudan Decoction combined with allopurinol tablets is more effective and safe than that with allopurinol tablets alone.
Objective:To observe the clinical effect of nourishing the heart and spleen on major depressive disorder(MDD)with suicidal ideation.Methods:From January 2017 to April 2018,366 MDD patients(deficiency of heart and spleen)in 13 hospitals including traditional Chinese medicine(TCM)hospitals,integrated Chinese and Western medicine hospitals and psychiatric hospi-tals were taken as the study subjects in this cohort.Three cohort groups(TCM,Western medicine,and integrated Chinese and West-ern medicine)were formed naturally according to treatment regime,and they were divided into high,medium and low levels based on the TCM exposure time(TCM-ET).The follow-up interval was 3 months,and the Montgomery-Asberg Depression Rating Scale(MADRS)was used in the follow-up to assess the dynamic status of MDD patients and the occurrence of the endpoint events.Re-sults:The 2-year incidence of suicidal ideation of MDD patients in the TCM,Western medicine,and integrated Western and Chi-nese medicine cohorts was 39.0%,49.1%,and 29.0%,respectively.In the multifactorial comparison of suicidal ideation,com-pared with the condition in Western medicine cohort,the risk of suicidal ideation was reduced by 28.1%(HR=0.719,95%CI 0.476 to 1.087,P>0.05)in TCM cohort,and by 42.1%(HR=0.579,95%CI 0.373 to 0.899,P<0.01)in integrated Chinese and Western medicine cohort.In TCM cohort,compared with the condition under low-level TCM-ET,the risk of suicidal ideation was reduced by 22.2%(HR=0.778,95%CI 0.404 to 1.500,P>0.05)under medium-level TCM-ET,and by 66.6%(HR=0.394,95%CI 0.167 to 0.931,P<0.05)under high-level TCM-ET.In integrated Chinese and Western medicine cohort,com-pared with the condition under low-level TCM-ET,the risk of suicidal ideation was lowered by 70.3%(HR=0.419,95%CI 0.189 to 0.927,P<0.05)with medium-level TCM-ET,and by 83.1%(HR=0.217,95%CI 0.094 to 0.501,P<0.01)with high-level TCM-ET.Conclusion:Western medicine and TCM method of nourishing the heart and spleen present the same effect,and integrat-ed Chinese and Western medicine is more significant in reducing the suicidal ideation of MDD patients(deficiency of heart and spleen).Compared with low-level TCM-ET,medium-level and high-level TCM-ETs are more clinically effective in reducing the sui-cidal ideation.
Objective:To preliminarily analyze the potential quality markers(Q-Markers)of Heishun Tablet by combining finger-print,network pharmacology and molecular docking.Methods:High performance liquid chromatography(HPLC)was used to estab-lish the fingerprint of Heishun Tablet and identify the common peaks,and cluster analysis and principal component analysis were performed at the same time.Network pharmacology was employed to construct the"active component-target-pathway"network to predict Q-markers,and molecular docking was used for verification.Results:The fingerprints of 25 batches of Heishun Tablet were established.Eight common peaks were calibrated and 6 of them were identified,which were benzoylmesaconine,benzoylaconine,benzoylaconine,mesaconitine,hypaconitine and aconitine.The similarity ranged from 0.922 to 0.999.Cluster analysis showed that the 25 batches of samples were roughly clustered into two groups,and principal component analysis found that S4,S10 and S11 samples were significantly different from the other batches.Network pharmacology found that six components,including hypoaconi-tine,were the main active components of Heishun Tablet,and mainly played anti-tumor,anti-inflammatory,and cardiovascular pro-tective functions.By molecular docking,the above components all had strong binding activity with the key targets,which might be the potential Q-markers of Heishun Tablet.Conclusion:In this paper,the quality information of Heishun Tablet was obtained from multiple levels,and the Q-markers were preliminarily predicted and verified.It is expected to facilitate the further research on the mechanism of Heishun Tablet and the overall quality control of decoction pieces.
This paper summarized professor JIA Yingjie's experience in treating pancreatic cancer.Pancreatic cancer is called"the king of cancer"due to its high malignancy and rapid progression.Professor JIA innovatively proposes the treatment principle of"Chuzhuo Peiben"(removing turbidity and building up root)for cancers.He believes that the clinical characteristics of pancre-atic cancer are consistent with those of cancer turbidity,and the core pathogenesis is deficiency in nature,turbid accumulation,earth stagnation and wood depression.The treatment should focus on removing turbidity,building up root,and transporting earth to wood.In clinical tests,fruitful results are obtained.
Chronic gastritis(CG)is a chronic inflammation or atrophic lesion of gastric mucosa caused by various reasons.It is mostly due to the repeated invasion of gastric mucosal epithelium by various pathogenic factors,and the reconstruction of the mucosa due to its specific regeneration function,which eventually leads to the irreversible atrophy or even disappearance of the intrinsic glands of the stomach.Modern research shows that chronic inflammation of gastric mucosa can cause malignant changes in cells and eventually lead to the occurrence of gastric cancer,but its mechanism is still unclear.It is currently believed that effective interven-tion of precancerous lesions such as intestinal metaplasia and dysplasia in the background of gastric mucosal atrophy is the key link in secondary prevention of gastric cancer in CG"inflammation-cancer transformation".In traditional Chinese medicine,spleen defi-ciency is an important pathogenesis of CG"inflammation-cancer transformation",and on this basis,factors such as phlegm damp-ness and blood stasis can accelerate its malignant transformation process.This process is highly consistent with mitochondrial energy metabolism in terms of microscopic performance and occurrence.
Objective:To evaluate the efficacy of different acupuncture methods for the treatment of postherpetic neuralgia by net-work Meta-analysis.Methods:The randomized controlled trials(RCTs)regarding different acupuncture methods for the treatment of postherpetic neuralgia were searched in PubMed,Web of science,China National Knowledge Infrastructure(CNKI),Chinese Sci-ence Citation Database(CSPD),Chinese Citation Database(CCD)and China Biology Medicine disc(CBM)from the inception of the databases to August 15,2021.Quality of the literature was assessed using the risk of bias tool in the Cochrane Handbook,and RevMan 5.4 and Stata 16 were used for data analysis.Results:A total of 31 RCTs were included,involving 13 intervention meas-ures and 2 330 patients,1 120 in the observation group and 1 210 in the control group.The network Meta-analysis showed that the efficacy probability of different methods was ranked from high to the low as fire needle+collateral-bloodletting puncture and cup-ping(0.903)>floating needling+fire needle(0.799)>electroacupuncturecircumpuncture(0.72)>circumpuncture+collateral-bloodletting puncture and cupping(0.619)>floating needling+collateral-bloodletting puncture and cupping(0.602)>electroacu-puncture+collateral-bloodletting puncture and cupping(0.591)>floating needling(0.518)>electroacupuncture+floating nee-dling(0.484)>fire needle(0.477)>collateral-bloodletting puncture and cupping(0.467)>circumpuncture(0.206)>electroa-cupuncture(0.092)>conventional western medicine(0.023).Conclusion:Acupuncture is favorable in the treatment of posther-petic neuralgia,and fire needle combined with collateral-bloodletting puncture and cupping has the optimal clinical efficacy.
Objective:To study the improvement effect of Zhibao Sanbian Pill on adenine-induced kidney yang deficiency in rat model.Methods:A total of 98 SD rats were selected.Two hours after the last administration,the body weight of rats in each group was measured,and the testis coefficient,epididymis coefficient and seminal vesicle coefficient were also detected.The number,mor-phology and motility of spermatozoa were observed by microscope.Serum levels of estradiol(E2),testosterone(T),follicle-stimula-ting hormone(FSH),luteinizing hormone(LH),dopamine(DA),5-hydroxytrypsin(5-HT)and norepinephrine(NE)were deter-mined by enzyme-linked immunosorbent assay(ELIS A).Hematoxylin and eosin(HE)staining was used to observe the pathological changes of kidney,testis and epididymis.Masson staining was used to detect renal fibrosis.Results:Compared with the normal group,the Zhibao Sanbian groups had alleviated kidney yang deficiency,increased body weight,testis coefficient,epididymis coeffi-cient and seminal vesicle coefficient,increased number of spermatozoa with low malformation,and enhanced motility.Additionally,the pathological changes of kidney,testis,epididymis and other reproductive organs as well as renal fibrosis were reduced.The pre-scription also lowered the contents of E2,FSH,LH,DA,NE and 5-HT,while increased T.Conclusion:Zhibao Sanbian Pill can im-prove the adenine-induced kidney yang deficiency in rat model based on the regulation of the hypothalamic-pituitary-gonadal(HPG)axis,showing a good effect of tonifying kidney to strengthen yang.
The theory of healthy qi and pathogenic qi is a basic model for understanding diseases in traditional Chinese medicine(TCM),which implies the philosophical thought of the contradictory unity.The occurrence and development of tumors is closely re-lated to genetic mutation,and the behaviors of antioncogenes and oncogenes at the molecular level can be considered as symptoms and summarized into the theory of healthy qi and pathogenic qi.This paper analyzed the relationship between healthy qi and patho-genic qi demonstrated by antioncogenes and oncogenes in the occurrence and development of tumors,which is conducive to connect-ing the research findings of modern medicine with the basic TCM theory and expanding the theoretical connotation of healthy qi and pathogenic qi.It is expected to provide evidence for the TCM treatment of tumors.
Objective:To investigate the effect of Zuogui Pill on the migration of human umbilical cord mesenchymal stem cells(hUC-MSCs)in vitro by regulating the expression of C-X-C chemokine receptor 4(CXCR4).Methods:The hUC-MSCs were isola-ted through tissue block method and identified by flow cytometry.CCK-8 method was used to analyze the proliferation of hUC-MSCs treated with different concentrations of Zuogui Pill.Cell scratch and Transwell assays were performed to detect the effect of Zuogui Pill on the migration of hUC-MSCs.The expression of CXCR4 in hUC-MSCs was detected by real-time PCR,Western blot,and flow cytometry.Results:The flow cytometry showed that hUC-MSCs highly expressed CD73 and CD90,while extremely lowly expressed CD34 and CD45 on their surfaces.CCK-8 assay indicated that 0.1 and 0.2 mg/mL Zuogui Pill significantly promoted the prolifera-tion of hUC-MSCs(P<0.05).By cell scratch and Transwell assays,it was found that 0.1 and 0.2 mg/mL(optimal concentration)Zuogui Pill enhanced the migration of hUC-MSCs,and CXCR4 antagonist AMD3100 attenuated its migration-promoting effect(P<0.05).Real-time PCR,Western blot,and flow cytometry demonstrated that Zuogui Pill up-regulated the CXCR4 mRNA and protein expression of hUC-MSCs,and promoted the expression of CXCR4 on cell surface(P<0.05).Conclusion:Zuogui Pill facilitates the migration of hUC-MSCs in vitro by up-regulating the expression of CXCR4.

Ovarian cancer,with the highest mortality among gynecological malignancies,poses a serious threat to the health and safety of women.In recent years,it has become more and more prominent in the treatment of ovarian cancer with traditional Chinese medicine(TCM),and abundant data in modern pharmacological studies have supported the effectiveness and feasibility.By explo-ring the relevant descriptions of ovarian cancer in ancient TCM literature,this paper summarized the core pathogenesis of ovarian cancer,and discussed the clinical application of TCM in perioperative period,chemotherapy and follow-up maintenance treatment of ovarian cancer,aiming to improve the comprehensive curative effect of patients with ovarian cancer and break through the bottleneck of clinical prognosis of the disease.

Objective:To observe the effects of traditional Chinese medicine(TCM)compound Tangnai Kang on blood sugar,body weight,interleukin-6(IL-6)and inhibitor of nuclear factor kappaB kinase beta(IKKβ)in Toll-like receptor 4(TLR4)signaling pathway and glucose transporter 4(GluT-4)in KKAy mice,and to discuss its possible mechanism in improving type 2 diabetes mel-litus(T2DM).Methods:SPF KKAy and C57BL/6J mice(spontaneous T2DM animals)were selected and fed with high fat food.After 8 weeks,blood sugar and body weight were evaluated and the levels of IL-6 and GluT-4 in plasma were measured by semi-quantitative ELISA.The protein expressions of TLR4 in liver tissue and IKKβin muscle tissue were detected by Western blot.Re-sults:High dose of Tangnai Kang reduced blood glucose(P<0.05)and controlled the growth trend of body weight.It also de-creased the level of IL-6 and increased the level of GluT-4(P<0.01,P<0.05),and inhibited the protein expressions of TLR4 in liver tissue and IKKβ in muscle tissue(P<0.01,P<0.05).Conclusion:Tangnai Kang may reduce the low-grade inflammation response by down-regulating the key inflammatory factors in the LPS-mediated TLR4 signaling pathway,and thus inhibit the inflam-matory cascade effect to some extent.It also enhances the transposition and activity of GluT-4 in the insulin signal transduction pathway,to jointly improve the insulin resistance.
Objective:To analyze the medication law of traditional Chinese medicine(TCM)compound prescriptions for the treat-ment of cancer-induced bone pain(CIBP)based on data mining.Methods:The clinical studies on the treatment of CIBP by TCM compound prescriptions were retrieved from four Chinese databases from the inception of the databases to December 31,2021.The effective TCM compound prescriptions were screened,and the frequency,properties,tastes,meridian tropism,and efficacy of the in-volved herbal medicines were analyzed.Association rule analysis,cluster analysis,and complex network visualization were conduc-ted.Results:A total of 62 randomized controlled trials(RCTs)were obtained,including 4 863 patients,2 413 in the experimental group and 2 450 in the control group.There were 62 TCM compound prescriptions,involving 139 herbal medicines,with a total fre-quency of 715.They were mostly pungent and warm and acted on the liver,kidney,and spleen,with the main effects of restoring qi,followed by activating blood and resolving stasis.The association rules and complex network suggested that"Radix Rhemanniae Praeparata→Rhizoma Drynariae"had the strongest correlation,and the high-frequency herbal medicines were divided into six cate-gories by cluster analysis.Conclusion:Tonifying and replenishing the liver and kidney,activating blood and dredging meridians are the priorities in the treatment of CIBP by TCM compound prescriptions,and"Radix Rhemanniae Praeparata→Rhizoma Drynariae"was the core combination,which reflects the characteristics of reinforcing healthy qi and eliminating pathogenic factor as well as simultaneous treatment of the symptoms and root cause.
Objective:To explore the potential active components and mechanism of Loki zupa(LKZP)for asthma treatment through pharmacodynamic evaluation,network pharmacology and molecular docking.Methods:The anti-asthmatic activity of LKZP was studied in ovalbumin(OVA)-induced asthma model mice.The chemical components of LKZP collected from literature and their targets were screened by Lipinsk's Rule of Five.After their intersection with the asthma targets,the anti-asthma targets of LKZP were determined.The core targets of LKZP for asthma treatment were determined by constructing a protein-protein interaction(PPI)network of therapeutic targets using the String.Then GO and KEGG enrichment analyses were proformed on the core targets by the DAVID.Finally,a component-target-pathway network was built by Cytoscape,and the binding energy of active components and tar-gets was calculated by molecular docking.Results:A total of 136 potential active compounds and 841 targets of LKZP were screened,with 66 core targets.According to the network analysis,the key targets of LKZP for asthma treatment were MAPK1,EG-FR,MAPK3,PIK3CA,AKT1,PIK3CB,PIK3CD,PIK3R1 and PRKCA.The signaling pathways were mainly related with the regula-tion of airway inflammation(such as PI3K-Akt signaling pathway,ErbB signaling pathway,and estrogen signaling)and the regula-tion of airway remodeling(such as HIF-1 signaling pathway,VEGF signaling pathway,and calcium signaling pathway).Finally,the molecular docking indicated a strong binding energy between key active components and targets.Conclusion:Sesquiterpenes,fla-vonoids and phenylpropanoids,the key active components in LKZP,regulate the airway inflammation and airway remodeling by mul-tiple targets and pathways,thereby treating asthma.
