Parathyroid hormone (PTH) is one of the osteophilic endocrine hormones, which is involved in a variety of cellular biological processes. Previous studies have shown that PTH can inhibit the hypertrophic differentiation and apoptosis of chondrocytes and stimulate their proliferation, which has a moderate protection function on the cartilage cells by multiple signal pathways. In this paper, the mechanism of PTH in osteochondral tissue was reviewed.
目的 探讨多学科综合诊疗(MDT)联合以问题为基础的学习(PBL)教学法对老年髋部骨折的临床教学效果.方法 选取2017年1月至2018年12月广州医科大学五年制临床医学系在广州医科大学附属第一医院骨科进行临床实习同级学生共90名,随机分为试验组(MDT模式下PBL组)45名和对照组[传统教学以讲授为基础的学习(LBL)组]45名.分别按照各组教学方式进行老年髋部骨折临床示教,比较2组的教学效果与教学满意度.结果 试验组理论考试成绩较对照组高,但差异无统计学意义(P>0.05);而试验组的病例分析成绩较对照组高,同时教学满意度亦明显高于对照组,差异均有统计学意义(P<0.05).结论 MDT模式下PBL教学法在老年髋部骨折临床教学中收到良好的效果,可提高学生的自学能力,值得在临床教学中进一步推广.
Objective To explore the clinical curative effect of the platelet-rich plasma approach to treat early osteoarthritis of knee .Methods The autologous platelet rich plasma was injected into articular-cavity of 19 patients with knee osteoarthritis from August 2016 and February 2017 in the First Affiliated Hospital of Guangzhou Medical University . All the patients were early knee osteoarthritis ,excluding late knee osteoarthritis and infectious arthropathy .The treatment was administered in series of three intra-knee-articular injections at 3-week interval .The visual analogue scale ( VAS ) ,International Knee Documentation Committee scale (IKDCS),and the Lysholm knee scale (LKS) were used assessing the effect of treatment .SPSS 20.0 was applied to carry out data processing analysis ,and analysis of variance about repeated measurement data was adopted .Results All the patients were followed up for six months.No severe complication related to the PRP injection was observed .Significant differences were found between pre-and post-injection VAS(F=32.183),IKDCS(F =66.005),and LKS(F=65.193) in 19 patients ( all P<0.01);the significant differences were also found among the postoperative 6th month,the postoperative 3rd and 1st months in all the patients ( all P <0.05 ).Conclusion The early osteoarthritis of knee can be treated with the PRP injection which could obtain short-term satisfactory clinical effect .
BACKGROUND:Parathyroid hormone can activate a series of physiological and biochemical reactions through specific receptors on the surface of target organs,to adjust the dynamic calcium balance.OBJECTIVE:To explore the mechanisms whereby parathyroid hormone maintains in vitro proliferation of bone marrow mesenchymal stem cells (BMSCs),by observing the effects of parathyroid hormone on calcium ions in BMSCs.METHODS:BMSCs at passage 4 were randomly divided into two groups.Experimental group was treated with parathyroid hormone,and control group was treated with routine culture medium.During 10 days of culture,MTT assay was used to detect BMSCs proliferation.At 7 and 14 days after treatment,laser scanning confocal microscopy was used to detect fluorescence intensity of calcium ions in BMSCs in the two groups.RESULTS AND CONCLUSION:(1) At 1 and 2 days after culture,the cell viability in the two groups was sluggish.At 3 days after culture,the cells turned into the rapid growth.At 7 and 8 days after culture,the cell growth turned into the plateau.At 10 days after culture,the cell proliferation ability had a weakening trend,which was lower in the control group than in the experimental group (P < 0.05).(2) At 7 days after culture,the fluorescence intensity of intracellular calcium ions was strong.As the induction time extended,the fluorescence intensity of intracellular calcium ions was declined at 14 days after-culture.At 7 and 14 days after culture,the fluorescence intensity of intracellular calcium ions in the experimental group was higher than that in the control group (P < 0.001).There was no difference in the fluorescence intensity of intracellular calcium ions between experimental group at 14 days and control group at 7 days.To conclude,parathyroid hormone could increase the fluorescence intensity of calcium ions in BMSCs,and function to maintain the BMSCs proliferation.
Objective To summarize the treatment experience of unstable Colles' fracture in elderly patients.Methods All cases in this group used volar T-plate internal fixation combined with calcium phosphate cement(CPC) for treatment.Results 30 cases in this group were followed up with osseous healing.According to Gartland Werley standard assessment,20 cases were optimal,8 cases were good and 2 cases were general,with the excellent rates of 93.33%.Conclusion Volar T-plate internal fixation combined with CPC for the treatment of unstable Colles'fracture in elderly patients is an effective method.
