目的:探讨经腹胆双镜术结合茵陈柴胡汤治疗胆囊结石合并胆总管结石的临床效果及对患者肝功能、免疫功能的影响.方法:选取丰城市中医院 2020 年 1 月-2021 年 12 月收治的胆囊结石并胆总管结石患者 82 例,按随机数字表法分为对照组和观察组,各 41 例.对照组予以经腹胆双镜术治疗,观察组在对照组基础上加用茵陈柴胡汤治疗,术后 14 d评价比较两组临床疗效、肝功能指标、免疫功能指标及并发症,术后 3 个月比较两组中医症候积分.结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05).观察组术后 14 d谷草转氨酶(GOT)、总胆汁酸(TBA)及谷丙转氨酶(GPT)水平分别为(57.63±5.49)U/L、(16.58±2.11)μmol/L、(52.96±5.42)U/L,均低于对照组的(65.47±5.76)U/L、(21.05±2.28)μmol/L、(60.24±6.19)U/L,差异均有统计学意义(P<0.05).观察组术后 14 d CD4+、CD4+/CD8+分别为(44.79±4.05)%、(1.67±0.21),均高于对照组的(40.31±3.88)%、(1.42±0.19),CD8+为(26.89±2.04)%,低于对照组的(28.32±2.11)%,差异均有统计学意义(P<0.05).观察组术后 3 个月上腹饱胀、恶心呕吐、腹痛、压痛积分分别为(0.65±0.11)、(0.52±0.11)、(0.71±0.12)、(0.68±0.12)分,均低于对照组的(0.87±0.14)、(0.79±0.12)、(0.89±0.14)、(0.82±0.13)分,差异均有统计学意义(P<0.05).观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05).结论:联用经腹胆双镜术及茵陈柴胡汤可提高胆囊结石合并胆总管结石患者疗效,减轻肝功能损害,加快免疫功能恢复,降低中医症候积分,减少并发症发生,安全可靠.
目的：对比腹腔镜和传统开腹手术治疗胆石症的疗效差异。方法回顾性分析我院124例胆石症患者的临床资料，随机均分为对照组（开腹胆囊切除术）和实验组（腹腔镜胆囊切除术），比较两组手术时间、切口长度、术中出血量、术后住院时间和并发症发生率。结果实验组手术时间、切口长度、术中出血量、术后住院时间、并发症发生率均明显低于对照组（P＜0.05），临床效果优于对照组。结论腹腔镜胆囊切除术具有出血少、切口小、恢复快、并发症少等优点，临床疗效优于传统开腹手术治疗胆石症。
Objective:To study the clinical effect of bedsore ointment in the treatment of decubital ulcer. Method:68 patients with decubital ulcer were randomly divided into control group and treatment group according to random number table method in our hospital From June 2009 to June 2011,the control group was treated by the conventional method of treatment of decubital ulcer,the treatment group was treated by bedsore ointment. The recent clinical efficacy and decubitus ulcer healing time were observed in the two groups of patients. Result:The treatment efficiency in the control group was 76.5%,and the treatment efficiency in the treatment group was 91.2%,the treatment efficacy in the treatment group was better than that in the control group,the differences was statistically significant (P<0.05). The decubital ulcer healing time in the treatment group was shorter than that in the control group,and the difference was statistically significant (P<0.05). Conclusion:The bedsore ointment has better clinical curative effect in the treatment of decubital ulcer,and shorten the healing time,it is worth in clinical promotion.
