The medial opening wedge high tibia osteotomy(HTO) is an effective treatment method in patients with osteoarthritis of knee and varus deformity.This study analyzed the relative factors that may influence the outcome of operation,such as age,weight,joint space narrowing and corrective angle,and then the handling strategies of all problems were presented simultaneously.

Objective: To investigate the expression of stathmin/ oncoprotein 18(Op18) in human osteosarcoma and its relation to biological behaviors of osteosarcoma.Methods: The stathmin protein expressions of 56 cases with human osteosarcoma and 10 cases of normal bone tissues were examined by immunohistochemical staining and Western blot methods.Results: The positive rate of stathmin in normal human bone tissues,G1 grade and G2 grade were 20%,65%,100% respectively.The difference between normal human bone tis-sues and G1 grade,G2 grade were significant(P0.05)respectively;the difference between G1 grade and G2 grade were significant(P0.05).Expression of stathmin in normal human bone tissues,G1 grade and G2 grade were(0.34±0.15),(0.68±0.21),(0.90±0.17)respectively.The difference be-tween normal human bone tissues and G1 grade,G2 grade were significant(P0.01) respectively;the difference between G1 grade and G2 grade were significant(P0.01).Conclusion: Overexpression of stathmin plays an important role in development and progression of human osteosarcoma.

[Objective]To observe the therapeutic effect of human parathyroid hormone1-34(rhPTH1-34)on osteoporosis of ovariectomized rats and its relation to blood calcium,phosphorus and growth factors. [Methods]Seventy-four ovariectomized(OVX)rats were divided into model control group(OVX group without any treatment),rhPTH1-34 treatment group(PTH group with treatment for 8 weeks),and sham operation group.BMD of femur were measured with Hologic dual energy X-ray 4 500 W bone desitometer in rats.Serum sclerostin levels and BGP concentrations were determined by ELISA. [Results]BMD of the femur in rhPTH1-34 group and sham group were higher than those in OVX control group(P0.01).the serum sclerostin and BGP in rhPTH1-34 group were significantly different with OVX control group(P0.01),but the calcium,phosphorus and ALP had no significent difference with OVX group. [Conclusion]rhPTH1-34 can effectively prevent the loss of BMD in femur,increase the serum BGP and decrease sclerostin levels without obvious effect on calcium,phosphorus and ALP contents.

[Objective]To evaluate the clinical and radiographic results of convalescent total hip arthroplasty in a bony fused hip.[Method]From August 1999 to April 2006,36 hips(32patients) underwent THA.There were 18 males and 14 females,aged from 21-53 years old(average 36.5 years old).Twenty hips were fused by spontaneous fusion and sixteen hips were fused by surgery.The average period of fusion was 18 years and 7 months.Preoperatively,16 patients complained of lower back pain,ipsilateral or contralateral knee or hip pain.Twenty-one patients were unable to do light house work,due to severe limb discrepancy and hip deformity,prior to the operation.We evaluated both the clinical and radiographic parameters.[Result]All the patients were followed up for a mean of 6 years and 11 months(range from 4 to 10 years).All incision healed by first intention.None deep infection occurred.The mean Harris hip score increased from preoperative 52.7 points to postoperative 85.1 points.The average range of hip motion were-1.4°±4.1°of flexion contracture,86°±10.1°of further flexion,25°±6.2°of external rotation,20.1°±5.3°of internal rotation,29.7°±6.3°of abduction and 21.3°±5.1°of adduction.In a radiographic evaluation,osteolysis was seen in 5 hips.One cup and one stem were revised because of the aseptic loosening.The heterotopic ossification was found in 10 hips.[Conclusion]The alescent THA is an effective method for reconstruction of fused hip,which could provide pain relief,improve the leg discrepancy,correct the fixed deformity,and obtain the hip joint motion.

Objective To study allogenic tendon transplantation to reconstruct interarticular ligament by dual beam ligament scilicet for anteromedial beam (AM) and posterolateral beam (PL) to re-establish anterior cruciate ligament(ACL), to achieve the condition that can restore or re-establish the ligament with the power of vitodynamics articular genu curvature movement is equilibrate and when the joint is convolution it can stabilize it. Methods 67 cases were undergone ACL reconstruction using allogenic tendon. Through elliptic tunnel on the side of condyles of femur, using transversalis nail fix method that Arthrax company provided, and through double tunnel tibia to reconstruct ACL for AM and PL with crushing nail. Results 67 cases were followed-up for 12～36(18.4±3.8) months. Before operation Lysholm score was 34.47±1.5, after operation 3 months it was 78.35±3.4, 6 months it was 81.88±3.3, 12 months it was 87.76±2.1, 18 months it was 89.70±3.5. These cases that have arthrodial cartilage injury influence the Lysholm score of after operation. Conclusions Allogenic tendon can achieve the requirement of ligament recipient site, dual beam ACL re-establish consistent with 4 connecting rod curvature biologic motor pattern of ligament of knee joint, gyratus stability is good. Operative procedure is simple, and effect is good.
