Zusammenfassung Kongenitale portosystemische Shunts (CPSS), auch bekannt als Abernethy Malformation, gehören zu den seltenen angeborenen Gefäßfehlbildungen des portalvenösen Kreislaufs. Das Blut aus den Bauchorganen fließt dabei ganz oder teilweise an der Leber vorbei. Diese Umgehung des Leberkreislaufes kann je nach Ausmaß und Lokalisation unterschiedliche klinische Folgen haben. Neben asymptomatischen Verläufen sind auch schwere metabolische, neurologische oder pulmonale Komplikationen beschrieben. Durch moderne Bildgebung wie der Duplexsonographie, der Magnetresonanztomographie und der Katheterangiographie lässt sich der Shunt heute genau beurteilen. Auch therapeutisch hat sich vieles verändert. Während früher die Lebertransplantation als einzige kurative Option galt, steht heute der oft sequenzielle Verschluss des Shunts im Vordergrund. In den meisten Fällen kann der physiologische portalvenöse Blutfluss durch interventionelle oder chirurgische, minimal-invasive Verfahren wiederhergestellt werden. Unter Umständen entsteht jedoch eine portale Hypertension aufgrund hypoplastischer Pfortadergefäße. Dieser Beitrag fasst aktuelle Erkenntnisse zu Entstehung, Diagnostik und Behandlung zusammen. Abstract Congenital portosystemic shunts (CPSS), also known as Abernethy malformation, are rare congenital vascular anomalies of the portal venous circulation. In this condition, blood from the abdominal organs is completely or partially diverted past the liver. Depending on its extent and location, the shunt can have various clinical consequences. In addition to asymptomatic courses, severe metabolic, neurological, or pulmonary complications have also been described. Modern imaging techniques such as duplex sonography, magnetic resonance imaging, and catheter angiography now allow for a much more accurate assessment of the shunt. Much has also changed in terms of treatment. Whereas liver transplantation used to be the only curative option for severe shunts the focus today is on often sequential closure of the shunt. In most cases physiological portal venous blood flow can be restored using interventional or surgical, minimally invasive procedures. In selected cases, shunt closure may be associated with the development of portal hypertension due to hypoplastic portal venous structures. This article summarizes current findings on the development, diagnosis, and treatment of this condition.
