目的 探讨持续静脉镇痛对食管癌手术患者苏醒质量及早期认知功能的影响.方法 选择2016年1月至2018年12月在南通大学附属肿瘤医院择期行食管癌手术患者46例,随机分为硬膜外镇痛组(A组)和持续静脉镇痛组(B组).比较2组视觉模拟量表(VAS)评分、布鲁格曼舒适量表(BCS)评分、术后恢复质量15项量表(QoR-15)评分等指标.结果 2组术毕自主呼吸恢复时间、苏醒时间、拔管时间、Steward苏醒评分达到6分时间比较,差异均无统计学意义(P>0.05).B组拔管后24、48、72 h苏醒期Ramsay镇静评分优于A组,差异均有统计学意义(P<0.05).B组术后谵妄、认知功能障碍、苏醒期间躁动及镇痛引起的不良反应(皮肤瘙痒除外)发生率优于A组,差异均有统计学意义(P<0.05).B组静息及运动时VAS、BCS评分及QoR-15评分优于A组,差异均有统计学意义(P<0.05).结论 食管癌手术患者行持续静脉镇痛可有效改善患者苏醒质量,降低术后谵妄、认知功能障碍、苏醒期间躁动及镇痛引起的不良反应发生率,有利于患者早期下床活动,可提高患者治疗满意度及舒适度.
在复杂碎屑岩储层中,测井解释的含水饱和度结果往往受井眼条件、测井质量等因素影响而存在较大偏差.为了更精确地表征含水饱和度的空间分布特征,以乌干达Albert盆地J油田为例,应用与储层物性和烃柱高度有关的SHF(Saturation Height Function)方法推导了该油田含水饱和度计算公式,即首先根据特殊岩心分析实验参数,将毛管压力转化到地层条件下,再对每个样品采用Lambda公式(Sw=ApcB+C)拟合,得到各条毛管压力曲线对应的系数A、B、C;然后通过对各系数与储层孔隙度和渗透率进行回归分析,获得各系数与对应物性的函数关系,从而得到含水饱和度计算公式,并通过对比计算结果与测井解释结果进行差异性检验.应用效果表明,本文公式得出的饱和度结果与测井解释结果具有极高的一致性,对于薄层砂岩能够提供更精确的结果,而且含水饱和度用数学公式表示后还能够更加方便地应用到三维地质模型和动态油藏模型中.
Objective To observe the effect of dexmedetomidine on the quality of recovery from general anaesthesia in the patients undergoing thoracic surgeries .Methods Sixty patients undergoing thoracic surgeries were equally randomized into three groups ,who anesthetized with same drugs and managements .During operation ,the patients were infused dexmedetomidine 0.2μg?kg-1 ?h-1 (group D1) ,0.4μg?kg-1 ?h-1 (group D2) or normal saline(group C) until the end of operation .The quality of recovery from general anaesthesia was evaluated and compared among three groups .Results The incidence of postoperative agitation was significantly lower in groups of D 1 and D2 than that in group C(P<0 .05) .The time for the patients to get the Steward recovery score of 6 points was shorter in group D1 than that in groups of C and D2(P<0 .05) .Compared to groups of C and D1 ,the times for the recovery of spontaneous breathing ,awakening and extubation in group D2 were longer(P<0 .05) . The Ramsay sedation score at 2 hours after extubation was better in group D1 than that in groups of C and D2 ( P<0 .05 ) .Conclusion Infusion of low-dose dexmedetomidine during operation may effectively improve the quality of recovery from general anaesthesia in the patients undergoing thoracic surgeries .
Because the heterogeneity of H oilfield is strong,so the single permeability interpretation model can′t meet the needs of heterogeneity reservoir permeability interpretation.In order to improve the accuracy of permeabili-ty interpretation,the lithology,property and pore structure characteristics were analyzed based on 36.5 m cores and 399 samples assay data.The reservoir rock types were divided into four categories:coarse sandstone,medium sand-stone,clay laminated sandstone and calcareous sandstone.The differences of pore structure and flow ability in each rock type were quantitative showed.And the permeability accurate interpretation model of each type has been estab-lished.The result shows that the heterogeneity of reservoir was revealed,and the model of each type has been used to calculated permeability,the accuracy of permeability interpretation was improved.The method of permeability ac-curate interpretation was verified by the well test of new well.
海外K油田内油层的电阻率变化范围较大,经取样证实同一套层系中高电阻率油层和低电阻率油层并存,给流体性质的测井解释带来很大的困难.本文通过对低阻油层的成因进行分析,根据不同原因造成的低阻油层进行分类识别.利用大量的测压资料回归流体密度可以识别地层水矿化度变化引起的低阻油层,利用三分量感应测井及成像、核磁等测井新技术可以准确的解释砂泥岩薄互层形成的低阻油层.含水饱和度是储量计算中的重要参数,针对低阻油层的含水饱和度计算,采用了不同地层水电阻率的计算模式,并通过毛管压力曲线对所计算的含水饱和度做了标定,证实计算的含水饱和度合理.
阿尔及利亚Ⅹ区块三叠系的主要目的层段是TAGI组,它属于典型的低幅圈闭油气藏,其储层类型为河流相的砂岩储层.在新完钻的两口井中均钻遇了低阻油层,其成因十分复杂,油水识别比较困难.本文基于低阻油层的测井曲线响应特征,深入分析了该区低阻油层的成因,主要为高地层水矿化度、双重孔隙系统并存、泥质含量重及砂泥岩薄互层的影响.在此基础之上,建立了基于岩心分析、成像测井和电缆测压资料的低阻油层饱和度的计算模型,能准确计算研究靶区低阻油层的饱和度,为海外油气储量的准确评估奠定了坚实的基础.
海外K油田并购后初步计算储量与外方储量有100多百万桶的差距,储量急待重新计算,储层参数作为储量计算的重要参数需要尽可能的准确.K油田的岩性比较简单为砂泥岩互层,但是受沉积环境影响,储层的非均值性比较强.在孔隙度的计算中存在部分井缺少中子密度曲线的情况,利用邻井资料确定压实系数利用单声波计算孔隙度.由于区块中高阻和低阻油层并存,在评价饱和度的过程中,优选了解释模型,并利用不同的地层水电阻率计算.利用测压资料确定了该区的储层参数下限值.重新计算的储层参数与岩心资料误差更小,计算的储量更加合理.
The fine log evaluation of complex reservoirs has been a problem of conventional log interpretation. This article introduces QC methods to improve the coincidence rate of complex reservoirs log interpretation. Through a comprehensive analysis, finds the main factors affect the accuracy of complex reservoirs log interpretation and takes appropriate measures to implement. Practice has proved that the QC method worked better in the complex reservoirs log interpretation and played a good project management role.
